Background: This study investigated the structural-environmental and individual-level social factors associated with the numbers of commercial sex clients of female sex workers (FSWs) per month (CPM). Methods: Data were analyzed from cross-sectional surveys of FSWs in 5 districts in Karnataka state, India (2004-2005). Bivariate and multivariable negative binomial regression models with generalized estimating equations were constructed for each district. Normalized weights were used to account for complex sampling design. Results: Median (average) CPM varied significantly (P < 0.001) across districts, from 40.0 (56.9) CPM in the district with the highest immunodeficiency virus prevalence (33.9%) to 16.0 (26.1) in the district with the lowest prevalence (9.7%). In multivariable analysis, there was a significant positive association between rates of CPM and sex work as sole income in 4 districts (adjusted incidence rate ratios, AIRR: [95% confidence intervals]: AIRR: 1.3 [1.0-1.7], P < 0.005-1.7 [1.2-2.5], P < 0.01); age, typology (place of solicitation), and marital status were significantly associated with CPM in 3 districts, with an inverse association between age and CPM (P < 0.01). FSWs soliciting clients in brothels had higher rates of CPM than home-based FSWs in 2 districts (AIRR: 1.5 [1.1-2.0]-1.6 [1.0-2.7], P < 0.05), whereas public places-based FSWs had higher rates in 1 district (AIRR: 1.4 [1.1-1.8], P < 0.01); cohabiting FSWs had higher rates than married FSWs in 3 districts, whereas single FSWs had higher rates in one district. Conclusions: These results provide support for the continued development of structural core group interventions that address common elements of sex work placing FSWs at higher risk for immunodeficiency virus, as well as geographically focused programs that account for local differences in sexual structure. Background: Syndromic case management remains the cornerstone for STI (sexually transmitted infection) treatment in many countries. We undertook this study to better understand the etiology of STIs in adults in south India and to inform STI management guidelines. Methods: Adult males and females presenting with genital complaints were recruited from clinics in Karnataka state, south India. A questionnaire was administered, physical examination performed, and blood collected for herpes simplex virus-type 2 (HSV-2) and syphilis serology. Men with urethral discharge (UD) and women with vaginal discharge were tested for Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT) and Trichomonas vaginalis (TV). Vaginal swabs were also tested for bacterial vaginosis and yeast infection. Participants with genital ulcers were tested for Treponema pallidum (TP), Haemophilus ducreyi (HD), and HSV-2. human immunodeficiency virus (HIV) testing was offered to all individuals. Results: There were 401 male and 412 female participants, and rates of HIV infection were high (men, 17%; women, 15%). HSV-2 was significantly associated with HIV in men and women. Among men with the complaint of UD, NG was identified in 35%, CT in 10.5%, and TV in 8.5%. Very little NG or CT was detected among women with vaginal discharge. However, bacterial vaginosis was identified in approximately 40% of women, with significant amounts of TV and Candida also detected. HSV-2 was the most commonly identified pathogen among participants with genital ulcer disease, and the clinical distinction of herpetic versus nonherpetic lesions was not helpful. Conclusions: Current STI management guidelines should be reevaluated in south India. Consideration should be given to treating all persons with GUD for both HSV-2 and syphilis, and to adding initial treatment for TV for men with UD in areas of high background prevalence of HSV-2 and TV, respectively. Objectives: To measure the determinants of syphilis among female sex workers (FSWs) in the state of Karnataka, South India. Methods: During 2004-2006, cross-sectional surveys were administered to 2312 FSWs across 5 districts in the state, in the context of a large-scale HIV preventive intervention program. Demographic and behavioral information, and serum (for syphilis, HSV-2 and HIV) and urine specimens (for Neisseria gonorrhoeae and Chlamydia trachomatis) were obtained. Results: The prevalences of lifetime (TPHA positive) and active (RPR and TPHA positive) syphilis were 25.3% and 9.6%. respectively. There was considerable variation in the prevalence between districts, ranging from 10.9% to 37.4% lifetime, and 3.4% to 24.9% active infection. Factors associated with lifetime syphilis were older age, longer duration of sex work, illiteracy, client volume, practising sex work in >1 city, and sex work typology (public solicitation followed by brothel or lodge-based sex). The same typology, client volume, illiteracy, and having been widowed, divorced or deserted, were predictive of active infection. Of the 976 women who had symptoms of an STI, 78.8% had sought medical treatment, behavior that was protective for both outcomes. HIV infection was strongly associated with lifetime (OR 2.0: 95% CI: 1.6-2.6) and active syphilis (OR 2.1; 95% CI: 1.5-2.9).

Population-based serological antibody test for SARS-CoV-2 infection helps in estimating the exposure in the community. We present the findings of the first district representative seroepidemiological survey conducted between 4 and 10 September 2020 among the population aged 5 years and above in the state of Uttar Pradesh, India. Multi-stage cluster sampling was used to select participants from 495 primary sampling units (villages in rural areas and wards in urban areas) across 11 selected districts to provide district-level seroprevalence disaggregated by place of residence (rural/urban), age (5–17 years/aged 18 +) and gender. A venous blood sample was collected to determine seroprevalence. Of 16,012 individuals enrolled in the study, 22.2% [95% CI 21.5–22.9] equating to about 10.4 million population in 11 districts were already exposed to SARS-CoV-2 infection by mid-September 2020. The overall seroprevalence was significantly higher in urban areas (30.6%, 95% CI 29.4–31.7) compared to rural areas (14.7%, 95% CI 13.9–15.6), and among aged 18 + years (23.2%, 95% CI 22.4–24.0) compared to aged 5–17 years (18.4%, 95% CI 17.0–19.9). No differences were observed by gender. Individuals exposed to a COVID confirmed case or residing in a COVID containment zone had higher seroprevalence (34.5% and 26.0%, respectively). There was also a wide variation (10.7–33.0%) in seropositivity across 11 districts indicating that population exposed to COVID was not uniform at the time of the study. Since about 78% of the population (36.5 million) in these districts were still susceptible to infection, public health measures remain essential to reduce further spread.

ObjectivesIn 2013, Kenya’s National AIDS and STI Control Programme established a Learning Site (LS) in Mombasa County to support and strengthen capacity for HIV prevention programming within organisations working with sex workers. A defining feature of LS was the use of a Programme Science approach throughout its development and implementation. We provide an overview of the key components of LS, present findings from 23 months of programme monitoring data, and highlight key Programme Science lessons from its implementation and monitoring.MethodsRoutine monitoring data collected from September 2013 through July 2015 are presented. Individual-level service utilisation data were collected monthly and indicators of interest were analysed over time to illustrate trends in enrolment, programme coverage and service utilisation among sex workers in Mombasa County.ResultsOver the monitoring period, outreach programme enrolment occurred rapidly; condom distribution targets were met consistently; rates of STI screening remained high and diagnoses declined; and reporting of and response to violent incidents increased. At the same time, enrolment in LS clinics was relatively low among female sex workers, and HIV testing at LS was low among both female and male sex workers.ConclusionLessons learnt from operationalising the Programme Science framework through the Mombasa LS can inform the development and implementation of similar LS in different geographical and epidemiological contexts. Importantly, meaningful involvement of sex workers in the design, implementation and monitoring processes ensures that overall programme performance is optimised in the context of local, ‘on-the-ground’ realities. Additionally, learnings from LS highlight the importance of introducing enhanced monitoring and evaluations systems into complex programmes to better understand and explain programme dynamics over time.

BackgroundWe evaluate the impact since 2003 of the Bill & Melinda Gates Foundation's Indian AIDS initiative, Avahan, in four districts of Karnataka, south India. The programme involved comprehensive outreach, clinical care, and other services to reduce the risk of HIV and other sexually transmitted infections (STIs) among high-risk women and men by promoting behavioural changes. Though there were reviews of the programme performance through periodic cross-sectional surveys, the actual impact of the programme could not be assessed in the absence of a suitable control group.
