The diagnosis of clinically significant portal hypertension is crucial for prognosis and treatment guidance in patients with compensated advanced chronic liver disease (ACLD). Spleen stiffness measurement (SSM) might improve the non-invasive diagnosis of clinically significant portal hypertension, but previous studies have reported heterogeneous SSM cutoffs. We aimed to evaluate the accuracy of SSM and SSM-based algorithms in this setting.

Alcohol-associated hepatitis (AH) corresponds to a severe entity with high short-term mortality; however, few studies have been published in patients with moderate AH. This study aimed to characterize patients with moderate AH in a global study, identifying prognostic factors and survival at 30, 90, and 180 days.

Alcohol-associated hepatitis (AH) is a severe entity with a mortality of up to 30–50% at 1 month. Pentoxifylline combined with steroids has not demonstrated benefits in severe AH. Some studies have suggested that pentoxifylline may be beneficial in the subgroup of patients with acute kidney injury (AKI) and AH. However, there is no solid evidence of its benefit in mortality in this setting. This study aimed to determine the benefit of the use of pentoxifylline in patients with severe AH and AKI.

In patients with compensated advanced chronic liver disease (cACLD), risk of clinically significant portal hypertension (CSPH) can be estimated by applying non-invasive tests such as liver stiffness measurement (LSM), platelet count, and, in some cases, BMI. We aimed to assess the diagnostic utility of spleen stiffness measurement (SSM) at 100 Hz as a standalone non-invasive test for CSPH and to evaluate its incremental value compared with the ANTICIPATE±NASH model in patients with cACLD.

Combined hepatocellular-cholangiocarcinoma is a primary liver tumor and its imaging patterns have been poorly investigated. Misdiagnosis for either hepatocellular carcinoma or benign lesions can occur. We aimed to evaluate the enhancement pattern of combined hepatocellular-cholangiocarcinoma in cirrhosis with imaging techniques and to estimate the risk of misdiagnosis for hepatocellular carcinoma. All histology-confirmed combined hepatocellular-cholangiocarcinoma in cirrhosis seen in two Italian centers between 2003 and 2016, in which at least one imaging technique had been performed, was retrospectively collected. The enhancement pattern was analyzed for all available imaging modalities. A total of 37 combined hepatocellular-cholangiocarcinoma nodules were identified. Contrast-enhanced ultrasound, computed tomography, and magnetic resonance imaging had been performed in 27, 34, and 17 nodules, respectively. Contrast-enhanced ultrasound was at higher risk of misdiagnosis for pure hepatocellular carcinoma than computed tomography (p?=?0.005) or magnetic resonance imaging (p?=?0.040). Corticosteroids might be used in cases with severe disease and several dynamic models can predict mortality and response to corticosteroids in AH patients. However, there is no consensus on the best of them. This study aimed to evaluate dynamic models to predict response to corticosteroid treatment based on short-term mortality in patients with severe AH based on a worldwide cohort.

•Endoscopy to screen varices needing treatment can be avoided by non-invasive markers in a minority of patients. A new LIP test, designed for mains etiologies, spared 33% of endoscopies in population screening and 54% in individual screening. A new BLIP test, designed for NAFLD, spared 41% of endoscopies in population screening and 75% in individual screening. This new strategy, adapted to clinical practice, should improve screening adherence which is currently poor.
