The purpose of this discussion paper is to explore how nurses can be strategically poised to advocate for needed policy change in support of greater income equality and other social determinants of health. We adapted Bronfenbrenner's social‐ecological model to highlight how four broad pervasive subsystems shape the opportunities that nurses have to engage in advocacy at the policy level. These subsystems include organizations (the microsystem), professional bodies (the mesosystem), public policies (the exosystem), and societal values (the macrosystem). On the basis of this adapted model, we recommend changes among modifiable elements of the microsystem and mesosystem that can help position nurses (ecologically and collectively) to advocate for public policy change and use examples from a Canadian context to illustrate these points. We believe that the ideas arising from this model can be widely used where policy action on the social determinants of health is needed to inform, guide, and frame change efforts and advocacy work. This paper examines how organisations have increasingly been portrayed in textbooks as solving social problems as well as contributing to national development. Findings from 527 Canadian and U.S. textbooks illustrate the rise of an organisational society during the time period between 1836 and 2011. Discussions of for-profit and non-profit forms of organisations rise early on in both countries, creating the foundation for an organisational society, which expands to incorporate global organisations in the post-World War II period. We argue that such portrayals in textbooks both reflect and legitimise the role of organisations in society, strengthening their taken-for-granted status as social actors. [ABSTRACT FROM AUTHOR]

Summary: A variety of hip protectors are available, but it is not clear which is the most effective and there is no standard test to evaluate their performance. This is the first study that uses a standard mechanical test on hip protectors. Some protectors perform well but others are almost ineffective, providing little to no protection to the wearer during a fall. Introduction: Each year, over 70,000 patients are admitted to hospital in the UK with hip fractures. There are a variety of commercial hip protectors currently available. However, it is not explicitly clear which is the most effective with regard to maximum force attenuation, whilst still being both comfortable for the user and providing reasonable force reduction if misplaced from the intended position. The numerous test methods reported in the literature have given conflicting results, making objective comparison difficult for users, researchers, and manufacturers alike. The Canadian Standards Association (CSA) has therefore published an express document (EXP-08-17) with a draft standard test method. This paper presents initial results for a range of hip protectors. Methods: Eighteen commercially available hip protectors were tested according to EXP-08-17. Each hip protector was impacted five times in correct anatomical alignment over the greater trochanter and once at 50 mm displacements in the anterior, posterior, and lateral directions. Results: Considerable differences were identified between individual hip protectors in their ability to reduce impact forces on the femur (between 3% and 36% reduction in peak force). The performance was reduced when misplaced in many cases (maximum reduction only 20%). Conclusions: This is the first study that uses a standard mechanical test on hip protectors. Previous studies have used a variety of methods, making it difficult to interpret results. We hope that these results using a standard test method will facilitate the effective comparison of results, as well as providing useful data for clinicians, users, and purchasers. Building information modeling (BIM) is emerging as a potential solution for facility owners to address the challenges of poor information fidelity, interoperability, and usability in project delivery to support the lifecycle of their assets' information. Despite the many benefits offered by BIM, its use for facility operations remains significantly limited. The reality is that implementing BIM in large owner organisations is a complex challenge. In particular, a significant barrier to BIM adoption for owners is the challenge of identifying and formalizing the information requirements needed to support model-based project delivery and asset management. This paper presents the results of a longitudinal research project that investigated two large owner organisations in Canada to better understand the process of developing and formulating BIM requirements to support the lifecycle of their assets. Specifically, the objectives were to formalize an iterative approach to the identification and characterization of owner requirements and to develop a conceptual framework that would relate digital and physical products to owner requirements and organisational constructs, to underpin the formalization process. As part of this research an array of requirements documentation were analysed, interviews were performed with numerous facility management personnel, and BIMs from four projects were analysed. A methodology is introduced to support a rigorous and detailed analysis of BIM requirements. The investigation of the owner requirements helped to develop an understanding of the required information content, and its alignment with BIM. Finally the relationships between organisational constructs, owner requirements, and BIM were mapped. As the construction industry shifts towards model-based project delivery, this research will inform owners about how to think about handover of digital facility models, and what to require in models based on their specific needs. In 1978 the Canadian Chiropractic Association recognized the need to establish an organization that would prepare chiropractors to treat athletic injuries and promote these services to sports organizations. Dr. Adrian Grice approached three chiropractors to establish such an organization. The Canadian Chiropractic Sports Academy (CCSA) was established in 1978. This was the start of the chiropractic sports movement which has seen chiropractors playing prominent roles as team doctors to professional and amateur teams and athletes and in the delivery of care at major national and international competitions. This paper will show the work done by the original founders of the CCS A which has helped to pave the way to the present level of acceptance of chiropractic sports injury management and performance enhancement and as the progenitor of the Royal College of Chiropractic Sports Sciences Canada. The significant role of the community in the lives of children and youth at-risk has become increasingly clear to social work academics and professionals over the last three decades. Alongside the more traditional individual and family responses, community interventions have been designed to catalyze change in the environment of children and youth at-risk and supply holistic and sustainable responses to their needs. Ten such community intervention programs were identified from the United States, Australia, Canada, and Israel. Most employed the community development model, focused on developing leadership and social capital (improving community networking) and advancing coordination between the organizations and sectors in the field of risk among children and youth. The diverse programs reviewed focused both on at-risk children and youth in general or specifically on child abuse and neglect. The programs originated from different health, education, and welfare disciplines and sponsoring authorities. The majority were funded originally by private foundations; however, government involvement was significant, particularly in the adoption and support of initiatives after their development. The current analysis of the programs refers to core issues that arose from the review: professional orientation, main target unit, main initiator, and research and evaluation. Analysis of program characteristics enables identifying relevant aspects of these programs for use by policy, governmental, and nonprofit sector stakeholders seeking to develop similar programs. Conclusions and recommendations to advance the field are suggested considering the current context of government cuts in welfare funds. [ABSTRACT FROM AUTHOR]

Background: Small and medium sized enterprises are yet uncharted territory in terms of presenteeism. In addition, the Demand-Control-Support (DCS) and Siegrist's Effort-Reward Imbalance (ERI) models are proposed to predict stress-related health problems, but not for sickness behaviors such as presenteeism. Objective: This study aims to examine the relationships between psychosocial risk factors, presenteeism, mental and physical health in the context of small and medium-sized enterprises (SMEs). This study also examines the moderating effect of company size on these associations. Methods: To test the association between psychosocial risks, presenteeism, and health, only people working in small and medium-sized enterprises (SMEs) of between 2 and 199 employees were included in the sample, giving a total of 2,525 SME employees surveyed. To test the differences in exposure to psychosocial risk and presenteeism, and the moderating impact of size on the relationship between psychosocial risks, presenteeism, and health, we took the original sample (4608) of the EQCOTESST. Results: The results confirm the associations between job demands, social support and effort-reward imbalance, and presenteeism. Also, the associations between presenteeism and health problems in SMEs' context. Multi-group analyses show that the business's size does not moderate the strength of the relationships between psychosocial risks, presenteeism and health. Conclusion: The current study highlights that SMEs are somehow protected from certain psychosocial constraints such as high job demands, and low social support, but are more exposed to others such as effort-reward imbalance. Mental health researchers and practitioners alike have recognized that there are special considerations and challenges involved in diagnosing and treating mental illnesses in athletes. However, very few clinical psychologists and psychiatrists in Canada specialize in sport, representing a significant gap in mental health care service provision for this population. In this study, a group of expert sport and mental health stakeholders (n = 17) employed a Participatory Action Research approach to design a specialized sport-focused mental health care model integrated within the Canadian Centre for Mental Health in Sport (CCMHS). Stakeholders engaged in focus group discussions to perform an environmental scan of the Canadian sport and mental health care contexts that laid the foundation for a Group Concept Mapping (GCM) exercise. Using the Concepts Systems software, stakeholders individually produced statements that described the elements to include in a sport-specific mental health care model implemented within the CCMHS. A total of 106 unique statements were organized into 6 themed clusters, focusing on: (1) service delivery [40 statements], (2) communications and promotion [20 statements], (3) business, policy, and operations [19 statements], (4) partnerships [9 statements], (5) research [6 statements], and (6) education and training [6 statements]. These findings were operationalized to establish a sport-centered mental health care model and the CCMHS itself - the first Centre of its kind in Canada. GCM is seldom used to conduct sport research, thus the validity and reliability of this methodology was assessed. Lay Summary: In this study, 17 sport and mental health expert stakeholders participated in group concept mapping to design a sport-focused mental health care delivery model. Despite increasing interest in child sexual abuse occurring in organisations, the perpetration of such abuse by females is largely ignored. This study examined situational factors in 136 cases of sexual abuse perpetrated by women working with children in the UK, Canada and the USA between 2000 and 2016. Qualitative and quantitative content analysis of court reports, professional regulator decisions, media reports and an online sentencing database was used, findings indicating that situational and contextual factors are highly relevant in perpetration. Much abuse occurs away from the organisational environment, particularly in perpetrators' homes and cars, and in virtual environments. However, it also occurs within organisations, generally in unsupervised areas, outside of operating hours and often during mentoring/tutoring or extra-curricular activities. Organisational and local culture can be a facilitator in this abuse and allow it to continue even when concerns are raised. Rationale, aims, and objectives: Clinical practice guidelines (CPGs) endeavour to incorporate the best available research evidence together with the clinically informed opinions of leading experts in order to guide clinical practice when dealing with a given condition. There has been increased interest in CPGs that are evidence based and that promote best practice, a central component of which is incorporating the best available research predicated on strong study designs. Despite this soaring interest, there remains heterogeneity in the methodological quality of many CPGs, which may have an effect on the quality of services that clinicians offer. In light of this, this study examined the quality of the methodology used to develop two CPGs of the Canadian Psychiatric Association (CPA). Method: The CPA's guidelines for the management of anxiety disorders (2006) and for the treatment of depressive disorders (2001) were assessed by trained raters using the Appraisal of Guidelines for Research and Evaluation II Instrument scale. Results: The blind ratings of three trained raters demonstrated that the anxiety and depression CPGs had a number of strengths and important weaknesses. Conclusion: Implications for the development of future CPGs on anxiety and depression, including recommendations to improve guideline quality in psychiatry in particular, are discussed. Voluntary self-exclusion (VSE) programs enable problem gamblers to engage in a break from casino-based gambling. The current study analyzed the effects of a VSE program in British Columbia, Canada on problem gambling symptoms and the comparative reductions in problem gambling symptoms when participants abstained from gambling, continued to participate in non-casino based gambling, or attempted to violate their exclusion contract. 269 participants completed two telephone interviews over a 6-month period. Participants were administered the Problem Gambling Severity Index (PGSI). Substantial reductions in PGSI scores were observed after 6 months. Program violators had significantly smaller PGSI Difference Scores by Time 2 compared to those who continued to gamble outside of the casino and those who completely abstained from all gambling. There were no significant differences between those who gambled informally and those who abstained. A multiple regression identified that while access to counselling and length of enrollment also contributed to the reduction in PGSI scores, violation attempts were most strongly associated with smaller reductions in symptoms of problem gambling. These results imply that some gamblers can successfully engage in non-casino based forms of gambling and still experience reductions in symptoms of problem gambling. Future analyses will explore characteristics associated with group membership that may help to identify which participants can successfully engage in non-casino based gambling without re-triggering symptoms of problem gambling. The article presents a study which aims to map the social organisation of health care in Toronto, Ontario, with particular focus on impact of literacy, or the lack of literacy, on access to health care by homeless individuals. Topics include connection between literacy and overall health; experiences of individuals who visited the homeless shelter Open Access in Toronto; and use of Brazilian educator Paulo Freire's critical standpoint which acknowledges interconnection of knowledge and power.

Background: The Eat Well Campaign (EWC) was a social marketing campaign developed by Health Canada and disseminated to the public with the help of cross-sector partners. The purpose of this study was to describe factors that influenced cross-sector partners' decision to adopt the EWC.Methods: Thematic content analysis, based primarily on an a priori codebook of constructs from Roger's diffusion of innovations decision process model, was conducted on hour-long semi-structured telephone interviews with Health Canada's cross-sector partners (n = 18).Results: Dominant themes influencing cross-sector partners' decision to adopt the EWC were: high compatibility with the organization's values; being associated with Health Canada; and low perceived complexity of activities. Several adopters indicated that social norms (e.g., knowing that other organizations in their network were involved in the collaboration) played a strong role in their decision to participate, particularly for food retailers and small organizations. The opportunity itself to work in partnership with Health Canada and other organizations was seen as a prominent relative advantage by many organizations. Adopters were characterized as having high social participation and positive attitudes towards health, new ideas and Health Canada. The lack of exposure to the mass media channels used to diffuse the campaign and reserved attitudes towards Health Canada were prominent obstacles identified by a minority of health organizations, which challenged the decision to adopt the EWC. Most other barriers were considered as minor challenges and did not appear to impede the adoption process.Conclusions: Understanding factors that influence cross-sector adoption of nutrition initiatives can help decision makers target the most appropriate partners to advance public health objectives. Government health agencies are likely to find strong partners in organizations that share the same values as the initiative, have positive attitudes towards health, are extremely implicated in social causes and value the notion of partnership. Background: Health systems globally promote appropriate prescribing by healthcare providers and safe and effective medicine use by consumers. Rx for Change, a publicly available database, provides access to systematic reviews regarding best practices for prescribing and using medicines. Despite the value of the database for improving prescribing and medicine use, its use remains suboptimal. This study aimed to develop a training program for five medicine-focused organisations in Canada and Australia to facilitate the use and understanding of the Rx for Change database.Methods: Four steps were undertaken: 1) key informant interviews were completed across all organisations to understand the knowledge user perspective; 2) a directed content analysis was completed of the interview transcripts and proposed training was developed; 3) a second round of feedback on the proposed training by knowledge users was gathered; and 4) feedback was integrated to develop the final training.Results: Sixteen key informant interviews with knowledge users were conducted. Themes for training content included the scope of, navigation and strategies for using Rx for Change (generic content) and practical examples on incorporating evidence within their workplace context (tailored content). The final training consisted of an informational video, a 60-minute face-to-face workshop and two post-training reminders.Conclusions: A method of engaging knowledge users in the development of a training program to improve the use of an on-line database of systematic reviews was established and used to design training. An essay is presented on the history of Friendship Centres, social and cultural institutions which provided social services to Aboriginal peoples in Canada, during the 1950s, 1960s, and 1970s. Particular focus is given to the politics and ideology of the centres, debates over their practices, and their relationship to public policy. According to the author, the institutional theme of friendship carried the potential for coalition-building. Other topics include social integration, racism, and imperialism.

The article presents a report by an international expert panel on peer review convened by the Canadian Institutes of Health Research (CIHR), on face-to-face peer review. Topics discussed include views of Dr. Jim Woodgett, director of research at the Lunenfeld-Tanenbaum Research Institute in Toronto on same; use of face-to-face review for rebuild people's confidence; and views of Matthew Herder, director of the Health Law Institute on same.
