Zusammenfassung: Allogene Transplantate sind in der rekonstruktiven Kniegelenkchirurgie bei ligamentären Verletzungen ein fester Bestandteil der Therapieoptionen. Während bei der primären vorderen Kreuzbandrekonstruktion die Bedeutung von allogenem Sehnenmaterial zumindest im europäischen Raum noch gering ist, steigt jedoch deren Gebrauch bei Revisionsoperationen am vorderen Kreuzband deutlich an. Bei Rekonstruktionen des hinteren Kreuzbandes und der posterolateralen Ecke bieten sich allogene Sehnentransplantate ebenfalls als eine nicht zu vernachlässigende Alternative an. Bei Multiligamentrekonstruktionen am Kniegelenk kann durch die Verwendung von allogenem Material das bereits traumatisierte periartikuläre Weichteilgewebe vor einer weiteren iatrogenen Traumatisierung geschützt werden. Der vorliegende Artikel beschäftigt sich mit den Einsatzmöglichkeiten und klinischen Resultaten von allogenen Transplantaten bei Bandrekonstruktionen des Kniegelenks. Des Weiteren soll auf die immunologischen und biologischen Grundlagen bei Verwendung von allogenem Sehnenmaterial, deren Verfügbarkeit, Aufbereitung und Sicherheit näher eingegangen werden.

Zusammenfassung: Die Infektion eines Gelenks nach vorangegangener arthroskopischer Operation ist mit einer Inzidenz von bis zu 0,42% zwar ein äußerst seltenes Ereignis, jedoch können die Folgen bei verspäteter Diagnose dramatisch sein. Neben einer genauen Anamneseerhebung ist zur Diagnostik die Bestimmung des C-reaktiven Proteins, insbesondere jedoch die diagnostische Gelenkpunktion erforderlich. Für Infekte früher Stadien hat sich die arthroskopische Therapie bewährt. Zusätzlich essentiell ist die kalkulierte und dann antibiogrammgerechte antibiotische Therapie. Bei persistierenden Infektzeichen muss die Indikation zur Rearthroskopie rasch und großzügig gestellt werden. Die Anzahl der notwendigen Revisionen hängt vom initialen Stadium des Infekts ab. Eine postoperative Immobilisation des betroffenen Gelenks kann gelegentlich zur Schmerztherapie erforderlich sein, ansonsten sollte die frühzeitige Mobilisation des Gelenkes durchgeführt werden.

Objective To evaluate whether a sandwich technique procedure for large osteochondral lesions (OCL) of the medial femur condyle reduces clinical symptoms and improves activity level as well as to assess repair tissue integration on MRI over 2 years.Design Twenty-one patients (median age: 29 years, 18-44 years) who received matrix-associated autologous chondrocyte transplantation (MACT) combined with cancellous bone grafting at the medial femur condyle in a 1-step procedure were prospectively included. Patients were evaluated before surgery (baseline) as well as 3, 6, 12, and 24 months postoperatively, including clinical evaluation, Lysholm score, Tegner Activity Rating Scale, and MRI with Magnetic Resonance Observation of Cartilage Repair Tissue (MOCART) score and a modified Whole-Organ Magnetic Resonance Imaging Score (WORMS).Results Seventeen patients were available for the 24-month (final) follow-up (4 dropouts). Lysholm significantly improved from 48 preoperatively stepwise to 95 at final follow-up (P< 0.05). Tegner improvement from 2.5 at baseline to 4.0 at final follow-up was not significant (P= 1.0). MOCART score improved significantly and stepwise from 65 at 3 months to 90 at 24 months (P< 0.05). Total WORMS improved from 14.5 at surgery to 7.0 after 24 months (P< 0.05). Body mass index and defect size at surgery correlated with total WORMS at final follow-up (P< 0.05) but did not correlate with clinical scores or defect filling.Conclusion MACT combined with cancellous bone grafting at the medial femoral condyle reduces symptoms continuously over 2 years. A 1-step procedure may reduce perioperative morbidity. However, despite improvements, patients’ activity levels remain low, even 2 years after surgery.

Die Ätiopathogenese der osteochondralen Läsion des Capitulum humeri ist bis heute nicht abschließend geklärt. Abzugrenzen von der osteochondralen Läsion ist der M. Panner, einer juvenile Osteonekrose des gesamten Kapitulums.

Chronische Akromioklavikulargelenk (ACG)-Stabilisierungen sowie Revisionen nach fehlgeschlagenen Initialstabilisierungen stellen weiterhin eine große Herausforderung für den behandelnden Orthopäden dar. In diesen Fällen ist von einer Heilung der korakoklavikulären (CC) Ligamente mit einer suffizienten ACG-Stabilität nicht auszugehen. Verschiedene Verfahren wurden zur Behandlung der chronischen Instabilitäten beschrieben, wobei sich diese in anatomische und nichtanatomische Techniken unterteilen lassen. Der Grundgedanke dieser Verfahren ist neben einer suffizienten Stabilität eine biologische Basis zur späteren Ligamentisation der CC-Ligamente zu schaffen. So entwickelte sich die anatomische, minimalinvasive Operationstechnik mittels TightRope™ (Fa. Arthrex, FL/USA) oder ähnlichen Flaschenzugsystemen in Kombination mit einem freien Sehnentransplantat in den letzten Jahren zu einer akzeptierten und sich zunehmend verbreitenden Behandlungsoption von chronischen ACG-Instabilitäten. Revision surgery following unsuccessful acromioclavicular (AC) joint stabilization and chronic joint instability presents a great challenge in orthopedics. In those cases, sufficient healing of the coracoclavicular (CC) ligaments cannot be expected. Different kinds of procedures are described for the operative treatment of chronic AC joint instability that can be divided into anatomic and non-anatomic techniques. The basic idea is to provide stability and a biological basis for the ligamentization process of the CC ligaments. The anatomic, minimally invasive operation techniques using TightRope™ or similar pulley systems in combination with a free tendon autograft have turned to a widely accepted and used treatment for chronic acromioclavicular (AC) joint separations.

Isolated avulsion fracture of the lesser tuberosity represents an extremely rare injury that occurs mainly in younger patients. This report outlines the case of a 35-year-old healthy man who sustained a bony avulsion of the subscapularis tendon during an abduction external rotation trauma to his left shoulder. The injury was diagnosed by physical examination, standard radiographs, and magnetic resonance imaging. An arthroscopic reconstruction including closed reduction and internal fixation using suture anchors was performed. After a follow-up of 6 months, the patient's Constant Score increased from 62.4 points preoperatively to 91.3 points. At the latest follow-up, the patient was completely pain free, had regained a full work activity level, and returned to overhead sports. On postoperative standard radiographs and magnetic resonance images, the bony fragment was shown to be consolidated in an anatomic position. In cases of displaced isolated fractures of the lesser tuberosity without significant subluxation or dislocation of the long head of the biceps arthroscopic reconstruction using suture anchors can lead to a good clinical and radiological result.

Recent evidence has shown that the superior glenohumeral ligament (SGHL) and coracohumeral ligament (CHL) are important static stabilizers. To clarify the function of these two ligaments, we studied their tensile properties with bone-ligament-bone complexes from fresh-frozen shoulders, 10 SGHLs and 10 CHLs. Each ligament's cross-sectional area was measured, and uniaxial tensile testing of each complex was performed. The stiffness, ultimate load, percent elongation, and energy absorbed to failure of each bone-ligament-bone complex were derived from its load-elongation curve. The cross-sectional area of the coracohumeral ligament was significantly greater than that of the superior glenohumeral ligament at their midportions (CHL, 53.7+/-3.2 mm^2 vs. SGHL, 11.3+/-1.6 mm^2, p<0.05). Results also reveal significant differences between the tensile properties for the two ligaments, with the coracohumeral ligament possessing greater stiffness (CHL, 36.7+/-5.9 N/mm vs. SGHL, 17.4+/-1.5 N/mm, p<0.05) and ultimate load (CHL, 359.8+/-40.3 N vs. SGHL, 101.9+/-11.5 N, p<0.05) than the superior glenohumeral ligament. Our findings confirm that the coracohumeral ligament is an important capsuloligamentous structure of the glenohumeral joint.
