Mobility is considered a central topic for urban regeneration in metropolitan cities, in relation to the impact on traffic congestion, air pollution, public space quality, social inclusion. During the Covid-19 pandemic, mobility patterns have been strongly affected by the spread of the virus and the social distancing measures. In the last months, many cities have adopted mobility strategies for urban resilience, to face the crisis by the reorganization of infrastructures and networks with a glance at a prevention of an unsustainable return to private transport in the post-covid phase. In this context, the research illustrated in this paper, developed within a collaboration between Sapienza University of Rome and Roma Tre University, aims to propose an "anti-fragile" strategy for "post covid Rome", adaptable to other contexts of European cities, starting from an integrated approach to urban planning and mobility. The research methodology has articulated the activities into three phases. The phase of analysis of the phenomena and the main scientific references relating to urban resilience and antifragility, highlighting the the relationship between urban form and mobility models. The second phase relates to the study of the main ongoing strategies and practices in some European metropolitan cities. The third phase proposes an operational hypothesis of an antifragile strategy for Rome highlighting the relevance of mobility transition. Purpose: The aim of the present work is to explore the impact of the COVID-19 pandemic on research activities in a vast multidisciplinary academic community to identify the most critical issues. Method: To this purpose we planned a survey addressed to the entire academic research staff at "Sapienza" University of Rome, which represents the largest Italian academic community. A questionnaire consisting of both open and closed-ended questions was delivered to 4118 individuals in April 2021. Results: A total of 544 responses were collected. All academic roles were sufficiently represented in the study cohort. The median number of critical issues experienced by academic research staff was three. Among these, the three most frequently reported were related to: "Access to libraries / laboratories / research sites" (21.9%), "Limitation to stay abroad / study / research periods" (17.6%), "Progress of experimental work" (14.7%), with variable prevalence according to academic position and gender. Older subjects reported issues with "Projects' financial reporting" and "Expiration of acquired consumable material more frequently". The most common critical aspects reported in relation to the economic burden were: being "Unable to allocate funds" (31.4%), a "Reduction in clinical and scientific activity" (26.3%) and experiencing "Increased expenses (comprising private costs)" (21.2%) with no differences between genders. Researchers in Applied Sciences and Natural Sciences reported a higher frequency of problems in clinical and scientific activities, whereas increased expenses were reported also by researchers operating in the Humanities field. As a possible solution aimed at improving these issues, most subjects, especially those aged >45 years, indicated "Economic aid" (22.6%), "Reduction in bureaucracy" (19.9%) or "Enhancement of the scientific and clinical activities", whereas those aged ≤45 years felt that an increased duration and better access to PhD programs were to be prioritized. Conclusion: Our findings highlight the most critical issues related to research activities during the COVID-19 pandemic in a large academic community. The information achieved may be useful to identify researchers' needs and to design appropriate policies aimed at preparing research institutions for unexpected catastrophic events and limiting the negative impact on academic research activities. In this paper, we start from a contextualization about the measures used to contain the COVID-19 diffusion and the need to promote geotechnological proposals, data sharing and homogenous centralised systems for data collection and analysis. Successively, we present the "Dynamic Space-Time Diffusion Simulator in a GIS Environment to Tackle the COVID-19 Emergency" that we have elaborated on the basis of the data provided by the UOC Hygiene and Public Health Service - Local Health Unit Rome 1. Particularly, after describing the main technical process able to predispose the dynamic simulator, we underline the possible added value that it can provide in terms of infection surveillance and monitoring, precision preparedness, support to decision making and territorial screening. For this demonstrative application, we have extracted from the simulator some groups of four digital screenshots which are able to show synoptic photographs in temporal perspective concerning the total number of cases of COVID-19 in Rome (Italy) for the period February 25th - September 26th. Specifically we have selected: - four screenshots for the period February 25th - June 11th, to provide significant evidence about the first three months and a half; - four screenshots for the period March 1st - March 29th, to add an insight into the geographically and statistically meaningful month of March; - four screenshots for the period June 12th - September 26th, to supply an efficacious geovisualisation of the last three months and a half available; - four screenshots for the period February 25th - September 26th, to show a cumulative elaboration aimed at geolocating all the cases recorded in the seven months examined; - four screenshots for the period March 26th - September 26th, with a distinction about the first and second data sets, for a detailed (cumulative) zoom. This simulator, elaborated for the COVID-19 emergency, can be replicated in any circumstance for which specific data and information are available for the scientific community, shared and progressively updated in order to provide a productive contribution to the identification of serious infectious disease clusters, patterns and trends, and quickly respond to specific needs. Endometrial cancer represents an ideal target to evaluate the impact of COVID-19 being the most frequent gynecological malignancy in Italy, generally detected at early stages and correlated with favorable oncological outcomes. The present comparative retrospective study carried out at Campus Bio-medico University Foundation in Rome aims to evaluate the impact of the COVID-19 pandemic on the presentation, diagnosis and treatment of EC. All women with a histological diagnosis of non-endometrioid and endometrioid endometrial cancer between 1 March 2018 and 31 October 2022 were included. The number of cases was higher in period 2 (95 vs. 64 cases). Time to diagnosis did not show statistically significant differences but in period 2, 92.06% of the diagnoses were made following abnormal uterine bleeding, while in period 1, only 67.02% were. The waiting time for the intervention was significantly shorter in period 2. Definitive histology, FIGO staging, surgical technique and adjuvant therapy did not show significant differences between the two periods. The study demonstrates that the impact of the COVID-19 pandemic did not have a direct effect on the diagnostic delay, tumor staging and type of therapy but rather on the presentation pattern of endometrial cancer. The COVID-19 pandemic and resulting containment measures have shown that energy consumption in buildings is linked to several factors, such as living habits, occupancy profiles, and heating ventilation and air conditioning (HVAC) systems. This paper addresses the influences of such factors on energy consumption in a residential building, analysing different scenarios (pre-COVID-19, lockdown, post-COVID-19), in terms of discomfort and energy needs, through the new hourly calculation method (UNI EN ISO 52016). Energy and environmental effects were studied in a real case study near Rome by varying occupancy profiles, lighting and appliance schedules, and HVAC systems. Results show that, during the heating period, the lockdown scenario led to the lowest hours of discomfort (−29% on average), but the highest in the cooling period (up to +154%, +28% on average). The same scenario led to reasonable reduction of energy needs for heating (−14%), but also highlighted a significant increase (+60%) for the cooling period. This study underlines how the pandemic has influenced the energy and environmental behaviours in buildings. Moreover, the new hourly calculation method points out the importance of analysing HVAC systems, in terms of hours of discomfort, which could provide results that are more reliable. The COVID-19 pandemic called for a reorganisation of the methods for providing health services. The aim of this paper is to describe the organisational model implemented by one of Rome's Local Health Units (LHU), ASL Roma 1, for the "at-home COVID-19 vaccination campaign" dedicated to a target population and to outline data related to vaccination coverage stratified by health districts. A cross-sectional study was designed to describe the strategies implemented by LHU to deliver at-home vaccination programmes. People eligible for the at-home vaccination programme included patients living in the area of the LHU, being assisted by the district home care centre or not transportable or individuals with social situations that make traveling difficult. Priority for vaccination was given to (I) age > 80 years, (II) ventilated patients with no age limit, (III) very seriously disabled people with no age limit. Patients' data were acquired from regional and LHU databases. From 5 February until the 16 May, 6127 people got at least one dose of Pfizer-Biontech Comirnaty® vaccine, while 5278 (86.14%) completed the necessary two doses. The highest number of vaccines was administered during the first week of April, reaching 1296 doses overall. The number of vaccines administered were similar across the districts. The average number of people vaccinated at home was 6 per 1000 inhabitants in the LHU. Co-infections during COVID-19 may worsen patients' outcomes. This study reports the results of a screening assessing the presence of co-infections among patients hospitalized for SARS-CoV-2 infection in the Infectious Diseases-Ward of the Policlinico Tor Vergata Hospital, Rome, Italy, from 1 January to 31 December 2021. Data on hepatitis B and C virus, urinary antigens for legionella pneumophila and streptococcus pneumoniae, pharyngeal swab for respiratory viruses, QuantiFERON®-TB Gold Plus assay (QFT-P), blood cultures and pre-hospitalization antibiotic prescription were recorded. A total of 482 patients were included, 61% males, median age of 65 years (IQR 52–77), median Charlson comorbidity index of 4 (IQR 2–5). The mortality rate was 12.4%; 366 patients needed oxygen supply. In total, 151 patients (31.3%) received home antibiotics without any association with the outcome. No significant association between mortality and the positivity of viral hepatitis markers was found. Out of 442 patients, 125 had an indeterminate QFT-P, associated with increased mortality. SARS-CoV-2 was the only respiratory virus detected among 389 pharyngeal swabs; 15/428 patients were positive for S. pneumoniae; none for L. pneumophila. In total, 237 blood cultures were drawn within 48 h from hospital admission: 28 were positive and associated with increased mortality. In our cohort, bacterial and viral co-infections in COVID-19 hospitalized patients were rare and not associated with higher mortality. [ABSTRACT FROM AUTHOR]

The Editors of the Virtual Special Issue (VSI) "New Research on Water, Waste and Energy Management, with Special Focus on Antibiotics and Priority Pollutants" (VSI WWEM-20) here present details corresponding to papers that have been accepted, as well as further comments on the matter. It should be noted that the VSI should be associated to a Conference that had been initially programmed to be held in Rome during the summer of 2020, Unfortunately, it was postponed due to the COVID-19 pandemic. That conference was one of those within the series called "International Congress on Water, Waste and Energy Management". Although the Conference was postponed, the Call for Papers for the VSI was maintained by this journal. As a result, a set of very interesting papers were accepted after a careful peer-review process. Background: Since its outbreak, Coronavirus disease 2019 (COVID-19), a life-threatening respiratory illness, has rapidly become a public health emergency with a devastating social impact. Lately, the Omicron strain is considered the main variant of concern. Routine blood biomarkers are, indeed, essential for stratifying patients at risk of severe outcomes, and a huge amount of data is available in the literature, mainly for the previous variants. However, only a few studies are available on early routine biochemical blood biomarkers for Omicron-afflicted patients. Thus, the aim and novelty of this study were to identify routine blood biomarkers detected at the emergency room for the early prediction of severe morbidity and/or mortality. Methods: 449 COVID-19 patients from Sapienza University Hospital of Rome were divided into four groups: (1) the emergency group (patients with mild forms who were quickly discharged); (2) the hospital ward group (patients that after the admission in the emergency department were hospitalized in a COVID-19 ward); (3) the intensive care unit (ICU) group (patients that after the admission in the emergency department required intensive assistance); (4) the deceased group (patients that after the admission in the emergency department had a fatal outcome). Results: ANOVA and ROC data showed that high-sensitivity troponin-T (TnT), fibrinogen, glycemia, C-reactive protein, lactate dehydrogenase, albumin, D-dimer myoglobin, and ferritin for both men and women may predict lethal outcomes already at the level of the emergency department. Conclusions: Compared to previous Delta COVID-19 parallel emergency patterns of prediction, Omicron-induced changes in TnT may be considered other early predictors of severe outcomes. The increase in the use of the Internet, strongly boosted by the spread of COVID-19, has amplified the risk of involvement in cyberbullying and online grooming among minors. To date, most research on these phenomena has focused on middle and high school students, with fewer studies on younger children. The present study aims to fill this knowledge gap by measuring the spread of cyberbullying and online grooming in a sample of 410 primary school students in the city of Rome and by identifying the main individual and environmental predictors associated with the involvement of children in these phenomena using factor analysis. Results indicate that both cyberbullying and online grooming are widespread among respondents, showing common traits within the four latent dimensions identified. Screen time is among the main predictors of children's involvement, together with parental supervision, phubbing behaviours, prosocial tendencies and family socio-economic background. These findings highlight the need for further studies on representative samples of this age group, as well as for a greater cooperative effort among schools, parents and caregivers to keep children safe in the virtual world. The lockdown was crucial to stop the COVID-19 pandemic in Italy, but it affected many aspects of social life, among which traditional live science cafés. Moreover, citizens and experts asked for a direct contact, not relying on mass-media communication. In this paper, we describe how the Florence and Rome science cafés, contacted by citizens and experts, either directly or through the Florence science shop, responded to these needs by organizing online versions of traditional face-to-face events, experiencing high levels of participation. The science café methodology was also requested by a high school that needed to conclude an engagement experience with students and their families. Aero-medical evacuation has been considered as a feasible and safe treatment option during COVID pandemic, particularly when the needs of affected patients exceed what local clinics and hospitals are supposed to provide. In this article, we analyzed the clinical course of 17 patients medically evacuated to the "L. Spallanzani" Institute in Rome, Italy from foreign countries, mainly Africa and Eastern Europe, who had COVID-19 pneumonia with, or without, coinfections such as malaria, HIV, tuberculosis and microbiologically confirmed sepsis syndrome. The aero-medical evacuation of patients with infectious diseases has become one of the greatest medical achievements we have reached during this pandemic; in fact, only two patients with life threatening coinfections have died. Although logistically difficult and cost consuming, medical evacuation should be considered as a treatment option more than a single extraordinary measure, especially among complex cases that require specific technical and human resources. The COVID-19 pandemic marked an important breakthrough in human progress: from working habits to social life, the world population's behaviours changed according to the new lifestyle requirements. In this changing environment, university courses and learning methods evolved along with other "remote" working activities. For this quasi-experimental study, we discuss the effectiveness of the changes made by the LUMSA University in Rome, comparing two different groups of students who attended a master's course with blended and fully remote methodologies. Here, we focused our attention on the paradigm shift, comparing the data gathered during the blended course in the 2019/2020 academic year with data gathered during the same course, but conducted fully online, in the academic year 2020/2021. Considering the sample size and type, the group comparison was made using a non-parametric test (U-test). The statistical analysis results suggest that there was no substantial difference between the students' performance, confirming that the course changes made to adapt to the pandemic situation were successful and that learning effectiveness was preserved. The COVID-19 pandemic affected the acoustic environment worldwide, entailing relevant reductions of equivalent noise levels (LAeq) during this exceptional period. In the context of the LIFE+ DYNAMAP project, two wireless acoustic sensor networks were deployed in Milan and Rome. Taking advantage of the built-in identification of anomalous noise events (ANE) in the sensors, this work analyses the effects of the COVID-19 lockdown in both urban and suburban acoustic environments from January to June 2020, considering the distribution of ANEs and the intermittency ratio (IR) as an indicator of the impact of noise on population. The results show statistically significant increments of ANEs in Rome during the lockdown, mainly on weekends, and especially at night, despite the significant decrease in salient events. Differently, ANEs decrease during the lockdown in Milan, mostly at daytime, as a result of population confinement. Although the IR increases in several urban locations, most sensed locations show a relevant decrease in IR during the confinement, which represents a noteworthy reduction of the negative impact of noise in the population of both cities. Critically ill patients with COVID-19 are at increased risk of developing a hypercoagulable state due to haemostatic changes directly related to the SARS-CoV-2 infection or to the consequence of the cytokine storm. Anticoagulation is now recommended to reduce the thrombotic risk. Ilio-psoas haematoma (IPH) is a potentially lethal condition that can arise during the hospitalization, especially in intensive care units (ICUs) and frequently reported as a complication of anticoagulation treatment. We report a case series of seven subjects with SARS-CoV-2 pneumonia complicated by Ilio-psoas haematomas (IPHs) at our COVID-Hospital in Rome, Italy. Over the observation period, 925 subjects with confirmed SARS-CoV-2 infection were admitted to our COVID-hospital. Among them, we found seven spontaneous IPHs with an incidence of 7.6 cases per 1000 hospitalization. All the reported cases had a severe manifestation of COVID-19 pneumonia, with at least one comorbidity and 5/7 were on treatment with low weight molecular heparin for micro or macro pulmonary thrombosis. Given the indications to prescribe anticoagulant therapy in COVID-19 and the lack of solid evidences on the optimal dose and duration, it is important to be aware of the iliopsoas haematoma as a potentially serious complication in COVID-19 inpatients. Critically ill patients with COVID-19 are at increased risk of hypercoagulability state and anticoagulation therapy is recommended. Ilio-psoas haematoma (IPH) is found to be a complication of anticoagulation regimen especially in severe COVID-19 cases. An incidence of 7.6 cases per 1000 admission of IPHs was reported. Hypoesthesia of the lower limbs, pain triggered by femoral rotation, hypovolaemia and anaemia are the most common symptoms and signs of IPHs that should alert physician. [ABSTRACT FROM AUTHOR]

Background: In the first wave of the COVID-19 pandemic in 2020, many countries made changes to the routine management of patients with non-communicable diseases, including neurocognitive disorders. Therefore, many "so-called" non-urgent elective procedures and outpatient appointments have been canceled or postponed, possibly impacting negatively on health and well-being of patients in the short- and long-term. Aim: Here, we aimed at describing numbers and types of outpatient appointments canceled as a result of government's restrictive measures in our memory clinic. Methods: The scheduled appointments at the memory clinic of the Santa Lucia Foundation IRCCS, Rome, Italy, are recorded in a comprehensive dataset under strict administrative control. Here, we compared appointments (first-time and follow-up) that were canceled from January to April 2020 with those of the corresponding months in 2019. Results: We observed a substantial decrease in appointments during 2020. The majority of scheduled appointments were follow-up, and about a quarter were first-time appointments. We estimated that 66.7% and 77.4% of patients missed out respectively their first and follow-up appointments in our memory clinic due to government's restrictive measures in March–April 2020. Conclusions: A large number of patients with neurocognitive disorders missed crucial appointments due to government's restrictive measures, and many experienced a delay in initial diagnosis and initiation of treatment. This has relevant impact on their treatment and consequently has (is still having and potentially will have) an increase on the healthcare service burden of clinics. Furthermore, as a second wave of COVID-19 affects Europe, and with winter approaching, it is a compelling priority to ensure easy and rapid access to appropriate assessment, care and treatment in the event of a new outbreak and potential subsequent lockdowns, with particular attention to the development of specific healthcare technologies customized to older persons with cognitive impairment. The coronavirus pandemic that has brought Italy (like many other countries) to its knees brings to the surface in a brutal way the criticality of the health policy choices taken by the government in the last thirty years, guided by the principle of subordination of the quality of care to cost efficiency. The priority has been to maximize profit by sacrificing the quality of care and health services. All this forces us to redefine the terms and categories with which we read and interpret today the issues related to the physical and mental health of citizens and the policies that govern it. Starting from this, the aim of this article is to reflect on the limits of contemporary health policies and the urgency of rethinking them in terms of a greater capacity to reconcile people's needs with a better use of public resources. To this end, the article will focus on bottom-up health policy practices that try to guarantee, at the same time, the quality of care and a better use of health spending, through the analysis of the experience of the Solaris association in Rome working in the field of mental health. The restriction measures linked to the COVID-19 shock suddenly highlighted the vulnerability of most socioeconomic systems, including the food sector. In a context in which the limitation to the movement of people and goods has put the longer and more structured supply chains in serious difficulty, many experiences and initiatives have emerged as viable alternatives. The aim of the research was to understand if and how the Solidarity Purchasing Groups (SPG) of Rome have contributed to the resilience of the food system of the metropolitan city during the lockdown. The research was based on the results of a questionnaire administered to the SPGs of Rome during the first period of the pandemic (April–July 2020), enriched by some in-depth interviews carried out by the authors. What emerged was that, despite the limited extent in terms of products conveyed within the whole food system, the SPGs represented an important food supply channel during the lockdown period, for two main reasons: a greater flexibility and agility in moving and in handling goods and the possibility of remunerating local farms, contributing to the resilience of the local agri-food fabric. The analysis of the results confirms the strong vitality of such Food Movements in Rome and, at the same time, allows for the identification of a series of interventions that the institutions could adopt to favor the spread of a food environment more compatible with more sustainable and fairer forms of food production and distribution.
