Teen dating violence (TDV) is a pervasive issue that can have a variety of negative effects on those who have been victimized. Prior research shows that state-level laws have an impact on the extent of TDV; however, scant research has analyzed the variations in the language used in the legislation. The present study analyzes the existing state legislation for addressing TDV in schools and compares the content of the legislation. This study provides a detailed analysis of each of the states’ legislation. It examines differences in how states conceptualize TDV and provides details demonstrating the variations of state legislation. We utilized qualitative descriptive content analysis and purposive sampling with maximum variation. A total of 27 states have legislation to address TDV in schools. Findings demonstrate that states have great variation in legislation addressing TDV in schools. Some states appear to take a minimalistic approach in specifying legislation on how schools should address TDV, other states provide their schools with general guidelines to address TDV, and a few states provide very specific guidelines for schools to follow. This study is an important step to understanding what components of state TDV legislation in schools are most universal and might impact the prevalence of TDV. Future research is needed to identify the components of state TDV legislation that are related to impacting TDV prevalence.

Breast cancer, a leading cause of death in women, can be treated conservatively or by radical mastectomy, removal of all breast tissue. Although the use of the conservative approach to breast cancer care is increasing, many mastectomies are still being performed. Up to 60 percent of patients opting for radical mastectomy have wound complications after surgery. To study the factors contributing to wound complications, the period following surgery was reviewed in 101 patients having a modified radical mastectomy. The influence of age, weight, type of operative technique (knife or electrocautery), amount of blood loss, amount and location of lymph node involvement, the number and duration of wound drains and the duration of hospital stay were evaluated. Seroma, a tumor-like collection of serum which can lead to tissue damage, infection, swelling and long-term problems were found in 10.9 percent of the patients. Of the other complications common after radical mastectomy surgery, 8.9 percent had wound infections, five percent had wound necrosis (tissue death), three percent developed hematomas (collections of blood masses), two percent had lymphedema (lymph swelling) and one percent had pneumothorax (an abnormal pocket of air in the lung space). The use of electrocautery, a surgical technique using intense heat to manipulate tissues, was associated with 44 percent more wound complications when compared with the knife technique of mastectomy. ABSTRACTNonprofits’ engagement with stakeholders ensures their existence since engagement guarantees support for the organization. Social work researchers, leaders, and educators must consider engagement within the context of geographic and online communities. Indeed, learning how to engage with individuals and communities is a core competency within social work education. We believe that it is important to interpret this competency as a mandate for educators who are preparing social work students for nonprofit leadership roles in an online society. After introducing this perspective, we segue into a discussion of our findings from a study of 145 nonprofits’ websites. Findings suggest that there are opportunities for nonprofit leaders to enhance stakeholder engagement through their website designs; we discuss implications for social work education.

ABSTRACTIn 2015, 81% of counties in Texas were designated as full or partial Mental Health Professional Shortage Areas. To address this problem, we worked with a community-based association of mental health practitioners, using the social theory of career selection, to explore factors that influence students to join the mental health field. Our goal was to better understand how to recruit and retain mental health-trained students. We interviewed bachelor and master-level mental health interns in social work, counseling, and psychiatric nursing and field instructors using semistructured, open-ended interviews. Five themes emerged from the interviews: 1) attraction to mental health; 2) professional experience; 3) impact capacity; 4) develop social work skills; and 5) intern preparedness.

ABSTRACTSocial enterprise is a burgeoning, although nebulously defined, strategy for linking market-based revenues with social good in nonprofit, for-profit, and public organizations. This article offers a unifying definition of social enterprise, tracks its development throughout traditional sectors, and takes a first step in extending the concepts of social enterprise to the higher education sector. We argue that administrators of university centers associated with schools of social work should employ social enterprise principles. Key benefits would be to gain revenue for the centers, break down disciplinary silos in the university, better link social work schools to the world outside academe, help train students in an emerging organizational model, and increase civic engagement in the university.

ABSTRACTPositive organizational psychology suggests that researchers should focus on the rewarding elements of work life, yet those in the fields of social work and nonprofit administration have not conducted research in line with this admonition. Indeed, the current focus on administrative challenges and problems may be part of the reason there is currently a leadership crisis in human services nonprofits. This article describes an exploratory qualitative study of human service executives’ perceptions of their work, focusing on what brings them joy in their jobs. We found five categories of joyful experiences. Implications for future practice, education, and research are included.

The perceived apathy of the US public towards elections is actually a result of their alienation from legislators. Voters feel they have no influence over legislators' actions once they have been elected to office. This feeling of helplessness may be eliminated through comparisons of incumbents' legislative votes with their campaign platforms. Using an evaluation form similar to a scorecard, voters can check if candidates acted on issues the way they promised to during the campaign. The form has been submitted to newspapers for possible inclusion in dailies.

The perceived wisdom in the social work education community, based on empirical research from the 1990s and the early part of this century, says that the master of social work (MSW) degree is not competitive with the master of business administration or the master of public administration to obtain top-level administration jobs in nonprofit agencies. This article reviews recent information on hiring preferences of board chairs that provides new hope that the MSW degree with a concentration in management or administration is perceived as a viable and useful degree for top-level positions in human services agencies. Thus, current board chairs are more inclined to value the MSW than administrators and academics from years past. The reason is that the values of social workers are seen as quite valuable in the current environment.

Government support for human services programs de creased considerably during the 1980s while defense spending rose dras tically. The shift in spending priorities is thought by some to be due to insufficiently active or incapable social welfare lobbyists. This paper ar gues that human services advocacy groups were as active as supporters of defense programs. Human services advocates were shut out of the de cision-making process primarily due to the lack of public support for their ideas. The implication, that social welfare advocates must build a solid base of public support for their proposals, can be applied to current issues.

Geographic Information Systems (GISs) are pictorial representations of information and are quite useful in understanding social welfare problems and showing social work clients in their environment. This paper presents GIS as a potential tool for social work research, planning, management, and administration. It includes sections defining GID, explaining the possibilties for GIS use in human services, presenting a case study using geographic information, advising potential GIS users, and discussing the implications for social work education.

IMPORTANCE: Conventional adjuvant radiotherapy for breast cancer given daily for several weeks is onerous and expensive. Some patients may be obliged to choose a mastectomy instead, and some may forgo radiotherapy altogether. We proposed a clinical trial to test whether radiotherapy could be safely limited to the tumor bed. OBJECTIVE: To determine whether delayed second-procedure targeted intraoperative radiotherapy (TARGIT-IORT) is noninferior to whole-breast external beam radiotherapy (EBRT) in terms of local control. DESIGN, SETTING, AND PARTICIPANTS: In this prospective, randomized (1:1 ratio) noninferiority trial, 1153 patients aged 45 years or older with invasive ductal breast carcinoma smaller than 3.5 cm treated with breast conservation were enrolled from 28 centers in 9 countries. Data were locked in on July 3, 2019. INTERVENTIONS: The TARGIT-A trial was started in March 2000; patients were randomized after needle biopsy to receive TARGIT-IORT immediately after lumpectomy under the same anesthetic vs EBRT and results have been shown to be noninferior. A parallel study, described in this article, was initiated in 2004; patients who had their cancer excised were randomly allocated using separate randomization tables to receive EBRT or delayed TARGIT-IORT given as a second procedure by reopening the lumpectomy wound. MAIN OUTCOMES AND MEASURES: A noninferiority margin for local recurrence rate of 2.5% at 5 years, and long-term survival outcomes. RESULTS: Overall, 581 women (mean [SD] age, 63 [7] years) were randomized to delayed TARGIT-IORT and 572 patients (mean [SD] age, 63 [8] years) were randomized to EBRT. Sixty patients (5%) had tumors larger than 2 cm, or had positive nodes and only 32 (2.7%) were younger than 50 years. Delayed TARGIT-IORT was not noninferior to EBRT. The local recurrence rates at 5-year complete follow-up were: delayed TARGIT-IORT vs EBRT (23/581 [3.96%] vs 6/572 [1.05%], respectively; difference, 2.91%; upper 90% CI, 4.4%). With long-term follow-up (median [IQR], 9.0 [7.5-10.5] years), there was no statistically significant difference in local recurrence-free survival (HR, 0.75; 95% CI, 0.57-1.003; P = .052), mastectomy-free survival (HR, 0.88; 95% CI, 0.65-1.18; P = .38), distant disease-free survival (HR, 1.00; 95% CI, 0.72-1.39; P = .98), or overall survival (HR, 0.96; 95% CI, 0.68-1.35; P = .80). CONCLUSIONS AND RELEVANCE: These long-term data show that despite an increase in the number of local recurrences with delayed TARGIT-IORT, there was no statistically significant decrease in mastectomy-free survival, distant disease-free survival, or overall survival. ObjectiveTo determine whether risk adapted intraoperative radiotherapy, delivered as a single dose during lumpectomy, can effectively replace postoperative whole breast external beam radiotherapy for early breast cancer.DesignProspective, open label, randomised controlled clinical trial.Setting32 centres in 10 countries in the United Kingdom, Europe, Australia, the United States, and Canada.Participants2298 women aged 45 years and older with invasive ductal carcinoma up to 3.5 cm in size, cN0-N1, eligible for breast conservation and randomised before lumpectomy (1:1 ratio, blocks stratified by centre) to either risk adapted targeted intraoperative radiotherapy (TARGIT-IORT) or external beam radiotherapy (EBRT).InterventionsRandom allocation was to the EBRT arm, which consisted of a standard daily fractionated course (three to six weeks) of whole breast radiotherapy, or the TARGIT-IORT arm. TARGIT-IORT was given immediately after lumpectomy under the same anaesthetic and was the only radiotherapy for most patients (around 80%). TARGIT-IORT was supplemented by EBRT when postoperative histopathology found unsuspected higher risk factors (around 20% of patients).Main outcome measuresNon-inferiority with a margin of 2.5% for the absolute difference between the five year local recurrence rates of the two arms, and long term survival outcomes.ResultsBetween 24 March 2000 and 25 June 2012, 1140 patients were randomised to TARGIT-IORT and 1158 to EBRT. TARGIT-IORT was non-inferior to EBRT: the local recurrence risk at five year complete follow-up was 2.11% for TARGIT-IORT compared with 0.95% for EBRT (difference 1.16%, 90% confidence interval 0.32 to 1.99). In the first five years, 13 additional local recurrences were reported (24/1140 v11/1158) but 14 fewer deaths (42/1140 v56/1158) for TARGIT-IORT compared with EBRT. With long term follow-up (median 8.6 years, maximum 18.90 years, interquartile range 7.0-10.6) no statistically significant difference was found for local recurrence-free survival (hazard ratio 1.13, 95% confidence interval 0.91 to 1.41, P=0.28), mastectomy-free survival (0.96, 0.78 to 1.19, P=0.74), distant disease-free survival (0.88, 0.69 to 1.12, P=0.30), overall survival (0.82, 0.63 to 1.05, P=0.13), and breast cancer mortality (1.12, 0.78 to 1.60, P=0.54). Mortality from other causes was significantly lower (0.59, 0.40 to 0.86, P=0.005).ConclusionFor patients with early breast cancer who met our trial selection criteria, risk adapted immediate single dose TARGIT-IORT during lumpectomy was an effective alternative to EBRT, with comparable long term efficacy for cancer control and lower non-breast cancer mortality. TARGIT-IORT should be discussed with eligible patients when breast conserving surgery is planned.Trial registrationISRCTN34086741, NCT00983684.
