Early administration of antiplatelet agents in acute ischemic stroke patients (AIS) receiving intravenous thrombolysis (IVT) is a potential therapeutic strategy, however, safety and efficacy are not well established. We hypothesize that antiplatelet pretreatment (AP) before IVT have a similarly role as initiation of AP within the first 24 hours following IVT. We aimed to explore the effect of AP on platelet aggregation and clinical outcomes in thrombolysis-treated AIS patients. We enrolled AIS patients treated with IVT at the Neurology Department of the Nanjing First Hospital from January 2016 to June 2018. Prior use of antiplatelet agent was recorded. Light transmittance aggregometry was used to estimate the maximum platelet aggregation (MPA). Linear regression model was performed to investigate the factors associated with MPA. Multivariate logistic regression was used to analyse the association between AP and clinical outcomes. A total of 59 patients were included; 23 (38.9 %) were taking antiplatelet agent before stroke. Prior AP (β = -20.209, SE mean=6.574; P=0.004) was significantly lower the arachidonic acid-induced MPA at the time point of 3h after thrombolysis. AP did not increase of the risk for sICH (OR=3.41, 95%CI 0.16-7.20, p=0.436) or mortality (OR=3.55, 95%CI 0.39-8.52, p=0.260). There were no associations between AP and improved clinical outcomes (all P>0.05). In thrombolysis-treated AIS patients, AP was associated with lower MPA after thrombolysis. AP is safe in these patients, however, further studies are required to confirm the efficacy.

Different ensemble strategies were compared in online near-infrared models for monitoring active pharmaceutical ingredients of Traditional Chinese Medicine. Bagging partial least square regression and boosting partial least square regression were adopted to near-infrared models, to determine hesperidin and nobiletin content during the extraction process of Pericarpium Citri Reticulatae in a pilot scale system. Different pretreatment methods were investigated, including Savitzky-Golay smoothing, derivatives, multiplicative scatter correction, standard normal variate, normalize, and combinations of them. Two different variable selection methods, including synergy interval partial least squares and backward interval partial least squares algorithms, were performed. Based on the result of the synergy interval partial least squares algorithm, bagging partial least square regression and boosting partial least square regression were adopted into the quantitative analysis. The results demonstrated that the established approach could be applied for rapid determination and real-time monitoring of hesperidin and nobiletin in Pericarpium Citri Reticulatae (Citrus reticulata) during the extraction process. Comparing the results, the boosting partial least square regression provided a slightly better accuracy than the bagging partial least square regression. Finally, this paper provides a promising ensemble strategy on online near-infrared models in Chinese medicine. [ABSTRACT FROM AUTHOR]

Aims Prone and supine positions for percutaneous nephrolithotomy are widely used but have their drawbacks. We report a new positioning method called “flank suspended supine position” (FSSP) for PCNL and describe our experience with PCNL in this position to evaluate its safety and efficacy.Methods Retrospective study of 150 cases of renal stone patients treated with PCNL in a new position called flank suspended supine position (FSSP) from June 2009 to July 2010. All patients were treated with PCNL in FSSP under epidural anesthesia. Operation time, bleeding rate, stone free rate, and complications were recorded.Results All patients tolerated FSSP. Mean operation time was 78.29±26.13 min. Initial stone-free rate was 83%. For those with residual stones (26 cases), 18 were stone-free after a second PCNL, 8 after extracorporeal shock wave lithotripsy (ESWL). Mean hospital stay was 7.63±2.39 days. No penetrating injury of the pleural cavity or injury to visceral organs was reported.Summary FSSP is an effective and safe position for PCNL in our hands and its effectiveness relative to traditional prone position needs to be determined in future randomized studies.

In view of the poor water supply system's network properties, the system's complicated network hydraulic equations were replaced by macroscopic nodal pressure model and the model of relationship between supply flow and water source head. By using pump-station pressure head and initial tank water levels as decision variables, the model of optimal allocation of water supply between pump-sources was developed. Genetic algorithm was introduced to deal with the model of optimal allocation of water supply. Methods for handling each constraint condition were put forward, and overcome the shortcoming such as premature convergence of genetic algorithm; a solving method was brought forward in which genetic algorithm was combined with simulated annealing technology and self-adaptive crossover and mutation probabilities were adopted. An application example showed the feasibility of this algorithm.

The effects of polychlorinated biphenyls (PCBs) on reproduction of adult cocks were studied by gavaging peanut oil or PCBs (Aroclor 1254, 50 mg/kg) once a week for six consecutive weeks. Physiological parameters were recorded and gonads were removed at the end of experiment for histological examination. The results showed that there was no significant difference between the control and treatment group in body weight, respiration rate, heart rate, body temperature, and the numbers of red and white blood cells. However, there was a marked decrease in the testicular weight and serum testosterone level after PCB treatment. Morphological studies manifested severe damage of the seminiferous tubules by PCB. The number of the germ cells at the different developmental stages was decreased and condensed nuclei were observed in most of these cells. This study revealed that the reproductive function of the adult cocks is sensitive to PCBs, which inhibited mainly spermatogenesis and testosterone secretion.

The results of systemic behavior analysis of cantilever arched retaining structure reported in this paper have significant value for guiding the design and application of such structure.

Voluminous statistical data on the breakage and leakage of pipelines occurring frequently in water supply and drainage engineering, were collected and analyzed to find the causes and solutions for the breakage and leakage. Then the original parameter method of foundation beam on elastic semi-infinite plane subgrade was applied to verify the statistical results. Numerical computational results showed that the performance of the original parameter method of foundation beam was satisfactory.

Abstract: Many problems in petroleum and chemical industry can be reduced to the solutions of the helical flow of Non-newtonian fluid in eccentric annular space. This paper studies the flow law of the laminar helical flow of the power law fluid and Bingham fluid in eccentric annular space and the determination of the flow state. In theory, by the principle of the fluid mechanics, with methematical methods, based on our theory of the helical flow in concentric annular space and through the infinite subdivision method for the flow field of eccentric annular space helical flow, the apparent viscosity distribution, the velocity distribution, the flow rate and the pressure drop equation of the field can be given, and then the stability parameter, characterizing the transition from laminar to turbulent flow, is established.
