Purpose: To compare, over a 48-hr follow-up period, the analgesia and side-effects of patient controiledivanalgesia (PCA) with morphine alonevscombined intrathecal and PCA morphine (IT+PCA) in patients undergoing major abdominal surgery. Methods: Sixty adult patients undergoing abdominal surgery for cancer were randomly allocated to receive preoperative IT (0,3 or 0,4 mg) plus postoperative PCA morphine or postoperative PCA morphine alone. Postoperative analgesia was tested at rest and while coughing on a visual analogue pain scale and morphine consumption was recorded. Patients’ satisfaction, arterial oxygen saturation, respiratory rate, episodes of nausea, vomiting and pruritus were also noted. Results: Analgesia at rest and while coughing was significantly better in the IT+PCA morphine group (rest:P= 0,01; coughing:P= 0,005) on the first postoperative day only, IT+PCA morphine constantly provided adequate analgesia during this period. Morphine consumption was lower in the IT+PCA morphine group during this period also (IT+PCA: 9 (17)vsPCA: 40 (26); mg of morphine, mean (SD),P= 0,0001), No difference was found in pain relief and morphine consumption between the groups on the second postoperative day. Nausea and vomiting were more frequent with IT+PCA morphine on the first postoperative day. No respiratory depression occurred in either group. Satisfaction was high in both groups. Conclusions: IT+PCA morphine improves patient comfort constantly during the first postoperative day after major abdominal surgery. However, after the first postoperative day, IT+PCA morphine provides no additional benefit.

Although keratinocyte growth factor (KGF) protects against experimental acute lung injury, the mechanisms for the protective effect are incompletely understood. Therefore, the time-dependent effects of KGF on alveolar epithelial fluid transport were studied in rats 48–240 h after intratracheal administration of KGF (5 mg/kg). There was a marked proliferative response to KGF, measured both by in vivo bromodeoxyuridine staining and by staining with an antibody to a type II cell antigen. In controls, alveolar liquid clearance (ALC) was 23 ± 3%/h. After KGF pretreatment, ALC was significantly increased to 30 ± 2%/h at 48 h, to 39 ± 2%/h at 72 h, and to 36 ± 3%/h at 120 h compared with controls (P< 0.05). By 240 h, ALC had returned to near-control levels (26 ± 2%/h). The increase in ALC was explained primarily by the proliferation of alveolar type II cells, since there was a good correlation between the number of alveolar type II cells and the increase in ALC (r= 0.92,P= 0.02). The fraction of ALC inhibited by amiloride was similar in control rats (33%) as in 72-h KGF-pretreated rats (38%), indicating that there was probably no major change in the apical pathways for Na uptake in the KGF-pretreated rats at this time point. However, more rapid ALC at 120 h, compared with 48 h after KGF treatment, may be explained by greater maturation of α-epithelial Na channel, since its expression was greater at 120 than at 48 h, whereas the number of type II cells was the same at these two time points. β-Adrenergic stimulation with terbutaline 72 h after KGF pretreatment further increased ALC to 50 ± 7%/h (P< 0.5). In summary, KGF induced a sustained increase over 120 h in the fluid transport capacity of the alveolar epithelium. This impressive upregulation in fluid transport was further enhanced with β-adrenergic agonist therapy, thus providing evidence that two different treatments can simultaneously increase the fluid transport capacity of the alveolar epithelium.

Purpose:To describe negative pressure pulmonary edema due to biting of the laryngeal mask tube at emergence from general anesthesia.

La chirurgie du sein est pourvoyeuse de douleurs aiguës de moyenne intensité et de douleurs chroniques dans 27 % des cas [1]. La douleur aiguë est un facteur de risque de douleur chronique postopératoire. Ainsi, depuis quelques années, l’ALR dans la chirurgie du sein s’est développée avec les infiltrations du site opératoire, les blocs para-vertébraux (BPV) et les blocs inter-pectoraux (PECS) dans le but de réduire la consommation de morphine, ses effets secondaires et au-delà la douleur chronique. Dans notre établissement, l’ALR s’est développée à partir de 2012 avec les BPV, réservés aux mastectomies±geste axillaire ou tumorectomies±curage axillaire et les PECS pour les tumorectomies±ganglion sentinelle. Ainsi, en 2015, toutes les patientes opérées d’un cancer du sein bénéficient d’une ALR dans notre hôpital. Nous avons évalué le coût de cette nouvelle activité.

Le bloc paravertébral (BPV), recommandé pour l’analgésie après chirurgie du sein, est de plus en plus pratiqué. Il peut être réalisé « à l’aveugle » par perte de résistance, sous neurostimulation ou à l’aide d’une sonde d’échographie (échoguidage ou échorepérage). Pour les blocs d’espace, l’échographie « est probablement recommandée » car elle permet d’administrer l’anesthésique local plus précisément qu’avec les autres techniques [1]. Les complications du BPV faites par repérage anatomique ou neurostimulation sont peu fréquentes et peu rapportées dans la littérature [2,3]. Par contre, aucune étude ne rapporte l’incidence des complications du BPV réalisé sous échoguidage. Le but de cette étude était donc de colliger de façon prospective sur une grande série de patients les complications du BPV réalisé sous échoguidage pour les tumorectomies+curage axillaire, mastectomies±geste axillaire et l’oncoplastie.

De 2009 à 2014, l’ARS a mis en place la tarification unique et la suppression de la borne basse. Le but de notre étude était d’évaluer l’impact de cette nouvelle tarification sur le développement de la chirurgie ambulatoire en sénologie dans notre établissement. En effet, depuis 2009, nous avons mis en place un circuit ambulatoire mais plusieurs facteurs aussi bien culturels, financiers qu’organisationnels sont apparus comme des freins à ce développement. En 2014, la nouvelle tarification concomitante à un changement de direction et de l’équipe chirurgicale a incité l’ensemble de l’équipe soignante de notre institut à repenser la prise en charge des patients, favorisant le développement de l’ambulatoire.

Le bloc paravertébral (BPV) est recommandé pour l’analgésie après chirurgie majeure du sein [1]. Or, il n’existe pas de moyen de prédiction objectif de l’efficacité du BPV, l’anesthésie au froid étant insuffisante [2]. La pupillométrie pourrait évaluer la profondeur analgésique des BPV sous anesthésie générale, à condition que la concentration de morphinique soit basse [rémifentanil au site effet (Ce)<1ng/mL] [3]. L’objectif de cette étude était d’évaluer l’intérêt de la pupillométrie pour prédire l’efficacité analgésique des BPV en chirurgie carcinologique du sein.
