A randomized trial and systematic review led by researchers at McMaster University in Canada found that proton pump inhibitor (PPI) prophylaxis can prevent upper gastrointestinal (GI) bleeding in patients undergoing mechanical ventilation in the intensive care unit (ICU). The trial compared pantoprazole with a placebo and found that pantoprazole significantly reduced the risk of clinically important upper GI bleeding without affecting mortality. The systematic review, which included 12 randomized controlled trials, also found that PPIs were associated with a reduced incidence of upper GI bleeding and may have little or no effect on mortality. However, the review suggested that the effect of PPIs on mortality may differ based on disease severity.
