1. 下腹部痛のみを訴えた鈍的外傷による精巣破裂の1例(Testicular rupture due to blunt trauma with complaints of lower abdominal pain alone: a case report)

精巣は白膜に保護されているが強い外力が加わると損傷を受け,陰嚢の痛みや腫脹を呈する。損傷した精巣を早期に同定し治療方針を決定することは,精巣機能温存のために重要とされる。われわれは精巣破裂でありながら,初発症状として陰嚢痛を訴えず下腹部痛のみを主訴とした症例を経験した。10歳代の高校生がバスケットボール中に相手選手と交錯し,下腹部痛を来して救急搬送された。腹腔内臓器損傷を疑いCT検査を行ったが,腹腔内臓器には明らかな損傷を認めなかった。経過観察後にも下腹部痛を訴えていたため再診察したところ陰嚢に圧痛を認めた。CTを見直すと撮影範囲に含まれていた右陰嚢内に血腫を認め,超音波検査では精巣輪郭の欠損を認めた。精巣破裂と診断し手術を施行した。下腹部痛を認めたため,患者も診察者も当初は下腹部を打撲したものと認識していたが,実際は陰部打撲による精巣破裂によって下腹部痛を来していたものと推測された。精巣破裂が下腹部痛を来す機序については,精巣捻転と同様に,陰嚢内を支配する精巣神経叢の構成によって説明されうる。咄嗟のことで患者自身が打撲部位を正確に認識できなかった鈍的外傷において,原因が判然としない下腹部痛を認めたときには,精巣損傷も念頭に診療にあたる必要がある。 The testes are protected by the tunica albuginea but can be damaged by a strong external force, resulting in scrotal pain and swelling. Identifying a damaged testis early to determine a treatment plan is important for preserving testicular function. We report a testicular rupture case with lower abdominal pain alone without scrotal pain. A teenage high school student was transported to our emergency department with lower abdominal pain after a collision with an opponent during basketball. An intraperitoneal injury was suspected, and a computed tomography (CT) scan was performed, although there were no obvious findings. After follow–up, the patient continued complaining of lower abdominal pain, and upon a second examination, scrotum tenderness was found. After examining the CT image again, a hematoma in the right scrotum was observed. Ultrasonography confirmed a defect in the testicular outline. The patient was diagnosed with testicular rupture and underwent surgery. Because of the lower abdominal pain, both the patient and the clinician initially thought that the lower abdomen was bruised; however, this pain was probably caused by testicular rupture due to perineum bruising. The mechanism by which testicular rupture causes lower abdominal pain may be explained by the configuration of the testicular plexus innervating the scrotum, as in testicular torsion. When a patient presents with lower abdominal pain with no apparent cause after blunt trauma in which the patient was unable to recognize the exact site of the bruise, a testicular injury should be considered.
