El tratamiento de las fracturas vertebrales metastásicas sin compresión neural se realiza con técnicas percutáneas de cementación. El aumento de presión intratumoral por estas técnicas puede enviar células tumorales al torrente sanguíneo. Para evitar esa diseminación y mejorar el tratamiento del dolor se han introducido las técnicas de ablación que permitirían crear una cavidad en el tumor previo a la cementación o directamente necrosar la metástasis cuando el tamaño es pequeño.

Treatment of metastatic vertebral fractures without neural compression is performed with percutaneous cementation techniques. The increase in intratumoral pressure by these techniques can send tumor cells into the bloodstream. To prevent this dissemination and improve pain treatment, ablation techniques have been introduced that would allow the creation of a cavity in the tumor prior to cementation or directly necrosing the metastasis when its size is small.

El concepto de artritis idiopática juvenil (AIJ) engloba un grupo heterogéneo de artritis crónicas de causa desconocida que aparecen antes de los 16 años, y que persisten durante más de 6 semanas una vez excluidas otras causas. Se caracterizan por la presencia de inflamación articular y se acompañan en mayor o menor medida de manifestaciones sistémicas. Es la enfermedad reumática inflamatoria crónica más frecuente en la infancia. Los grupos para su clasificación aceptados en la actualidad son los descritos por la International League of Associations for Rheumatology(ILAR); sin embargo, recientemente se han propuesto nuevas modificaciones a estos criterios. La AIJ tiene una etiopatogenia multifactorial, mecanismos inmunológicos y ambientales actúan en un individuo genéticamente predispuesto. Salvo el subtipo de AIJ sistémica que tiene una base autoinflamatoria, el resto de las formas de AIJ poseen un claro componente autoinmune. El diagnóstico precoz y un adecuado tratamiento resultan cruciales para evitar las complicaciones. Este tratamiento debe ser individualizado en función de las manifestaciones clínicas y el grado de afectación orgánica. Además de los corticoides y los fármacos modificadores de la enfermedad sintéticos (FAMES), desde la introducción de los FAME biológicos el pronóstico de la AIJ ha experimentado una notable mejora.

This case report describes an 11-year-old boy with spastic diplegia whose reflex status, range of motion (ROM), strength, and motor performance were measured before and after implantation of an indwelling system for delivery of intrathecally administered baclofen. Before baclofen use, the subject experienced clonus that interfered with walking, needed assistance with transfers, and was unable to independently put on underwear and socks. Measures of spasticity, kinematics and electromyographic activity during voluntary movements, ROM, Gross Motor Function Measure (GMFM) scores, and self-reports of change were obtained at baseline, before and after bolus baclofen injection, during a double-blind placebo-controlled clinical trial of baclofen administration via an indwelling pump, and after 1 and 2 years of baclofen therapy. Spasticity, Babinski reflexes, clonus, strength, and coactivation of antagonist muscles during voluntary movement were decreased shortly after baclofen administration began. Hip and ankle ROM increased, upper-extremity movement speed increased, independence in dressing and transfers improved, and orthoses were discarded. After 1 and 2 years, GMFM scores were 7.8% and 6.4% above baseline, respectively; the subject won a fitness award. After 2 years, ROM was worse than at baseline and concerns regarding hip subluxation arose. Background. As computer hardware becomes less expensive and computer software more sophisticated and easy to use, more physicians arc using computers to take notes and keep records. Although computer use offers many benefits, there is concern about whether the use of computers in the examination room will interfere with the patient-physician relationship. This experiment surveyed patient satisfaction following examination by either of two physicians, one using pen-and-pencil note-taking, the other taking notes on a computer. Methods. Sixty patients consented to participate in this prospective, randomized, crossover study. In the first phase, 15 randomly selected patients were examined by Physician A, who made a written record, and 15 by Physician B, who made a computer record. In the second phase, the physicians switched roles. After the examination, patients completed a questionnaire to assess their degree of satisfaction. Results. There were no significant differences in satisfaction between the group whose physician made a handwritten record as compared with those whose physician used a computer, nor was there an interaction between type of note-taking and physician. There was also no correlation between patient satisfaction and previous patient exposure to and use of computers. Conclusions. This study demonstrated no decrease in patient satisfaction when a computerized patient record was introduced. Key words. The purpose of this article is to review the literature on the effects of intrathecally administered baclofen on impairment in spasticity and muscle activation patterns, on functional limitations in mobility and self-care, and on disability in daily life roles. We found plentiful evidence of improvement in spasticity, spasms, and bladder function and some reports of improved patterns of muscle activation and kinematics of single-joint movement. Improved ability to accomplish transfers, self-care, and locomotion is less consistently studied but has also been reported in about 60% to 70% of patients. Evidence of improved quality of life is primarily anecdotal but may be found in 10% to 30% of patients. Background and Purpose. The purpose of this study was to investigate the effects of prolonged practice of a simple motor task (a fast, unidirectional single-joint movement) on different indexes of motor performance in individuals with Down syndrome. Subjects. Eight individuals with Down syndrome were tested before and after practice involving 1,100 movements. Methods. The test consisted of three series of elbow flexion movements. In the first series, the subjects were asked to move "as fast as possible" over four distances. In the second series, the subjects moved over one distance at a "comfortable speed." The initial position of the elbow joint for these two series of movements was 55 degrees into flexion (full elbow extension equals 0 [degrees]). In the third series, the subjects were also asked to move "as fast as possible" over two distances, but from another initial elbow position (73 [degrees] into flexion). Results. After training over a 2-week period, all subjects improved their performance on all tasks as reflected by both kinematic and electromyographic data. In particular, they increased the quantity of the agonist activity decreased the antagonist onset latency, and doubled their peak velocity. They were able to transfer the improvement in their performance to the nontrained distances and to the different starting position. Subjects decreased their movement time by proportionally decreasing both the acceleration and deceleration times. Conclusion and Discussion. This study supports the idea that subjects with Down syndrome can use patterns of muscle activation that are qualitatively indistinguishable from those used by individuals who are neurologically normal. In this article, several solar energy application possibilities in forestry and industrial processes are foreseen and discussed, focused on the case of cork harvesting and processing, in world areas where solar radiation is high. Cork processing to obtain several cork-based products has several and different operational steps in which electricity, heat and thermal fluids are necessary. Different types of existing solar technologies are considered, for the specific forestry and industrial steps of cork transformation. Most of these technologies are not yet being used in the cork sector. Besides this, the use of different cork derived products in several solar energy technologies and applications is also assessed and discussed. Cork is a sector where solar energy may be used in various ways and cork material could be a component in various solar energy systems/devices, increasing its domain of applications.

Acute myeloid leukemias (AMLs) result from a series of genetic events occurring in a stem or progenitor hematopoietic cell that gives rise to their clonal expansion and an impaired capacity to differentiate. To circumvent the genetic heterogeneity of AML patient cohorts, we have developed a model system, driven by the MLL-AF9 (MA9) oncogene, to generate multiple human leukemias using progenitor cells from a single healthy donor. Through stepwise RNA-sequencing data generated using this model and AML patients, we have identified consistent changes associated with MA9-driven leukemogenesis and demonstrate that no recurrent secondary mutations are required. We identify 39 biomarkers whose high expression level is specific to this genetic subtype of AML and validate that many of these have diagnostic utility. We further examined one biomarker, the receptor tyrosine kinase (RTK) RET, and show through shRNA knockdowns that its expression is essential for in vivoand in vitrogrowth of MA9-AML. These results highlight the value of novel human models of AML derived from single donors using specific oncogenic fusions to understand their biology and to uncover potential therapeutic targets.

Reconocer la importancia de prevenir y tratar precozmente las náuseas y los vómitos postoperatorios (NVPO) es fundamental para evitar complicaciones postoperatorias, mejorar la satisfacción del paciente y permitir el desarrollo de la cirugía mayor ambulatoria y de la cirugía fast-track.El tema de las NVPO podría parecer estancado, pero seguimos avanzando. Aparecen nuevos conceptos y problemas como las náuseas y vómitos postalta, nuevos factores de riesgo y nuevos fármacos. Por otro lado, siguen existiendo ideas erróneas, como asociar las NVPO con la estancia en la unidad de recuperación postanestésica o asumir como factores de riesgo características del paciente, de la anestesia o de la cirugía que realmente no lo son. Debemos enfrentarnos a las NVPO de otro modo, implementando el uso de las guías clínicas en nuestros centros y apostando por una profilaxis más agresiva en determinados grupos de pacientes. Presentamos a continuación una amplia revisión del tema.

There is an increasing interest in Faecalibacterium prausnitzii, one of the most abundant bacterial species found in the gut, given its potentially important role in promoting gut health. Although some studies have phenotypically characterized strains of this species, it remains a challenge to determine which factors have a key role in maintaining the abundance of this bacterium in the gut. Besides, phylogenetic analysis has shown that at least two different F. prausnitzii phylogroups can be found within this species and their distribution is different between healthy subjects and patients with gut disorders. It also remains unknown whether or not there are other phylogroups within this species, and also if other Faecalibacterium species exist. Finally, many studies have shown that F. prausnitzii abundance is reduced in different intestinal disorders. It has been proposed that F. prausnitzii monitoring may therefore serve as biomarker to assist in gut diseases diagnostics. In this mini-review, we aim to serve as an overview of F. prausnitzii phylogeny, ecophysiology and diversity. In addition, strategies to modulate the abundance of F. prausnitzii in the gut as well as its application as a biomarker for diagnostics and prognostics of gut diseases are discussed. This species may be a useful potential biomarker to assist in ulcerative colitis and Crohn’s disease discrimination.

Naturally occurring and sustainable materials can be used as a template to create biomimetic/biomorphic ceramics, known as Ecoceramics (environmentally conscious ceramics). In this work, cork was chosen as template to produce novel ceria (CeO2) ecoceramics, for applications in water splitting for H2production via direct concentrated solar thermochemical fuel production (TCFP). The cork powder was pyrolised at 900°C and the resulting carbon skeleton was infiltrated with an aqueous CeO2precursor, and then heated at 1000°C for 2h to produce the ecoceramic. The cellular structure of the cork was maintained, with hexagonal cell dimensions of 20-30μm in diameter, but the grains were nanoscale at ≤100nm. XRD data confirmed that CeO2was the only crystalline phase obtained. An important feature was that, while the rectangular side walls were maintained to hold the three-dimensionally ordered macroporous (3DOM) cellular cork structure, the rear hexagonal walls were pierced repeatedly through the structure, unlike in the original cork structure, which will allow gasses such as H2to permeate well into the structure, greatly increasing the reactive area available for catalysis. The next step will be to test the capabilities of both the regular, porous 3DOM structure and the nanoscale grains for thermochemical water splitting to produce hydrogen under direct concentrated solar energy.

We compute the normal form description of a semiconductor laser near its threshold, with the sole assumption that the polarization be related to the electric field through the susceptibility (dependent on laser frequency and population inversion). We prove both analytically and numerically the possible existence of a phase-unstable regime, characterized by a periodic oscillation of the optical frequency and a constant intensity. This regime bears close resemblance to the mode-switching behavior, with constant total output power, experimentally observed in semiconductor lasers. In addition, our model predicts the appearance of a phase- and amplitude-turbulent regime, compatible with experimental observations. Some equilibrium configurations of the cholesteric unwinding transition problem are numerically computed using the Landau-de Gennes expression of the liquid crystal free energy density. The classical TIC and modulated TIC configurations are numerically obtained and the cholesteric finger structure is found to be in good agreement with the previous theoretical guesses. Moreover, we report the existence of new solutions, specifically associated with the local $\bf n\rightarrow -n$symmetry, and which seem to be experimentally observed.

When a cholesteric liquid crystal is sandwiched between two horizontal glass plates with homeotropic anchoring, chiral patterns named cholesteric fingers of first class, are known to occur. When a DC vertical electric field is applied, the Cholesteric Finger is then observed to move perpendicularly to its long-axis with a velocity V⊥. In this paper, we prove analytically that this velocity may be related with the Lehmann rotation effect, and that it may provide a new way to measure the electromechanical coupling coefficient.

Probabilistic networks can be a useful tool for reliability modelling and risk analysis in the operation of technical facilities. In the article, the authors present the method of using probabilistic networks in the analysis of threats to the elements of the electric bus power supply system with a hydrogen fuel cell. Both the use of probabilistic networks as a tool supporting decision-making processes and the use of the latest achievements in the field of non-emission propulsion indicate a new area of research. The article presents the construction of a hydrogen bus with particular emphasis on the hydrogen storage and transmission installation. Next one specific case is considered where there is a leak in the hydrogen system with the simultaneous appearance of an ignition initiator in the form of a short circuit in the electrical system. Authors considered Fault Tree-Event Tree method in context of Bayesian networks technology. The conducted research has shown that there is a real risk of hydrogen explosion in a hydrogen fuel cell-powered bus and that it can be accurately calculated.

Tumor necrosis factor receptor–associated periodic syndrome (TRAPS) is a rare autosomal dominant disease included in the group of autoinflammatory syndromes. It is characterized by recurrent episodes of fever and inflammation in different regions of the body. The main clinical manifestations are myalgia, migratory erythematous rash, periorbital edema, and abdominal pain. The diagnosis is reached using gene analysis and prognosis depends on the appearance of amyloidosis secondary to the recurrent episodes of inflammation. Tumor necrosis factor inhibitors and corticosteroids are the most widely used treatments. In recent years, significant advances have been made in the diagnosis and treatment of TRAPS, thanks to a better understanding of its pathogenesis. Dermatologists must be aware that the skin manifestations of TRAPS are particularly important, as they are often diagnostic.

We carefully numerically simulate the full three-dimensional (3D), two-level Maxwell Bloch equations in a ring configuration, beyond the rotating wave and slowly varying envelope approximations. The 3D electromagnetic spatial and temporal variations are then well described up to the micrometer and femtosecond scales. Genuine dielectric waveguide boundary conditions together with no-reflection, perfectly matching layers are implemented. Besides the already known and well-described transverse instabilities of a traveling-wave solution, we observe steady zipper-state configurations characterized by the coexistence, in the laser transverse plane, of two longitudinal counter-propagating waves, with their separation plane parallel to the ring laser optical axis. The predictive value of coronary computed tomographic angiography (cCTA) in subjects without chest pain syndrome (CPS) has not been established. We investigated the prognostic value of coronary artery disease detection by cCTA and determined the incremental risk stratification benefit of cCTA findings compared with clinical risk factor scoring and coronary artery calcium scoring (CACS) for individuals without CPS.

Abstract: We investigated how richness and composition of vascular plant species in the understory of a mixed hardwood forest stand varied with respect to the abundance and composition of the overstory. The stand is in central Spain and represents the southernmost range of distribution of several tree and herbaceous species in Europe. Understory species were identified in 46 quadrats (0.25 m2) where variables litter depth and light availability were measured. In addition, we estimated tree density, basal area, and percent basal area by tree species within 6-m-radius areas around each plot. Species richness and composition were studied using path analysis and scale-dependent geostatistical methods, respectively. We found that the relative abundance of certain trees species in the overstory was more important than total overstory abundance in explaining understory species richness. Richness decreased as soil litter depth increased, and soil litter increased as the relative proportion of Fagus sylvatica in the overstory increased, which accounted for a negative, indirect effect of Fagus sylvatica on richness. Regarding understory species composition, we found that some species distributed preferentially below certain tree species. For example, Melica uniflora was most frequent below Fagus sylvatica and Quercus petraea while the increasing proportion of Q. pyrenaica in the overstory favored the presence of Cruciata glabra, Arenaria montana, Prunus avium, Conopodium bourgaei, Holcus mollis, Stellaria media and Galium aparine in the understory. Overall, these results emphasize the importance of individual tree species in controlling the assemblage and richness of understory species in mixed stands. We conclude that soil litter accumulation is one way through which overstory composition shapes the understory community.

The ac Stark effect is a well-known, well-established effect associated with the displacement of the atomic energy levels in the presence of an electromagnetic (em.) field. As the origin of the e.m. field is irrelevant, a self-induced ac Stark effect should be present in a laser. However, not only has this effect never been reported but it is not included in any laser model, Here, we show that a self-induced ac Stark shift does exist in the theoretical description of a two-level laser, provided that the reduced equations are derived not from the rotating-wave approximation, but from the full density matrix formulation. Interesting fundamental consequences are discussed and quantitative estimates of the self-induced frequency shift resulting from the laser field's ac Stark shift are obtained for a variety of lasers. We find that the effect is generally small but (at least in some cases) measurable and that the transfer of laser amplitude noise into frequency noise may play a role in future, ultrahigh-precision metrological applications. We theoretically describe the spontaneous formation of stable pulses in a GaAs bulk semiconductor microring laser. These pulses are obtained without active or passive mode locking. We show that the parameter regime associated with their existence is limited on one side by the phase instability of the continuous-wave solution, and on the other side by the failure of Lamb's mode-locking criterion. The present study has been carried out to evaluate the corrosion performance of three different (Ti1–xAlx)N/TiN multilayers coatings commercially deposited on WC–Co inserts. Two of these coatings were nanostructured, and all of them presented different Ti/Al atomic ratios varying from 2 : 3 up to 7 : 3 and were deposited by employing a commercial physical vapour deposition cathodic arc process. X-ray diffraction analysis indicated that the grain size for both the TiAlN and TiN layers in the coatings architecture, varied between approximately 9 to 14 nm. The corrosion performance was evaluated in a 3 wt-%NaCl solution using both open circuit potential and potentiodynamic polarisation measurements. Scanning electron microscopy and energy dispersive spectroscopy surface characterisation before and after corrosion testing allowed the determination that all the coatings under study show a corrosion current density of approximately one order of magnitude smaller than the uncoated substrate, indicating thus their beneficial effect from the corrosion protection viewpoint.
