Simple Summary: Horse sports are popular worldwide, providing spectator enjoyment, benefiting human health, and contributing substantially to national economies. Training and management practices used to care for sporthorses are generally based on tradition rather than science; this combined with the high-risk nature of equestrian activities led to the public questioning if sporthorse health and welfare are being compromised. To understand better how sporthorses are being managed, experts, actively involved with national and international horse sports (dressage, showjumping, and eventing) were consulted across four rounds of a Delphi study. This approach allowed participants to interact to reach a point where everyone agreed on core areas (or domains) that they felt were essential to sporthorse management. Five areas were rated as essential: training management, competition management, young horse management, health status and veterinary management, and the horse–human relationship. Stable and environmental management, and welfare assessment were rated as important but not essential, as most experts felt that these areas were already managed well. Experts called for increased education and research to support riders, trainers, and federations. A welfare charter and evidence-based guidelines to inform management practices were advised to ensure sporthorses have a good life and to safeguard the future of equestrian sports. The public is increasingly questioning equestrianism's social license to operate. While the focus historically centered on horseracing, increased scrutiny is now being placed on how dressage, showjumping, and eventing are addressing equine management and welfare concerns. Nominated equestrian federation and equestrian organization experts (n = 104) directly involved in international and/or national-level horse sports took part in a four-stage, iterative Delphi to obtain consensus on what factors should be considered essential to manage sporthorse health and welfare. Five core domains were agreed as essential: training management, competition management, young horse management, health status and veterinary management, and the horse–human relationship. Two further domains: stable and environmental management, and welfare assessment were rated as important but not essential, as most respondents felt that these areas were already managed well. Participants felt increased education and guidance combined with further policy development and regulation are needed to support stakeholders to optimize sporthorse management. An appetite to engage with research to generate evidence that promotes sporthorse welfare was evident. The development of a sporthorse welfare charter and evidence-based guidelines to inform the management and monitoring of sporthorses' health and welfare are recommended to provide horses with a good life and to safeguard the future of equestrian sports. In the past decade, numerous ethical frameworks have been developed to support public health decision‐making in challenging areas. Before the COVID‐19 pandemic began, members of the authorship team were involved in research programmes, in which the development of ethical frameworks was planned, to guide (a) the use of new technologies for emerging infectious disease surveillance; and (b) the allocation of scarce supplies of pandemic influenza vaccine. However, as the pandemic evolved, significant practical challenges emerged that led to our questioning the value of these frameworks. We now believe that a normative instrument, such as a framework, cannot adequately or reliably provide the ethical guidance that needs to be incorporated into public health decision‐making during natural disasters or infectious disease emergencies. Recently it has been suggested that there are potentially more dynamic, flexible, and effective ways to navigate decisions involving complex considerations entailed in policies and practices during a public health emergency. In this paper, we first outline the key functions of a public health ethics framework, before describing why we believe it would not be fit for purpose during a crisis. Background: Physical activity benefits physical and mental health. However, limited research investigates if physical activity can improve outcomes from the grieving process following the death of a parent. Methods: Semi-structured interviews were conducted with 14 individuals (n = 8 female; age M = 31.2 years), who had experienced the death of a parent when they were aged between 10 and 24 years old, using retrospective recall. Data were analysed inductively using thematic analysis. Results: Six themes were identified. Physical activity was seen as; 1) 'Therapeutic'; providing an 2) 'Emotional Outlet' and created a strong sense of 3) 'Social Support'. Alongside it 4) 'Builds Confidence', and led to 5) 'Finding Yourself' and 6) 'Improved Health and wellbeing' (physical and psychological). Conclusion: Physical activity has the potential to provide positive experiences following a parental bereavement. It can provide a sense of freedom and was seen to alleviate grief outcomes, build resilience, enable social support and create a stronger sense of self. Bereavement support services for young people who have experienced death of a parent should consider physical activity as a viable intervention to support the grieving process. Variation in equine hoof conformation between farriery interventions lacks research, despite associations with distal limb injuries. This study aimed to determine linear and angular hoof variations pre- and post-farriery within a four to six week shoeing/trimming interval. Seventeen hoof and distal limb measurements were drawn from lateral and anterior digital photographs from 26 horses pre- and post-farriery. Most lateral view variables changed significantly. Reductions of the dorsal wall, and weight bearing and coronary band lengths resulted in an increased vertical orientation of the hoof. The increased dorsal hoof wall angle, heel angle, and heel height illustrated this further, improving dorsopalmar alignment. Mediolateral measurements of coronary band and weight bearing lengths reduced, whilst medial and lateral wall lengths from the 2D images increased, indicating an increased vertical hoof alignment. Additionally, dorsopalmar balance improved. However, the results demonstrated that a four to six week interval is sufficient for a palmer shift in the centre of pressure, increasing the loading on acutely inclined heels, altering DIP angulation, and increasing the load on susceptible structures (e.g., DDFT). Mediolateral variable asymmetries suit the lateral hoof landing and unrollment pattern of the foot during landing. The results support regular (four to six week) farriery intervals for the optimal prevention of excess loading of palmar limb structures, reducing long-term injury risks through cumulative, excessive loading. Chemotherapy is a commonly integrated treatment option within human and animal oncology regimes. Limited research has investigated pet owners' treatment decision-making in animals diagnosed with malignant neoplasia. Dog and cat owners were asked to complete an online questionnaire to elucidate factors which are key to the decision making process. Seventy-eight respondents completed the questionnaire in full. Fifty-eight percent of pet owners would not elect to treat pets with chemotherapy due to the negative impact of the associated side effects. Seventy-two percent of respondents over estimated pet survival time post chemotherapy, indicating a general perception that it would lead to remission or a cure. Vomiting was considered an acceptable side effect but inappetence, weight loss and depression were considered unacceptable. Owners did expect animals' to be less active, sleep more and play less, but common side effects were not rated as acceptable despite the potential benefits of chemotherapy. Based on the results, veterinary teams involved with oncology consultations should establish if clients have prior experience of cancer treatments and their expectations of survival time. Quality of life assessments should also be implemented during initial oncology consultations and conducted regularly during chemotherapy courses to inform client decision making and to safe guard animal welfare. Simple Summary: In the U.S., thousands of pigs had to be destroyed on the farms when illness caused by COVID-19 greatly reduced pork slaughter plant capacity. Some of the methods used to destroy pigs on the farms severely compromised animal welfare. Reliance on a few large slaughter plants created a fragile supply chain. Animal welfare auditing conducted by large meat buyers was also hindered by COVID-19. Many live in-person audits were stopped and replaced by a combination of stationary video cameras and live streamed videos from mobile phones. To insure high standards of animal welfare, video methods should never completely replace in-person visits. In the U.S., the most severe animal welfare problems caused by COViD-19 were in the pork industry. Thousands of pigs had to be destroyed on the farm due to reduced slaughter capacity caused by ill workers. In the future, both short-term and long-term remedies will be needed. In the short-term, a portable electrocution unit that uses scientifically validated electrical parameters for inducing instantaneous unconsciousness, would be preferable to some of the poor killing methods. A second alternative would be converting the slaughter houses to carcass production. This would require fewer people to process the same number of pigs. The pandemic revealed the fragility of large centralized supply chains. A more distributed supply chain with smaller abattoirs would be more robust and less prone to disruption, but the cost of pork would be greater. Small abattoirs can coexist with large slaughter facilities if they process pigs for specialized premium markets such as high welfare pork. The pandemic also had a detrimental effect on animal welfare inspection and third party auditing programs run by large meat buyers. Simple Summary: On 11 March 2020, the World Health Organization declared coronavirus disease (COVID-19) outbreak a global pandemic and an ongoing public health emergency. In the United Kingdom, quarantine and social distancing measures were implemented with immediate effect on 17 March 2020, resulting in a rapid change to the way owners managed and interacted with their horses. We surveyed 6259 horse owners to evaluate the impact of COVID-19 on themselves and their horses. The majority of horse owners were visiting and riding their horses less, with increased restrictions experienced by owners who kept their horse at private livery yards. Whilst social distancing and visiting restrictions were in place at livery yards, nearly half were not providing hand sanitization or disinfection protocols for shared areas/equipment to prevent the spread of the virus between owners. Horse owners expressed concern that equine health and welfare would be negatively affected by the restrictions put in place and of financial consequences as a result of the pandemic. The majority of respondents also felt their own mental health and wellbeing was being adversely affected by not being able to visit/interact as they normally would with their horses. Equestrian influencers and national bodies should engage with increased communication and education to support horse owners through the pandemic in the short, medium, and long-term. COVID-19 was declared a global pandemic on 11 March 2020; the United Kingdom (UK) implemented quarantine measures shortly afterward, resulting in rapid changes in how owners managed and interacted with their horses. This study provides a rapid analysis of the initial impact of the COVID-19 outbreak on the management of UK leisure and competition horses. A 17 question online survey was distributed via equestrian social media sites to ascertain the impact of COVID-19 on horse and yard management and on human–horse interactions. Frequency analysis combined with Chi-squared and thematic analyses identified the impact of COVID-19 on UK horse owners. Major changes within horse management and horse–human interactions were reported for the majority of horse owners (>65%), regardless of the establishment type or region. Social distancing and visiting restrictions were implemented at most yards, but nearly half were not providing hand sanitization or disinfection protocols for the shared areas/equipment to prevent cross-contamination between users. The financial impact of the pandemic combined with restricted access to veterinary professionals resulted in owners expressing concerns that horse health and welfare may be compromised as a result. Aim: To evaluate an innovative paediatric neurorehabilitation model in relation to improving quality of neurorehabilitation and reducing length of stay (LOS) for children with acquired brain injury.Method: A process evaluation approach was conducted in line with Medical Research Council evaluation of complex interventions guidance. Analysis was conducted on routinely collected patient data from 2017 to 2018, including LOS and family feedback. Descriptive and inferential statistics were used for quantitative analysis and qualitative data was analysed thematically.Results: Outcomes for 70 children (0-16y, median age 5y, IQR 1-11y, 46 males, 24 females) referred to the service indicated improved function and reduced complexity of need. The mean LOS was 10.6 days compared to baseline mean LOS of 41 days (2011-2012). High satisfaction from the families was recorded; however, ongoing needs and service gaps regarding long-term support were identified.Interpretation: This service model is effective in delivering quality paediatric neurorehabilitation, demonstrating a sustained impact on LOS, and positive patient outcome data and family feedback for this group of patients. Background: Recent deliberations by Australian public health researchers and practitioners produced an ethical framework of how decisions should be made to distribute pandemic influenza vaccine. The outcome of the deliberations was that the population should be considered in two categories, Level 1 and Level 2, with Level 1 groups being offered access to the pandemic influenza vaccine before other groups. However, the public health researchers and practitioners recognised the importance of making space for public opinion and sought to understand citizens values and preferences, especially First Nations peoples. Methods: We conducted First Nations Community Panels in two Australian locations in 2019 to assess First Nations people's informed views through a deliberative process on pandemic influenza vaccination distribution strategies. Panels were asked to make decisions on priority levels, coverage and vaccine doses. Results: Two panels were conducted with eighteen First Nations participants from a range of ages who were purposively recruited through local community networks. Panels heard presentations from public health experts, cross-examined expert presenters and deliberated on the issues. Both panels agreed that First Nations peoples be assigned Level 1 priority, be offered pandemic influenza vaccination before other groups, and be offered two doses of vaccine. Reasons for this decision included First Nations people's lives, culture and families are important; are at-risk of severe health outcomes; and experience barriers and challenges to accessing safe, quality and culturally appropriate healthcare. We found that communication strategies, utilising and upskilling the First Nations health workforce, and targeted vaccination strategies are important elements in pandemic preparedness and response with First Nations peoples. Conclusions: First Nations Community Panels supported prioritising First Nations peoples for pandemic influenza vaccination distribution and offering greater protection by using a two-dose full course to fewer people if there are initial supply limitations, instead of one dose to more people, during the initial phase of the vaccine roll out. Simple Summary: Employee mental health is a strategic initiative for global organizations and maintaining staff wellbeing is a key focus for British horseracing. Workforce wellbeing is increasingly linked to employee recruitment, retention, and productivity, challenges currently facing the horseracing sector. Improving staff wellbeing is paramount to maintaining high standards of equine welfare, ensuring the industry's social license to operate is upheld. Research in horseracing has identified a range of factors influencing staff wellbeing; however, the role of injury in anxiety and depression scores is unknown for this population. Over two thirds of injured staff were experiencing anxiety and over half were experiencing depression. Anxiety and depression scores were related to employment status, working hours, and type of injury. Higher anxiety and depression scores were negatively associated with help-seeking and pain management behaviors during injury, as well as increased risk of using alcohol as a coping method, both for pain-relief at work and socially. The findings from this study may provide opportunities to influence mental health post-injury within horseracing, through the development of educational resources aimed at reducing stigma, improving mental health literacy, and developing industry-wide early screening protocols for mental health in injured staff. Horseracing has identified several factors influencing staff wellbeing; however, the relationship between injury, anxiety, and depression is yet to be established. This study investigated anxiety and depression scores and their association to pain management, coping, and help-seeking behaviour in injured British horseracing staff. An online retrospective survey was completed by 175 participants, identifying injury prevalence, coping strategies, occupational risk factors, and Hospital Anxiety and Depression Scale (HADS) scores. Analysis identified 65.14% (n = 114) of staff reported anxiety scores above the threshold (≥8) and 59.52% (n = 104) of staff reported depression scores ≥8. Median anxiety and depression scores were higher for staff who viewed their employer as unhelpful (anxiety p = 0.001; depression p = 0.020). Heightened anxiety and depression were associated with an increased likelihood to use pain medication to manage at work, including non-steroidal anti-inflammatory drugs (NSAID's), alcohol, nicotine, and prescription drugs (p < 0.05). Implications for staff wellbeing is evident; anxiety and depression risks are high following injury, which may influence help-seeking behaviour, perceived job security, and coping mechanisms. Simple Summary: In the sport of Para dressage, the combined performance of the Para athlete and the horse is judged subjectively. To provide a level playing field for athletes with a wide range of impairments, Para dressage athletes are classified based on the degree to which their impairment impacts sports performance. This study aimed to evaluate the effects of impairment on objective performance measures in Para athletes as a step towards developing an evidence-based classification system. Two groups of athletes comprising twenty-one elite Para athletes (classified grades I to V) and eleven non-disabled athletes (competing at Prix St. Georges or Grand Prix) were measured while performing a 2 min custom dressage test on a riding simulator and clinically assessed using a battery of impairment assessment tools. Statistical analysis considered the extent to which performance could be predicted from impairment measures. Impairment assessment tools related to sitting function could predict the impact of impairment on performance in Para athletes but not in non-disabled athletes. These findings provide evidence of sport-specific impairment assessments that could be used to enhance athlete classification. These findings pave the way for further studies using similar approaches to enhance the objectivity of classification between grades. This study follows a previously defined framework to investigate the impact of impairment on performance in Para dressage athletes. Twenty-one elite Para dressage athletes (grades I to V) and eleven non-disabled dressage athletes (competing at Prix St. Georges or Grand Prix) participated. Data were collected in two phases: performing a two minute custom dressage test on a riding simulator while kinematic data were synchronously collected using inertial measurement units (2000 Hz) and optical motion capture (100 Hz), and clinically assessed using a battery of impairment assessment tools administered by qualified therapists. Impairment and performance measures were compared between Para and non-disabled athletes. Significant differences between athlete groups were found for all impairment measures and two performance measures: simulator trunk harmonics (p = 0.027) and athlete trunk dynamic symmetry (p < 0.001). Impairment assessments of sitting function and muscle tone could predict 19 to 35% of the impact of impairment on performance in Para athletes but not in non-disabled athletes. These findings provide the basis for a robust, scientific evidence base, which can be used to aid in the refinement of the current classification system for Para dressage, to ensure that it is in line with the International Paralympic Committee's mandate for evidence-based systems of classification. Aims: To pilot prospective data collection by paediatricians at the point of care across England using a defined terminology set; demonstrate feasibility of data collection and utility of data outputs; and confirm that counting the number of needs per child is valid for quantifying complexity.Method: Paediatricians in 16 hospital and community settings collected and anonymized data. Participants completed a survey regarding the process. Data were analysed using R version 3.1.2.Results: Overall, 8117 needs captured from 1224 consultations were recorded. Sixteen clinicians responded positively about the process and utility of data collection. The sum of needs varied significantly (p<0.01) by level of gross motor function ascertained using the Gross Motor Function Classification System for children with cerebral palsy; epilepsy severity as defined by level of expertise required to manage it; and by severity of intellectual disability.Interpretation: Prospective data collection at the point of clinical care proved possible without disrupting clinics, even for those with the most complex needs, and took the least time when done electronically. Counting the number of needs was easy to do, and quantified complexity in a way that informed clinical care for individuals and related directly to validated scales of functioning. Background: In response to the threat of COVID-19 infection, Australia mandated a 14 day quarantine period in a designated facility for all travellers returning from overseas from late March 2020. These facilities were usually hotels, or hotel-like serviced apartments, and also included a repurposed former mining village in the Northern Territory. This paper aimed to investigate the experiences of risk of people quarantined in designated supervised facilities in Australia, which has not been systematically explored before.Methods: In this qualitative study semi-structured interviews were conducted with 58 participants quarantined between March 2020 and January 2021. Participants were returned Australian citizens and residents who were required to undergo mandatory supervised quarantine for COVID-19. Interviews were conducted using video teleconferencing (via Zoom), transcribed and coded, then analysed thematically.Results: While participants generally supported the concept of quarantine to protect the Australian public, they were critical of elements of it where they felt exposed to risk (COVID-related or not). They also described instances where infection control within the system seemed inadequate. For some, particularly those quarantined with small children, they reported that the facilities were inadequate or inappropriate for health and wellbeing. Using thematic analysis, three major themes were identified that related to problems in the existing system: perception of being subjected to high risk through lax standards of COVID protection in the quarantine process; risks to the community identified in quarantine; and risk in non-hotel managed quarantine facilities.Conclusions: There are systemic issues with infection control in hotel quarantine, which can be further undermined by individual non-compliance. Risks to safety for those in quarantine can be reduced, both in terms of infection control within hotel quarantine and, in the case of the Northern Territory facility, timely in-person medical care as needed for non-COVID conditions. Systems of infection control need ongoing review to ensure that people entering quarantine are protected from known risks of infection at every stage. Background: The purpose of this study was to investigate the usefulness of salivary cortisol (SC) and eye temperature measured by infrared thermography (IRTET) as biomarkers to manage competitions more effectively and monitor horse welfare in endurance competitions. Based on previous studies, it was hypothesised that pre-exercise baseline SC and IRTET would be higher in younger or less experienced horses, and that post-exercise variation from baseline would be higher in the top finishers. Results: Salivary cortisol measured in 61 competing at qualifier 40 km and 80 km rides showed an abrupt variation (93–256% rise) of the baseline SC levels [median ± interquartile range (IQR) = 0.27 ng/dl ± 0.36] obtained at the Pre-Inspection (PI) into Vet Gate (VG)1 independently of the covered distance, but modest or even lower in the subsequent Vet Gates, e.g. VG2 or VG3. The IRTET measured concomitantly in 16 horses showed significant (p < 0.05) higher levels at the PI in less experienced horses participating in the 40 km ride (median ± IQR = 35.7 °C ± 1.4) than their counterparts in the 80 km ride (median ± IQR = 35.0 °C ± 1.5), but not SC. Baseline SC levels at the PI of horses classifying in the Top5 in the 40 km ride category were significantly (p < 0.05) higher median ± IQR = 0.90 ng/ml ±0.61) when compared to horses positioned from 10th position on (median ± IQR = 0.16 ng/ml ±0.40). A lower IRTET in the PI was correlated with better placement (p < 0.05) and those in the Top5 (median ± IQR = 33.9 °C ± 0.0) had a significantly (p < 0.5) higher variation (+ 10.65%) into the last VG. Conclusion: Pre-exercise baseline IRTET levels, but not SC, were higher in less experienced horses in the 40 compared to their counterparts in the 80 km ride competitions. SC and IRTET showed different indications according to the competition. In the40 km ride competition, higher baseline pre-exercise SC levels seemed to be linked to a better classification outcome. In contrast, in the 80 km ride horses, the higher IRTET variation from pre-exercise into final Vet Gate was the parameter associated with a better performance. Simple Summary: Forming relationships with horses is commonly understood as an important part of amateur and elite equestrian sports. Horse-human relationships have historically been romanticized and the success of sporting horse-human combinations has been attributed to a close relationship between horse and rider. Elite equestrian sports differ from amateur equestrian sports, especially where an elite rider earns their primary income from riding and competing horses, and third-party sponsors and owners are financially invested in a horse-rider combination. In this study, 36 elite equestrian athletes were interviewed about their interspecies relationships. Results indicated that a strong horse-rider relationship could, in some instances, inhibit a rider's ability to compete successfully and engage in a commercialized sporting context, due to tensions between an instrumental approach to animals and meaningful horse-rider interaction. Results also suggested that horse-rider relationships may be peripheral to performance outcomes, or conversely, essential to performance success. The relationship between sporting outcomes and the horse-rider relationship in an elite setting is clearly complex and multifaceted. An increase in transient, instrumental horse-rider relationships may be resulting in a shift towards a commercial, detached model of relating to horses, raising ethical questions around the professionalization of equestrianism and the management of competition horses. The horse-rider relationship is fundamental to ethical equestrianism wherein equine health and welfare are prioritized as core dimensions of sporting success. Equestrianism represents a unique and important form of interspecies activity in which relationships are commonly idealized as central to sporting performance but have been largely unexplored in the sport psychology literature. Horse-rider relationships warrant particular consideration in the elite sporting context, given the tension between constructions of "partnership" between horse and rider, and the pragmatic pressures of elite sport on horse and rider and their relationship. The current study examined the link between sporting performance and the horse-rider relationship in an elite equestrian sporting context. Thirty-six international elite riders from eight countries and six equestrian disciplines participated in a single in-depth interview. A social constructionist, grounded theory methodology was used to analyze this data. The horse-rider relationship was positioned in three different ways in relation to elite sporting outcomes: as pivotal to success; non-essential to success; or as antithetical to success. Participants shifted between these positions, expressing nuanced, ambivalent attitudes that reflected their sporting discipline and their personal orientation to equestrianism. Competitive success was also defined in fluid terms, with participants differentiating between intrinsic and extrinsic markers of success. These findings suggest a complex and multifaceted connection between interspecies performance and relationships in elite sport. Where strong horse-rider relationships are antithetical to performance, a threat to the welfare and ethics of equestrian sport exists. Simple Summary: Although coronavirus stopped horseracing in March 2020, most staff were classified as essential workers due to equine care and continued to work throughout lockdown. The physical and psychological impact of working during lockdown is unknown, and staff stress could have negative implications for racehorse welfare. Racing grooms and stud staff were more likely to be working than most sectors, due to the timing of lockdown with the racing calendar and foaling season. Administrative staff were busier during lockdown, completing additional risk assessments or paperwork. Most staff reported that workplace changes were successful in protecting health and safety, but flat racing grooms felt that work-based changes were less effective. Negative perceptions of work-based COVID-19 changes may affect the staff's ability to complete daily tasks and thus influence the quality of care provided to horses in their charge. Trainers and part-time staff were concerned about job security, highlighting a need for further employee support structures following the pandemic. The racing industry has prioritised staff health and safety but continued reflection on staff well-being, demands and working practices will maximise staff's ability to care for horses under their charge, and allow racing to maintain the highest standards of equine welfare. Due to COVID-19, horseracing was required to cease all activity in March 2020; however, little is known about the pandemic's impact on staff working practices. This study investigated the impact of COVID-19 on staff working practices during the initial lockdown phases. An online survey about working conditions during lockdown was answered by 287 participants. Chi-squared tests for independence and binary logistic regression (BLR) analysis was undertaken. A total of 53.7% (n = 154) of staff were working during lockdown. Pandemic-specific workplace changes were reported as effective by 87.8% (n = 115) of staff. Flat grooms reported workplace changes as less effective (χ2 (52, n = 131) = 92.996, p < 0.001). A total of 67.2% (n = 193) of staff were positive about job security. Trainers and grooms were significantly less likely to report jobs as secure (χ2 (52, n = 287) = 75.653, p < 0.05). The findings suggest that most of the racing industry positively received changes made by their employers to tackle the pandemic, and for staff still working during lockdown, their health and safety was prioritised. Continued development of employee support structures to promote job security and workforce stability is advised, which will minimise the disruption of staff changes on the care and welfare of the horses. Simple Summary: The Equine Behaviour Assessment and Research Questionnaire (E-BARQ) is an online questionnaire collecting information from horse-owners and riders on their horses' training, management and behaviour. In order to compare different populations of domestic equids—for example, according to their breed, discipline, training or management type—it is important that the questionnaire administered to participants has been validated and found to be reliable. The questionnaire must reflect a true assessment of the horse's behaviour, and, when taken by different users or the same user over time, the results should remain consistent. This article describes the process of validating the E-BARQ questionnaire, using three separate procedures. The results confirmed that the E-BARQ represents a standardised and validated behavioural assessment tool for horses. The Equine Behaviour Assessment and Research Questionnaire (E-BARQ) was developed to obtain quantitative data on the domestic equine triad: training, management and behaviour. It can be taken repeatedly, thus collecting longitudinal data to enable evaluation of how changes in a horse's training and management are reflected in that horse's behaviour over time and how these changes can impact horse welfare in the longer term. Questionnaire validation and reliability were tested by determining (a) whether an owner's subjective ratings of their horse's problematic behaviours or undesirable temperament traits were reflected in the questionnaire scores obtained for that horse (construct validity), (b) whether two respondents, equally familiar with a particular horse, reported comparable scores for that horse through the questionnaire (inter-rater reliability), and (c) whether the same respondent, scoring the same horse after a known interval of time, recorded similar responses (intra-rater reliability). Construct validity testing of 1923 responses showed significant alignment between owners' reported experience of focal horses' behaviour and those horses' E-BARQ scores, with scores varying from 1.13 to 1.34 for ridden horse behaviour (all p < 0.001) and from 1.06 to 1.43 for non-ridden horse behaviour (all p < 0.001). Inter-rater reliability testing of ten horse–rider pairs revealed that 203 of the 215 question items were significantly aligned (p < 0.001) when tested by two independent raters. Of the remaining 19 items, four had fair alignment (ĸ = 0.174–0.316; p = 0.281) and ten items, largely related to whether the horse shows behavioural signs related to anxiety when taken away from home, did not align (ĸ = 0; p = 1). Intra-rater reliability tests showed that the responses significantly aligned on all 215 question items tested (p < 0.001). Medicine and medical device donations have the potential to improve access to healthcare in some of the poorest parts of the world, but can do more harm than good. World Health Organization guidelines advise donors on how to make effective and useful donations. Our objective was to assess compliance of recent medicine or medical device donations with WHO guidelines from 2009 onwards. We searched media, academic and gray literature, including industry and organizational documents, to identify reports describing specific incidences of the donation of medicines or devices. We collected data on donation characteristics and guideline compliance. We identified 88 reports describing 53 donations. Most did not comply with at least some items in WHO guidelines and no reports provided sufficient information to assess compliance against all items. Donations that fail to comply with guidelines may be excessive, expired and/or burden recipient countries with storage and disposal costs. It was estimated that 40–70% of donated medical devices are not used as they are not functional, appropriate, or staff lack training. More effective donations involved needs assessments, training and the use of existing distribution networks. Storm surges are coastal sea-level variations caused by meteorological conditions. It is vitalthat they are forecasted accurately to reduce the potential for financial loss and loss oflife. An area historically prone to destructive surges is the North Sea, thanks largely to itsshallow nature, semi-enclosed geometry and large surrounding areas of low-lying land.In this study, we investigate how effectively the variational assimilation of sparse sealevel observations from tide gauges can be used for operational forecasting. A new shortest pathmethod based on Dijkstra's algorithm is introduced and evaluated for dealing withcoastal boundaries and a dynamic covariance model, incorporating information from themodel state itself, is also considered. For our experiments, a specific case study is used: theDecember 2013 Cyclone Xaver event in the North Sea.We validate our covariance models by removing selections of tide gauges from the assimilation.These experiments show widespread improvements in RMSE and correlation,reaching up to 16cm and 0.7 (respectively) at some locations, implying our assimilationsetup is reasonable. Mock forecasts show RMSE improvements of up to 5cm are found forthe first 24 hours of forecasting, which is useful operationally. Beyond 24 hours, improvementsquickly diminish however. During all experiments, the dynamic covariance modelperforms better than the non-dynamic covariance models however there is little differencebetween the Dijkstra and Euclidean based setups.This work places an upper bound on how effective variational assimilation of sea leveldata can be for storm surge forecasting in semi-enclosed, tidal, shallow seas. Aims: To develop a Disabilities Terminology Set and quantify the multifaceted needs of disabled children and their families in a district disability clinic population.Method: Data from structured electronic clinic letters of children attending paediatric disability clinics from June 2007 to May 2012 in Sunderland, north-east England collected at the point of clinical care were analysed to determine appropriate terms for consistent recording of each need and issue. Terms were collated to count the number of needs per child.Results: A Systemized Nomenclature of Medicine - Clinical Terms subset of 296 terms was identified and published, and 8392 consultations for 1999 children were reviewed. The required number of clinic appointments correlated strongly with the number of needs identified. Children with intellectual disabilities in addition to cerebral palsy and epilepsy had more than double the number of conditions, technology dependencies, and family-reported issues than those without. Disabled children who subsequently died had the highest burden of needs (p=0.007).Interpretation: Detailed data about needs generated outputs useful for local care pathway development and service planning. Sufficient evidence was provided for successful business cases leading to the appointment of additional paediatric disability consultants. The aim of this study was (i) to seek a shared vision between families and clinicians regarding key aspects of health as outcomes, beyond mortality and morbidity, for children with neurodisability, and (ii) to appraise which multidimensional patient reported outcome measures (PROMs) could be used to assess salient health domains. Methods: Relevant outcomes were identified from (i) qualitative research with children and young people with neurodisability and parent carers, (ii) Delphi survey with health professionals, and (iii) systematic review of PROMs. The International Classification of Functioning Disability and Health provided a common language to code aspects of health. A subset of stakeholders participated in a prioritisation meeting incorporating a Q-sorting task to discuss and rank aspects of health. Results: A total of 33 pertinent aspects of health were identified. Fifteen stakeholders from the qualitative and Delphi studies participated in the prioritisation meeting: 3 young people, 5 parent carers, and 7 health professionals. Aspects of health that emerged as more important for families and targets for health professionals were: communication, emotional wellbeing, pain, sleep, mobility, self-care, independence, mental health, community and social life, behaviour, toileting and safety. Whilst available PROMs measure many aspects of health in the ICF, no single PROM captures all the key domains prioritised as for children and young people with neurodisability. The paucity of scales for assessing communication was notable. Conclusions: We propose a core suite of key outcome domains for children with neurodisability that could be used in evaluative research, audit and as health service performance indicators. Simple Summary: The majority of the UK's leisure horses are kept at livery yards, under the oversight of a livery yard manager or owner. Livery yard managers/owners therefore have a major impact on the potential wellbeing of horses within each yard, but their role has not been previously studied. This study used the COVID-19 pandemic as a lens to view the priorities and decisions of livery yard managers at a time when everything was already subject to change. Yard managers/owners of 24 very diverse yards were interviewed over a period of nine months, repeating interviews quarterly. Discussion forum threads on relevant topics about livery yards during COVID-19 were also collected. These data were analysed using a qualitative approach called Grounded Theory. The results showed that running a yard required a careful balance between conflicting priorities, which made change difficult within each individual yard culture. During the pandemic, maintaining usual horse care routines and standards was prioritised above human health and the business model of the yard, and above horse–human relationships. However, each yard adopted individual approaches to managing human health alongside maintaining equine management. These findings are important for future initiatives which aim to support livery yard management and change. Approximately 60% of the UK's leisure horses are kept at livery yards under the management and oversight of a livery yard owner or manager (LYO/M), yet their role has received little research attention. This study used the COVID-19 pandemic as a lens through which to view LYO/Ms' decisions around equine care and management at a time when changes to usual practice were necessary. Qualitative research methods were used. Up to 3 interviews were conducted with 24 different LYO/Ms over nine months (n = 48). Discussion threads from open-access UK discussion fora were also analysed. All data were anonymised and analysed using a Grounded Theory methodology. Prior to the pandemic, equine care and management practices varied greatly across yards, and yard cultures were a product of LYO/Ms' construction of good equine care, their business model, and the need to balance human and equine contentment. The role of the LYO/M was to maintain an equilibrium between those interlinked factors. During the pandemic, LYO/Ms adopted new measures designed to influence the movement of horse owners and other people on yards to minimise the risk of COVID-19 transmission. During this time, LYO/Ms reported prioritising equine wellbeing by limiting change to equine routines and management wherever possible. Instead of altering equine management, there was an expectation that the lives of humans would be moulded and re-shaped to fit with the government COVID-19 guidelines. These results highlight the importance of routines, traditions and cultures in each individual yard. Simple Summary: The COVID-19 pandemic has exerted a substantial impact on small-scale broiler production systems in developing countries and put their supply chains at risk of disruption. Drawing on a survey of 205 small-scale commercial broiler farms (SCBFs) in Egypt, this study identifies the primary pathways through which the pandemic has affected these farms and investigates the determinants of their perception of COVID-19 effects. The empirical results revealed that the pandemic affected SCBFs heterogeneously based on their management and production systems and resource endowment. In particular, individually owned farms and those with membership of poultry producer organizations and larger total asset values perceived significantly fewer effects. In addition, SCBFs operating in both local and provincial markets were less likely to perceive negative effects from the pandemic. Despite that the adoption of strict containment measures was essential for protecting public health, our results indicate that policy responses to COVID-19 must consider the likely effects on small businesses such as SCBFs since disruptions to such socioeconomically important supply chains will intensify human suffering from the pandemic. These findings of our study provide important implications for enhancing the preparedness and resilience of small-scale broiler production systems in developing countries to future pandemics and natural hazards. As in many other countries, the outbreak of the COVID-19 pandemic, together with subsequent government containment measures, posed significant challenges to small-scale broiler production systems in Egypt. Based on a survey of 205 specialist small-scale commercial broiler farms (SCBFs) consisting of both farm-based and household-based production systems, this study identifies the primary pathways through which COVID-19 has affected SCBFs and investigates the determinants of farm perception of these effects. Next, five ordered logit models were constructed to examine the determinants of the SCBFs' perception of each category of these effects. Generally, the empirical results revealed that COVID-19 affected SCBFs heterogeneously based on their management and production systems and resource endowment. Female-led and household-based SCBFs perceived significantly greater COVID-19 effects. In contrast, individually owned farms and those with membership of poultry producer organizations and larger total asset values perceived fewer effects. In addition, SCBFs operating in both local and provincial markets were less likely to perceive negative effects from the pandemic on their broiler farming activities. Although the adoption of strict and immediate containment measures was essential for controlling the virus and protecting public health, our results indicate that policy responses to COVID-19 must consider the likely effects on small businesses such as SCBFs since disruptions to such socioeconomically important supply chains will intensify human suffering from the pandemic. Simple Summary: COVID-19 caused unprecedented lifestyle changes, with unknown effects on pets' quality of life. We studied the role of personality, the human–animal relationship, COVID-19-related factors, and demographics on feline and canine quality of life (QoL). We used the novel Milan Pet Quality of Life instrument (MPQL), derived from previous scales, which summarises four QoL domains: physical, psychological, social, and environmental. Our findings indicate that pets' demographics, life experience, and personality could explain a large part of the physical QoL. Conversely, the owners' demographics, COVID-19-related changes, and the owners' personality mostly explained the pets' psychological QoL. Predictably, outdoor access in the home environment and the country of residence mostly explained the pets' environmental QoL. Finally, the pets' social QoL was explained by all previous aspects: pets' characteristics and personality, environment and COVID-19-related changes, and the pet–human relationship. We suggest that these results may be explained by the effects of the COVID-19 pandemic on the owners' psychological state and lifestyle, which in turn alter the way the owners interact with their pets and look after them. However, the owners' personality and mood may also influence the way they interpret and report their pets' behavior and emotional state. These findings highlight the importance of the One Health–One Welfare perspective. The COVID-19 pandemic caused lifestyle changes, with unknown effect on pets' quality of life (QoL). Between May and July 2020, we distributed an online survey to investigate the role of several factors on feline and canine QoL, including lockdown-related factors. We used existing scales to measure human and pets' personalities (Reinforcement Sensitivity Theory Personality Questionnaire, RST-PQ; RST-Dog; RST-Cat) and the human–animal relationship (Lexington Attachment to Pets Scale, LAPS) and the Milan Pet Quality of Life instrument (MPQL). Overall, 235 participants reported about 242 adult pets (Ncats = 78, Ndogs = 164). Factor analysis confirmed the structure and internal reliability of the existing scales (RST-PQ, RST-Dog, RST-Cat, LAPS) and suggested a four-factor structure for the MPQL (physical, psychological, social, environmental). The results indicate that the pets' psysical QoL was largely explained by pet-related elements (pets' demographics and life experience, and pets' personality). Conversely, the pets' psychological QoL was explained mostly by owner-related elements, such as the owners' demographics, COVID-19-related changes, and the owners' personality. Predictably, the pets' environmental QoL is mostly explained by environmental factors, such as the outdoor access in the home environment and the country. Finally, the pets' social QoL was explained by the larger combination of models: pets' characteristics and personality, environment and COVID-19-related changes, and the pet–human relationship. These findings can be explained by two non-mutually exclusive mechanisms. The reported changes may be a by-product of the COVID-19 pandemic's psychological and lifestyle effects on the owners, which in turn alter the way the owners interact with their pets and look after them. Simple Summary: On 23 March 2020, the UK Government introduced its first nationwide lockdown as part of efforts to reduce the impact of COVID-19. There have since been two more. "Lockdown" control measures meant both racehorse trainers and veterinary surgeons (vets) had to make changes to the way they worked. Beneficial practices which aided veterinary care have been reported anecdotally, as has an increase in the use of electronic communication and information technologies. The aim of this study was to investigate if these claims could be supported by exploring any changes to racehorse veterinary care that occurred due to the implementation of the first COVID-19 lockdown restrictions. Data collection involved carrying out ten semistructured interviews with racehorse trainers and ten equine vets from November 2020 to January 2021. Reflexive thematic analysis was used to try and make sense of how vets and trainers interacted with each other before and during a period of rapid change and how both groups found alternative ways to ensure beneficial veterinary care was not compromised. Data Analysis revealed four themes threading through the data. These were, firstly, the trainer–vet relationship is built upon a good working relationship, secondly, there had been little or no change in the vet–trainer relationship during the first "lockdown" period. Thirdly, when COVID-19 restrictions were in force, more remote consultations took place using images or videos as well as telephone consults, and the fourth and final theme identified the way connectivity and poor-quality images and videos limited their effectiveness. In March 2020, the World Health Organisation called for countries to take urgent and aggressive action against a global pandemic caused by COVID-19. Restrictions were introduced in many countries to reduce transmission of COVID-19 and ultimately deaths. Such restrictions have been colloquially referred to as "lockdown". Anecdotal evidence of the beneficial practices that facilitated safe veterinary treatment and equine care had been reported together with an increase in the use of electronic communication and information technologies during the first "lockdown". Thus, the aim of this qualitative study was to capture any beneficial changes to racehorse veterinary care that were implemented during the first "lockdown" period in the UK that lasted from 23 March to 12 May 2020. Ten equine veterinary surgeons who primarily treat racehorses and 10 racehorse trainers were interviewed either by telephone or by videoconferencing. After using thematic analysis from a critical realist social constructionist perspective, four themes were identified. These were, firstly, according to our participants, the trainer–vet relationship is predicated upon a good working relationship, secondly, there had been little or no change in the vet–trainer relationship during the first "lockdown" period. Thirdly, when COVID-19 restrictions were in force, more remote consultations took place using images or videos as well as telephone consults, viewed favourably by both trainers and vets, and finally, intermittent connectivity and poor-quality images and videos limited their effectiveness. Simple Summary: COVID-19 spread across the UK in early 2020. On 16 March 2020, the government announced measures that could be taken by the public to reduce the spread of the virus. On the 18 March 2020 schools across the UK were closed and by 23rd March 2020, the UK entered its first mandatory lockdown. During this time, UK equestrians had to balance meeting their horse's welfare needs with government advice and guidance. A survey was distributed to equine owners at the end of March 2020 via social media to try to understand their equine-related decision-making at this time. Data from 452 respondents indicated differences between the types of changes made following the government announcements on 16 March and 18 March. Early changes were focused on implementing biosecurity measures at equine establishments and opting to stop riding the horse, while those after 18 March were largely aimed at reducing the risk associated with various equine-related activities. The findings suggest that few equestrians had emergency plans in place should they be unable to care for their horses, donkeys or mules, and also raise questions about risk perception. In the UK, March 2020 was a time of great uncertainty as COVID-19 became increasingly widespread. The government responded by making suggestions about how people could reduce the risk of spread on 16 March, moved swiftly into closing schools on the 18 March before announcing a mandatory lockdown on the 23rd March. This was a challenging time for UK equestrians who had to balance maintaining their equine's routine and daily care alongside the increasing biosecurity measures. A cross-sectional survey was distributed to UK equestrians via social media over two days (28 and 29 March 2020) to better understand the decisions made by UK horse, pony and donkey owners during this time. Data from 452 respondents were generated across all four countries comprising the UK, although there were no significant differences in owner response to the pandemic between locations. The changes respondents made differed between the 16th and the 18th of March 2020, with an early emphasis on improving yard biosecurity and opting to stop riding, as well as reducing the time spent at the yard. After the 18 March, respondents placed more emphasis on risk reduction by changing the activities they did with their horse, including riding, with common examples including avoiding "high risk" activities such as riding on busy roads, jumping, riding young or nervous horses. Few respondents reported having an emergency plan in place should they become ill or otherwise unable to care for their equine. The findings highlight areas that would significantly benefit from in-depth investigation in future research. Equestrian behaviour and mindset around risk-taking and risk perception have already been researched in relation to equestrian activities and sport but have received little attention in the context of wider health challenges. Understanding the uptake of emergency planning and preparation in the UK equestrian community also warrants consideration. Simple Summary: COVID-19 is the disease caused by a coronavirus: SARS-CoV-2. The disease was declared by WHO in March 2020 as a pandemic and is still affecting countries around the world. Although considered a human disease, it is thought to have its origins in bats, and transmitted to humans potentially through an intermediate animal. What is not fully understood is the likelihood of the virus transmitting back to animal populations, either to companion or wild animals. For the virus to enter host cells, it needs to interact with particular proteins on the cell surface. Here we review the susceptibility of animals to the SARS-CoV-2 virus through the comparison of proteins between animals and humans. The most studied is the angiotensin-converting enzyme 2 receptor (ACE2), which has been used to rank the viral susceptibility of a range of vertebrates. Here, we also assess three other proteins. Of these, TMPRSS2 may be helpful in determining susceptibility, whereas the other two would appear to be of limited use. We propose that future work should examine changes seen in these proteins which alter the ease by which the virus can enter cells. This type of analysis may contribute limited evidence in predicting if animals are safe from such viruses and may help to guide future welfare concerns. COVID-19, caused by SARS-CoV-2, is a world-wide problem for the human population. It is known that some animal species, such as mink, can become infected and transmit the virus. However, the susceptibility of most animals is not known. Here, we review the use of sequence analysis of the proteins which are known to interact with SARS-CoV-2 as a way to estimate an animal's susceptibility. Although most such work concentrates on the angiotensin-converting enzyme 2 receptor (ACE2), here TMPRSS2 (Transmembrane Serine Protease 2), neuropilin-1 and furin are also considered. Polymorphisms, especially ones which are known to alter viral/host interactions are also discussed. Analysis of ACE2 and TMPRSS2 protein sequences across species suggests this approach may be of some utility in predicting susceptibility; however, this analysis fails to highlight some susceptible animals such as mink. However, combined with observational data which emerges over time about which animals actually become infected, this may, in the future, be a useful tool to assist the management of risks associated with human/animal contact and support conservation and animal welfare measures. Simple Summary: This investigation explored the impact of the COVID-19 pandemic lockdown and following alert levels on pets in New Zealand. Pet owners were surveyed during the last week of the first Alert Level 4 lockdown (highest level of restrictions) and then three months later during Alert Level 1 (lowest level of restrictions). During lockdown, just over half of those surveyed thought that their pet's wellbeing was better than usual, and most owners could list at least one benefit of lockdown for their pets. These included more company, play and exercise. Owners expressed that they were concerned about their pet's wellbeing after lockdown, with pets missing company/attention and separation anxiety being major themes. The Alert Level 1 survey indicated that owners continued to play with their pets more but that higher levels of exercise were not maintained. Just over one-third of owners took steps to prepare their pets to transition out of lockdown. The results indicate that pets may have enjoyed improved welfare during lockdown due to the possibility of increased human-pet interaction. The steps taken by owners to prepare animals for a return to normal life may enhance pet wellbeing long-term if maintained. The influence of the COVID-19 pandemic on human-pet interactions within New Zealand, particularly during lockdown, was investigated via two national surveys. In Survey 1, pet owners (n = 686) responded during the final week of the five-week Alert Level 4 lockdown (highest level of restrictions—April 2020), and survey 2 involved 498 respondents during July 2020 whilst at Alert Level 1 (lowest level of restrictions). Simple Summary: The increase in the heart rate and various heart parameters play an important role in assessing the fitness of sport horses. The fitness of a horse, that particular horse's competition routine, and the resulting lower stress affect various cardiac parameters. The aim of this study was to examine the effect of the phases of competition (warm-up, resting period, show jumping course riding, cool-down) and the difficulty of a course (100, 120, 130 cm) on the heart rate and selected heart rate variability parameters of show jumping horses. The heart rate was monitored with a "Polar Equine heart rate monitor" before, during, and after a show jumping course was completed. Neither analysis of the average heart rate nor that of the maximum heart rate were able to detect a statistically proven difference among competition levels. In contrast, according to heart rate variability measures, such as maximum RR intervals, SD1, RMSSD, pNN50, and %VLF picked up differences in workload level. It has been confirmed that lower-class show jumping (up to 120 cm height) is not a strenuous exercise for horses. Heart rate is one of the gold standards used to assess the workload level and fitness of horses. However, when slight differences need to be detected, it is not sensitive enough. Therefore, the aim of this study was to test the effect of competition level and phase of exercise on the heart rate and heart rate variability parameters in show jumpers. Fourteen horses were examined competing on three different levels: 100 cm (n = 4), 120 cm (n = 6), and 130 cm (n = 4). The length of work (min); average and maximum heart rate; average, maximum and minimum RR intervals (ms); SD1 and SD2 (ms); RMSSD (ms) and pNN50 (%); VLF, LF, HF (%) were analyzed. The measurement was divided into four phases: warm-up, resting period, show jumping course riding, and cool-down. The level of the course had no significant effect on average and maximum heart rates throughout the entire exercise. The maximum RR interval, RMSSD, pNN50, SD1, and %VLF values were significantly different (p < 0.05) in horses competing at 100 cm height from those competing in the 120 cm group. The SD1 value was sensitive for the level of competition, while the SD2 parameter was sensitive for detecting exercise phases. The global disconnect between the Sustainable Development Goals (SDGs) and the Convention on the Rights of the Child (CRC), has been described as 'a missed opportunity'. Since devolution, the Welsh Government has actively pursued a 'sustainable development' and a 'children's rights' agenda. However, until recently, these separate agendas also did not contribute to each other, although they culminated in two radical and innovative pieces of legislation; the Rights of Children and Young Persons (Wales) Measure (2013) and the Well-being and Future Generations (Wales) Act (2015). This article offers a case study that draws upon the SDGs and the CRC and considers how recent guidance to Welsh public bodies for implementation attempts to contribute to a more integrated approach. It suggests that successful integration requires recognition of the importance of including children in deliberative processes, using both formal mechanisms, such as local authority youth forums, pupil councils and a national youth parliament, and informal mechanisms, such as child-led research, that enable children to initiate and influence sustainable change. Water treadmill exercise sessions can be tailored to the individual horse and the training/rehabilitation goals by altering the frequency, duration of exercise, water depth and belt speed. Recent work suggests that there are large variations in current modes of use between users, despite shared training or rehabilitation goals. In 2019, a group of researchers and experienced water treadmill users met in the UK to establish what was commonly considered to be best practice in the use of the modality. The result of these discussions was the production of 'Water treadmill guidelines—a guide for users', released in 2020 via various equestrian websites. The purpose of this article is to describe the development of these guidelines and propose them as a starting point for further collaboration between researchers and practitioners in the pursuit of 'best practice' in water treadmill exercise for horses. Water treadmill exercise has become popular in recent years for the training and rehabilitation of equine athletes. In 2019, an equine hydrotherapy working group was formed to establish what was commonly considered to be best practice in the use of the modality. This article describes the process by which general guidelines for the application of water treadmill exercise in training and rehabilitation programmes were produced by the working group. The guidelines describe the consensus reached to date on (1) the potential benefits of water treadmill exercise, (2) general good practice in water treadmill exercise, (3) introduction of horses to the exercise, (4) factors influencing selection of belt speed, water depth and duration of exercise, and (5) monitoring movement on the water treadmill. The long-term goal is to reach a consensus on the optimal use of the modality within a training or rehabilitation programme. Collaboration between clinicians, researchers and experienced users is needed to develop research programmes and further guidelines regarding the most appropriate application of the modality for specific veterinary conditions. Simple Summary: Within rehabilitation, measurements taken before, during and after treatments are used to judge patient progress and the effectiveness of prescribed treatments. To know which measurements to use for a given health condition, practitioners must have knowledge of what should be measured, which measurement tools are available and accurate, alongside what they intend to measure. Composite outcome measures (OMs) are tools which use grouped measurement tests to monitor patient progress; they have been tested for a variety of human and canine conditions but none have been designed or tested for use in physical rehabilitation in horses. This study asked leading equine veterinarians, physiotherapists and researchers which measures should be included in an OM for use in the rehabilitation of horses. Using a process to evaluate agreement, ten areas of measurement were included in the final model: lameness, pain at rest, pain during exercise, behaviour during exercise, muscular symmetry, performance/functional capacity, behaviour at rest, palpation, balance and proprioception. Existing reliable tests used to measure these areas were evaluated and potential new measures discussed and now should be taken forward to testing as a composite outcome score to see if they are effective in measuring effectiveness of treatment. Outcome measures (OMs) are a requirement of professional practice standards in human and canine physiotherapy practice for measurement of health status. Measures such as pain and functional capacity of specific regions are used to track treatment impact and can be used to develop optimal management strategies. To achieve comparable patient care in equine physiotherapy, OMs must be incorporated into practice; however, no reliable and valid OMs exist for equine rehabilitation. This study utilised the experience and opinion of a panel of experts working in the equine rehabilitation sphere to gain consensus on the core areas (domains) to be included in a model, to lead to an OM scale for horses undergoing rehabilitation. The Delphi method and content validity ratio testing was used to determine agreement with domains reaching the critical value required for inclusion. The expert panel agreed on ten domains to be included in the OM scale: lameness, pain at rest, pain during exercise, behaviour during exercise, muscular symmetry, performance/functional capacity, behaviour at rest, palpation, balance and proprioception. Simple Summary: Despite the increased demand for breeding stallions to be performing at elite levels of competition, little research has been conducted into how equestrian disciplines and competition level affect seminal quality. Using statistical analysis, we found that competing stallions have lower quality semen than non-competing stallions and competitive activity may have a greater negative impact on spermatogenesis than age. In addition, dressage stallions recorded improved semen characteristics when compared to show jumping and eventing stallions. Stallions at elite levels of competition recorded higher total sperm count and progressive motility compared to those competing at lower levels of competition. We suggest that appropriate management of the competition stallion may reduce the negative impacts which exercise can induce on semen quality. Most stallions within breeding programmes are expected to breed and compete concurrently. The exercising of stallions with regards to training regimes during the breeding season is a controversial subject. Daily exercise at low intensities is important for the mental and reproductive well-being of the stallion, however higher intensities of exercise, as seen in competing stallions, may have detrimental effects on seminal quality. To calculate if competition does affect semen quality, this study investigated the effect that equestrian discipline and timing of competition had on a range of stallion semen characteristics. This was a retrospective study that evaluated the seminal data of 1130 stallion semen collections from two UK based stud farms between 2009 and 2016. Competing stallion semen quality was significantly lower with regards to concentration (p < 0.05) and progressive motility (p < 0.05) than non-competing stallions. Semen volume was higher in competing stallions (p < 0.05) than non-competing stallions. There was a significant difference in seminal attributes among disciplines and competition levels (p < 0.05). The difference in semen quality among competing and non-competing stallions, as well as the difference among disciplines suggests endocrinological and physiological changes occur in relation to training intensity and competition. Background: Health services are increasingly focused on measuring and monitoring outcomes, particularly those that reflect patients' priorities. To be meaningful, outcomes measured should be valued by patients and carers, be consistent with what health professionals seek to achieve, and be robust in terms of measurement properties. The aim of this study was (i) to seek a shared vision between families and clinicians regarding key aspects of health as outcomes, beyond mortality and morbidity, for children with neurodisability, and (ii) to appraise which multidimensional patient reported outcome measures (PROMs) could be used to assess salient health domains.Methods: Relevant outcomes were identified from (i) qualitative research with children and young people with neurodisability and parent carers, (ii) Delphi survey with health professionals, and (iii) systematic review of PROMs. The International Classification of Functioning Disability and Health provided a common language to code aspects of health. A subset of stakeholders participated in a prioritisation meeting incorporating a Q-sorting task to discuss and rank aspects of health.Results: A total of 33 pertinent aspects of health were identified. Fifteen stakeholders from the qualitative and Delphi studies participated in the prioritisation meeting: 3 young people, 5 parent carers, and 7 health professionals. Aspects of health that emerged as more important for families and targets for health professionals were: communication, emotional wellbeing, pain, sleep, mobility, self-care, independence, mental health, community and social life, behaviour, toileting and safety. Whilst available PROMs measure many aspects of health in the ICF, no single PROM captures all the key domains prioritised as for children and young people with neurodisability. The paucity of scales for assessing communication was notable.Conclusions: We propose a core suite of key outcome domains for children with neurodisability that could be used in evaluative research, audit and as health service performance indicators. Future work could appraise domain-specific PROMs for these aspects of health; a single measure assessing the key aspects of health that could be applied across paediatric neurodisability remains to be developed.
