SUMMARY So far brain metastases represent a critical stage of a disease course and the frequency is increasing over the years. The treatment of brain metastases should be individualized for each patient: in case of single brain metastasis, surgery or radiosurgery should be considered as first options of treatment; in case of multiple lesions, whole-brain radiotherapy is the standard of care in association with systemic therapy or surgery/radiosurgery. Chemotherapy should be considered when surgery or radiation therapy are not possible. In the last decades, TKIs or monoclonal antibodies have shown increase in overall response rate and overall survival in Phase II–III trials. The aim of this paper is to make an overview of the current approaches in management of patients with brain metastases.

Percutaneous lung biopsies (PLBs) performed for the evaluation of pulmonary masses require image guidance to avoid critical structures. A new CT navigation system (SIRIO, “Sistema robotizzato assistito per il puntamento intraoperatorio”) for PLBs was validated.The local Institutional Review Board approved this retrospective study. Image-guided PLBs in 197 patients were performed with a CT navigation system (SIRIO). The procedures were reviewed based on the number of CT scans, patients’ radiation exposure and procedural time recorded. Comparison was performed with a group of 72 patients undergoing standard CT-guided PLBs. Sensitivity, specificity and overall diagnostic accuracy were assessed in both groups.SIRIO-guided PLBs showed a significant reduction in procedure time, number of required CT scans and the radiation dose administered to patients ($$p<0.001$$). In terms of diagnostic accuracy, SIRIO proved to be more accurate for small-sized lesions ($$<$$20 mm) than standard CT-guidance.SIRIO proved to be a reliable and effective tool when performing CT-guided PLBs and was especially useful for sampling small ($$<$$20 mm) lesions.

Background:The aim of the study was to evaluate tolerance and efficacy of preoperative treatment with capecitabine in combination with radiation therapy (RT) in patients with locally advanced, resectable, rectal cancer.

Background: At early stages, the diagnosis of local recurrence of rectal cancer is often difficult and magnetic resonance imaging (MRI) is currently considered the most accurate method for diagnosing recurrence. We evaluated the role of unhenhanced and gadolinium-enhanced MRI for the diagnosis of local recurrence of rectal cancer. Methods: Thirty-six patients, suspected of having a pelvic recurrence of rectal cancer, were evaluated by a high field strength MRI unit. Unenhanced spin-echo T1- and T2-weighted sequences and gadolinium-enhanced dynamic fast multiplanar spoiled gradient recalled sequences were performed in all patients. The dynamic images were re-elaborated with semiquantitative postprocessing by plotting intensity-time curves and calculating the percentage of signal increase at the end of the first postcontrast dynamic sequence. The pelvic lesions were classified as recurrent or not recurrent by applying the following diagnostic criteria: (a) morphology and signal intensity of the lesion in unenhanced sequences and (b) percentage of enhancement in dynamic enhanced sequences. Diagnosis was confirmed by computed tomography-guided needle biopsy (12 patients), surgery (four patients), clinical and imaging follow-up (20 patients). Results: The diagnosis was local recurrence in 15 patients and noncancerous lesions in 21 patients. Unenhanced MRI had 80% sensitivity and 86% specificity. Analysis of the percentage of enhancement showed 87% sensitivity and 100% specificity. Conclusion: In agreement with the literature, our results showed a high sensitivity and specificity for dynamic MRI. This technique thus can be considered an important adjunct to unenhanced MRI, especially in selected cases in which unenhanced MRI cannot rule out local recurrences. However, these results must be validated by further investigations.

Urinary neopterin levels were studied in 96 patients with malignant lymphomas. Twenty-eight had Hodgkin's disease and 68 non-Hodgkin's lymphoma. Neopterin excretion was significantly related to the clinical stage of the disease. Mean neopterin excretion in patients with active disease (634 ± 527 μmol neopterin/mol creatinine) was significantly higher (p = 0.000) than in patients in complete remission (198 ± 105 μmol neopterin/mol creatinine). Mean neopterin levels of patients in stage III-IV were higher than for patients in stage III. These findings were the same in patients with Hodgkin's disease and those with non-Hodgkin's lymphoma (659 ± 593 - 425 ± 316 μmol neopterin/mol creatinine), regardless of the histological subtype. A significant correlation was found between neopterin excretion, ESR (r=0.31; p=0.003) and hemoglobin (r=–0.40; p=0.000). Longitudinal analysis showed a trend towards a correlation between response to therapy and neopterin excretion. These findings suggest that neopterin may be a useful prognostic marker in non-Hodgkin's lymphoma.

Abstract:  Thirty-seven colorectal cancer patients with grade 1–4 diarrhea (NCICTC) caused by chemotherapy with 5-FU-containing regimens, received oral loperamide at the initial dose of 4 mg followed by 4 mg every 8 h (total dose 16 mg/24 h). Twenty-five patients (69%) were diarrhea-free and were considered to be treatment responders. Eight-four percent of the patients with grade 1 or 2 diarrhea achieved a response, but only 52% of those with grade 3–4 diarrhea. These data seem to suggest that high-dose loperamide is effective in patients with moderate diarrhea and can be regarded as the treatment of choice. The patients with more severe diarrhea did not respond so well, and should, perhaps, be given first-line treatment with more effective drugs, such as somatostatin analogues (e.g., octreotide).
