1. Ghost Fishing Threatens Biodiversity in an African Great Lake

Abstract

We conducted an investigation into ghost fishing, a phenomenon in which abandoned fishing equipment remains operational for extended durations, in the southern region of Lake Tanganyika, Republic of Zambia. The perils associated with ghost fishing have achieved international recognition; however, there exists a conspicuous dearth of scholarly investigations in particular regions, notably inland aquatic environments and developing countries. Employing scuba diving for field observations, we discovered that ghost fishing was solely evident in monofilament nylon nets, despite the presence of four different net types in the littoral area of the lake. Notably, endemic crabs Platythelphusa armataand P. maculatafrom Lake Tanganyika constituted 65.2% of the organisms captured by abandoned nets, with the remainder comprising various fish species. The number of captured organisms exhibited a dependence on the interplay between the length and mesh size of the abandoned nets, indicating that the risk of ghost fishing is heightened when both net and mesh size are substantial. Furthermore, we encountered predatory Spiny Eels Mastacembelus mooriiensnared in nets around their jaws and teeth. Intriguingly, the digestive tract of one of the Spiny Eels contained a partially digested cichlid species entangled in the same nets, strongly suggesting the occurrence of secondary ghost fishing.

Abstract

BackgroundAutoimmune pulmonary alveolar proteinosis (aPAP) is a rare disease, predisposing to an increased risk of infection. A complete picture of these infections is lacking.Research questionDescribe the characteristics and clinical outcomes of patients diagnosed with aPAP, and to identify risk factors associated with opportunistic infections.MethodsWe conducted a retrospective cohort including all patients diagnosed with aPAP between 2008 and 2018 in France and Belgium. Data were collected using a standardised questionnaire including demographics, comorbidities, imaging features, outcomes and microbiological data.ResultsWe included 104 patients, 2/3 were men and median age at diagnosis was 45 years. With a median follow-up of 3.4 years (IQR 1.7–6.6 years), 60 patients (58%), developed at least one infection, including 23 (22%) with opportunistic infections. Nocardiaspp was the main pathogen identified (n=10). Thirty-five (34%) patients were hospitalised due to infection. In univariate analysis, male gender was associated with opportunistic infections (p=0.04, OR=3.88; 95% CI (1.02 to 22.06)). Anti-granulocyte macrophage colony-stimulating factor antibody titre at diagnosis was significantly higher among patients who developed nocardiosis (1058 (316–1591) vs 580 (200–1190), p=0.01). Nine patients had died (9%), but only one death was related to infection.InterpretationPatients with aPAP often presented with opportunistic infections, especially nocardiosis, which highlights the importance of systematic search for slow-growing bacteria in bronchoalveolar lavage or whole lung lavage.

Abstract

Brugada syndrome (BrS) is a cardiac arrhythmia disorder associated with sudden death in young adults. With the exception of SCN5A, encoding the cardiac sodium channel NaV1.5, susceptibility genes remain largely unknown. Here we performed a genome-wide association meta-analysis comprising 2,820 unrelated cases with BrS and 10,001 controls, and identified 21 association signals at 12 loci (10 new). Single nucleotide polymorphism (SNP)-heritability estimates indicate a strong polygenic influence. Polygenic risk score analyses based on the 21 susceptibility variants demonstrate varying cumulative contribution of common risk alleles among different patient subgroups, as well as genetic associations with cardiac electrical traits and disorders in the general population. The predominance of cardiac transcription factor loci indicates that transcriptional regulation is a key feature of BrS pathogenesis. Furthermore, functional studies conducted on MAPRE2, encoding the microtubule plus-end binding protein EB2, point to microtubule-related trafficking effects on NaV1.5 expression as a new underlying molecular mechanism. Taken together, these findings broaden our understanding of the genetic architecture of BrS and provide new insights into its molecular underpinnings.

Abstract

Le tachicardie sopraventricolari (TSV) corrispondono ad un gruppo di patologie ritmiche frequentemente riscontrate nella pratica clinica e originate negli atri o alla giunzione atrioventricolare (AV). Per definizione, sono opposte alle tachicardie di origine ventricolare, con le quali non condividono il rischio emodinamico e di morte improvvisa. Le principali aritmie raggruppate sotto questo termine sono la fibrillazione atriale, i flutter atriali, le tachicardie atriali, le tachicardie giunzionali e le tachicardie sinusali. Il meccanismo fisiopatologico, la prevalenza e l’associazione con una cardiopatia sottostante differiscono notevolmente tra queste diverse TSV. Tuttavia, questi diversi aspetti devono essere ben integrati poiché condizioneranno le rispettive modalità di gestione per ciascuna di queste patologie. La loro diagnosi è spesso elettrocardiografica, ma può richiedere indagini più complesse come l’esplorazione elettrofisiologica. Il loro trattamento può basarsi sul solo monitoraggio, sull’introduzione di trattamenti farmacologici antiaritmici o sull’attuazione di procedure di ablazione a seconda del tipo di aritmia presente e della sua tolleranza.

Abstract

Las taquicardias supraventriculares (TSV) son un grupo de patologías rítmicas frecuentes en la práctica clínica y que se originan en las aurículas o en la unión auriculoventricular (AV). Por definición, se contraponen a las taquicardias de origen ventricular con las que no comparten el riesgo hemodinámico y de muerte súbita. Las principales arritmias agrupadas bajo este término son la fibrilación auricular, los cuadros de flutter auricular, las taquicardias auriculares, las taquicardias de la unión y las taquicardias sinusales. El mecanismo fisiopatológico, la prevalencia y la asociación con la cardiopatía subyacente difieren mucho entre estas distintas TSV. Sin embargo, estos diferentes aspectos deben estar bien integrados ya que condicionarán las respectivas modalidades de tratamiento de cada una de estas patologías. Su diagnóstico suele ser electrocardiográfico, pero puede requerir investigaciones más complejas como la exploración electrofisiológica. Su tratamiento puede basarse únicamente en la vigilancia, en la introducción de tratamientos farmacológicos antiarrítmicos o en la aplicación de procedimientos de ablación, dependiendo del tipo de trastorno del ritmo presente y de su tolerabilidad.

10. Localization of Residual Conduction Gaps After Wide Antral Circumferential Ablation of Pulmonary Veins

Abstract

Ablation of atrial fibrillation (AF) is the cornerstone therapy for patients with symptomatic AF resistant to anti-arrhythmic drugs or as first-line therapy, and is based on permanent pulmonary vein (PV) isolation. The presence of a conduction gap in a wide antral circumferential ablation lesion around PVs is often sufficient to transform an initially successful ablation into a procedural failure, thus necessitating a redo intervention. The strategy during a redo procedure is based on the detection and ablation of the reconnection gap. Finding gaps is often simple, but also sometimes challenging, because gaps may be difficult to detect, resulting in unnecessary radiofrequency delivery. The present review aimed to describe the various techniques published thus far to detect residual reconnections along the encircling ablation lines around PVs, to help electrophysiologists to detect and ablate reconnection gaps.

Congenital anomalies of the inferior vena cava (IVC) are rare and very often diagnosed in asymptomatic patients during computed tomography performed for other purposes. These anomalies can have significant clinical implications, for example if electrophysiology procedures are needed. Diagnostic and ablation procedures are difficult since catheter manipulation and positioning are more complex. We present here a case of successful atrioventricular nodal reentrant tachycardia ablation in a patient with unexpected IVC agenesis, using an azygos route.

24. Patient perspective on remote monitoring of cardiovascular implantable electronic devices: rationale and design of the REMOTE-CIED study

Abstract

Remote patient monitoring is a safe and effective alternative for the in-clinic follow-up of patients with cardiovascular implantable electronic devices (CIEDs). However, evidence on the patient perspective on remote monitoring is scarce and inconsistent. The primary objective of the REMOTE-CIED study is to evaluate the influence of remote patient monitoring versus in-clinic follow-up on patient-reported outcomes. Secondary objectives are to: 1) identify subgroups of patients who may not be satisfied with remote monitoring; and 2) investigate the cost-effectiveness of remote monitoring. The REMOTE-CIED study is an international randomised controlled study that will include 900 consecutive heart failure patients implanted with an implantable cardioverter defibrillator (ICD) compatible with the Boston Scientific LATITUDE® Remote Patient Management system at participating centres in five European countries. Patients will be randomised to remote monitoring or in-clinic follow-up. The In-Clinic group will visit the outpatient clinic every 3–6 months, according to standard practice. The Remote Monitoring group only visits the outpatient clinic at 12 and 24 months post-implantation, other check-ups are performed remotely. Patients are asked to complete questionnaires at five time points during the 2-year follow-up. The REMOTE-CIED study will provide insight into the patient perspective on remote monitoring in ICD patients, which could help to support patient-centred care in the future.

La cardiologie est un domaine où la télémédecine devrait connaître un essor rapide. L’application la plus avancée aujourd’hui est le télésuivi des stimulateurs et défibrillateurs cardiaques implantables, permettant de limiter les suivis en face à face au centre d’implantation et d’accéder plus facilement aux données mémorisées dans la prothèses. Après une phase de validation technologique, des larges études prospectives randomisées (TRUST, CONNECT, COMPAS, ECOST, EVATEL) ont démontré la sécurité d’utilisation du télésuivi sur des critères durs de morbi-mortalité avec une valeur ajoutée sous la forme d’une réduction du nombre des consultations, d’une détection anticipée des évènements et d’une réduction des thérapies inappropriées. Ce nouveau standard de suivi s’intègre parfaitement dans la loi française sur la télémédecine. Néanmoins, certains obstacles doivent encore être levés, notamment les aspects financiers de remboursement des actes pour les équipes médicales et de prise en charge d’un service pour les industriels, avant de passer dans la routine. L’acceptation par les patients et la communauté médicale ne devrait pas être un réel problème sous réserve de bien définir les objectifs cliniques et les modalités de partage de l’information. D’autres domaines de la cardiologie devraient à moyen terme s’ouvrir à la télémédecine.

34. Cardiac resynchronisation therapy in heart failure: Current status

Abstract

Cardiac resynchronization in heart failure already has a history of 12 years. However, the major advances have been the result of large multi center trials dating from 2001. In all these trials patients with a LVEF ≤ 35% were included, and a QRS above 120 msec. Follow up was from 3–36 months. The majority of these trials showed a positive effect in reduction of composite and points of death or hospitalization for major cardiovascular events. Many of these trials also showed a diminution of left ventricular and systolic diameter or volume. Even in NYHA class II patients an improvement was seen. Some unanswered questions still remain as regards the agreement on electrical or electromechanical dyssynchrony criteria. There is a number of patients with “wide” QRS who do not improve and conversely a number of patients with a narrow QRS who witness improvement. The benefit in patients with atrial fibrillation also remains unanswered. Finally the value of this modality in patients with mild heart failure or asymptomatic left ventricular systolic dysfunction, NYHA class I–II remains to be determined in large on going trials. Another question is whether biventricular or left ventricular patient is preferable. Finally whether biventricular patient should be complemented by a defibrillator insertion is being currently studied. Cardiac resynchronization therapy along or in combination with an ICD improves symptoms, reduces major morbidity and mortality in patients with a left ventricular EF<35%, ventricular dilatation and a QRS ≥ 120 msec in NYHA class III–IV. Further indications are currently being examined.

35. A pilot experience with permanent biventricular pacing to treat advanced heart failure

Abstract

BackgroundThe prognosis and quality of life of patients with advanced heart failure remain poor. The purpose of this study was to evaluate new nonpharmacologic approaches. Biventricular pacing was proposed in this indication, based on the encouraging results of acute hemodynamics studies. MethodsFifty patients with drug-resistant heart failure (New York Heart Association [NYHA] class III/IV, 16 of 34) were consecutively implanted with biventricular pacemakers. All patients had severe dilated cardiomyopathy and intraventricular conduction delay. Survival, NYHA class, electrocardiogram, echocardiographic data, and exercise tolerance were assessed over a mean follow-up period of 15.4 ± 10.2 months. ResultsAt the end of follow-up, 55% of patients were alive without heart transplantation or left ventricular assistance device. The mortality rate was significantly lower in class III (12.5%) than in class IV patients (52.5%). In survivors, biventricular pacing significantly improved symptoms (NYHA class 2.2 ± 0.5 at follow-up vs 3.7 ± 0.5 at baseline) and exercise tolerance (·VO2peak 15.5 ± 3.4 mL/min per kilogram at follow-up vs 11.1 ± 3 mL/min per kilogram at baseline). ConclusionsBiventricular pacing appears to improve the functional status of patients with dilated cardiomyopathy with advanced heart failure. The technique appears to be attractive as an additive treatment, especially in class III patients. Controlled randomized studies are needed to validate this novel concept. 38. Pacemaker lead infection: echocardiographic features, management, and outcome

ObjectiveTo compare transthoracic and transoesophageal echocardiography (TTE, TOE) in patients with permanent pacemaker lead infection and to evaluate the safety of medical extraction in cases of large vegetations. Methods TTE and TOE were performed in 23 patients with definite pacemaker lead infection. Seventeen patients without previous infection served as a TOE reference for non-infected leads. Results TTE was positive in seven cases (30%) whereas with TOE three different types of vegetations attached to the leads were visualised in 21 of the 23 cases (91%). Of the 20 patients with vegetations and lead culture, 17 (85%) had bacteriologically active infection. Left sided valvar endocarditis was diagnosed in two patients. In the control group, strands were visualised by TOE in five patients, and vegetations in none. Medical extraction of vegetations ≥ 10 mm was performed in 12 patients and was successful in nine (75%) without clinical pulmonary embolism. After 31.2 (19.1) months of follow up (mean (SD)), all patients except one were cured of infection; three died from other causes. Conclusions Combined with bacteriological data, vegetations seen on TOE strongly suggest pacemaker lead infection. Normal TTE examinations do not exclude this diagnosis because of its poor sensitivity. Medical extraction of even large vegetations appeared to be safe.

40. Technical Considerations in Computation of Spectral Harmonics-to-Noise Ratios for Sustained Vowels

Abstract

This paper explores technical issues affecting computed measures of the relative level of noise in the frequency spectrum of a vowel. This type of measure has been proposed for quantification of hoarseness in pathological speakers. An analysis of synthesized vowels was used to test the influence of vowel type, fundamental frequency, perturbation type, perturbation level and quantization. The algorithms were shown to be highly sensitive to errors in pitch-period demarcation, and a dependency on jitter perturbations, fundamental frequency, and vowel type was demonstrated. Relationships between algorithm performance and methods of spectrum estimation were discussed, and approaches for reducing the dependencies were proposed. Finally, a method for achieving a significant reduction in computation time was described.

41. Electrophysiological and Inotropic Characterization of a Novel Class III Antiarrhythmic Agent, GLG-V-13, in the Mammalian Heart

Abstract

GLG-V-13, a novel 3,7-diheterabicyclo(3.3.1)nonane, was examined both in vivo and in vitro to characterize its electrophysiological, hemodynamic, and inotropic properties. In anesthetized guinea pigs, GLG-V-13 [0.5-500 μg/kg intravenously (i.v.), n = 6] lengthened the epicardial monophasic action potential (MAP) duration, the atrioventricular (AV) conduction time and the RR interval in a dose-dependent manner. At the highest dose, these variables were increased by 30, 13, and 23%, respectively. No significant effects were noted on QRS duration or blood pressure (BP). In rabbit atrial and papillary muscle preparations, GLG-V-13 (0.32-3.2 mg/L) did not exert a negative inotropic action and in isolated rabbit cardiomyocytes the agent blocked the rapidly activating delayed rectifier K+current (IKr, EC50= 48 μg/L). In 10 intact anesthetized mongrel dogs, the left ventricular (LV) endocardial MAP was measured during atrial pacing before and after administration of GLG-V-13 (3 and 6 mg/kg i.v.). As compared with the drug-free state, the agent induced a significant prolongation of the MAP at all pacing frequencies (2.0-4.5 Hz). In 15 anesthetized dogs studied 1-4 days after two-stage ligation of the left anterior descending coronary artery (LAD), the antiarrhythmic/proarrhythmic potential of GLG-V-13 was compared with that of lidocaine. ECG, His bundle, LV (IZepi), and composite and normal zone composite electrograms were recorded. Programmed electrical stimulation (PES) and burst pacing (4.0-7.0 Hz) were delivered to the right ventricular outflow tract. In the drug-free state, sustained monomorphic ventricular tachycardia (SMVT) was inducible in 6 dogs (6 of 15). After lidocaine, SMVT was induced in 7 other dogs (13 of 15). GLG-V-13 prevented induction of SMVT in 5 of 6 dogs; a proarrhythmic action was noted in 1 dog only. GLG-V-13 slowed the heart rate (HR), increased the AH and the HV intervals, prolonged the paced (2.5 Hz) QT interval, and increased the ventricular effective refractory period (VERP). These effects were associated with 2:1 block of late potentials in the IZepielectrograms, a phenomenon also observed during rapid atrial pacing (2.5-3.5 Hz), suggestive of a marked prolongation of refractoriness in the ischemically damaged myocardium. In light of the recent Cardiac Arrhythmia Suppression Trial (CAST) study, the antiarrhythmic efficacy, together with the low proarrhythmic potential and lack of cardiodepressant properties of GLG-V-13, may merit further investigation of this novel class III antiarrhythmic agent.

Abstract

During the beam test of a tracker prototype for the Compact Muon Solenoid detector proposed for the LHC, the time response of the Microstrip Gas Chambers was studied using different gases and chamber gaps. The subsequent efficiency to identify the bunch crossing at LHC is discussed for several algorithms used in the off-line signal processing of the data.

44. Evaluation of bepridil efficacy by electrophysiologic testing in patients with recurrent ventricular tachycardia: Comparison of two regimens

Abstract

The purpose of the study was to evaluate this effect of different doses of intravenous and oral bepridil on the induction of ventricular tachycardia. Thirty-eight patients underwent electrophysiologic evaluation for recurrent ventricular tachycardia (VT). Sustained monomorphic VT was induced by programmed ventricular stimulation, using up to three extrastimuli in all patients. The effects of intravenous bepridil (2 mg/kg) were evaluated during the initial study. Intravenous bepridil prevented the induction of sustained VT in eight patients (21%). Electrophysiologic study was repeated after oral bepridil. In six patients the study was stopped because of adverse effects or VT recurrence. Thirty-two patients underwent repeat study 7 days later, taking oral bepridil, 500 mg/day (n=16) or 900/day (n=16). A dose of 500 mg/day of bepridil prevented the induction of sustained VT in only one patient. A dose of 900 mg/day of bepridil prevented the induction of sustained VT in eight patients. There were no significant clinical adverse effects, except in one patient receiving intravenous bepridil. The response to intravenous bepridil did not predict the response to oral bepridil. The response to intravenous or oral bepridil was not related to the plasma level of bepridil but was related to a higher left ventricular ejection fraction. Eight patients (21%) in whom VTs were noninducible on oral bepridil were discharged on 300 mg/day of bepridil if their initial loading dose was 500 mg/day or on 600 mg/day if their initial loading dose was 900 mg/day. They remained free of VT during a follow-up of at least 6 months. In conclusion, this study suggests that oral bepridil at the dose of 600 mg/day may be of value in patients with recurrent VT.

Based on our findings, long-term prognosis of ARVD would appear to be more favorable than reported earlier. Wide use of β blockers may have had a beneficial effect on prognosis, suggesting that medical treatment is of major importance in reducing the risk of sudden death.

Abstract

Clinical PET acquisitions of the heart suffer from artefacts and drops in image quality due to the poor spatial resolution of the PET system. Moreover, cardiac PET images are further degraded by the blur caused by the breathing and beating motions, thus hampering diagnosis and evaluation of myocardial pathologies. Anatomy-enhanced PET reconstruction, using a high-resolution CT, has proven useful in brain imaging. In cardiac datasets however, due to the motion artefacts, the application of any restoring technique on datasets affected by motion blur needs to be preceded by the validation of the proposed method on realistic static datasets. In this work, the validation is performed using static cardiac ex vivo datasets obtained from a number of sacrificed sheep, scanned on a clinical PET/CT scanner. The aim of this work is to assess the effectiveness of reconstructions of the acquired datasets with different CT-based anatomical priors, in comparison to reconstructions currently applied in clinical practise. The gold standard to which all reconstructions are compared consists of images of the same hearts scanned on a small-animal PET scanner, whose high spatial resolution allows for almost artefact-free images. Encouraging results were obtained so far, with improvements in volume delineation and uniformity of activity values when anatomical information was used.
