Metabolic changes often occur long before pathologies manifest and treatment becomes challenging. As key elements of energy metabolism, α-ketocarboxylic acids (α-KCA) are particularly interesting, e.g., as the upregulation of pyruvate to lactate conversion is a hallmark of cancer (Warburg effect). Magnetic resonance imaging with hyperpolarized metabolites has enabled imaging of this effect non-invasively and in vivo, allowing the early detection of cancerous tissue and its treatment. Hyperpolarization by means of dynamic nuclear polarization, however, is complex, slow, and expensive, while available precursors often limit parahydrogen-based alternatives. Here, we report the synthesis for novel 13C, deuterated ketocarboxylic acids, and a much-improved synthesis of 1-13C-vinyl pruvate-d6, arguably the most promising tracer for hyperpolarizing pyruvate using parahydrogen-induced hyperpolarization by side arm hydrogenation. The new synthesis is scalable and provides a high yield of 52%. We elucidated the mechanism of our Pd-catalyzed trans-vinylation reaction. Hydrogenation with parahydrogen allowed us to monitor the addition, which was found to depend on the electron demand of the vinyl ester. Electron-poor α-keto vinyl esters react slower than “normal” alkyl vinyl esters. This synthesis of 13C, deuterated α-ketocarboxylic acids opens up an entirely new class of biomolecules for fast and cost-efficient hyperpolarization with parahydrogen and their use for metabolic imaging.

Balancing stroke prevention and risk of bleeding in patients with atrial fibrillation (AF) is challenging. Direct oral anticoagulants (DOACs) are by now considered standard of care for treating patients with AF in international guidelines. Our objective was to assess the safety of long-term intake of DOACs in older adults with AF. We included RCTs in elderly (≥ 65 years) patients with AF. A systematic search in MEDLINE and EMBASE was performed on 19 April 2022. For determination of risk of bias, the RoB 2 tool was applied. We pooled outcomes using random-effects meta-analyses. The quality of evidence was assessed using GRADE. Eleven RCTs with a total of 63,374 patients were identified. Two RCTs compared apixaban with either warfarin or aspirin, four edoxaban with either placebo, aspirin, or vitamin K antagonists (VKAs), two dabigatran with warfarin and three rivaroxaban with warfarin. DOACs probably reduce mortality in elderly patients with AF (HR 0.89 95%CI 0.77 to 1.02). Low-dose DOACs likely reduce bleeding compared to VKAs (HR ranged from 0.47 to 1.01). For high-dose DOACS the risk of bleeding varied widely (HR ranged from 0.80 to 1.40). We found that low-dose DOACs probably decrease mortality in AF patients. Moreover, apixaban and probably edoxaban are associated with fewer major or clinically relevant bleeding (MCRB) events compared to VKAs. For dabigatran and rivaroxaban, the risk of MCRB varies depending on dose. Moreover, subgroup analyses indicate that in the very old (≥ 85) the risk for MCRB events might be increased when using DOACs.

In this work, we utilized the proteolysis targeting chimera (PROTAC) technology to achieve the chemical knock-down of histone deacetylase 6 (HDAC6). Two series of cereblon-recruiting PROTACs were synthesized via a solid-phase parallel synthesis approach, which allowed the rapid preparation of two HDAC6 degrader mini libraries. The PROTACs were either based on an unselective vorinostat-like HDAC ligand or derived from a selective HDAC6 inhibitor. Notably, both PROTAC series demonstrated selective degradation of HDAC6 in leukemia cell lines. The best degraders from each series (denoted A6and B4) were capable of degrading HDAC6 via ternary complex formation and the ubiquitin–proteasome pathway, with DC50values of 3.5 and 19.4 nM, respectively. PROTAC A6demonstrated promising antiproliferative activity via inducing apoptosis in myeloid leukemia cell lines. These findings highlight the potential of this series of degraders as effective pharmacological tools for the targeted degradation of HDAC6.

Quality indicators to assess the quality of primary care have only been applied on a national or regional level in European countries, and there have been no comparisons between regions of different countries. In the interventional pre-post-study “Improvement of Quality by Benchmarking – IQuaB” (level of evidence: 3), we aimed to improve and compare quality of process care in 57 participating general practices in Salzburg, Austria, and South Tyrol, Italy.

Plasmon sensors respond to local changes of their surrounding environment with a shift in their resonance wavelength. This response is usually detected by measuring light scattering spectra to determine the resonance wavelength. However, single wavelength detection has become increasingly important because it simplifies the setup, increases speed, and improves statistics. Therefore, we investigated theoretically how the sensitivity toward such single wavelength scattering intensity changes depend on the material and shape of the plasmonic sensor. Surprisingly, simple equations describe this intensity sensitivity very accurately and allow us to distinguish the various contributions: Rayleigh scattering, dielectric contrast, plasmon shift, and frequency-dependent plasmon bulk damping. We find very good agreement of theoretical predictions and experimental data obtained by single particle spectroscopy.

Frage: Miktionsbeschwerden sind insbesondere bei älteren Männern häufig auf eine benigne Prostatahyperplasie (BPH) zurückzuführen. Diese können durch Alpharezeptorblocker gelindert werden. Sollten daher zur Behandlung der benignen Prostatahyperplasie generell Alpharezeptorblocker verordnet werden? Antwort: Für die Alpharezeptorblocker Tamsulosin, Alfuzosin, Silodosin und Naftopidil konnte ein geringer bis mäßiggradiger Effekt in der Behandlung von Beschwerden des unteren Harntrakts (Lower Urinary Tract Symptoms – LUTS) nachgewiesen werden, der zu einem guten Teil auf Placeboeffekten beruht. Es gibt keinen Nachweis für eine positive Wirkung auf die Progression der BPH oder die Entstehung von Komplikationen (akuter Harnverhalt, Operation). An wichtigen Nebenwirkungen sind hypotone Kreislaufdysregulation, Schwindel, erektile Dysfunktion und Ejakulationsstörungen zu nennen. Aus großen Kohortenstudien gibt es Hinweise, dass Alpharezeptorblocker das Risiko für Stürze, Frakturen und Demenz erhöhen. Eine Behandlung mit Alpharezeptorblockern sollte daher nur bei entsprechendem Leidensdruck nach einer partizipativen Entscheidungsfindung mit adäquater Aufklärung über möglichen Nutzen und Risiken erfolgen.

Histone deacetylase 6 (HDAC6) is an emerging target for the treatment of cancer, neurodegenerative diseases, inflammation, and other diseases. Here, we present the multicomponent synthesis and structure–activity relationship of a series of tetrazole-based HDAC6 inhibitors. We discovered the hit compound NR-160 by investigating the inhibition of recombinant HDAC enzymes and protein acetylation. A cocrystal structure of HDAC6 complexed with NR-160 disclosed that the steric complementarity of the bifurcated capping group of NR-160 to the L1 and L2 loop pockets may be responsible for its HDAC6-selective inhibition. While NR-160 displayed only low cytotoxicity as a single agent against leukemia cell lines, it augmented the apoptosis induction of the proteasome inhibitor bortezomib in combination experiments significantly. Furthermore, a combinatorial high-throughput drug screen revealed significantly enhanced cytotoxicity when NR-160 was used in combination with epirubicin and daunorubicin. The synergistic effect in combination with bortezomib and anthracyclines highlights the potential of NR-160 in combination therapies.

We compare the decay of plasmons and “conventional” hot electrons within the same series of gold/metal oxide interfaces. We found an accelerated decay of hot electrons at gold-metal oxide interfaces with decreasing band gap of the oxide material. The decay is accelerated by the increased phase space for electron scattering caused by additional interfacial states. Since plasmons decay faster within the same series of gold-metal oxide interfaces, we propose plasmons are able to decay into the same interfacial states as hot electrons. The similarity of plasmon damping to conventional hot electron decay implies that many classical surface analysis techniques and theoretical concepts are transferable to plasmonic systems. Our results support the mechanism of direct decay of plasmons into interfacial hot electron pairs but the utility of these interfacial states for charge transfer reactions remains to be investigated.

Ziel dieser Arbeit ist die exemplarische methodische Überprüfung der Qualität eingeschlossener Studien zur medikamentösen Therapie älterer Menschen anhand einer publizierten Reihe von sechs systematischen Reviews (SRs). Diese sechs SRs inkludierten 48 systematische Übersichtsarbeiten, 65 Interventions- und 33 Beobachtungsstudien. Die Reihe der SRs wurde im Rahmen des PRIMA-eDS-Projekts (www.prima-eds.eu) erstellt, um Empfehlungen zur Behandlung von älteren Menschen mit Polypharmazie zu entwickeln.

We present a study that allows us to explain the chemical changes behind the often observed but so far ununderstood drift of the plasmon resonance of chemically prepared gold nanorods in microfluidic devices. We systematically monitored the evolution of the plasmon scattering signal from thousands of single nanoparticles in parallel, from different gold nanorod batches, prepared by both, silver-assisted and silver-free seeded growth. By varying the experimental conditions, we confirmed the presence of a silver surface layer for nanorods prepared by the silver-assisted method. Although the silver layer is initially protected by the surfactant CTAB, it gets oxidized as soon as CTAB desorbs because of a reduced concentration in the flowing solution. We propose that these findings should be taken into account at the time of devising technological applications of gold nanorods, including the design of new nanorod synthetic routes, but in particular for applications that depend on the precise plasmon resonance position, for the plasmon sensing capabilities and to consider the nanoparticle surface reactivity.

Frage: Derzeit stehen zwei Impfstoffe gegen Herpes zoster zur Verfügung, Zostavax® als attenuierter Lebendimpfstoff und Shin-grix® als adjuvantierter, rekombinanter Totimpfstoff. Soll überhaupt gegen Herpes zoster geimpft werden, und wenn ja, wer, ab welchem Alter und mit welchem Impfstoff? Antwort: Beide Impfstoffe sind effektiv in der Prävention von Herpes zoster und postherpetischer Neuralgie (PHN), mindestens für drei bis vier Jahre. Für den Lebendimpfstoff zeigte sich aber in Langzeitstudien ein rascher Effektivitätsverlust. Langzeitdaten für den Totimpfstoff liegen noch nicht vor. In den ersten vier Jahren war die Impfeffektivität des Totimpfstoffs derjenigen des Lebendimpfstoffs überlegen und der Effektivitätsverlust fiel geringer aus. Dieser Vorteil wird aber durch eine höhere Rate an lokalen und systemischen Nebenwirkungen erkauft. In Anbetracht der eher schlechten Verträglichkeit der Impfung muss in einer partizipati-ven informierten Entscheidung mit den Patient*innen diskutiert werden, ob eine Impfung erfolgen soll. Die STIKO empfiehlt die Impfung mit dem Totimpfstoff ab dem 60. Lebensjahr. In Deutschland wird die mit 226,80 Euro nicht ganz billige Impfung durch die Sozialversicherung bezahlt, in Österreich nicht.

Frage: Wir sind bereits mitten in der Influenzasaison und es stellt sich die Frage, ob es zusätzlich zu oder statt der Impfung therapeutische Optionen gibt, welche die Erkrankung verhindern oder verkürzen und vor allem Komplikationen und Todesfälle vermeiden helfen. Virustatische Arzneimittel gegen das Influenzavirus werden hier häufig als ursächliche Behandlung einer vermuteten oder bewiesenen Infektion verschrieben. Wie sieht es mit der Evidenz für die Wirksamkeit beziehungsweise für das Verhältnis von Nutzen zu Risiken dieser Therapie aus? Antwort: Die Evidenz für eine Effektivität der Behandlung einer vermuteten oder bewiesenen Influenza mit Neuraminidasehemmern ist aufgrund der mangelhaften Qualität der vorliegenden Studien schwach. Die Gabe von Oseltamivir und Zanamivir sollte derzeit nur für Patienten mit hohem Risiko für Komplikationen erwogenwerden (wegen Influenza hospitalisierte Patienten, Patienten mit schwerem oder progressivem Verlauf oder hohem Komplikationsrisiko, Patienten mit relevanten chronischen Erkrankungen, Immunsupprimierte, Kinder unter 2 Jahren, Erwachsene ≥ 65 Jahren, Schwangere und Frauen innerhalb von zwei Wochen nach Entbindung). Beide Substanzen können allenfalls die Zeit bis zum Beginn der Symptomlinderung etwas verkürzen. Es gibt keine belastbare Evidenz, dass Komplikationen verhindert werden oder die Mortalität reduziert wird. Die prophylaktische Gabe kann möglicherweise das Risiko für einen Krankheitsausbruch vermindern. Ob eine Reduktion des Risikos für Komplikationen wie Pneumonien erzielt werden kann, ist aus den vorliegenden Studien nicht abzuleiten. Nebenwirkungen wie Übelkeit, Erbrechen, psychiatrische und renale Komplikationen bei Erwachsenen und Erbrechen bei Kindern müssen bei der Entscheidung berücksichtigt werden.

Frage: Die Erhöhung systolischer Blutdruckwerte im Sinne einer arteriellen Hypertonie ist als wichtiger Risikofaktor für die Entstehung kardiovaskulärer Erkrankungen bekannt und stellt indirekt weltweit eine der häufigsten Todesursachen dar. Zur Senkung eines erhöhten systolischen Blutdrucks haben sich zwei Strategien gut etabliert. Einerseits sehen Guidelines die pharmakologische Blutdrucksenkung vor, daneben werden allgemeine Lebensstilmodifikationen zur Vermeidung kardiovaskulärer Begleitereignisse empfohlen. Es stellt sich daher die Frage, inwieweit regelmäßiges körperliches Training als Teil der Lebensstilmodifikation wirksam ist, um eine arterielle Hypertonie zu behandeln. Welche Studienevidenz liegt vor, um die Empfehlung zu mehr körperlicher Aktivität zu begründen? Antwort: Neben der pharmakologischen Therapie spielt körperliche Aktivität als Lebensstilmodifikation zur Blutdrucksenkung in der Behandlung der arteriellen Hypertonie eine wichtige Rolle. Durch körperliche Aktivität – optimalerweise von mindestens 10–20 MET (metabolisches Äquivalent) x Std. pro Woche – kann der systolische Blutdruck um ca. 5–15 mmHg gesenkt werden. Die Blutdrucksenkung liegt damit in einem ähnlichen Bereich wie die Senkung durch eine medikamentöse Monotherapie. Es ist wahrscheinlich, dass hierdurch auch ähnliche Effekte auf Mortalität und kardiovaskuläre Ereignisrate erzielt werden wie durch eine medikamentöse Monotherapie.

Frage: Pregabalin wird in der Mehrzahl der Leitlinien zur Behandlung der schmerzhaften diabetischen Polyneuropathie als eine der medikamentösen Erstlinientherapie-Optionen angesehen. Andererseits gibt es wachsende Bedenken hinsichtlich der Nebenwirkungen und des möglichen Missbrauchs. Was ist der aktuelle wissenschaftliche Stand hinsichtlich Nutzen und Risiken von Pregabalin zur Behandlung der diabetischen Neuropathie? Antwort: Die Wirkung von Pregabalin ist dosisabhängig und beginnt ab einer täglichen Gabe von 300 mg. Mit steigender Dosis nehmen auch unerwünschte Wirkungen wie Schwindel und Schläfrigkeit zu. Zudem besteht ein Missbrauchspotenzial. Einer NNT (number needed to treat) von 6 für eine 50-prozentige Schmerzlinderung steht eine NNH (number needed to harm) von 5–10 für Schläfrigkeit und Schwindel gegenüber. Nutzen und Risiken der Behandlung müssen daher vor der Verordnung kommuniziert werden, um einen partizipativen Entscheidungsprozess zu ermöglichen. Um das Missbrauchspotenzial zu verringern, sollte Pregabalin vor allem bei Risiko personen mit Abhängigkeitspotential mit Vorsicht angewendet werden. Ärztinnen/Ärzte sollten sich der miss-bräuchlichen Verwendung von Pregabalin bewusst sein.

Diazocines are bridged azobenzenes with superior photophysical properties. In contrast to azobenzenes the Zconfiguration is thermodynamically stable and the Eisomer is metastable. We present a new class of nitrogen bridged diazocines with bathochromically shifted switching wavelengths and remarkably high quantum yields (−NH–CH2− bridged diazocine: ΦZ→E= 0.57, ΦE→Z= 0.8). Zto Eisomerization is induced by irradiation with blue light, whereas switching back to the Zisomer is accomplished with light in the near-infrared window (up to 740 nm), which is important for medical applications like photopharmacology (deep tissue penetration). Furthermore, substitution at the bridging nitrogen should provide access to widely applicable tricyclic, photoswitchable pharmacophores. The −NAc–CH2− bridged derivative is soluble in water, and all photophysical properties (conversion rates, quantum yields, and thermal half-lives) are largely retained. Hence, this diazocine is an ideal photoswitch for applications in biochemical systems and in photopharmacology.

Fe(III) porphyrins bridged with 1,2,3-triazole ligands were synthesized. Upon deprotonation, the triazolate ion coordinates to the Fe(III) ion, forming an overall neutral high-spin Fe(III) porphyrin in which the triazolate serves both as an axial ligand and as the counterion. The second axial coordination site is activated for coordination and binds p-methoxypyridine, forming a six-coordinate low-spin complex. Upon addition of a phenylazopyridine as a photodissociable ligand, the spin state of the complex can be reversibly switched with ultraviolet and visible light. The system provides the basis for the development of switchable catalase- and peroxidase-type catalysts and molecular spin switches.

Frage: Zur Behandlung eines unkomplizierten Harnwegsinfektes von erwachsenen Frauen werden gemäß aktuellen Leitlinien antibiotische Regimes mit Fosfomycin, Nitrofurantoin, Nitroxolin, Pivmecillinam oder Trimethoprim als prinzipiell geeignete Substanzen empfohlen. Aufgrund einer zunehmenden Resistenzentwicklung ist der gut ausgewählte Einsatz des Therapeutikums in entsprechender Dosierung und verordneter Dauer von essentieller Bedeutung. Da für den Einsatz von Pivmecillinam niedrige Resistenzraten und Kollateralschäden beschrieben wurden, gingen wir der Fragestellung nach, wie wirkungsvoll die Therapie mit Pivmecillinam in der Therapie unkomplizierter Infektionen des unteren Harntraktes bei erwachsenen Frauen ist. Dabei interessierte uns die Evidenzlage zur Effektivität sowie zur optimalen Dosierung bzw. Dauer der antibiotischen Behandlung mit dieser Substanz. Antwort: Die vorliegende Studienliteratur spricht für eine hohe Effektivität von Pivmecillinam in der Behandlung unkomplizierter Harnwegsinfektionen bei erwachsenen Frauen. Insbesondere das Risiko für Pyelonephritiden als Komplikation wird gegenüber Placebo signifikant gesenkt. Bezüglich der optimalen Dosierung und Therapiedauer ist die Studienlage uneinheitlich. Längere und höhere Dosierung scheinen keinen klinisch relevanten Vorteil zu bieten, sodass eine niedrige Dosierung, z.B. 3 x 200 mg tgl. mit einer Initialdosis von 400 mg oder 2–3 x 400 mg tgl. für drei Tage als ausreichend angesehen wird.

Single-particle plasmon spectroscopy has become a standard technique to detect and quantify the presence of unlabeled macromolecules. Here, we extend this method to determine their exact distance from the plasmon sensors with sub-nanometer resolution by systematically varying the sensing range into the surrounding by adjusting the size of the plasmonic nanoparticles. We improved current single-particle plasmon spectroscopy to record continuously for hours the scattering spectra of thousands of nanoparticles of different sizes simultaneously with 1.8 s time resolution. We apply this technique to study the interaction dynamics of bacterial Min proteins with supported lipid membranes of different composition. Our experiments reveal a surprisingly flexible operating mode of the Min proteins: In the presence of cardiolipin and membrane curvature induced by nanoparticles, the protein oscillation occurs on top of a stationary MinD patch. Our results reveal the need to consider membrane composition and local curvature as important parameters to quantitatively understand the Min protein system and could be extrapolated to other macromolecular systems. Our label-free method is generally easily implementable and well suited to measure distances of interacting biological macromolecules.

We use plasmon rulers to follow the conformational dynamics of a single protein for up to 24 h at a video rate. The plasmon ruler consists of two gold nanospheres connected by a single protein linker. In our experiment, we follow the dynamics of the molecular chaperone heat shock protein 90 (Hsp90), which is known to show “open” and “closed” conformations. Our measurements confirm the previously known conformational dynamics with transition times in the second to minute time scale and reveals new dynamics on the time scale of minutes to hours. Plasmon rulers thus extend the observation bandwidth 3–4 orders of magnitude with respect to single-molecule fluorescence resonance energy transfer and enable the study of molecular dynamics with unprecedented precision.

The strong interactions between light and plasmons (in metal nanoparticles) allow to observe chemical and physical processes on and around the particle on nanometer length scales as well as they allow to use plasmons for various applications. While electrodynamic theory predicts such effects very accurately, it is too complex to intuitivelyconnect the underlying processes with the observed changes. The much simpler description of particle plasmons as harmonically driven dipole antennas has already been successfully used to describe many plasmonic effects. Here, these insights are combined and complemented to form a coherent and simple description of particle plasmons as dipole antennas. Unlike electrodynamic theory, this description connects fundamental plasmon properties such as shape, charges, environment, or surface coverage with spectroscopic properties like line width or resonance position. Therefore, this connection uniquely allows to identify the chemistry and physics behind the plasmonic responses and to intuitively predict the effects of various processes. The intuitive understanding is important to estimate the effects of complex processes, which occur in applications employing plasmonic nanostructures. To demonstrate the utility of this description, we untangled physical and chemical processes of changes in the plasmon observables during alkanethiol adsorption.

Dehydroepiandrosterone (DHEA) is the most abundant circulating steroid hormone in humans, produced by the adrenals, the gonads and the brain. DHEA was previously shown to bind to the nerve growth factor receptor, tropomyosin-related kinase A (TrkA), and to thereby exert neuroprotective effects. Here we show that DHEA reduces microglia-mediated inflammation in an acute lipopolysaccharide-induced neuro-inflammation model in mice and in cultured microglia in vitro. DHEA regulates microglial inflammatory responses through phosphorylation of TrkA and subsequent activation of a pathway involving Akt1/Akt2 and cAMP response element-binding protein. The latter induces the expression of the histone 3 lysine 27 (H3K27) demethylase Jumonji d3 (Jmjd3), which thereby controls the expression of inflammation-related genes and microglial polarization. Together, our data indicate that DHEA-activated TrkA signaling is a potent regulator of microglia-mediated inflammation in a Jmjd3-dependent manner, thereby providing the platform for potential future therapeutic interventions in neuro-inflammatory pathologies.

The plasmonic nanoparticle sensitivity, sensing volume, and the signal-to-noise ratio are strongly dependent on the nanoparticle dimensions. It is difficult to chemically produce or purify nanoparticles with a size variation of less than 10%. This size variation induces a systematic error in sensing experiments that can be reduced when the exact size of each individual nanoparticle is known. In this work, we show how the size of gold nanorods can be estimated directly from the optical spectra of single nanoparticles by using the increase of radiation damping with the nanoparticle size. We verify our approach by comparing these spectrally estimated sizes with the precise sizes of exactly the same gold nanoparticles using scanning electron microscopy. Our method of estimating individual particle dimensions from spectroscopic data reduces the error in plasmonic sensing experiments by as much as 30%.

We use plasmon rulers made from two connected gold nanoparticles to monitor the conformation and stiffness of single PEG molecules and their response to cations. By observing equilibrium fluctuations of the interparticle distance, we obtain the spring constants or stiffness of the connecting single-molecule tether with pico-Newton sensitivity. We observe a transition of the PEG molecules’ extension and stiffness above about 1.2 mM K+ion concentration which is specific to potassium ions. Molecular dynamics simulations reveal the formation of crown-like structures as the most likely molecular mechanism responsible for this specific effect.

Game species in the cultural landscapes of Europe are exposed to several sources of anthropogenic pressure. Whilst hunting is part of coordinated wildlife management, poaching is difficult to quantify, and hence, relatively little is known about its impact on survival patterns of game species. We estimated survival and cause-specific mortality rates of 31 roe deer Capreolus capreolusin the surroundings of the Białowieża Primeval Forest, eastern Poland, from 2010 to 2013. The study area consisted of large, extensively used agricultural fields and a mosaic of pine plantations, which were interspersed with natural forest stands and small agricultural fields and meadows. Roe deer were regularly hunted and the forested part of the study area was traversed by large carnivores. Annual survival rate of all deer was 0.69 and did not differ between seasons. Annual survival rates of females and males in each age class (young and adult) were similar. Poaching was a major source of mortality (6 out of 13 roe deer deaths), and poachers did not prefer any sex or age. Adult survival (0.77) was higher than survival of young individuals (0.53); this trend was stronger when poaching was excluded from the analysis. All cases of poaching were recorded between mid-September and February. Although the two parts of the study area were exposed to a similar degree of anthropogenic pressure, all poached individuals occupied home ranges in the mosaic, where the forest cover in home ranges of roe deer was significantly higher than in the field.

Frage: Neuropathische Schmerzen, vor allem bedingt durch eine diabetische Polyneuropathie, gehören zu den häufigen Beratungsanlässen in der hausärztlichen Praxis. Die adäquate Behandlung dieser Schmerzen stellt eine große Herausforderung dar. Häufig ist die medikamentöse Therapie entweder nicht effektiv oder kann wegen unerwünschter Wirkungen nicht in geeigneter Form appliziert werden. Neben der medikamentösen Therapie kommen daher auch nicht-pharmakologische Verfahren für eine optimierte Kontrolle chronischer Schmerzsymptome zum Einsatz. TENS, die transkutane elektrische Nervenstimulation, ist eine verbreitete Methode zur erweiterten Analgesie. Es stellt sich daher die Frage nach der vorhandenen Studienevidenz für die Effektivität dieser Behandlungsoption. Antwort: Zur Wirksamkeit einer TENS-Behandlung für eine effiziente Schmerzkontrolle bei diabetischer Neuropathie gibt es aus heutiger Sicht keinen belastbaren Nachweis aus qualitativ hochwertigen klinischen Studien. Die gemessenen Effekte entsprechen am ehesten Placeboeffekten, die bei längerer Beobachtung nicht mehr nachweisbar sind. Der Einsatz von TENS erscheint gerechtfertigt bei Patientinnen und Patienten, deren Schmerzen mit einer medikamentösen Standardtherapie nicht ausreichend behandelt werden können. Der Einsatz des Verfahrens sollte keinesfalls eine indizierte wirksame medikamentöse Behandlung hinauszögern.

Frage: Immer wieder kommen Patienten mit Heilsmeldungen wie „Selen schützt vor Krebs“, die in Zeitschriften und Werbeanzeigen verbreitet werden, in die Praxis. Doch was ist wirklich dran an diesen Behauptungen? Gibt es belastbare Studienevidenz für den Einsatz von Selen-Präparaten zur Prävention maligner Tumoren? Antwort: Von einer regelmäßigen Supplementierung der Nahrung mit Selen sollte abgeraten werden. Trotz des möglicherweise bestehenden epidemiologischen Zusammenhangs zwischen Selenspiegel und Krebsinzidenz konnte ein positiver Effekt einer Selensupplementierung bisher nicht in qualitativ hochwertigen Studien nachgewiesen werden, weder hinsichtlich der Krebsinzidenz noch hinsichtlich der Mortalität. Möglicherweise begünstigt eine längere Selensupplementierung die Entstehung eines Diabetes mellitus Typ 2.

Metallic nanoparticles show extraordinary strong light absorption near their plasmon resonance, orders of magnitude larger compared to nonmetallic nanoparticles. This “antenna” effect has recently been exploited to transfer electrons into empty states of an attached material, for example to create electric currents in photovoltaic devices or to induce chemical reactions. It is generally assumed that plasmons decay into hot electrons, which then transfer to the attached material. Ultrafast electron–electron scattering reduces the lifetime of hot electrons drastically in metals and therefore strongly limits the efficiency of plasmon induced hot electron transfer. However, recent work has revived the concept of plasmons decaying directly into an interfacial charge transfer state, thus avoiding the intermediate creation of hot electrons. This direct decay mechanism has mostly been neglected, and has been termed chemical interface damping (CID). CID manifests itself as an additional damping contribution to the homogeneous plasmon line width. In this study, we investigate the size dependence of CID by following the plasmon line width of gold nanorods during the adsorption process of thiols on the gold surface with single particle spectroscopy. We show that CID scales inversely with the effective path length of electrons, i.e., the average distance of electrons to the surface. Moreover, we compare the contribution of CID to other competing plasmon decay channels and predict that CID becomes the dominating plasmon energy decay mechanism for very small gold nanorods.

We demonstrate the potential of the NanoSPR (nanoscale surface plasmon resonance sensors) method as a simple and cheap tool for the quantitative study of membrane protein–protein interactions. We use NanoSPR to determine the effectiveness of two potential drug candidates that inhibit the protein complex formation between FtsA and ZipA at initial stages of bacterial division. As the NanoSPR method relies on individual gold nanorods as sensing elements, there is no need for fluorescent labels or organic cosolvents, and it provides intrinsically high statistics. NanoSPR could become a powerful tool in drug development, drug delivery, and membrane studies.

Hintergrund: Patient Reactions Assessment (PRA) ist ein im englischen Sprachraum etabliertes Befragungsinstrument zur differenzierten Erhebung der patientenseitig wahrgenommenen Qualität der Arzt-Patienten-Beziehung. Ziel der Studie war es, den 15 Items umfassenden Fragebogen mit den Subskalen: Information, Affektivität und Kommunikation in den deutschen Sprachraum zu übertragen und an einer Stichprobe in der Primärversorgung zu evaluieren. Methoden: Im Rahmen einer multizentrischen Querschnittsstudie wurden 506 hausärztlich versorgte Patienten in 19 Praxen an vier Studienzentren in Deutschland, Österreich und der Schweiz rekrutiert. Zur Auswertung wurden deskriptive Daten sowie Verteilungscharakteristiken der Items berechnet. Die interne Konsistenz wurde über Cronbach’s a für das gesamte Instrument und die Subskalen beschrieben und die Faktorenstruktur bestimmt. Ergebnisse: In Anlehnung an das Originalinstrument ergaben sich auch beim PRA-D mittlere bis hohe interne Konsistenzwerte und es bestätigte sich eine Dreifaktorengrundstruktur. Schlussfolgerungen: Mit dem PRA-D steht ein Instrument zur Verfügung, das die Qualität der Arzt-Patienten-Beziehung auf den Subskalen Information, Affektivität und Kommunikation erfassen kann.
