It is a heroic enterprise to develop a systematic reading of Hegel's Doctrine of Being, the first of three books in the Science of Logic. The basic idea of Houlgate's grand interpretation of Hegel's three parts of this Doctrine, which I call the Logic of Quality, Quantity and Measure, is that Hegel demands a ‘presuppositionless derivation of categories’ (II: ix). Hegel on Being(in short: HoB) presents a thoroughgoing and rigorous interpretation in a kind of dialogue(as I would characterize what HoBmarks as ‘Excursus’) with Kant (I: 19–46, 307–73; II: 171–79) and especially Frege's formal logic and his attempts of a predicative definition of numbers (II: 25–138). In any case, HoBmakes us aware of the really problematic presuppositions in post-Fregean Analytic Philosophy.

The Eurotransplant Senior Program (ESP) has expedited the chance for elderly patients with kidney failure to receive a timely transplant. This current study evaluated survival parameters of kidneys donated after brain death with or without matching for HLA-DR antigens. This cohort study evaluated the period within ESP with paired allocation of 675 kidneys from donors 65 years and older to transplant candidates 65 years and older, the first kidney to 341 patients within the Eurotransplant Senior DR-compatible Program and 334 contralateral kidneys without (ESP) HLA-DR antigen matching. We used Kaplan-Meier estimates and competing risk analysis to assess all cause mortality and kidney graft failure, respectively. The log-rank test and Cox proportional hazards regression were used for comparisons. Within ESP, matching for HLA-DR antigens was associated with a significantly lower five-year risk of mortality (hazard ratio 0.71; 95% confidence interval 0.53-0.95) and significantly lower cause-specific hazards for kidney graft failure and return to dialysis at one year (0.55; 0.35-0.87) and five years (0.73; 0.53-0.99) post-transplant. Allocation based on HLA-DR matching resulted in longer cold ischemia (mean difference 1.00 hours; 95% confidence interval: 0.32-1.68) and kidney offers with a significantly shorter median dialysis vintage of 2.4 versus 4.1 yrs. in ESP without matching. Thus, our allocation based on HLA-DR matching improved five-year patient and kidney allograft survival. Hence, our paired allocation study suggests a superior outcome of HLA-DR matching in the context of old-for-old kidney transplantation.

AbstractIn order to understand Hegel's gnomic oracle according to which the ‘I’ is a ‘We’, the notion of a personalsubject is explained by its competence to perform personal roles in a pre-given partition of roles. Explicit divisions of labour by contractual promises are special cases that presuppose the general case of an already established social practice. On the other hand, methodological individualism is right to stress that we actualize joint intentions only via corresponding instantiations. In performing our parts, we form a plural subject, a we-group. The result of what each of us does is what we do, and the generic ‘We’ turns into the generic ‘I’.

HERAKLESwas a 1‐year randomized, multicenter trial. Patients were randomized at 3 months after kidney transplantation to remain on cyclosporine‐based therapy, switch to everolimus without a calcineurin inhibitor (CNI), or switch to everolimus with low‐exposure cyclosporine. Overall, 417 of 497 (83.9%) patients from the core study entered a 4‐year extension study. The randomized regimen was continued to year 5 in 75.9%, 41.9% and 24.6% of patients in the standard‐CNI,CNI‐free and low‐CNIgroups, respectively. Adjusted estimated GFRat year 5 was significantly higher in the CNI‐free group versus standard CNI(difference 7.2 mL/min/1.73 m2, P< .001) or low CNI(difference 7.6 mL/min/1.73 m2, P< .001). For patients who continued randomized therapy for 5 years, differences were 14.4 mL/min/1.73 m2and 10.1 mL/min/1.73 m2, respectively. Biopsy‐proven acute rejection occurred during the 4‐year extension study in 7.6%, 8.6%, and 9.0% of patients in the standard‐CNI,CNI‐free and low‐CNIgroups, respectively (P= .927). In conclusion, conversion to a CNI‐free everolimus regimen 3 months after kidney transplantation improved long‐term graft function, particularly in patients who continued the CNI‐free regimen. Low CNIwith everolimus did not improve renal function. Efficacy was comparable between groups but frequent immunosuppression changes should be taken into account. Five‐year follow‐up data from patients randomized at three months after kidney transplantation to receive standard calcineurin inhibitor (CNI) therapy, everolimus‐based CNI‐free therapy, or everolimus with low‐exposure CNI show that early CNI withdrawal maintains better long‐term renal function without an increase in biopsy‐proven acute rejection, with a similar rate of infections and a low rate of discontinuations due to adverse events in all three treatment groups.

In a 24‐month, multicenter, open‐label, randomized trial, 715 de novokidney transplant recipients were randomized at 10–14 weeks to convert to everolimus (n = 359) or remain on standard calcineurin inhibitor (CNI) therapy (n = 356; 231 tacrolimus; 125 cyclosporine), all with mycophenolic acid and steroids. The primary endpoint, change in estimated glomerular filtration rate (eGFR) from randomization to month 12, was similar for everolimus versus CNI: mean (standard error) 0.3(1.5) mL/min/1.732versus −1.5(1.5) mL/min/1.732(p = 0.116). Biopsy‐proven acute rejection (BPAR) at month 12 was more frequent under everolimus versus CNIoverall (9.7% vs. 4.8%, p = 0.014) and versus tacrolimus‐treated patients (2.6%, p < 0.001) but similar to cyclosporine‐treated patients (8.8%, p = 0.755). Reporting on de novodonor‐specific antibodies (DSA) was limited but suggested more frequent anti‐HLAClass I DSAunder everolimus. Change in left ventricular mass index was similar. Discontinuation due to adverse events was more frequent with everolimus (23.6%) versus CNI(8.4%). In conclusion, conversion to everolimus at 10–14 weeks posttransplant was associated with renal function similar to that with standard therapy overall. Rates of BPARwere low in all groups, but lower with tacrolimus than everolimus. In a multicenter, open‐label, randomized trial of de novokidney transplant recipients, conversion to everolimus at 10–14 weeks posttransplant is associated with similar renal function as in patients who continue to standard tacrolimus or cyclosporine therapy, with low rates of biopsy‐proven acute rejection in all groups, but lower rates with tacrolimus than everolimus.

Renal transplant glomerulitis (G) is associated with acute antibody-mediated rejection (ABMR) in the presence of donor-specific antibodies. However, the long-term prognosis of isolated G (isG) in the absence of donor-specific antibodies or G in combination with T cell–mediated rejection (TCMR) remains unexplored.

AbstractThe problem of evaluating philosophical texts is not only an institutional problem for philosophers. It transcends our own discipline. In a globalized word, English seems to be the lingua franca of all the sciences, much more than Latin ever was. But if we do not appreciate the systematic differences between articulating the ‚results‘ in the ‚natural‘ sciences (which focus on facts and technical know-how) on the one hand and normative and philosophical disputes about reasonable developments of forms of articulation and understanding in the ‚Geisteswissenschaften‘ on the other, there is a great danger of transnational regionalism — especially if we limit philosophical thinking and research to one language. Instead, we should treasure the diverse traditions and experiences as they are manifested in different languages. In consequence, the policy of evaluating international visibility of philosophic journals, as proposed by the European Science Foundation (ESF), must be changed.

Zusammenfassung: □ Hintergrund: Die vorliegende Studie hatte das Ziel, die Wirksamkeit und Verträglichkeit einer siebentägigen Behandlung mit Pantoprazol, Amoxicillin und Metronidazol bei der Behandlung der Helicobacter-(H.-)pylori-Infektion zu prüfen. □ Patienten und Methoden: 50 Patienten (26 Männer, 24 Frauen, Alter 18 bis 86, Median 54 Jahre) mit floridem Zwölffingerdarm- (n=25) oder Magengeschwür (n=25) wurden in die Studie aufgenommen, bei 48 wurde endoskopisch (Ureasetest, Histologie) und im13C-Harnstoff-Atemtest eine H.-pylori-Infektion gesichert. Die Patienten wurden über insgesamt sieben Tage mit Pantoprazol (2mal 40 mg/d), Amoxicillin (2mal 1 g/d) und Metronidazol (2mal 500 mg/d) behandelt. Anschließend erfolgte die Weiterbehandlung mit Pantoprazol (1mal 40 mg/d) über 21 Tage. Vier Wochen nach Ende der Medikamenteneinnhme wurden die Patienten erneut endoskopisch untersucht und der H.-pylori-Status wieder mittels Ureasetest, Histologie und13C-Harnstoff-Atemtest beurteilt. □ Ergebnisse: Bei der Intention-to-treat-Analyse war die Eradikationstherapie in 39 von 48 Fällen erfoldgreich, entsprechend 81% (95%-CI=67 bis 91%). Bei Auswertung „per protocol” war die Eradikationsrate 85% (35 von 41, 95%-CI=71 bis 94%). Bei 45 von insgesamt 48 Patienten (94%; 95%-CI=83 bis 99%) waren die Geschwüre nach acht Wochen endoskopisch abgeheilt. Sechs von 50 Patienten (12%) berichteten über leichte bis mäßige Symptome, die wahrscheinlich Nebenwirkungen der Therapie zuzuordnen waren. Die Heilung der H.-pylori-Infektion führte auch zu einer histologisch nachweisbaren deutlichen Reduktion von Grad und Aktivität der Gastritis. □ Schlußfolgerung: Die Kurzzeit-Tripeltherapie mit Pantoprazol, Amoxicillin und Metronidazol ist eine wirksame und kostengünstige Alternative zu Standardtherapieschemata mit Clarithromycin.

Summary According to Kant, arithmetic judgements are not analytic since they are about our practice of operating with figures and things in a certain way. Hence the empiricist thesis that any meaningful assertion is either analytic or synthetic a posteriori seems to be refuted (§§ 1, 2). Using syntax and semantics of truth-conditional logic Frege nevertheless shows that arithmetic can be understood as a system of quasi-analytic sentences speaking about numbers as abstract entities (§§ 3, 4). Axiomatic set theory, however, conceals the connection between (internal) truth-functional arithmetic and our (external) practice of counting and computing (§§ 5–7). — In spite of the insights truth-conditional semantics provides for a non-psychological understanding of mathematical thinking, it is neither a general theory of meaning and analyticity nor a foundation of a general sense-criterion (§§ 8, 9).

Wenn wir nach H. v. Höker oder A. Steuer unter dem Namen, Bodenwasser" (oder von Keilhack, Kayser u. a. als, Grundwasser" bezeichnet) alles Wasser verstehen, das im Boden, also unter der Erdoberfläche, enthalten ist, so muß man bei demselben zwei Hauptabteilungen - zwei übereinanderliegende, jedoch an Mächtigkeit zumeist sehr verschiedene Wasserschichten - unterscheiden: Das der Oberfläche nahe Fließwasser und das darunter liegende mächtigere Standwasser (genauer Tiefenstandwasser). Beide sind im allgemeinen vadosen Ursprungs. Wo in der Tiefe aus dem Erdinnern juvenile Wasser emporsteigen, werden sie sich allerdings zumeist dem zunächst liegenden Tiefenstandwasser beimengen, wenn nicht bestimmte Voraussetzungen ihren Aufstieg bis zur Erdoberfläche ermöglichen. Das Fließwasser reicht nach abwärts soweit, als eine natürliche Abflußmöglichkeit für dasselbe besteht, also äußerstens bis zum Meeresspiegel. Seine Charakteristik liegt in seiner durch die Schwerkraft bedingten Fließbewegung, eben bis zum erreichbaren nächsten oder tiefsten Abflußpunkt an der Erdoberfläche. Es ist das Wasser, das den Menschen als Grundwasser im engen Sinne, oder unter einer sonstigen Bezeichnung, zunächst weitaus am meisten interessiert, da es zusammen mit dem Oberflächenwasser zu den Lebensnotwendigkeiten desselben gehört. Was darunter liegt, ist das Tiefenstandwasser. Seine Charakteristik ist im Gegensatz zu ersterem normal der Stillstand, da ihm an sich eben jede Abflußmöglichkeit fehlt, es sei denn, daß an einer Stelle irgendeine zusätzliche Kraft der Gravitation entgegenwirkt und sie überwindet. Es ist zum größten Teil seiner ursprünglichen Herkunft nach sicherlich ebenfalls vadoses Wasser; ein Zufluß von oben erfolgt aber so ohne weiteres wohl nicht mehr, da angenommen werden muß, daß der Erdboden, soweit er für Wasser überhaupt zugänglich ist und solches aufnehmen kann, seit den ungeheuren Zeiträumen seines Bestehens, und vielfach seines Entstehens (unter Wasser), restlos vom Wasser bereits durchtränkt und erfüllt worden ist. Das von oben kommende atmosphärische Wasser kann daher nicht mehr eindringen und geht infolgedessen schon mit dem Fließwasser ab. Ob und wieviel juveniles Wasser es andererseits dabei aus der Erdtiefe aufgenommen hat und noch aufnimmt, ist unbekannt. ...

Quellen der genannten Art kommen in Südbayern in drei Zonen vor, die parallel mit den Alpen sich von West nach Ost erstrecken: Einmal der äußerste Nordrand der Alpen selbst, aus oberostalpinem Flysch und helvetischen Deckenresten bestehend; dann der diesen vorgelagerte schmale, beiderseits von tiefgreifenden Störungsflächen begrenzte Streifen von oligozäner Molasse; und endlich das gegen Norden vorliegende, bis zum Abbruchrand des Frankenjura reichende, flach gelagerte miozäne Vorland des oberen Donaubeckens. Alle drei sind von ganz wesentlich verschiedenem Bau, so daß die erwähnten Quellen unter den abweichendsten Verhältnissen des Untergrundes zutage treten: Von dem Flysch und Helvetikum des Alpenrandes wird nach der heutigen Deckenlehre angenommen, daß sie sich in zwei getrennten Urgebirgswannen abgelagert haben, und dann gegen Norden zu in weitgehendster Weise aufeinander und durcheinander geschoben wurden. Daß das gleiche krystalline Urgestein auch unmittelbar unter dem oligozänen Streifen vermutet werden kann, wurde schon bei mehrfachen Gelegenheiten ausführlich dargetan; ebenso, daß das Mitteloligozän wahrscheinlich direkt auf diesem von der böhmischen Masse her als Untergrund herüberstreichendem Urgestein abgelagert und später, am Ausgang des Miozäns, durch die schiebende Wirkung der genannten Decken stark gefaltet und zusammengeschoben wurde. Es folgt daraus zwangsläufig, daß diese Deckenteile sich nunmehr auf dem ehemaligen Ablagerungsraum und daher der krystallinen Unterlage der oligozänen Molasse befinden müssen. Von dem oberdanubischen Becken kennt man von älteren Schichten fast nur das Miozän als Oberflächensediment. Was hier unter dem Miozän liegt, davon ist kaum etwas bekannt, ebenso wie die Grundlage selbst. Südlich des großen Donau-Abbruches liegt in der Tiefe wohl die Fortsetzung des Krystallins der böhmischen Masse, zunächst jedoch noch überlagert von den gleichen Gesteinen des schwäbisch-fränkischen Mesozoikums nördlich davon, darüber dann das Miozän. So deuten dies wenigstens direkt die zwei Bohrungen im Norden der Donau bei Straubing, in der unmittelbaren Nachbarschaft des Abbruchrandes, an. Wie weit diese niedergebrochenen Schichten des Frankenjura unter dem Tertiär noch nach Süden gehen, ist unbekannt. Jedenfalls wird irgendwo im Untergrunde gegen Süden zu die krystalline Unterlage sich heben, und die genannten Sedimente der germanischen Fazies daran als Südufer abstoßen. ...
