- Abstract
The pathophysiology of multisystem inflammatory syndrome (MIS) in children (MIS-C) is unknown. It occurs several weeks after COVID-19 infection or exposure; however, MIS is rarely reported after COVID-19 vaccination, and cases are mostly in adults. Herein, we present a 12-year-old male who had no prior COVID-19 infection or exposure and developed MIS-C after his first dose of COVID-19 mRNA vaccine.

- Abstract
80% of individuals with cancer will require a surgical procedure, yet little comparative data exist on early outcomes in low-income and middle-income countries (LMICs). We compared postoperative outcomes in breast, colorectal, and gastric cancer surgery in hospitals worldwide, focusing on the effect of disease stage and complications on postoperative mortality.

- Abstract
The aim of the study was to predict loneliness with regard to parental acceptance-rejection and sibling relationship quality after controlling for gender, age, grade, school type, field of study, birth order, and number of siblings. The UCLA Loneliness Scale, Maternal and Paternal Acceptance-Rejection Questionnaire, and Sibling Relationship Questionnaire were administered to 1,451 high school students, and multiple hierarchical regression analysis was conducted. Results showed that 1.7% of variance was explained by model 1, including demographic variables, but model 1 was not significant. After adding mother and father acceptance to model 2, the explained variance increased to 18.1%. In model 3, sibling variables were added and the explained variance was 19.6%, which were both significant results. Mother acceptance was the best predictor of loneliness, followed by father acceptance. Parental variables explained loneliness better than sibling and demographic variables. Understanding the family predictors of loneliness may be seen as the first step for a subsequent intervention.

- Abstract
This study is devoted to the empirical analysis by second generation panel data analysis of the effects of the R&D investment variables in different qualifications in OECD countries grouped as OECD-20 and OECD-9 based on the income levels of the economic growth for the period of 1996-2015. Within this context, the purpose of this study is to evaluate whether or not the economic growth performances of OECD-20 and OECD-9 countries have a sustainable structure that endogenizes the technological advancements and occurs by the increments in average factor productivity. At the end of the paper it is determined that all the R&D variables in different qualifications of the OECD-20 group have a higher income level in sample period and have positive and statistically significant effects on the economic growth. On the other hand, only the private sector, universities and the total R&D investments have positive and statistically significant effects on the economic growth of the OECD-9 group which has comparatively lower income level. However, it is specified that the size of the positive and statistically significant effects of the R&D investment variables in different qualifications is more than two times bigger in the OECD-20 group as opposed to the OECD-9 group. These results reveal that the economic performances of OECD-20 countries in the investigated period have a more substantial relation with the qualified and sustainable structure that endogenizes the technologic advancements and occurs by the increments in average factor productivity. All of this shows that the R&D investments also are substantially sufficient to change the long-term economic growth performances and income levels of the countries in OECD-20 and OECD-9 groups.

- Abstract
In this study, traffic noise in Seferihisar, which is a holiday resort by the Aegean Sea in the West of Turkey, was investigated. The noise occurring in summer and winter, on weekdays, and at weekends was mapped separately. Besides, land uses exposed to traffic noise were specified. In the carried out method, the land uses were primarily mapped by using the satellite images of the study area. Then, the noise was measured at 46 points during ten days in summer and winter, on weekdays and at weekends, and it was mapped by using Inverse Distance Weighting (IDW) method and by means of Geographic Information System (GIS) techniques. The measurements were taken in the daytime (07:00-19:00), in the evening (19:00-23:00), and at night (23:00-07:00) as stated in the Turkish Regulation of Assessment and Management of Environmental Noise. Since the time interval of the daytime measurements was large, the noise was measured in three different periods for the daytime measurements. In the formation of noise maps, logarithmic averages of all measurements taken at each point were used. The noise maps were overlapped with the land use maps by the help of the GIS techniques; the land use affected by the noise was analyzed. The results showed that the road traffic noise varied from summer to winter seasons in the study area depending on heavy or light traffic. The noise occurring in summer showed an increase both on weekdays and at weekends. While the limit value was exceeded in 49 of the measurement averages taken in winter, the limit value was exceeded in 96 measurements taken in summer. The noise maps formed according to the IDW method displayed that the areas in which the limit value was exceeded in the daytime in summer took up 14.27% of the study area while this rate went down to 4.06% in winter. The study area is one of the most important destinations for summer holiday. 20.12% of the area that exceeded the limit value in summer was builtup and 10.69% of them was tourism area. The noise together with population and traffic that increase in summer season is an important environmental problem against which some precautions should be taken. The findings of the study present significant results which might guide local governments in preventing and managing the noise.

- Abstract
This study aimed to investigate the effects of pharmacomechanical thrombectomy on novel complete blood count parameters in deep venous thrombosis. This retrospective study included 242 patients aged >18 years who were treated for deep venous thrombosis. Patients were grouped as follows: group 1 was accepted as having interventional operations (n = 123) and group 2 was accepted as having only medical advice (n = 119). Routine complete blood count parameters, the neutrophil-to-lymphocyte ratio (NLR), and the platelet-to-lymphocyte ratio (PLR) were compared. There was no difference between the groups in terms of admission hemoglobin, hematocrit, mean platelet volume, NLR and PLR (P= .11, P= .24, P= .55, P= .93, and P= .96, respectively). In the pharmacomechanic thrombectomy group, NLR and PLR were significantly reduced after intervention when compared to the admission values (P< .001 and P< .001, respectively). However, the NLR and PLR values of medically treated patients did not differ significantly from their baseline values (P= .16 and P= .08, respectively). In this study, we effectively removed the thrombus load in blocked proximal veins using pharmacomechanical thrombectomy and observed a significant decrease in NLR and PLR, which are current, inexpensive, and accessible parameters.

- Abstract
A 1-day-old female newborn presenting with a severe left proptosis was found, on imaging, to have a cranial mass extending in both orbits and ethmoid sinuses. Tumor debulking and biopsy were performed through a lateral orbitotomy. Based on histologic findings, a diagnosis of infantile myofibroma was made. No involvement was found in other areas of the body. The patient died because of respiratory arrest after intracranial surgery that was performed 45 days after the orbital surgery. To the authors' knowledge, only one similar case of cranio-orbital myofibroma has been reported previously. This tumor should be considered in the differential diagnosis of congenital proptosis and cranio-orbital tumor. In such cases, tumor debulking can be performed through orbitotomy.

- Abstract
Being the cause of the loss of life and damaging property, landslide is an important natural hazard. Therefore, landslides have to be monitored and preventive measures taken accordingly. In Geodesy, landslides can be determined with static, kinematic and dynamic geodetic models. The aim of this study is to develop a dynamic geodetic model for landslides and compare it with static and kinematic geodetic models. A study area was selected in the Northeastern Black Sea Region of Turkey where landslides are the most effective natural hazard. Movements were determined with static, kinematic and dynamic geodetic models using geodetic, geologic and geophysical measurements made in the study area. Groundwater levels changes were regarded as causative forces in the formulation of the dynamic model. The dynamic model delivered more detailed information (direction, values, velocity, acceleration of movements) about landslide movements. It is possible to formulate more realistic strategies about prevention of landslides by using this information. As a result, it can be suggested that dynamic geodetic models are more useful in landslide studies.

- Abstract
Objective. Systemic lupus erythematosus (SLE) is a chronic systemic disease, which can involve multiple organs such as kidney, skin and brain. Lung is another organ that can be affected. A number of pulmonary complications including pleuritis, pneumonitis, infectious pneumonia, pulmonary haemorrhage, pulmonary hypertension and pneumothorax have been reported in patients with SLE. Pulmonary involvement is relatively frequent in adult patients; it has infrequently been reported in children with SLE. However, pulmonary manifestations may be an initial and/or life-threatening complication of SLE in children. In this paper we aim to emphasize the pulmonary involvement in childhood-onset SLE via description of our patients.Methods. The patients, who were diagnosed with SLE at the Children’s Hospital of Ankara University Medical School between 1993 and 2002, were retrospectively evaluated for evidence of pulmonary involvement. All patients fulfilled at least four of the classification criteria of the American Rheumatism Association. Using a standardized form, we obtained data regarding the age, sex and presenting complaints of the patients, previous therapies given, clinical and laboratory features, treatment and outcome. Informed consent was obtained from all patients.Results. During the 10-yr study period, 16 patients were diagnosed with childhood-onset SLE. Five of them (31%) had pulmonary involvement including acute lupus pneumonitis, invasive pulmonary aspergillosis, cytomegalovirus pneumonia and pulmonary haemorrhage (in two patients). These 5 patients with lupus lung disease are presented in more detail.

- Abstract
Objective—The mechanisms of the different haemodynamic and clinical responses to dobutamine infusion in mitral stenosis (MS) are not clearly established. The aim of this study was to evaluate the relation between left atrial (LA) function and haemodynamic response in patiens with MS during dobutamine infusion. Methods and results— Forty-two consecutive moderately symptomatic patients (33 women, 9 men; mean age 46±9, range from 26 to 66), NewYork Heart Association (NYHA) class II with MS (mean mitral valve area 1.7±0.1cm2) were evaluated with dobutamine stress echocardiography. Haemodynamic measurements were obtained at rest and during peak dobutamine infusion. LA fractional shortening at rest was used as an index of global LA function. Group I consisting of patients with significantly elevated pulmonary artery pressure (>60 mm Hg) and mean transmitral gradient (>15 mm Hg) at peak dobutamine infusion were defined as haemodynamically serious MS. Group II consisted of the remaining 30 patients whose haemodynamic data were below these levels. While baseline haemodynamic parameters and mitral valve characteristics were not different between the two groups, LA fractional shortening was significantly lower (18.9±2.8 vs. 32.3±5.1%, p<0.0001) and left atrial dimension was significantly larger in group I (49.7±2.3mm vs 43.6±5.3 mm, p<0.0001). Left atrial fractional shortening was negatively correlated with the increase in mean transmitral gradient (r:-0.58, p<0.01). When the patients were divided using a LA fractional shortening level of 25% as the cut-off point, we observed that the patients with low LA fractional shortening had a greater increase in mean transmitral gradient (7.3±3.1 mm Hg vs. 4.6±1.4 mm Hg), p = 0.005) and pulmonary artery pressure (22.4±3.5 mm Hg vs. 16.1±8.5 mm Hg, p = 0.001) compared to the patients with high LA fractional shortening. Based on these haemodynamic results, management was changed in 12 patients (28%): 5 underwent percutaneous mitral balloon commissurotomy and 7 received intensive medical treatment. Conclusions—The present study demonstrates that haemodynamic response during dobutamine stress echocardiography correlates with LA fractional shortening in patients with MS. The evaluation of left atrial function at rest in patients with ambiguous symptoms and mild mitral stenosis may be useful in clinical decision making. Atrial dysfunction at rest may predict the haemodynamic response during stress echo in these patients.

- Abstract
In this study, we investigated the effects of erythropoietin (Epo), and pentoxifylline (Ptx) on the oxidant and antioxidant systems in the experimental short bowel syndrome. Spraque-Dawley rats were divided into four groups and all animals underwent 75% small bowel resection. Group E was treated with 500 IU kg− 1 Epo subcutaneously (s.c.), group P with 50 mg kg− 1 day− 1 s.c. Ptx and group E+P with 500 IU kg− 1 s.c. Epo plus 50 mg kg− 1 day− 1 s.c. Ptx for a period of 28 days. In group C, which is the control group, no drug treatment was given. At the end of 28 days the experimented rats were killed and ileum samples excised for biochemical and histopathological testing. Malondialdehyde (MDA), superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) levels were determined in ileum homogenates. When compared to group C, the MDA and GSH-Px levels were significantly decreased (p < 0.05), but SOD activitiy was not changed (p > 0.05) in groups P and E+P, whereas both MDA and SOD and also GSH-Px activities were not changed significantly in group E (p > 0.05). The average villous length, crypt depth, muscular thickness and mucosal length were measured in all groups. The average crypt depth and mucosal length were statistically higher in the group P than group C (p < 0.001, p < 0.01, respectively). In addition, the crypt depth was statistically higher in both E and E+P groups as compared to group C (p < 0.001, p < 0.01, respectively). Therefore, our study indicates that Ptx may be more effective than Epo in reducing lipid peroxidation. Moreover, we considered that Ptx may give this protective effect by inhibiting the free oxygen radicals to a greater extent than developing the antioxidant capacity. - Abstract
Properties and regulation of the human organic cation (OC) transporter type 2 (hOCT2) expressed in HEK-293 cells were extensively characterized using the fluorescent OC 4-[4-(dimethylamino)styryl]-N-methylpyridinium (ASP+). ASP+uptake was electrogenic and inhibited by TPA+(EC50= 2.7 μM), tetraethylammonium (EC50= 35 μM), cimetidine (EC50= 36 μM), or quinine (EC50= 6.7 μM). Stimulation with carbachol or ATP decreased initial uptake by 44 ± 3 (n= 14) and 34 ± 4% (n= 21), respectively, independently of PKC but dependent on phosphatidylinositol 3-kinase (PI3K). PKA stimulation decreased uptake by 18 ± 4% (n= 40). Inhibition of calmodulin (CaM), Ca2+/CaM-dependent kinase II, or myosin light chain kinase decreased uptake by 63 ± 2 (n= 15), 40 ± 4 (n= 30), and 31 ± 4% (n= 16), respectively. Inhibition of CaM resulted in a significant change in the EC50value for the inhibition of ASP+uptake by tetraethylammonium. In conclusion, we demonstrate that the hOCT2 is inhibited by PI3K and PKA and activated by a CaM-dependent signaling pathway, probably via a change in substrate affinity.

- Abstract
We present a polymer-based mechanical sensor with an integrated strain sensor element. Conventionally, silicon has been used as a piezoresistive material due to its high gauge factor and thereby high sensitivity to strain changes in the sensor. By using the fact that the polymer SU-8 [1] is much softer than silicon and that a gold resistor is easily incorporated in SU-8, we have proven that a SU-8-based cantilever sensor is almost as sensitive to stress changes as the silicon piezoresistive cantilever. First, the surface stress sensing principle is discussed, from which it can be shown that the SU-8-based sensor is nearly as sensitive as the silicon based mechanical sensor. We hereafter demonstrate the chip fabrication technology of such a sensor, which includes multiple SU-8 and gold layer deposition. The SU-8-based mechanical sensor is finally characterized with respect to sensitivity, noise and device failure. The characterization shows that there is a good agreement between the expected and the obtained performance.

- Abstract
Cystic leukoencephalopathy with megalencephaly is a newly described entity with mild clinical involvement. Patients suffer from developmental problems and seizures in childhood. Progression is gradual into adulthood. Typical magnetic resonance imaging findings include subcortical cysts and diffuse leukoencephalopathy. The etiology is unknown with possibly autosomal-recessive inheritance. We present two pairs of siblings with this disease and emphasize the characteristic and variable patterns even within the same family. (J Child Neurol2000;15:690-693).

- Abstract
We compared the frequencies of seven MEFV mutations (M694V, M680I, V726A, M694I, K695R, R761H, E148Q) and the clinical findings in 20 Turkish FMF patients who had not developed amyloidosis by the age of 40 years in the absence of colchicine therapy, with those in 27 Turkish amyloidosis patients. No mutation frequency, including that of M694V, was different between the two groups. Family history of amyloidosis and parental consanguinity were noted to be higher in the amyloidosis group. The seven mutations do not appear to be sufficient to explain the development of amyloidosis in Turkish FMF patients. Other genetic factors may be important for this association.

- Abstract
As some apparently idiopatic epilepsies may occasionally pose diagnostic difficulties in regard to their precise status of etiology, evoked potentials, particularly visual evoked potential (VEP), may contribute to the diagnosis of childhood epilepsy with occipital paroxysms (CEOP) as a subsidiary method of evaluation. This study includes 19 children (10 boys 52.6%; 9 girls 47.4%) ranging in age from 5 to 17 years (mean SD = 9.68 3.28)suffering from CEOP and a control group of 30 normal children, matched for chronological age and sex. Peak amplitudes and latencies of the P100 component for pattern-shift VEP (PVEP) and of major positivity for flash VEP (FVEP) are measured, respectively. The results from this study demonstrate that amplitude and latency values in patients with CEOP differs insignificantly when compared with controls. Although, non-significantly, mean values of amplitudes for both PVEP and FVEP were higher in the patients than in the normal children, whereas latencies in FVEP were somewhat longer. There may be some tendency for the amplitudes to increase and the latencies to be delayed in VEPs in patients with CEOP, when an overall interpretation of our and similar studies are considered. In certain cases of diagnostic difficulty, VEP values may provide further information for the clinician, regarding either a symptomatic or an idiopathic nature of the underlying disorder.

- Abstract
The purpose of this study was to investigate the informative value of single photon emission tomography (SPECT) in relation to the pathophysiological functioning of the brain during absence seizures and the origin of ictal discharges in idiopathic generalized epilepsies (IGEs). Six patients with childhood absence epilepsy (CAE) were selected for the study and two consecutive SPECT sessions were performed concomitant with EEG recordings revealing normal results and during hyperventilation (HV) studies where the ictal discharges were induced either alone or accompanied by clinical absence seizures. All six patients had ictal discharges in their EEGs during HV and two of them also had clinical absences. SPECT findings during HV revealed an overall increase in the cerebral blood flow (CBF) with significantly higher values as compared to the baseline data. There was no indication for any focal origin in either the interictal or the ictal SPECT findings. Results of the study were supportive for the concept of subcortical origin for the absence seizures and they were also promising for the diagnostic value of ictal SPECT in epileptic cases with undetermined origin as to whether they were localization-related or generalized.

- Abstract
Background: In patients with familial Mediterranean fever (FMF) renal involvement is usually in the form of AA amyloidosis. There is increasing evidence that renal involvement may be due to diseases other than amyloid as well. Methods: Amongst 302 children with FMF we observed and followed 28 with typical clinical and laboratory features of vasculitis. The diagnosis of FMF was established according to the Tel Hashomer criteria. Results: Polyarteritis nodosa, protracted febrile attacks and Henoch-Schonlein purpura were diagnosed in 4, 13, and 11 patients, respectively. The presentation was often difficult to distinguish from FMF attacks, but protracted febrile attacks lasting several weeks, hypertension, thrombocytosis, and dramatic responses to corticosteroid therapy that were observed in many cases were different from what is observed in classical FMF.Conclusions: We suggest that FMF, perhaps as a consequence of impaired control of inflammatory responses, predisposes to vasculitis with renal involvement.

- Abstract
The effect of a single treatment with cigarette smoke on the blood levels and hemodynamic effects of propranolol in rats was studied. Pentobarbital sleep time was not affected whereas zoxazolamine paralysis time was shortened 72% in rats, 24 h after the cigarette smoke exposure. The gb-adrenoceptor blocking effect of propranolol observed at 10 and 20 min time intervals was abolished in rats exposed to cigarette smoke 24 h after the exposure. The blood propranolol concentrations were decreased in rats pretreated with phénobarbital, 3,4-benzpyrene and ethanol as well as in cigarette smoke exposed rats. Among several factors that could influence propranolol metabolism, in this study, enzyme induction is suggested to be dominant.

- Abstract
We have obtained ESR ages on 18 teeth from archaeological levels AH16-AH27 exposed at the east profile in compartment E at Karaïn Cave. This sequence exhibits a transition in lithic assemblage from a Classic Charentian to a Karaïn (Zagros-type) Mousterian between levels AH27 and AH25, and two calcite-cemented soils at levels AH22 and AH16 which have been interpreted as climatic warm phases. Since we found no systematic increase in tooth ages with depth, we assigned this sequence a mean early uptake (EU) age of 108 ± 23 ka and mean linear uptake (LU) age of 121 ± 27 ka, on the basis of 37 subsamples from 16 teeth. In this site, the precision of individual tooth ages suffered little from the problems associated with uranium uptake. However, this advantage was offset due to uncertainties arising from the rather inhomogeneous nature of the enclosing strata which contained various sizes of limestone blocks with intervening sediment. This led to difficulties in ascribing a precise annual gamma radiation dose to individual teeth in such a "lumpy" gamma radiation field. Considerable scattering of the individual tooth ages occurred mainly because the annual gamma dose comprised greater than 50% of the total annual dose to the individual teeth. On the basis of the ESR dating results, we infer that deposition of the two different lithic assemblages and both the penultimate and the final climatic warm phases at Karaïn probably occurred during oxygen isotope stage 5. We also conclude that these lithic assemblages are significantly different than contemporaneous Levantine Mousterian industries which occur about 700 km to the southeast. - Abstract
OBJECTIVE: . To investigate the prevalence of MEFV gene mutations in Turkish patients with Henoch-Schönlein purpura (HSP) but with no symptoms of familial Mediterranean fever (FMF). In addition, we assessed the clinical and laboratory characteristics of HSP patients with and without MEFV mutations. METHODS:. Eighty pediatric patients with HSP (44 boys and 36 girls) were enrolled. Blood for mutation analysis was obtained either at the time of the diagnosis of HSP or during followup visits in previously diagnosed patients. No patient had the diagnosis of FMF in their history and in the followup period. Exon 10 of the MEFV gene was screened, together with p.E148Q mutation analysis. RESULTS: Twenty-seven (34%) patients were found to be heterozygous for one of the screened MEFV mutations; p.M694V in 16, p.M680I in 5, p.V726A in 3, and p.E148Q in 3 patients. Patients with MEFV mutations were younger than those without mutations and they had edema and arthritis more frequently. Also, the frequencies of elevated erythrocyte sedimentation rate and C-reactive protein values were found to be significantly higher in patients who had MEFV mutations. CONCLUSION: Alterations in the MEFV gene are important susceptibility factors for the development of HSP and also affect the clinical presentation of it.

- Abstract
Background and Purpose: To evaluate the success and complications of percutaneous suprapubic cystolithotripsy (PCCL) in pediatric and adult patients with neurogenic bladder.Patients and Methods: Between 2000 and 2004, 72 patients, all male (30 children aged 2 to 7 years [mean 4.7 years] and 42 spastic paraplegic adults aged 34 to 62 years [mean 53 years]), with sterile urine underwent PCCL under general anesthesia in one sitting. An 18-gauge needle, Amplatz dilatation set, 30F Amplatz sheath, rigid nephroscope, lithotripter (pneumatic, mechanic), and stone forceps were used. Fluoroscopy was not. A suprapubic catheter was placed in the first two patients only. Results: The dimensions of the stones were on average 3.2 cm (range 1–5 cm) for the pediatric patients and 5.5 cm (4–10 cm) for the adult patients. The operating time was 20 minutes (10–35 minutes). In all cases, the stones were taken out. No serious intraoperative or postoperative complications were observed. In all cases, the transurethral catheter was removed on postoperative day 5. No recurrence was observed during the follow- up period (mean 20 months). Conclusion: As urethral diameters are narrow in pediatric patients and adult spastic paraplegic patients in whom an endoscopic approach could not be performed, PCCL is a safe alternative with low morbidity and complication rate. The technique is also more advantageous than open surgery with regard to cosmetic outcome and length of the hospital stay.

- Abstract
Mayer-Rokitansky-Küster-Hauser anomaly originates from agenesis of the Müllerian duct including agenesis of the uterus and the vagina because of abnormal development of the uterine ducts. This syndrome may be accompanied by the upper urinary tract anomalies such as unilateral renal agenesis, ectopia of 1 or both kidneys, renal hypoplasia, horseshoe kidney, and hydronephrosis. We report a 16-year-old girl, with unilateral renal agenesis, herniating ovary, and renal cell carcinoma in her solitary kidney, associated with Mayer-Rokitansky-Küster-Hauser syndrome—the first case in the literature to our knowledge.

- Abstract
A 28-year-old man presented with proptosis and medial displacement of the right eye, intermittent pain, and diplopia of 1 month's duration. Radiologic examination demonstrated a mass in the right lateral orbit resembling an aneurysmal bone cyst (ABC). Histopathologic evaluation of the surgically excised lesion via orbitotomy confirmed the radiologic diagnosis. Two months later, the patient presented again with a mass at the same location as the previous one, with a major component that was solid. The recurrent lesion was excised via frontal craniotomy. Histopathologic examination of the surgical specimen led to a diagnosis of ABC secondary to trabecular ossifying fibroma. During the follow up of 13 months, the lesion did not recur. To the best of the authors' knowledge, this is the first case of ABC secondary to ossifying fibroma in the orbit. ABC can dominate the radiologic appearance of the lesion and veil the primary neoplasm.
