The present study aims to study the immune role of human leukocyte antigens CD4 & IL-23 in patients with Beta thalassemia, by Enzyme-Linked Immunosorbent Assay (ELISA) technique. The level of immunological markers (CD4 & IL-23) was determined in the serum of patients with thalassemia, using the ELISA test (Sandwich ELISA test) for (80) people, 40 samples from thalassemia patients, 20 male samples and 20 female samples. 20 control group samples were 20 male samples and 20 female samples. The results showed that the level of the results of the current study indicated the presence of significant differences and at the probability level (P<0.05), and a high level of CD4 was recorded in patients' 88,97 ng/ml compared to the control group 83.123 ng/ml. While the percentage of males was higher than that of females, and it reached 99,88 ng/ml, 79.29 ng / ml, respectively. As for age groups, its percentage in the age group (27-19 years) was 100.234 ng/ml higher than age groups (1-9) and (10-18) years was 80.124 ng/ml, 75.567 ng / ml, respectively. The results showed a highly significant difference at the probability level (P < 0.01), and between the high level of IL-23 in patients 65.500 ng / ml compared to the control group 45.165 ng / ml. And, for age groups, its percentage was in the 10 -18-year age group and it reached 75.501 ng/ml higher than the age groups (1-9), (19-27) years and the ng/ml was 65.255 ng/ml, 52.955, respectively.
