AbstractGeneral purpose technologies (GPTs) like AI enable and require significant complementary investments. These investments are often intangible and poorly measured in national accounts. We develop a model that shows how this can lead to underestimation of productivity growth in a new GPTs early years and, later, when the benefits of intangible investments are harvested, productivity growth overestimation. We call this phenomenon the Productivity J-curve. We apply our method to US data and find that adjusting for intangibles related to computer hardware and software yields a TFP level that is 15.9 percent higher than official measures by the end of 2017. Objective: Mental health (MH) care in remote areas is frequently scarce and fragmented and difficult to compare objectively with other areas even in the same country. This study aimed to analyze the adult MH service provision in 3 remote areas of Organization for Economic Cooperation and Development countries in the world.Methods: We used an internationally agreed set of systems indicators, terminology, and classification of services (Description and Evaluation of Services and DirectoriEs for Long Term Care). This instrument provided a standard description of MH care provision in the Kimberley region (Australia), Nunavik (Canada), and Lapland (Finland), areas characterized by an extremely low population density and high relative rates of Indigenous peoples.Results: All areas showed high rates of deprivation within their national contexts. MH services were mostly provided by the public sector supplemented by nonprofit organizations. This study found a higher provision per inhabitant of community residential care in Nunavik in relation to the other areas; higher provision of community outreach services in the Kimberley; and a lack of day services except in Lapland. Specific cultural-based services for the Indigenous population were identified only in the Kimberley. MH care in Lapland was self-sufficient, and its care pattern was similar to other Finnish areas, while the Kimberley and Nunavik differed from the standard pattern of care in their respective countries and relied partly on services located outside their boundaries for treating severe cases.Conclusion: We found common challenges in these remote areas but a huge diversity in the patterns of MH care. The implementation of care interventions should be locally tailored considering both the environmental characteristics and the existing pattern of service provision.

Objective: The objective of this study was to assess the acceptability, content validity and usability of the My Medicines and Me (M3Q) self-report side effect questionnaire.Methods: Eight focus groups consisting of mental health patients, carers, general practitioners, psychiatrists, mental health nurses and pharmacists were conducted, involving 78 participants. Two researchers independently examined the transcriptions and analysed the data thematically using an inductive method.Results: The findings supported changes to the formatting, length and phrasing of questions in the original version of the questionnaire. Although the groups provided differing views on the usability of the M3Q in clinical practice, the patient and carer groups were unconditionally in favour of such a tool to be used systematically to describe patients’ subjective experiences with side effects.Conclusion: The differing contribution made by all groups involved in the administration and completion of the M3Q assisted with content validity of the questionnaire. The acceptability and usability of this novel side effect questionnaire was also explored, with many participants agreeing it was a necessary tool for a patient centred approach to treatment. Following implementation of the changes to the current format of the questionnaire, investigation into the uptake and use in clinical practice should be carried out.

We aimed to construct and assess the psychometric properties of the My Medicines and Me Questionnaire (M3Q), a self-report side-effect questionnaire for mental health patients. Thirty individuals taking a psychotropic medication completed the M3Q side-effect checklist along with the Liverpool University Neuroleptic Side Effect Rating Scale. Thirty healthy comparison individuals also completed the M3Q checklist. Data were analyzed using factorial analysis. Psychometric evaluations including validity and reliability testing were carried out on the questionnaire’s checklist. The original 54-item checklist of the M3Q was shortened to include only those items found to be significant in the factorial analysis. The revised 32-item checklist of the M3Q was found to be valid and reliable. The M3Q is a simple, valid and reliable questionnaire that can be used routinely in clinical practice for detecting bothersome side effects of a range of psychotropic medications. This will provide a platform for improved communication between the clinician and the patient.

Neuritin 1 (NRN1), an activity‐regulated gene with multiple roles in neurodevelopment and synaptic plasticity, is located within the 6p24‐p25 interval on chromosome 6, previously identified as linked to a subtype of schizophrenia (SZ) characterized by pervasive cognitive deficit (CD). We have tested the effect of NRN1sequence variation on susceptibility to SZ and on general cognitive ability in patients and non‐psychiatric control subjects by re‐sequencing the coding regions of NRN1and its flanking sequences, and genotyping 19 single‐nucleotide polymorphisms (SNPs) in 336 SZ patients and 172 healthy control individuals. All participants completed comprehensive neurocognitive assessment, including tests estimating premorbid/prior IQ and current IQ. Logistic regression analyses found no significant association for any of the 19 SNPs with SZ or its CD subtype. However, linear regression analysis gave significant association (P= 0.024 and P= 0.027 after correction for multiple testing) for polymorphisms rs1475157 and rs9405890 with current IQ in the patient group. In SZ, the rs1475157–rs9405890 haplotypes revealed a highly significant association with the abstraction component of current (“fluid”) intelligence (P= 0.0014), and with percentage loss of IQ points between premorbid and current intelligence (P= 0.0041). Results in the control group were not significant after correction. This is the first analysis of association between variation in NRN1and SZ. The findings suggest a role of NRN1as a modifier of cognitive functioning in SZ, with implications for future research into the impact of the environment on the development and maintenance of “fluid” intelligence. © 2009 Wiley‐Liss, Inc.

Groups at seasonal risk for deliberate self-harm (DSH) vary according to their geographic location. It is unknown, however, if seasonal risk factors for DSH are associated with place of birth or place of residence as these are confounded in all studies to date. In order to disaggregate place of birth from place of residence we examined general and seasonal risk factors for DSH in three different population birth groups living in Western Australia: Australian Aborigines, Australian born non-Aborigines, and UK migrants. We found Aborigines are at much higher general risk for DSH than non-Aborigines, but are not at seasonal risk, whereas non-Aboriginal Australians and UK migrants are. For UK migrants, this is only found for females. For all groups at seasonal risk this peaks during the austral (southern hemisphere) spring/summer. Furthermore, non-Aboriginal Australians and UK migrants show a consistent pattern of increased case fatality with increasing age. In contrast, case fatality does not increase with age among Australian Aborigines. Overall, despite living in the same environment, the three birth groups show different patterns of seasonal risk for DSH. In particular, the sex difference found between UK migrants and non-Aboriginal Australian birth groups suggests that predisposition toward seasonal risk for DSH is established early in life, but when present this is expressed according to local conditions.

This paper presents an analysis of Tuberculosis (TB). Specifically addressed are the history of its development and treatment; its current resurgence; the global impact of this resurgence; the social issues responsible for the resurgence including poverty, homelessness, substance abuse, the HIV/AIDS epidemic, and increased immigration from TB endemic areas of the world; and the political, economic and ethical issues affecting its incidence today including inadequate funding, California's Proposition 187, pharmaceutical corporation profit motives, directly observed therapy and forced confinement of non-compliant patients.

Analysis of 94 kb of DNA, located between map positions 88 and 182 kb in the 330-kb chlorella virus PBCV-1 genome, revealed 195 open reading frames (ORFs) 65 codons or longer. One hundred and five of the 195 ORFs were considered major ORFs. Twenty-six of the 105 major ORFs resembled genes in the databases including three chitinases, a chitosanase, three serine/threonine protein kinases, two additional protein kinases, a tyrosine protein phosphatase, two ankyrins, an ornithine decarboxylase, a copper/zinc-superoxide dismutase, a proliferating cell nuclear antigen, a DNA polymerase, a fibronectin-binding protein, the yeast Ski2 protein, an adenine DNA methyltransferase and its corresponding DNA site-specific endonuclease, and an amidase. The genes for the 105 major ORFs were evenly distributed along the genome and, except for one noncoding 1788-nucleotide stretch, the genes were close together. Unexpectedly, a 900-bp region in the 1788-bp noncoding sequence resembled a CpG island.

The tetrahedron model of clinical judgment (Rock, Bransford, Maisto, and Morey, Clin. Psyckol. Rev. 7645-661,1987; Rock, Bransford, Morey, and Maisto, Clin. Psychol. Rev. 8411- 416, 1988) provides a framework for identifying factors that may influence the judgments of psychotherapists (e.g., clinical and counseling psychologists, psychiatrists, social workers, etc.). Three parameters of the model were experimentally manipulated mode of presentation of clinical information, patient, and judgment task. Sixteen clinical psychology graduate student therapists evaluated two patients on axis I and axis II of the DSM-III. Judgmental accuracy was influenced by main effects and two- and three-way disordinal interactions among the three model parameters. Additionally, we found that judgmental accuracy was positively related to experience and training. This relationship was evident only when experience was assessed with specific rather than general measures, and when the clinical materials were presented in an audio video format and hence resembled the conditions under which the clinicians in the study acquired that training and experience. Implications for the design of training programs that facilitate competency in clinical judgment are discussed.

Forty-five kilobases of DNA, including the previously sequenced 2.2-kb inverted repeat region, located at the left terminiof the 330-kb Chlorella virus PBCV-1 genome were sequenced and analyzed. Eighty-five complete open reading frames (ORFs) larger than 195 nucleotides were identified. Thirty-seven of the 85 ORFs, which are densely packed on both strands of the DNA, were considered major ORFs. Fifteen of the major ORFs have similarity to genes in the databases, including bacterial glycerophosphoryl diester phosphodiesterase, bacteriophage T4 endonuclease V, d-isomer specific 2-hydroxyacid dehydrogenases, and β-alanine synthetase and bacterial nitrilases. Two major ORFs resemble the virus major capsid protein. Three major ORFs contain three or more ankyrin-like repeat elements and four ORFs encode proline-rich proteins.

Forty-three kb of DNA, located at the left end (45 to 88 kb) of the 330-kb Chlorella virus PBCV-1 genome, was sequenced and analyzed. Eighty-six open reading frames (ORFs) 65 codons or longer were identified; 47 were classified as major ORFs. These 47 major ORFs are densely packed on both strands of PBCV-1 DNA. Seventeen of these major ORFs resemble genes in the sequence databases, including three putative gene products involved in manipulating sugars (glucosamine synthetase, GDP-D-mannose dehydratase, and N-acetylglucosaminyltransferase), two transcription factors, β-1,3-glucanase, aspartate transcarbamylase, ubiquitin carboxy terminal hydrolase, RNA guanyl transferase, an exonuclease, and a helicase. This is the first time some of these putative PBCV-1 genes have been found in a virus genome. One of the transcription factor-like genes contains a type IB self-splicing intron. Since a spliceosomal processed intron was reported previously in the PBCV-1 DNA polymerase gene, PBCV-1 is the first virus known to contain two different types of introns. Analysis of 76 kb of newly sequenced DNA, located between map positions 182 and 258 kb in the 330-kb chlorella virus PBCV-1 genome, revealed 175 open reading frames (ORFs) of 65 codons or longer. One hundred and five of these 175 ORFs were considered major ORFs. Twenty-one of the 105 major ORFs resembled proteins in databases including ribonucleotide reductase small subunit, RNase III, thioredoxin, glutaredoxin, protein disulfide isomerase, deoxynucleoside kinase, frog virus 3 ATPase,Acetobactercellulose synthase, a bacteriophage encoded endonuclease, and two C-5 cytosine DNA methyltransferases. One of the ORFs was the PBCV-1 major capsid protein. The 105 major ORFs were evenly distributed along the genome. One set of ORFs was separated by 543 nucleotides whereas 75 of the ORFs were separated by fewer than 100 nucleotides. Nineteen of the 175 ORFs resembled other PBCV-1 ORFs, suggesting that they represent either gene duplications or gene families.

The bovine herpesvirus type 1 (BHV-1) open reading frame (ORF) UL3.5 is similar to ORFs found in pseudorabies virus, infectious laryngotracheitis virus, equine herpesvirus type 1, and varicella zoster virus, but clearly absent from herpes simplex virus. The published sequence for this ORF predicts a 126-amino-acid (13.2 kDa) protein product with an isoelectric point of 12.3. We confirmed the UL3.5 sequence, expressed the ORF as a glutathione-S-transferase fusion protein, and made rabbit antibodies against the purified fusion protein. The antiserum detected a 13-kDa protein in Western blots of MDBK cells infected with BHV-1, but not with other herpesviruses or uninfected cells. The BHV-1 UL3.5 protein was characterized as a component of the virion envelope or tegument because it was expressed as a late protein, it was present in the cytoplasm but not the nucleus of infected cells, and it was removed from purified virions by detergent extraction.

This report completes a preliminary analysis of the sequence of the 330,740-bp chlorella virus PBCV-1 genome, the largest virus genome to be sequenced to date. The PBCV-1 genome is 57% the size of the genome from the smallest self-replicating organism,Mycoplasma genitalium.Analysis of 74 kb of newly sequenced DNA, from the right terminus of the PBCV-1 genome, revealed 153 open reading frames (ORFs) of 65 codons or longer. Eighty-five of these ORFs, which are evenly distributed on both strands of the DNA, were considered major ORFs. Fifty-nine of the major ORFs were separated by less than 100 bp. The largest intergenic distance was 729 bp, which occurred between two ORFs located in the 2.2-kb inverted terminal repeat region of the PBCV-1 genome. Twenty-seven of the 85 major ORFs resemble proteins in databases, including the large subunit of ribonucleotide diphosphate reductase, ATP-dependent DNA ligase, type II DNA topoisomerase, a helicase, histidine decarboxylase, dCMP deaminase, dUTP pyrophosphatase, proliferating cell nuclear antigen, a transposase, fungal translation elongation factor 3 (EF-3), UDP glucose dehydrogenase, a protein kinase, and an adenine DNA methyltransferase and its corresponding DNA site-specific endonuclease. Seventeen of the 153 ORFs resembled other PBCV-1 ORFs, suggesting that they represent either gene duplications or gene families.

Four hundred fifty-three chronic pain patients completed a Symptom Checklist 90 (SCL-90) and a comprehensive pain evaluation questionnaire. All patients were evaluated by a physician and rated on degree of pain pathology and pain behavior. The SCL-90 data were analyzed using two clustering procedures and replicated over two similar samples. Three distinct profiles emerged and represented high, medium, and low scores on the SCL-90. No differences were found between subgroups on demographic characteristics, compensation status, pain duration, or pain ratings. Patients in the high-profile subgroup showed the most emotional distress, reported that their pain interfered the most with all activities, and were most often judged to have high pain behavior. Little evidence was found to support a “denial” profile, as previously predicted. Further support was found for using the SCL-90 in assessing chronic pain patients.

The present descriptive study profiles a number of variables related to outcome, i.e., show vs no-show, for routine intake appointments at a large urban community mental health center. Data obtained from 156 individuals at their initial contact with the community mental health center were analyzed using general loglinear and chi-squared procedures. Method of contact (in person, telephone) and latency for intake appointment (number of days between initial mental health center contact and date of intake appointment) are statistically significantly related to outcome. Individuals who contact the community mental health center by telephone and have relatively short latencies for intake appointments are more likely to “show” for their intake appointments than individuals who make contact in person and have relatively long latencies. Results are discussed in relation to previous research involving appointment keeping. Suggestions for future research are presented.

We report the nucleotide sequence of the 19-kb HindIII fragment B of bovine herpesvirus 1 (BHV-1) DNA and adjacent parts of the HindIII A and L fragments, which together span a still completely uncharted 30-kb region located between the glycoprotein H gene and the right end of the unique long segment The analysis revealed 17 complete open reading frames (ORFs) and 2 ORFs that were interrupted by potential splice donor and acceptor sites. All of these ORFs exhibited strong amino acid sequence homology to the gene products of other alphaherpesviruses, The BHV-1 ORFs were arranged colinearly with the prototype sequence of herpes simplex virus 1 in the range of the UL21 to UL4 genes. Colinearity was also observed with the genes of betaherpesviruses and gamma herpesviruses, although not all ORFs exhibited clear sequence homology The possible functions of the proteins encoded within the sequenced region are assessed and features found are discussed. Unexpected findings include the following high amino acid sequence conservation among alphaherpesviruses despite large differences in G + C content, ranging from 45% for varicella zoster virus to 72% for BHV-1; high similarity with other UL20 proteins at the predicted structural level in spite of relatively low amino acid homology; and a 2-kb open reading frame overlapping UL19 in the opposite sense and exhibiting high amino acid similarity to the same area of pseudorabies virus. Computerized psychological assessment is potentially a viable alternative to the traditional method of examiner based assessment. 15 children, 4 females and 11 males, ranging in age from 7 to 14 yr., were administered a computerized version of the Raven's Coloured Progressive Matrices test. The necessary software for computerizing this test was developed and programmed for use with an Apple II Plus microcomputer. Scores obtained from the computerized and traditional versions of the Raven's test were compared on total mean score, correlation with the WISC-R, Raven's subscale intercorrelations, and Raven's total mean score composition. In general, results indicate that the two versions of the Raven's Matrices do not statistically significantly differ on the dimensions tested in the present study. The relatively small number of subjects and the lack of rigorous experimental control limit generalization of the results. Continued research into the use of computerized psychological assessment is warranted.

Herpes simplex virus type 1 (HSV-1) can establish latent infections in peripheral nerve ganglia and the central nervous system (CNS) of experimentally infected mice. Latent infections of peripheral nervous tissue are characterized by the ability to recover infectious virus from explant cultures of most of the latently infected ganglia1–4. In contrast, infectious virus is infrequently recovered from the CNS of latently infected mice following explant culture2–4, although viral DNA can be detected in CNS tissue4. The presence of incomplete or defective viral genomes during the latent CNS infections has been proposed to explain this difference between the two latent infections4. To characterize the completeness and the physical state of the latent viral genome, we used Southern blot hybridization to analyse the viral DNA from latently infected mouse ganglia and brains. Our results, reported here, indicate that most, if not all, of the HSV-1 genome is present in latently infected CNS tissue and that the latent viral genome exists in a form other than linear, unit length DNA.
