The article focuses on Duncan Sandys, chair of the government rocket and flying bomb countermeasures CROSSBOW committee, confidently announced that the Battle of London, comprising the V–1 flying bomb attacks, was over and that the public could relax, and because of Allied advances through northern France, discounted the apocalyptic predictions of rocket attacks. Topics include the grave concern because of alarmist intelligence assessments of the size of warheads and predicted scale of attacks.

Chondromyxoid fibroma (CMF) is a rare, benign, cartilaginous bone tumor that commonly affects the metaphysis of the lower-extremity long bones in young adults. Involvement of the craniofacial bones, especially the temporal bone, is exceedingly rare. We report a case of a 64-year-old man who presented with a left ear mass, but on subsequent imaging was found to have a more extensive mass centered in the temporal bone. Initial biopsy raised the concern for a low-grade chondrosarcoma. However, following surgical resection, a diagnosis of CMF was rendered. The clinical findings, radiographic features, histology, and differential diagnoses are discussed.

The Upper Jurassic Norphlet Formation is an eolian sandstone and important hydrocarbon reservoir that overlies the Louann Salt in the Gulf of Mexico (GOM). Because the sand was concentrated into dunes formed by Late Jurassic winds, determining the source areas and paleotransport direction of the sand can improve predictions of the distribution of the dune facies around the GOM. Paleo-wind-blown sediment transport into the proto-GOM was controlled by wind direction and magnitude and the extant topography of the basin and adjacent uplands. Analysis of the Norphlet Formation in the eastern GOM shows that wadis and alluvial fans controlled by the location of highs were the primary route for introducing sediment of varied provenance into the eolian erg. Eolian transport directions interpreted from dip-log analyses are south directed in southern Alabama and west to northwest directed in western Florida. Interpretations of regional, two-dimensional, prestack-depth-migrated seismic data show that erosional incision of the Middle Ground arch occurred prior to and during the time of Norphlet deposition; this as well as preexisting lows in the basement topography may have facilitated basinward sand transport of sediment that fed the Norphlet Formation erg preserved in the deep-water subsurface eastern GOM. Oncocytic adenocarcinoma of the orbit is a rare tumor, with 1 case of nonlacrimal sac, nonlacrimal gland origin, and a poor outcome previously reported. An 85-year-old man with a 2-month history of left-sided epiphora, enlarging eyelid nodules, and diplopia in left gaze was found on imaging to have a poorly circumscribed, nodular mass of uniform radiodensity in the inferomedial orbit. Incisional biopsy revealed morphologic and immunohistochemical features of oncocytic adenocarcinoma with origin in the caruncle suspected, and CT of the neck, chest, abdomen, and pelvis showed no metastases or remote primary tumor source. Based on multidisciplinary consensus, orbital exenteration with adjuvant radiation therapy was performed, and there was no evidence of residual or recurrent tumor 2 years after treatment.A patient with orbital oncocytic adenocarcinoma, the second reported case of non-lacrimal sac, non-lacrimal gland origin, was treated with orbital exenteration and adjuvant radiation remaining clinically and radiographically tumor free at two years.

IMPORTANCE Epistaxis is the most common otolaryngologic emergency in the United States. Most cases are controlled with first-line measures, but intractable epistaxis can be a challenging clinical problem requiring posterior nasal packing and surgical or endovascular intervention. Bleeding from nasal telangiectasias is well known in hereditary hemorrhagic telangiectasia, but there are no reports in the literature of recurrent epistaxis due to isolated telangiectasias not associated with systemic disease. This report describes a series of cases in which intractable epistaxis due to isolated primary telangiectasias was effectively controlled with bipolar electrosurgery. OBSERVATIONS We describe a patient with intractable epistaxis that had failed management with posterior packing and embolization. We also report a series of 16 cases of epistaxis, 6 of which were intractable, in adults without hereditary hemorrhagic telangiectasia who received treatment between 2009 and 2012. These cases reveal a common pattern of bleeding from telangiectasias on the anterior septum (8 cases [42%]), nasal sidewall (3 [16%]), inferior meatus (2 [10%]), posterior septum (2 [10%]), nasal floor (2 [10%]), middle turbinate (1 [5%]), and inferior turbinate (1 [5%]). CONCLUSIONS AND RELEVANCE Telangiectasias not associated with systemic disease are a previously unreported source of significant nasal bleeding that, when identified endoscopically, can be treated successfully with bipolar electrosurgery rather than with more invasive and costly surgical and endovascular measures.

Objectives:Although oropharyngeal neoplasia can often lead to dysphagia, salivary gland tumors rarely grow within the tongue base. We present the case of a 75-year-old man with adenoid cystic carcinoma of the base of the tongue causing profound dysphagia and weight loss, and provide a current literature review and update on the management of these rare tumors.Methods:We present a case report and a literature review.Results:Physical examination performed at the initial visit revealed a firm right base-of-tongue mass with no palpable lymphadenopathy. Flexible fiberoptic laryngoscopy confirmed a large submucosal mass at the right base of the tongue that obscured the right vallecula. Histopathologic analysis of the operative biopsy specimens revealed the classic features of adenoid cystic carcinoma. Treatment included radical pharyngotomy with wide local excision and primary closure, followed by postoperative radiation treatment.Conclusions:We demonstrate the clinical examination findings and histopathologic characteristics of this disease, and review the literature for clinical treatment recommendations for this rare cause of dysphagia.
