Introduction: The aim of this clinical registry is to record the use of CytoSorb® adsorber device in critically ill patients under real-life conditions. Methods: The registry records all relevant information in the course of product use, e.?g., diagnosis, comorbidities, course of the condition, treatment, concomitant medication, clinical laboratory parameters, and outcome (ClinicalTrials.gov Identifier: NCT02312024). Primary endpoint is in-hospital mortality as compared to the mortality predicted by the APACHE II and SAPS II score, respectively. Results: As of January 30, 2017, 130 centers from 22 countries were participating. Data available from the start of the registry on May 18, 2015 to November 24, 2016 (122 centers; 22 countries) were analyzed, of whom 20 centers from four countries provided data for a total of 198 patients (mean age 60.3 ± 15.1 years, 135 men [68.2%]). In all, 192 (97.0%) had 1 to 5 Cytosorb® adsorber applications. Sepsis was the most common indication for CytoSorb® treatment (135 patients). Mean APACHE II score in this group was 33.1 ± 8.4 [range 15–52] with a predicted risk of death of 78%, whereas the observed mortality was 65%. There were no significant decreases in the SOFA scores after treatment (17.2 ± 4.8 [3–24]). However interleukin-6 levels were markedly reduced after treatment (median 5000?pg/ml before and 289?pg/ml after treatment, respectively). Conclusions: This third interim report demonstrates the feasibility of the registry with excellent data quality and completeness from 20 study centers. The results must be interpreted with caution, since the numbers are still small; however the disease severity is remarkably high and suggests that adsorber treatment might be used as an ultimate treatment in life-threatening situations. There were no device-associated side effects.

Wir berichten über den Fall einer 48-jährigen Frau, bei der vermutlich durch eine intramuskuläre Glukokortikoidinjektion eine Phäochromozytomkrise ausgelöst wurde. Im Rahmen dieser Krise kam es zur Entwicklung eines kardiogenen Schocks mit Reanimationspflichtigkeit, woraufhin die Implantation einer venoarteriellen extrakorporalen Membranoxygenierung (VA-ECMO, Cardiohelp®, Maquet, Rastatt) notwendig wurde. Zur Behandlung eines durch die VA-ECMO entstandenes Lungenödem wurde eine Impella-CP® (Abiomed, Aachen) implantiert. Im weiteren Verlauf führten wir eine Koronarangiographie, die Entnahme von linksventrikulären Endomyokardbiopsien sowie eine Computertomographie (CT) des Thorax und des Abdomens durch. Letztendlich zeigte sich eine 7 cm mal 5 cm große Tumorformation im Bereich der linken Nebenniere. Im Serum konnten wir deutlich erhöhte Katecholaminwerte nachweisen, sodass wir bei der Patientin die Diagnose eines Phäochromozytoms stellen konnten. Als endgültige Therapie wurde im Verlauf eine operative Tumorexzision durchgeführt. We report the case of a 48-year old woman where probably the intramuscular administration of glucocorticoids by an orthopedist induced a pheochromocytoma crisis. The development of a cardiogenic shock with a cardiac arrest made the use of a venoarterial extra corporeal membrane oxygenation (VA-ECMO, Cardiohelp®, Maquet, Rastatt) necessary. To treat a pulmonary edema under VA-ECMO an Impella-CP® (Abiomed, Aachen) was implanted. A coronary angiography, endomyocardial biopsies and a computer tomography were performed. Finally a 7 cm × 5 cm tumor mass in the left adrenal medulla was detected. The blood values presented high levels of catecholamines. The total results of the examinations led us to the final diagnosis of a pheochromocytoma which was finally surgical removed.

Abstract:  Two cases of lymphoma-associated acquired C 1-inhibitor deficiency are described. In both patients, C 1-inhibitor deficiency and related symptoms preceded the diagnosis of the underlying neoplasm by several months. C 1-inhibitor deficiency was most likely due to consumption following immunocomplex formation. In both patients, a close relationship between low levels of C 1-inhibitor and tumor relapse was observed during follow-up. These findings indicate that measurement of C 1-inhibitor and complement factor C4 can be used as markers of disease activity in affected patients.

Zusammenfassung Auf oszillographischem Wege wird die Geschwindigkeit und die Beutellänge der Ummagnetisierungswelle bei gespanntem Permalloydraht (60% Ni) gemessen und daraus die kritische FeldstärkeH0 bestimmt. Mit diesem Wert wird aus dem Zusammenhang Keimdicke-Startfeldstärke die Wandenergie ermittelt. An einer Reihe von Keimen wird der Gültigkeitsbereich der Döringschen Keimtheorie untersucht.

Zusammenfassung Der von W. Westphal behauptete Potentialsprung unmittelbar vor der GlÜhkathode einer Fadenstrahlentladung wird nachgeprÜft, und es wird festgestellt, da\ ein solcher Sprung nicht existiert. Die Experimente von Westphal sowie eigene Experimente können durch einen erst steil und dann flach ansteigenden Potentialverlauf mit gro\er Genauigkeit gedeutet werden. Einige bei dieser Untersuchung gemachte Beobachtungen an der Fadenstrahlentladung werden mitgeteilt.
