2. Brugada syndrome associated with J waves in multiple leads and “pseudo-epsilon” wiggle waves in lateral leads: Possible conduction delay in J-wave syndrome

Abstract

We present a case of Brugada syndrome associated with augmentation of multiple J waves and the presence of “pseudo-epsilon” wiggle waves in the lateral leads. Just after ventricular fibrillation (VF), J waves were prominent in leads V1 through V4 and in the inferior leads. Moreover, pseudo-epsilon wiggle waves were recorded at the terminal portion of the QRS-complexes in leads V5 and V6. Oral administration of quinidine eliminated the epsilon-like waves and J waves, thus completely suppressing VF. In this case, both the pseudo-epsilon wiggle waves in the lateral leads and the J waves were thought to be associated with VF. In this case report, we discuss the etiology of J-wave syndrome from the perspective of the appearance of epsilon-like delayed waves.

Abstract

We report a rare case of slow-fast form of atrioventricular nodal reentrant tachycardia with delayed activation in the low septal right atrium (His bundle area). During supraventricular tachycardia (SVT), electrocardiogram showed “pseudo-positive P waves” in II, III, and aVF leads. SVT was induced by atrial extrastimulus with marked AH prolongation (i.e., jump phenomenon). Ventricular pacing showed a decremental retrograde conduction without jump phenomenon. Double atrial potentials were observed in the His bundle area during SVT and during ventricular pacing. The first electrogram of these split potentials, which was the earliest activation during SVT and during ventricular pacing, showed a dull and small deflection, whereas the second electrogram was sharp and clear. The interval of these discrete potentials was 70 msec during SVT. After a standard slow pathway ablation, SVT could never be induced by any programmed stimuli. It was concluded that in this case, the conduction disturbance across the tendon of Todaro was likely to cause the delayed atrial activation in the His bundle area, which created the pseudo-positive (biphasic. retrograde P wave.

7. Kinetics of FrequencyDependent Conduction Delay by Class I Antiarrhythmic Drugs in Human Atrium

Abstract

We investigated use-dependent prolongation of interatrial conduction time (IACT) by class I antiar-rhythmic drugs in 16 patients. Changes in IACT at the initiation of atrial pacing were used to evaluate the onset kinetics. We examined recovery kinetics by giving a single extra stimulus with a varying coupling interval after discontinuing train stimulation. Time constants of the onset kinetics were 1.52 ± 0.15/n(fast) and 0.087 ± 0.031/ n(slow) for mexiletine, 0.075 ± 0.015/n for aprindine, 0.078 ± 0.019/n for disopyramide, and 0.050 ± 0.006/n for pilsicainide. The recovery time constants were 203 ± 66 ms for mexiletine, 1,021 ± 162 ms for aprindine, 993 ± 101 ms for disopyramide, and 2,930 ± 569 ms for pilsicainide. Class I antiarrhythmic drugs produced use-dependent IACT prolongation in humans, with characteristic kinetics for each agent similar to that of depression of the maximum upstroke velocity of cardiac action potential (Vmax) reported in in vitro studies.

8. Electrophysiological effects of flecainide acetate on stretched guinea pig left atrial muscle fibers

Abstract

The electrophysiological effects of flecainide acetate (3×10-6 M) on stretched atrial tissue were investigated using guinea-pig left atrial muscle fibers. Before stretching, the resting membrane potential was not affected by flecainide at 1 Hz, although the overshoot potential (Eov) and the action potential duration at 50% repolarization (APD50) were slightly but significantly decreased by 2±1 mV and 2±1 msec, respectively. The effective refractory period (ERP) was increased by 3±1 msec. The reduction of $$\dot V$$ max was 20.6±1.2%. The half-maximum potential (Vh) of the relationship between $$\dot V$$ max and the resting potential was shifted to become more negative by flecainide (from -60.6±2.1 mV to -63.2±1.7 mV). After 90–120 min of washout with drug-free Tyrode's solution, the tissue was mechanically stretched to 150% of its slack length. Stretching significantly decreased the $$\dot V$$ max by 16.9±3.1%, along with a slight but significant increase in ERP (3±1 msec) and shifted Vh to become more negative (from -60.6±2.1 to -63.1±1.8 mV). In the presence of flecainide, $$\dot V$$ max further decreased by 20.2±2.6%, and Vh shifted from -63.1±1.8 to -65.0±1.5 mV. Comparison with the control unstretched fibers showed that flecainide significantly decreased $$\dot V$$ max by 34.0±2.7%, reduced the resting membrane potential by 3±1 mV, decreased Eov by 4±1 mV, and shifted Vh from -60.6±2.1 to -65.0±1.5 mV, while the APD50 and ERP did not change. In conclusion, the reduction of $$\dot V$$ max in the presence of flecainide was much greater in the stretched atrial muscle fibers than in the unstretched fibers, because the $$\dot V$$ max-resting potential relationship was shifted towards more negative potentials by both flecainide and stretching. These results suggest that flecainide exerts a stronger antiarrhythmic action on stretched atrial muscle fibers than on normal fibers.
