Tuberculosis (TB) of the thyroid gland is extremely rare, even in areas where the prevalence of TB is very high. Only a few such cases have been reported in the literature. The clinical course may resemble toxic goiter or acute thyroiditis. We report the case of a 23-year-old male who presented with painless swelling of the thyroid gland associated with mild fever and weight loss. On ultrasound neck, a large echogenic solid lesion was seen. Fine-needle aspiration cytology was suggestive of colloid goiter, but histopathology study suggested TB of the thyroid gland along with colloid goiter. We report this case to emphasize that TB of the thyroid gland, although rare, should be kept in the differential diagnosis of thyroid nodules, especially in countries such as India which are endemic for TB. Background: This study aimed to evaluate the characteristics of incidental findings (IFs) on computed tomography (CT) scans of trauma admissions, examine associations between IFs and gender and age-groups, and discuss the management strategies. Methods: The CT reports were retrospectively reviewed to evaluate IFs. Cases were divided into five age-groups (0--19, 20--39, 40--59, 60--79, ≥ 80). IFs were classified as "Group 1": congenital anomalies that do not require further investigation, nondegenerative/minor degenerative findings; "Group 2": findings that do not require immediate intervention, require outpatient follow-up/in case of symptoms; and "Group 3": findings that require immediate intervention/further investigation. Results: There were 2385 CT scans and 1802 incidental findings (IFs) in 783 trauma cases. CT scans with IFs constituted 50.2%. The percentage of IFs was 75.6% in males and 24.4% in females, and they occurred in 4.8%, 27.6%, 44.3%, 20.9%, and 2.4% of age groups 1 to 5, respectively. Group 1 had 34.6%, group 2 had 54.6%, and group 3 had 10.8% IFs. There was not any significant association between the classification and gender or age-groups. In terms of organs, IFs of the thyroid and gall bladder & bile ducts were significantly higher among females (P = 0.044 and P < 0.001, respectively), while IFs in the head & neck region were significantly higher in males (P < 0.001). Incidental findings in the kidney, liver, adrenal gland, and vascular structures differed significantly across age-groups (P < 0.05). Conclusion: There were no significant relationships between the classification of IFs and gender or age-groups. However, the distribution of IFs was significantly associated with gender and age-groups in terms of organs. Background: Thyroid ultrasound has undergone a dramatic transformation from the cryptic deflections on an oscilloscope produced in A-mode scanning, to barely recognizable B-mode images, followed by initial low resolution gray scale, and now modern high resolution images. Aim: The aim of the present study was to evaluate the diagnostic role of ultrasonography (USG) and multi-detector computed tomography (MDCT) in thyroid swellings, compare the ultrasonographic findings with multi-detector computed tomographic findings and to correlate the radiological findings with histopathological examination (HPE). Materials and methods: The present study was carried out in the Department of Radiology, Kamineni Institute of Medical Sciences, Narketpally. In this study, 50 patients with thyroid gland swellings diagnosed clinically, referred to Radiology Department were selected during the period from October 2008 to September 2010. Histopathological examination was acquired in 35 cases. The study was carried out to observe the sensitivity, specificity and diagnostic accuracy of USG and MDCT in thyroid gland swellings. Results: Maximum number of patients belonged to the age group of 21-40 years that was 27 cases (54.00%) and maximum number of patients were females - 41 cases (82.00%). Most common diagnosis was multi-nodular goitre on USG-16 cases (32.00%) and MDCT-14 cases (28.00%), solitary thyroid nodule is common in the right lobe of thyroid by USG-5 cases (62.50%) and MDCT-5 cases (62.50%). Most common malignancy was papillary carcinoma of thyroid-5 cases (62.50%) and most common inflammatory disorder was hashimoto's thyroiditis-5 cases (10.00%). Conclusion: The present study has concluded that USG is the fast and cost-effective modality of imaging investigation of choice in thyroid diseases and for differential diagnosis of thyroid nodules. MDCT is superior to ultrasound in evaluating retrosternal extension, relations and infiltrations in large lesions. It is also very helpful in evaluating extra-capsular, mediastinal, vascular invasion, lymph nodal involvement and metastasis. Purpose This study aims to establish normative reference values for thoracic aortic diameter (AD) in participants in the National Lung Screening Trial. Methods Thoracic AD on 322 prevalence computed tomography was recorded at the sinotubular junction, mid-ascending, transverse arch, mid-descending, and diaphragmatic hiatus. Results Mean AD (cm) and upper limits of normal for men and women were recorded for at each location. Smoking did not correlate with AD. Age, gender, and body surface area (BSA) were the most significant factors. Conclusions Thoracic AD reference values are reported. 7. Computed tomography features of benign and malignant solid thyroid nodules.

Background: No previous study has investigated computed tomography (CT) features of benign and malignant solid thyroid nodules using detailed CT features and histopathological results.Purpose: To assess CT features of benign and malignant solid thyroid nodules in patients who underwent thyroid surgery.Material and Methods: From January to April 2014, 151 consecutive patients underwent neck CT before thyroid surgery. In each case, neck CT was retrospectively examined by a single radiologist. We evaluated the diagnostic accuracy of specific CT features for differentiating malignant from benign thyroid nodules using histopathological results as a reference standard.Results: Of 173 thyroid nodules in 142 patients, 162 (mean diameter, 12.8 ± 10.3 mm; range, 5.0-93.7 mm) were visualized on neck CT. Of 162 nodules in 133 patients, 116 were malignant and 46 were benign as confirmed by histopathology. A multivariate logistic regression analysis showed a significant difference between malignant and benign thyroid nodules in the degree and pattern of nodular enhancement, but there were no significant differences in other CT features. In particular, thyroid nodules with exophytic configuration, irregular margin, taller-than-wide shape, punctate calcifications, or homogeneously decreased enhancement showed a high malignancy rate.Conclusion: The study demonstrated that the degree and pattern of nodular enhancement are helpful CT features for differentiating malignant from benign solid thyroid nodules. [Display omitted] Background and Objective: As one of the largest endocrine organs in the human body, the thyroid gland regulates daily metabolism. Early detection of thyroid disease leads to reduced mortality rates. The diagnosis of thyroid disease is usually made by radiologists and pathologists, which heavily relies on their experience and expertise. To mitigate human false-positive diagnostic rates, this paper proves that deep learning-driven techniques yield promising performance for automatic detection of thyroid diseases which offers clinicians assistance regarding diagnostic decision-making. Method: This research study is the first of its kind, which adopts two pre-operative medical image modalities for multi-classifying thyroid disease types (i.e., normal, thyroiditis, cystic, multi-nodular goiter, adenoma, and cancer). Using the current state-of-the-art performing deep convolutional neural network (CNN) architecture, this study builds a thyroid disease diagnostic model for distinguishing among the disease types. Results: The model obtains unprecedented performance for both medical image sets, and it reaches an accuracy of 0.972 and 0.942 for ultrasound images and computed tomography (CT) scans correspondingly. Conclusion: The experimental results illustrate that the selected CNN can be adapted to both image modalities, indicating the feasibility of the deep learning model and emphasizing its further applications in clinics. 9. Clinical values of of gemstone spectral CT in diagnosing thyroid disease.

OBJECTIVE: To quantitatively evaluate the diagnostic value of gemstone spectral CT in thyroid disease.PATIENTS and METHODS: A total of 123 patients with thyroid diseases were enrolled in the retrospective study. All the patients underwent spectral CT scan, and 39 of them underwent dual-phase enhanced scan. Iodine concentration (IC) and normalized IC (NIC) were compared between benign and malignant nodules. The optimal threshold to predict malignancy was obtained by receiver operating characteristic curve (ROC). Multivariate ROC analysis was performed to evaluate the efficacy of combining the IC (NIC) and conventional morphological characteristics.RESULTS: Ten diffuse diseases and 113 nodular diseases were confirmed by clinical laboratory examination and histopathology. In total, 122 nodules (87 benign and 35 malignant) were detected, 41 nodules in enhanced cases. The IC and NIC
_{VP} of malignant nodules were significantly lower than benign nodules in noncontrast and venous phase (VP) (P=0.000). The optimal IC and NIC_{VP} threshold was 0.68 mg/mL and 0.38, achieving 74.3% and 76.9% sensitivity, 66.7% and 80.0% specificity, respectively. For multivariate analysis, the area under ROC (AUC) was 0.88 and 0.94 in noncontrast and VP, respectively. Sensitivities of 77.1% and 84.6%, and specificities of 84.5% and 88.0% were achieved. CONCLUSION: Gemstone spectral CT can generally evaluate the abnormal iodine content in thyroid disease. Lower iodine can help differentiating malignant from benign nodules. A comprehensive analysis combining spectrum parameters and morphological characteristics is suggested in differential diagnosis. [ABSTRACT FROM AUTHOR]- Published

Abstract: The aim of this study was to compare the diagnostic values of thyroid ultrasound (US) and neck computed tomography (CT) in incidentally detecting diffuse thyroid disease (DTD). A single radiologist made US and CT diagnoses of incidentally detected DTD in 130 consecutive patients before thyroidectomy for various malignancies. Histopathologic examinations confirmed normal thyroid (n = 80), Hashimoto thyroiditis (n = 20), non-Hashimoto lymphocytic thyroiditis (n = 28) and diffuse hyperplasia (n = 2). Receiver operating characteristic curves revealed that the best diagnostic indices of both imaging methods were achieved on the basis of two or more abnormal imaging findings. The sensitivity, specificity and accuracy of US and CT in incidentally detecting DTD by this classification were 72% and 72%, 87.5% and 91.3% and 81.5% and 83.8%, respectively. Thyroid US and neck CT have similar diagnostic values for differentiating incidental DTD from normal thyroid. [Copyright &y& Elsevier]

Developmental morphological abnormalities of the thyroid gland are very rare. Thyroid lobe hemiagenesis is a very rare abnormality, in which one thyroid lobe fails to develop. Ectopic thyroid is one of the common developmental anomalies of thyroid characterized by the presence of thyroid tissue in a site other than its usual. Of all, lingual thyroid is the most common manifestation of benign ectopic thyroid tissue, but it is still a rare clinical entity. Both of them seen together is even rarer. These developmental defects may or may not be associated with thyroid dysfunction. Ultrasonography (USG) is the initial modality of choice for thyroid hemiagenesis but further evaluation by computed tomography (CT) scan or nuclear scan should be done to look for other thyroid ectopic tissue rest. We report a case of right thyroid hemiagenesis with ectopic lingual thyroid in a 13-year-old female. The article discusses differential diagnosis of expanding thyroid mass and the immunoglobulin G4 (IgG4)-related systemic disease (RSD).
