A monofilament fibre debridement pad has been found to be a rapid and effective mechanical method of removing dry skin, biofilm and debris from acute and chronic wounds with minimal patient discomfort. Evidence of its impact on prescribing and wound healing, however, has been more limited. The aim of this audit was to show evidence of the monofilament fibre debridement pad's impact on wound treatment costs through an analysis of NHS wound-care prescribing data in England. A dataset for 486 uniquely identified patients who had been newly prescribed the monofilament fibre debridement pad was obtained from the NHS Business Services Authority. All data were anonymised. Costs were identified for the six months before and six months after the month of first prescription of the monofilament fibre debridement pad. The total cost of wound-care prescribing fell by 14% or £101,723 in the six months after the intervention compared with the six months before. The average monthly expenditure per patient fell from £244 before the intervention to £209 (n=486) after. These results indicate that use of the monofilament fibre debridement pad could reduce prescribing costs and the use of antimicrobial and negative pressure therapies. Further research is warranted to investigate the clinical role of the monofilament fibre debridement pad in wound healing

The late 1960s were a turbulent time on Canadian postsecondary campuses. University students were both the subjects of and participants in the broader discourse regarding the counterculture and drug use. Many participants in this discourse presumed a close relationship between students, drug use, and countercultural perspectives, but the records of the Le Dain commission challenge some of these associations. Held throughout 1969 and 1970, the Le Dain commission transcripts from hearings held on university campuses across Canada suggest that while Canadian postsecondary students broadly endorsed drug policy reforms, they simultaneously resisted the more radical aspects of the late-1960s countercultural discourse.

Swine medicine resources and caseloads for teaching and supporting extracurricular training activities vary widely among veterinary colleges and are concentrated in specific regions. Student interest and demand for swine medicine training is broader in geographical distribution. This is illustrated by student membership and attendance at the American Association of Swine Veterinarians (AASV) annual meetings, for example. To explore how concentrated resources might be made more widely available in a cost-effective manner, the Swine Medicine Education Center (SMEC) at Iowa State University’s College of Veterinary Medicine looked for ways to leverage existing extracurricular resources with a broader geography of schools and students. This article describes the organization of student chapters of the AASV and the outcomes of a multi-session live audio and video webcast focused on swine medicine topics across North America over a 3-year period. SMEC organized the series with funding provided by the AASV and AASV Foundation. The broadcast series covered a wide range of swine-related topics, including pet pigs, emerging diseases, and regulation of antimicrobials. In its third year, 25 North American and 4 international veterinary schools participated in the series and provided feedback from attendees.

JAK1, JAK2, JAK3, and TYK2 belong to the JAK (Janus kinase) family. They play critical roles in cytokine signaling. Constitutive activation of JAK/STAT pathways is associated with a wide variety of diseases. Particularly, pSTAT3 is observed in response to the treatment with inhibitors of oncogenic signaling pathways such as EGFR, MAPK, and AKT and is associated with resistance or poorer response to agents targeting these pathways. Among the JAK family kinases, JAK1 has been shown to be the primary driver of STAT3 phosphorylation and signaling; therefore, selective JAK1 inhibition can be a viable means to overcome such treatment resistances. Herein, an account of the medicinal chemistry optimization from the promiscuous kinase screening hit 3to the candidate drug 21(AZD4205), a highly selective JAK1 kinase inhibitor, is reported. Compound 21has good preclinical pharmacokinetics. Compound 21displayed an enhanced antitumor activity in combination with an approved EGFR inhibitor, osimertinib, in a preclinical non-small-cell lung cancer (NSCLC) xenograft NCI-H1975 model.

Breast cancer affects 1 in 8 women. As the treatment of breast cancer evolves, breast reconstruction does as well. Implant-based reconstructions are increasing, leading to increased use of acellular dermal matrix (ADM) for better implant positioning. Acellular dermal matrices are derived from cadaveric skin and are processed to be immunologically inert. However, ADM can be costly and can have complications such as seroma and infection. This has led to the development of dermal autografts. These were first used in postmastectomy breast reconstruction in women with redundant breast skin that was deepithelialized and used for lower pole coverage of tissue expanders and implants. This evolved into harvesting dermal autografts from the abdomen. Later studies evaluated the use of meshed dermal autografts. Histological analysis of ADM versus dermal autografts shows that there are increased vessels within dermal autografts compared with ADM. This potentially contributes to the decreased complication rate seen with autografts. In addition, one study showed equivalent results in aesthetic outcomes and capsular contracture between ADM and dermal autograft. Analysis of cost has shown that ADM is significantly more costly than harvesting a dermal autograft. Physician reimbursement is also higher for dermal autografts. This review article seeks to summarize key studies that highlight the feasibility of using dermal autografts in breast reconstruction.

Journalists of former Yugoslavia agree in a meeting at Dusseldorf, Germany, that the central source of political manipulation in Serbia, Montenegrom Croatia and Bosnia is control of the media from Belgrade. Both local and foreign journalists are unable to broadcast the true situation in former Yugoslavia because of censorship in the former and misinformation in the latter case. The political situation in these states can only be resolved with the reform of the media which should have begun when repression of Kosovo's Albanian population occured in the 1980s.

Albanian journalists in Serbian-occupied Kosovo lost their jobs with the closure of the daily paper Rilindja and five radio stations. Activities of the Albanian-language media have been curtailed since July 1990 by the Belgrade-run administration. The only journalists left with jobs are those who work for the foreign press, the broadcast stations Croatian News Agency, Czech and Macedonian Radio and those who publish periodicals like the Bujiku and the Zeri. But these journalists have to deal with police harassment. Moreover, readers found in possession of Albanian-language magazines also face arrest.

Athlete safety and concussion injury have garnered considerable attention recently, and appropriate evaluation of athletes following head impacts depends, in part, on athletes’ self-reporting of the symptoms. The National Collegiate Athletic Association has focused primarily on concussion injury education to encourage self-reporting; however, such efforts have not been especially effective and many potential injuries continue to go unreported. This research investigates cultural narratives, derived from sports media and popular culture, and how their narrative logics contribute to the context in which student-athletes make head injury reporting decisions and how these narratives offer templates for understanding potential consequences. We argue that performance-oriented narratives are more prevalent and showcase pathways to more immediate satisfaction of desires or goals. Ultimately, we argue that not only does analysis of prevailing cultural narratives illuminate the context in which athletes make reporting decisions but also that such understanding could inform narrative-based interventions in order to emphasize and model recommended behaviors, such as injury reporting, and values, such as long-term brain health and player wellness.

Oxaliplatin-induced peripheral neuropathy (OIPN) is a dose-limiting toxicity characterised by mechanical allodynia and thermal hyperalgesia, without any licensed medications. ART26.12 is a fatty acid-binding protein (FABP) 5 inhibitor with antinociceptive properties, characterised here for the prevention and treatment of OIPN. ART26.12 binds selectively to FABP5 compared to FABP3, FABP4, and FABP7, with minimal off-target liabilities, high oral bioavailability, and a NOAEL of 1,000 mg/kg/day in rats and dogs. In an established preclinical OIPN model, acute oral dosing (25–100 mg/kg) showed a cannabinoid receptor type 1 (CB1)-dependent anti-allodynic effect lasting up to 8 hours (persisting longer than plasma exposure to ART26.12). Antagonists of cannabinoid receptor type 2 (CB2), peroxisome proliferator-activated receptor alpha, and transient receptor potential cation channel subfamily V member 1 (TRPV1) may have also been implicated. Twice daily oral dosing (25 mg/kg bis in die (BID) for 7 days) showed anti-allodynic effects in an established OIPN model without the development of tolerance. In a prevention paradigm, coadministration of ART26.12 (10 and 25 mg/kg BID for 15 days) with oxaliplatin prevented thermal hyperalgesia, mitigated mechanical allodynia, and attenuated OXA-induced weight loss. Multi-scale analyses revealed widespread lipid modulation, particularly among N-acyl amino acids in the spinal cord, including potential analgesic mediators. Additionally, ART26.12 administration led to upregulation of ion channels in the periaqueductal grey, and broad translational upregulation within the plasma proteome. These results show promise that ART26.12 is a safe and well-tolerated candidate for the treatment and prevention of OIPN through lipid modulation.

Janus kinases (JAKs) have been demonstrated to be critical in cytokine signaling and have thus been implicated in both cancer and inflammatory diseases. The JAK family consists of four highly homologous members: JAK1–3 and TYK2. The development of small-molecule inhibitors that are selective for a specific family member would represent highly desirable tools for deconvoluting the intricacies of JAK family biology. Herein, we report the discovery of a potent JAK1 inhibitor, 24, which displays ∼1000-fold selectivity over the other highly homologous JAK family members (determined by biochemical assays), while also possessing good selectivity over other kinases (determined by panel screening). Moreover, this compound was demonstrated to be orally bioavailable and possesses acceptable pharmacokinetic parameters. In an in vivo study, the compound was observed to dose dependently modulate the phosphorylation of STAT3 (a downstream marker of JAK1 inhibition).

OBJECTIVETo investigate an outbreak of Burkholderia cepaciacomplex and describe the measures that revealed the source.SETTINGA 629-bed, tertiary-care, pediatric hospital in Houston, Texas.PATIENTSPediatric patients without cystic fibrosis (CF) hospitalized in the pediatric and cardiovascular intensive care units.METHODSWe investigated an outbreak of B. cepaciacomplex from February through July 2016. Isolates were evaluated for molecular relatedness with repetitive extragenic palindromic polymerase chain reaction (rep-PCR); specific species identification and genotyping were performed at an independent laboratory. The investigation included a detailed review of all cases, direct observation of clinical practices, and respiratory surveillance cultures. Environmental and product cultures were performed at an accredited reference environmental microbiology laboratory.RESULTSOverall, 18 respiratory tract cultures, 5 blood cultures, 4 urine cultures, and 3 stool cultures were positive in 24 patients. Among the 24 patients, 17 had symptomatic infections and 7 were colonized. The median age of the patients was 22.5 months (range, 2–148 months). Rep-PCR typing showed that 21 of 24 cases represented the same strain, which was identified as a novel species within the B. cepaciacomplex. Product cultures of liquid docusate were positive with an identical strain of B. cepaciacomplex. Local and state health departments, as well as the CDC and FDA, were notified, prompting a multistate investigation.CONCLUSIONSOur investigation revealed an outbreak of a unique strain of B. cepaciacomplex isolated in clinical specimens from non-CF pediatric patients and from liquid docusate. During a study of the early marine survival of Chinook Salmon Oncorhynchus tshawytschaand Coho Salmon O. kisutchin the Strait of Georgia from 1998 to 2010, moderate abundances of juvenile Sockeye Salmon O. nerkawere observed to remain in the strait much longer than previously thought. In 2008, DNA stock identification showed that these juveniles were from the Harrison River, a population with a sea-type life history in which juveniles enter the ocean during the year of emergence from the gravel. Using information collected in 1998–2010, we describe the early ocean life and production of Harrison River sea-type Sockeye Salmon. Juveniles entered the Strait of Georgia from the Fraser River over an extended period, with most entering after mid-July—about 8 weeks later than lake-type juveniles, which had virtually all left the strait by that time. The September diets of sea-type juveniles were highly selective for hyperiid amphipods, which were not abundant in the zooplankton. Interaction with juvenile Pink Salmon O. gorbuschaduring this early marine period was identified as potentially affecting the age structure of returning adults. Juvenile Sockeye Salmon remained in the strait until the fall and then moved offshore, where they remained until returning as age-3 or age-4 adults. There was a strong positive relationship between the September CPUE of juveniles in the Strait of Georgia and the total adult return of Harrison River Sockeye Salmon, indicating that total production in recent years was likely related to conditions in the strait. From 2005 to 2011, Harrison River Sockeye Salmon production exhibited a large increase in comparison with the total production of Sockeye Salmon in the Fraser River system. This recent increase in production of late-ocean-entering juveniles with a sea-type life history identifies the importance of managing for biodiversity of Sockeye Salmon populations within the Fraser River drainage. A new data source from ground‐based stations that track global navigation satellite system (GNSS) transmitters has been implemented in the Navy Global Environmental Model (NAVGEM) with the NRL Atmospheric Variational Data Assimilation System‐Accelerated Representer. The observable is the ground‐based zenith total delay (ZTD) at each ground station. This ground‐based GNSS ZTD represents the tropospheric delay of the propagation of GNSS radio L‐band signal between the transmitters and the ground stations after correcting for ionospheric effects. Here, we present the implementation procedure, quality control, single observation test, bias correction scheme, and data impact assessment. One of the critical elements in the quality control scheme is to correct for the difference between the observing ground station elevation and the numerical model terrain. A single observation test shows comparable impact from this new data source to a single radiosonde observation at a single level. The ZTD biases estimated from a 6‐month experiment run are generally small, but have a dependency on the processing center and are computed separately for each station. The forecast sensitivity to observation impact diagnosis from a 3‐month experiment demonstrates that a single ZTD observation shows similar impact as the average impact from a single tangent point of a GNSS Radio Occultation profile. The assimilation of ZTD observations also significantly improves the forecast skill by 0.25%–0.75% for wind, temperature, and precipitable water and by 1%–2% for geopotential height in the Southern Hemisphere beyond 3 days. Adequate sampling of water vapor in meteorological and climate observing systems is critical to environmental characterization and climate monitoring. Currently, atmospheric humidity observations in both space and time are not adequately sampled. The ground‐based global navigation satellite system (GNSS) network offers a cost‐effective pathway to retrieve high‐quality humidity observations. Observations from the GNSS network can serve as anchoring observations for the data assimilation and numerical weather prediction. These observations complement other satellite measurements to further improve forecast quality. This study outlines the implementation procedure for the ground‐based GNSS zenith total delay and assesses its impact on Navy’s global numerical weather forecasts. Our results indicate that the data can have a significant positive impact on the Southern Hemisphere’s forecasts beyond 3 days. When evaluating each observation’s impact on the 24‐hr model forecasts, the impact of a single zenith total delay measurement is comparable to that of the average impact of a single GNSS Radio Occultation data at a single tangent point.
