2. Seltene ZNS-Metastasierung eines peripheren T-Zell-Lymphoms

- Abstract
Die Metastasierung in das zentrale Nervensystem (ZNS) bei T‑Zell-Lymphomen ist ein seltenes klinisches Szenario. Zu den häufigsten Subtypen mit ZNS-Beteiligung gehören periphere T‑Zell-Lymphome (PTCL) und anaplastische großzellige Lymphome (ALCL). Neurologische Symptome bei einer sekundären ZNS-Beteiligung entwickeln sich in der Regel subakut bis akut. Die diagnostischen Maßnahmen umfassen die klinisch neurologische Untersuchung, eine Gadolinium-verstärkte MRT der gesamten Neuroachse, eine Liquoranalyse und gegebenenfalls eine Hirnbiopsie. Die ZNS-Metastasierung von T‑Zell-Lymphomen stellt, aufgrund ihrer Seltenheit, eine diagnostische Herausforderung dar und muss von anderen Erkrankungen, wie infektiologischen, autoimmunen oder anderen neoplastischen Ursachen, abgegrenzt werden. Zu den Behandlungsoptionen gehören die systemische Chemotherapie, die intrathekale Chemotherapie und die Strahlentherapie. Die Prognose für eine sekundäre ZNS-Beteiligung bei T‑Zell-Lymphomen ist, trotz der zahlreichen Behandlungsoptionen, in der Regel ungünstig. In diesem Fallbericht wird über eine durch Biopsie nachgewiesene ZNS-Metastase eines peripheren T‑Zell-Lymphoms berichtet. Ziel der Literaturübersicht inklusive des Fallberichts ist es, das Verständnis für die klinischen Merkmale, die diagnostischen Modalitäten, die Behandlungsmöglichkeiten und die Prognose der ZNS-Metastasierung von peripheren T‑Zell-Lymphomen zu verbessern.

Patients who have had a stroke with unknown time of onset have been previously excluded from thrombolysis. We aimed to establish whether intravenous alteplase is safe and effective in such patients when salvageable tissue has been identified with imaging biomarkers.

- Abstract
Glioblastoma is characterized by widespread genetic and transcriptional heterogeneity, yet little is known about the role of the epigenome in glioblastoma disease progression. Here, we present genome-scale maps of DNA methylation in matched primary and recurring glioblastoma tumors, using data from a highly annotated clinical cohort that was selected through a national patient registry. We demonstrate the feasibility of DNA methylation mapping in a large set of routinely collected FFPE samples, and we validate bisulfite sequencing as a multipurpose assay that allowed us to infer a range of different genetic, epigenetic, and transcriptional characteristics of the profiled tumor samples. On the basis of these data, we identified subtle differences between primary and recurring tumors, links between DNA methylation and the tumor microenvironment, and an association of epigenetic tumor heterogeneity with patient survival. In summary, this study establishes an open resource for dissecting DNA methylation heterogeneity in a genetically diverse and heterogeneous cancer, and it demonstrates the feasibility of integrating epigenomics, radiology, and digital pathology for a national cohort, thereby leveraging existing samples and data collected as part of routine clinical practice.

12. P450 enzyme inducing and non-enzyme inducing antiepileptics in glioblastoma patients treated with standard chemotherapy

- Abstract
Summary The co-administration of antiepileptic drugs (AED) and chemotherapeutic agents in patients with glioblastoma multiforme (GBM) is common. Interactions of chemotherapeutic agents and AED have not been investigated sufficiently. The purpose of this study is to evaluate the effects of enzyme inducing (EI-AED) and non-EI-AED in patients with GBM treated with standard chemotherapeutic agents on survival and haematotoxicity. One hundred and sixty eight glioblastoma patients with standard treatment including surgery, radiotherapy and chemotherapy were retrospectively analysed. Patients were separated into three groups: Group A patients without AED (n=88), Group B patients with EI-AED (n=43), and Group C patients with non-EI-AED (n=37). CCNU was the most frequently used first-line drug in all three groups (Group A: 77%; Group B: 81%; Group C: 78%). Second line treatment, mainly temozolomide, was applicated in 58 of patients and third-line treatment in 9. Carbamazepine was the most frequently administered AED in Group B (81%) and valproic acid in Group C (85%). For statistical analysis, only patients with CCNU first line treatment were calculated. A significant difference regarding survival was detected between Group B (10.8 month) and Group C (13.9 month), as well as increased haematotoxicity for Group C. These results indicate that AED influence the pharmacokinetics of chemotherapeutic drugs in patients with GBM. Valproic acid might be responsible for increasing haematotoxicity. Whether the difference regarding survival between Group B and Group C is due to a decrease of efficacy of chemotherapeutic agents by EI-AED, or due to increased efficacy of chemotherapeutic agents caused by the enzyme inhibiting properties of valproic acid, has to be evaluated in future studies.

13. Neuromuscular symptoms in cancer patients

- Abstract
The neuromuscular system is often affected in patients with cancer. This can be due to a direct effect of cancer, neurotoxic treatment, radiation therapy or paraneoplastic phenomena. This review focuses on the sites of involvement, such as cranial nerves, nerve roots, and plexus, mono- and polyneuropathies disorders at neuromuscular transmission, and muscle disease. At each site of the peripheral nervous system, the most common types of involvement and therapeutic issues are discussed. Knowledge of neuromuscular symptoms is important for medical oncologists, neurologists and neuro-oncologist alike, as precise diagnosis may have therapeutic consequences for the patients. Immunologic therapies, or in some cases anticancer treatment may decrease the individual’s burden of disease in paraneoplastic syndromes. Conventional cancer therapies, such as chemotherapy and radiation therapy, also have neurotoxic side effects which need to be outweighed against their therapeutic effect. Several other less well-defined complications such as infection, hemorrhage or metabolic disorders have to be considered as a differential diagnosis. Complications of surgical and other interventions will not be discussed in this context.

- Abstract
Acquired neuromyotonia is characterized by hyperexcitability of motor nerves resulting in continuous muscle fiber activity. It occurs most often as a paraneoplastic syndrome in patients with cancers of the immune system. Antibodies against voltage‐gated potassium channels (VGKCs) have been detected in some patients. Peripheral neuropathy is sometimes present. We report on a patient with Hodgkin's lymphoma in complete remission who developed paresthesias followed by neuromyotonia with bulbar involvement. Peripheral sensorimotor neuropathy was diagnosed electrophysiologically and evidence of axonal degeneration and demyelination was detected by sural nerve biopsy. 16. Epilepsy in the end of life phase of brain tumor patients: a systematic review

- Abstract
Epileptic seizures are common in patients with primary or secondary malignant brain tumor. However, current knowledge on the occurrence of seizures during the end of life (EOL) phase of brain tumor patients is limited. Because symptom management with preservation of quality of life is of major importance for patients with a malignant brain tumor, particularly in the EOL, it is necessary to gain a deeper understanding of seizures and their management during this phase. We performed a systematic review of literature related to epilepsy in the EOL phase of brain tumor patients, based on the electronic resources PubMed, Embase, and Cinahl. The search yielded 442 unique records, of which 11 articles were eligible for further analysis after applying predefined inclusion criteria. Seizures occur relatively frequently in the EOL phase, particularly in patients with high-grade glioma. However, seizure management is often hampered by swallowing difficulties and impaired consciousness. Treatment decisions are largely dependent on expert opinion because a standardized approach for treating seizures in the terminal stage of brain tumor patients is still lacking.
