Although end‐stage renal disease related to AA amyloidosis nephropathy is well characterized, there are limited data concerning patient and graft outcome after renal transplantation. We performed a multicentric retrospective survey to assess the graft and patient survival in 59 renal recipients with AA amyloidosis. The recurrence rate of AA amyloidosis nephropathy was estimated at 14%. The overall, 5‐ and 10‐year patient survival was significantly lower for the AA amyloidosis patients than for a control group of 177 renal transplant recipients (p = 0.0001, 0.028 and 0.013, respectively). In contrast, we did not observe any statistical differences in the 5‐ and 10‐ year graft survival censored for death between two groups. AA amyloidosis‐transplanted patients exhibited a high proportion of infectious complications after transplantation (73.2%). Causes of death included both acute cardiovascular events and fatal septic complications. Multivariate analysis demonstrated that the recurrence of AA amyloidosis on the graft (adjusted OR = 14.4, p = 0.01) and older recipient age (adjusted OR for a 1‐year increase = 1.06, p = 0.03) were significantly associated with risk of death. Finally, patients with AA amyloidosis nephropathy are eligible for renal transplantation but require careful management of both cardiovascular and infectious complications to reduce the high risk of mortality.

Long-term use of calcineurine inhibitors (CNIs) may contribute to the development of chronic allograft dysfunction (CAD). We investigate the impact of the introduction of MMF combined with cyclosporine (CsA) 50% dose reduction. An open, randomized, controlled, multicenter, prospective study was conducted in 103 patients, receiving a CsA-based therapy with a serum creatinine between 1.7–3.4 mg/dL, more than 1 year after transplantation. They were randomized to receive MMF with half dose of CsA (MMF group) or to continue their maintenance CsA dose (control group). A total of 96 weeks after randomization, the evolution of renal function assessed by regression line analysis of 1/SeCr improved in the MMF group (positive slope) vs. the control group (negative slope), 4.2 × 10−4vs. −3.0 × 10−4, respectively (p < 0.001). Concurrently, the absolute renal function improved significantly in the MMF group. No episode of biopsy-proven acute rejection occurred. One patient in each group lost his graft because of biopsy-proven chronic allograft nephropathy. There was a significant decrease of triglycerides level in the MMF group. Anemia and diarrhea were statistically more frequent in the MMF group.

This retrospective study aimed to evaluate the benefit of switching from calcineurin inhibitors (CnI) to sirolimus in posttransplant Kaposi's sarcoma (KS). Fourteen patients monitored in five French departments who had developed posttransplant KS were switched from CnI to sirolimus either abruptly (n = 9) or progressively (n = 5) with trough levels 5–12 ng/mL. Two patients had a complete remission, eight a partial response, and five no significant improvement of KS. The mean time to response was 3.9 months. After a mean follow-up of 16 months, 3 partial responders, with previous severe and refractory KS, suffered again from KS progression despite the lack of concomitant infectious or neoplastic event. These relapses occurred 5–9 months after switching. The tolerance of sirolimus has been excellent except for in one patient who developed severe interstitial pneumonitis. Sirolimus is usually useful in the management of posttransplant KS. It may be, however, ineffective or transiently effective in some patients with severe KS. Prospective studies with pharmacodynamic evaluation are important in order to better assess the duration of responses and the mechanisms of primary and acquired drug resistance.

We report 3 cases of acute polyradiculoneuritis associated with an active cytomegalovirus (CMV) infection that occurred in 1 patient with a renal transplant and in 2 patients with simultaneous renal and pancreatic transplants. The time association with CMV infection was shown by the detection of positive viremia and viruria and by the presence of specific anti‐CMV IgM at the onset of the neurological signs. It was a recurrent infection in the 3 patients. One patient developed a recurrent polyradiculoneuritis which was associated with a relapse of the CMV infection. In all the patients, immune complexes were present together with a decrease in serum complement. All 3 patients underwent an immunosuppressive therapy, which consisted in a quadruple therapy: steroids, azathioprine, cyclosporine A, and a second treatment against a steroid‐resistant rejection, by either a monoclonal antibody (OK.T3) or rabbit antithymocyte globulin at the time of the polyneuritis. The electrophysiological analysis showed findings characteristic of a CMV polyneuropathy with mixed axonal and demyelinating lesions. Several factors might have triggered the neuropathogenicity of the CMV: the presence of specific anti‐CMV IgM involved in the formation of the immune complexes, the second course of globulin therapy, and the existence of a previous diabetic and uremic neuropathy. The clinical course was positive for the 3 patients undergoing ganciclovir therapy and without plasmapheresis, including the 2 patients who required assisted ventilation because of a respiratory failure and CMV pneumonitis.

The role of platelet transfusion as a preparative method for kidney transplantation is still a matter of debate. Two groups of 28 male patients transplanted between 1983 and 1988, paired for age, date of transplant, absence of anti-HLA antibody and immunosuppressive therapy have been compared. Group I was given 5 purified platelet transfusions at 1-week intervals before transplantation. Each transfusion contained 7.6x106platelets contaminated by less than 1 leukocyte in 105plateletes. Group II received from 3 to 5 whole blood transfusions. In all cases it was a first transplant from cadaveric donors and previously untransfused patients before entering the protocol. No patient in group I developed cytotoxic antibodies. Acute tubular necrosis occurred with the same incidence in group I and in group II but was more severe and longer in group I, requiring hemodialysis in 62.5 and only 22 in group II. ATN was significantly associated with graft loss in group I (P<0.05). The total number of rejections and the number of patients undergoing rejection were not significantly different in both groups. However, the intensity of rejection was significantly higher in group I with 41 (21/51) of severe or irreversible rejections versus 9/46 (19.5) in group II (P<0.05). The first rejection occurred significantly earlier in group I than in group II since 75 of the first rejection episodes occurred in the first 10 days versus 38 in group II (P<0.02) with a mean delay of 12.8±3.2 and 19.10±3.3 days, respectively. Although platelet transfusions are devoid of leukocytes the incidence of CMV infection was not significantly different in both groups: 57 in group I and 68 in group II. Purified platelet transfusions did not induce humoral immunization but lack of sensitization does not imply indefinite graft prolongation. Because platelets do not carry class II antigens, purified platelets transfusions represent a useful model to analyze the role of class I antigens alone in the induction of unresponsiveness in organ transplantation.

The actuarial survival curve of patients with inborn errors of metabolism treated with fetal liver transplantation shows a 12-year survival of 77%. The condition of many of these patients has been improved by the treatment, but transplantation has had to be repeated in order to maintain clinical amelioration. Enzyme levels were not significantly and durably increased in peripheral blood but the quantities of substrates detected in sera and urines were significantly reduced and tissue deposits were stabilized.

The aim of the study was to evaluate the virological parameters associated with the severity of cytomegalovirus (CMV) disease in renal and simultaneous renal and pancreatic transplantation. The association of the viral profile and the severity of the viral disease was analysed taking into account different confounding variables susceptible to linkage with the severity of the CMV infection and the viral parameters. All the patients transplanted between 1 January 1989 and 31 December 1990, a total of 242, were prospectively followed by viral cultures in blood and urine and by serological methods using the detection of CMV‐specific IgM and the complement fixation (CF) test. The samples were taken systematically each week for the first month and then at day 90, 180 and every 6 months and also in cases of clinical manifestations related to viral disease. CMV infection was diagnosed virologically by the presence of viraemia, viruria, IgM, or a significant rise in CMV antibody titre in CF. CMV disease was classified as asymptomatic, mild (fever and/or leukopenia), moderate (fever, leukopenia and liver abnormalities), severe (CMV pneumopathy and/or gastrointestinal disease) or fatal. The incidence of CMV infection was 65% (157/242): 32% asymptomatic, 36% mild, 30% moderate and 2% severe. The presence of IgM was associated with the severity of CMV disease: 51.4% of moderate and severe CMV infections in the group with IgM versus only 16% in the group without IgM (P< 0.0001). The risk of having severe or moderate CMV disease was 3.28 times higher in patients with positive IgM. However the serological changes in CF were not significantly associated with the severity of the viral disease since 34.6 % of the patients with CF changes had a severe form versus 20.8% in the group without CF modification. Viruria was significantly associated with moderate or severe infection: 43.6% of the patients with viruria had severe infection versus only 12.5% in the patients without viruria (P< 0.0002). The risk of having moderate or severe CMV disease was 3.48 times higher in the patients with viruria. Viraemia was also associated with more severe CMV infection: 48.6% of moderate or severe CMV infection in the group of patients with viraemia versus 19% in the group without viraemia (P< 0.0001). The risk of having severe or moderate CMV infection was 2.58 times higher in the patients with viraemia. Viraemia was not more associated with severe CMV infection than viruria. Using the maximum likelihood ratio method and the logistic regression model, CMV‐specific IgM, viruria and viraemia were each shown to be associated with the severity of CMV disease and the addition of one parameter to the other(s), whatever the type (except the CF changes) and whatever the order of this addition, did not remove the link between the severity and IgM, viruria and viremia. The incidence of severe and moderate CMV disease increased with the number of positive viral parameters (PVP) from 2% of moderate and severe infections in the group with one PVP, to 28% in the group with two PVP, to 39% in the group with three PVP and 68% in the group with four PVP (trend, 35.95; P< 0.0001). Taking the absolute risk of the group of patients without IgM, viruria or viraemia as the basal level, the observed relative risk of severe CMV infection varied from 6.45 in the group with positive IgM without viruria or viraemia, to 10.74 in the group with positive IgM and viruria without viraemia and to 22.5 in the group with the three positive parameters IgM, viruria and viraemia. The different potential confounding factors (recipient and donor serology, renal or renal and pancreatic transplantation, DR compatibility, rejection before CMV infection) did not modify the link between the viral profile and the severity of CMV disease. This study suggests that the severity of CMV disease might be linked to the overspread of the virus as well as to the consequences of a CMV‐specific humoral immune response.

Bacterial hospital-acquired infections (HAI) are frequent complications of the early post-transplantation period. The most frequent infections are urinary tract infection, bacteremia and surgical site infection. The objective was to report the incidence of early HAI among kidney, pancreas or kidney-pancreas transplant recipients in a French university hospital.

In organ transplantation, virus transmitted by the donor is associated with a higher risk of severe primary infection after transplantation in the seronegative recipient. In this study, the risk of hepatitis‐C virus (HCV) transmission by the kidney was determined, and the morbidity in the recipient assessed. Serum samples from all kidney donors of our Transplantation Unit between 1983 and 1988 were screened for antibodies to anti‐HCV by first enzyme‐linked immunosorbent assay (Ortho ELISA) and positive samples were confirmed by a second‐generation ELISA and the CHIRON RIBA HCV test. Of the 164 kidney donors whose sera were available, five were positive (3%) and all of them were positive with the RIBA test. Liver function was normal in the five donors. Seven recipients received a renal transplant from the anti‐HCV‐positive donors. Two patients had a follow‐up too short to draw any conclusions. Two patients remained anti‐HCV‐negative up to 36 and 48 months, respectively, but one of them had chronic hepatitis. One patient was anti‐HCV‐positive before transplantation and remained positive over the 4‐year follow‐up. The two last patients seroconverted and acute hepatitis occurred at 16 and 101 days after transplantation, respectively. In both cases, no peroperative or postoperative transfusion was given and no other cause of hepatitis could be determined. A cirrhotic evolution was observed within 15 and 36 months in both cases. Thus HCV can be transmitted by a kidney transplant and cadaveric donors positive for anti‐HCV antibodies should be excluded from kidney donation.

The aim of the study was to analyse the influence of co-infection by hepatitis B virus (HBV) and hepatitis C virus (HCV) as compared with HCV infection alone in 1098 patients who received a kidney transplant between 1 January and 31 December 1991. At transplantation, the prevalence of anti-HCV antibodies was 21.40% (235/1098) while the prevalence of HBV infection was 9.85% (108/1096); 46 patients were co-infected with HBV and HCV, either 19.70% of HCV-infected patients and 42.60% of HBV-infected patients. Liver tests, galactose clearance and liver biopsy were compared in the 46 co-infected patients (HCV+HBV+) and in the 189 HCV-infected patients (HCV+HBV−). At the time of transplantation, cytolysis was present in 31.45% of HCV+HBV− patients (50/159) and in 40% of HCV+HBV− patients (16/40); cholestasis was present in 34.18% of HCV+HBV− patients (34/158) and 42.11% of HCV+HBV+ patients (16/38). At 6 months the incidence of biological abnormalities increased to 37% in HCV+HBV− patients (55/150) and to 52.5% in HCV+HBV− patients (21/40), suggesting a more deleterious effect of the immunosuppressive therapy in the co-infected group. Over the course of transplantation, chronic hepatitis was present in 50% of HCV+HBV− patients and in 64.1% of HCV+HBV+ patients. Liver failure occurred in 7% of HCV+HBV− patients (12/156) and 17% of HCV+HBV+ patients (7/41). Galactose clearance was performed as a functional test in 68 patients: it was not significantly different in either group. Liver biopsy was performed in 108 patients at least once. Minimal changes were more frequent in HCV+HBV− patients while the incidence of cirrhosis was 10% in HCV+HBV− patients (8/81) and 26% in HCV+HBV+ patients (7/27). Serum HCV RNA detected by polymerase chain reaction was present in 81% of the HCV+HBV− patients (43/53) and 70% of the HCV+HBV+ patients (14/20). Patient survival rate was not significantly different in both groups at 12 years.
