Während es in der Diagnostik und Früherkennung der Alzheimer-Demenz in den letzten Jahren große Fortschritte gab, hinken die Therapiemöglichkeiten noch hinterher. Neue Therapieansätze, die in die Entwicklung der Erkrankung eingreifen, könnten einen Paradigmenwechsel in der Behandlung der Alzheimer-Demenz einläuten. Wir umreißen in Kürze die aktuelle Entwicklung.

Bei der Fatigue handelt es sich um einen umfassenden Mangel an körperlicher oder mentaler Energie, der zu einer deutlichen Beeinträchtigung von Berufs- und Alltagsleben führt. Obwohl auch bei anderen chronischen Krankheiten vorkommend, hat das Symptom Fatigue bei der Multiplen Sklerose (MS) einen besonderen Stellenwert, der sich aus der Häufigkeit des Auftretens und der Beeinträchtigung des Alltagslebens gerade bei jüngeren und aktiven Patienten ergibt. Die Ursachen sind komplex, jedoch scheinen Diskonnektionen zwischen diversen kortikalen und subkortikalen Hirnarealen, bedingt durch die Gesamtheit der MS-Läsionen, die Hauptrolle zu spielen. Die Behandlung ist vielschichtig, umfasst medikamentöse und nichtmedikamentöse Maßnahmen. Nach neueren Untersuchungen scheinen u. a. die kognitive Verhaltenstherapie Erfolg versprechend zu sein. Fatigue is a comprehensive lack of physical or mental energy that results in significant impairment of work and daily living in general. Although the symptom fatigue is also present in several chronic diseases, the impact in multiple sclerosis (MS) is of particular significance, because of the frequency of occurrence and the impairment of everyday life, especially in young and active patients. The causes are complex; however, dysconnectivity among the various cortical and subcortical brain regions, as a result of the collective MS lesions, seems to play a major role. Treatment is complex, comprising pharmacological and nonpharmacological interventions. According to recent studies, cognitive behavioral therapy, among other forms, seems promising.

Myelin oligodendrocyte glycoprotein antibody disease (MOGAD) is a relatively new entity of demyelinating diseases, clinically presenting with optic neuritis, transverse myelitis, or encephalic symptoms. Typical radiological features include demyelinating cerebral and spinal lesions, cortical involvement, leptomeningeal enhancement, or tumefactive lesions. Here we present a rare case of a young patient with extensive brain stem lesion on the MRI while exhibiting nystagmus, singultus and somnolence.

A multidisciplinary patient tailored approach is essential during the first hours in the acute phase and also the further management on a specialized unit as well as in early rehabilitation.

A 51-year-old man presented to our emergency department with persistent right cervical pain. Neurologic examination was unremarkable, and no anisocoria was reported. Using a linear ultrasound probe, the pupillary diameters were measured at rest, under direct/consensual light stimulus, and after elicitation of the ipsilateral ciliospinal reflex (CR), evoked by applying a painful stimulation (pinching) at the base of the neck on the trapezius muscle.1,2

At the 67th annual meeting of the American Academy of Neurology (AAN), there was a roundtable discussion with representatives from the International Working Group of Young Neurologist and Trainees–World Federation of Neurology (IWGYNT-WFN), Asia and Oceanian Association of Neurology, Association of South East Asian Nations Neurological Association, European Association of Young Neurologists and Trainees (EAYNT), and the AAN's Consortium of Neurology Residents and Fellows (AAN-CNRF). The purpose of the meeting was to provide updates on the developments affecting young neurologists throughout the world, as well as to discuss current initiatives focused on the improvement of neurology training on the international stage.
