Aim: This article is a report of a study protocol designed to explore what factors inform healthcare professionals' clinical decision‐making when providing end of life care for people with dementia in primary care. Design: The proposed study will adopt a qualitative study design, utilizing an ethnographic approach. Methods: A mixed sample group of registered healthcare professionals, including district nurses, specialist nurses and general practitioners, will be purposively sampled and recruited from one health board in Wales, which will cover three settings in primary care. A multi‐method approach will be utilized to provide rich data, including non‐participant observation, semi‐structured interviews, and review of key documents. Data will be managed using NVivo 12 and analysed thematically. Ethics approval was gained in April 2022. Discussion: Little is known about the end of life care decision‐making process of healthcare professionals caring for people with dementia in primary care settings. This study will therefore address this pertinent gap. The study findings may also be transferable to healthcare professionals working in similar clinical settings across the UK. Impact It is anticipated that this study will inform the subsequent development of an intervention that can be used by healthcare professionals to support decision making and subsequently the provision of quality end of life care in primary care for people with dementia. In principle, it is possible for a couple to get married in a register office in England or Wales for £127 (including the cost of giving notice and a certificate). In this article, we draw on empirical research to show how limited this option is in practice. Its availability is constrained by the scarcity of register offices limited slots for weddings, and the addition of other fees not provided for in the regulations. Its accessibility is often not obvious from local authorities' websites, and the administration of such a wedding varies hugely, with some local authorities treating it as a no-frills legal procedure, and others regarding it as a significant ceremony that is incomplete without music or a reflective introductory speech by the superintendent registrar – even if the couple wanted neither. With significant numbers of couples having a register office wedding because the marriage ceremony they choose to have to reflect their beliefs is not legally binding, there is a need to address these issues of availability, accessibility and administration so that couples are not put off or discriminated against. Adopting a children's rights perspective, a critique and analysis underpinned by documentary research methodology was undertaken in order to assess the extent to which the government's Green Paper (Department of Health and Social Care and Department of Education, 2017. Transforming children and young people's mental health provision: a green paper. Available at: https://www.gov.uk/government/consultations/transforming-children-and-young-peoples-mental-health-provision-a-green-paper (accessed 7 December 2017)) addresses the mental health and well-being needs of refugee children and young people in England and Wales, identifying strengths, limitations and challenges for future policy and practice. Findings suggest that there is much of potential benefit to refugee children and young people's future mental health and well-being. However, a paradigm shift, explicit in implications, scale and time frame, will be required, if the Green Paper is to achieve those changes in attitudes, practice and service delivery which it anticipates. We argue that this Green Paper's overarching challenge is that it is premised on Western-centric models in its understanding of the experiences of refugee children and young people, and management of trauma and mental health. It fails to recognize the meanings and significance of culture, and of diversity and difference, and the need to invest in all communities in facilitating engagement and support for children and young people's mental health issues.

Primary care services in or alongside emergency departments look and function differently and are described using inconsistent terminology. Research to determine effectiveness of these models is hampered by outdated classification systems, limiting the opportunity for data synthesis to draw conclusions and inform decision-making and policy. We used findings from a literature review, a national survey of Type 1 emergency departments in England and Wales, staff interviews, other routine data sources and discussions from two stakeholder events to inform the taxonomy. We categorised the forms inside or outside the emergency department: inside primary care services may be integrated with emergency department patient flow or may run parallel to that activity; outside services may be offered on site or off site. We then describe a conceptual spectrum of integration: identifying constructs that influence how the services function-from being closer to an emergency medicine service or to usual primary care. This taxonomy provides a basis for future evaluation of service models that will comprise the evidence base to inform policy-making in this domain. The last 30 years has seen an exponential increase in Historical Institutional Abuse Inquiries.1 One feature of these has been to place adult survivor voices at the center of Inquiry work, meaning that child abuse victims and survivors2 are engaging with Inquiries, sharing their experiences, with this participation often presented as empowering and healing. This initiative challenges long held beliefs that child sexual abuse survivors are unreliable witnesses, which has led to epistemic injustice and a hermeneutical lacunae in survivor testimony. However to date there has been limited research on what survivors say about their experiences of participation. The Truth Project was one area of work of the Independent Inquiry into Child Sexual Abuse3 in England and Wales. It invited survivors of Child Sexual Abuse to share their experiences including the impacts of abuse and their recommendations for change. The Truth Project concluded in 2021 and heard from more than 6,000 victims of child sexual abuse. The evaluation of the Trauma Informed Approach designed to support survivors through their engagement with the project was a mixed methods, two phase methodology. A total of 66 survey responses were received. Follow-up interviews were conducted with seven survey respondents. The Trauma Informed Approach was found to be predominantly helpful in attending to victim needs and minimizing harm. However, a small number of participants reported harmful effects post-session. The positive impacts reported about taking part in the Truth Project as a one-off engagement challenges beliefs that survivors of child sexual abuse cannot safely talk about their experiences. It also provides evidence of the central role survivors should have in designing services for trauma victims. This study contributes to the epistemic justice literature which emphasizes the central role of relational ethics in the politics of knowing, and the importance of developing a testimonial sensibility when listening to marginalized groups. This introduction outlines the research context from which this special number arises. It opens with a discussion of the interdisciplinary conference 'Reading, Writing and Collecting: Books and Manuscripts in Wales, 1450--1850' and our rationale in our selection of papers from that conference. There follows an exploration of relevant historiography on this topic, drawing out areas of interest but also those where further work is needed. Purpose: Despite a known risk of cellulitis recurrence, the management of the wider impact and risk factors has been neglected. The innovative National Cellulitis Improvement Programme (NCIP) addresses this by providing evidence-based and individualised care to improve patient reported outcomes and reduce the risk of recurrence. The aim of this paper is to examine the longer-term impact of cellulitis and to identify a suitable and clinically relevant Patient Reported Outcome Measure (PROM). Methods: A review of existing cellulitis-specific PROMs was undertaken, alongside literature detailing the patient-focused impact of cellulitis, to identify a suitable PROM for clinical use. A group of expert therapists and patient representatives (n = 14) shared their individual and collective experiences over a series of events to discuss and debate the impact of cellulitis and review available PROMs. CELLUPROM© is introduced with anonymised PROM data and case study information reported to establish the impact of CELLUPROM© within usual NCIP care. Results: No cellulitis-specific PROMs were identified. Literature focused on the signs and symptoms of an acute episode of cellulitis, with outcome measures primarily used to evidence the impact of an intervention. An enduring physical, social and emotional impact of cellulitis was identified in this study, providing the basis for the new cellulitis-specific PROM (CELLUPROM©), which has been implemented with good effect in clinical care. Conclusion: This study has highlighted the lasting impact of cellulitis. Using CELLUPROM© within the risk-reduction NCIP has helped develop Value-Based Healthcare and support programme evaluation. Across the United Kingdom public (and private) resources have been targeted on improving broadband infrastructure. While this has served to provide new opportunities for households and firms, there has been some debate around the ability of firms to take full advantage of the opportunities that arise through this evolving infrastructure. In this respect, there has been particular debate on how far small- and medium-sized enterprises (SMEs) have taken up the challenge of effectively engaging with the resource. Drawing on the case of Wales, this paper explores the digital maturity of a sample of Welsh SMEs. The paper provides evidence of how far SMEs are successfully engaging with the new opportunities available through broadband, and develops a typology of firms according to their engagement. Background/aims: The value of using qualitative methods within clinical trials is widely recognised. How qualitative research is integrated within trials units to achieve this is less clear. This paper describes the process through which qualitative research has been integrated within Cardiff University's Centre for Trials Research (CTR) in Wales, UK. We highlight facilitators of, and challenges to, integration. Methods: We held group discussions on the work of the Qualitative Research Group (QRG) within CTR. The content of these discussions, materials for a presentation in CTR, and documents relating to the development of the QRG were interpreted at a workshop attended by group members. Normalisation Process Theory (NPT) was used to structure analysis. A writing group prepared a document for input from members of CTR, forming the basis of this paper. Results: Actions to integrate qualitative research comprised: its inclusion in Centre strategies; formation of a QRG with dedicated funding/roles; embedding of qualitative research within operating systems; capacity building/training; monitoring opportunities to include qualitative methods in studies; maximising the quality of qualitative research and developing methodological innovation. Facilitators of these actions included: the influence of the broader methodological landscape within trial/study design and its promotion of the value of qualitative research; and close physical proximity of CTR qualitative staff/students allowing sharing of methodological approaches. Introduction of innovative qualitative methods generated interest among other staff groups. Challenges included: pressure to under-resource qualitative components of research, preference for a statistical stance historically in some research areas and funding structures, and difficulties faced by qualitative researchers carving out individual academic profiles when working across trials/studies. Conclusions: Given that CTUs are pivotal to the design and conduct of RCTs and related study types across multiple disciplines, integrating qualitative research into trials units is crucial if its contribution is to be fully realised. We have made explicit one trials unit's experience of embedding qualitative research and present this to open dialogue on ways to operationalise and optimise qualitative research in trials. NPT provides a valuable framework with which to theorise these processes, including the importance of sense-making and legitimisation when introducing new practices within organisations. Devolution to Scotland and Wales was a central pillar of the legislative agenda of the Labour government elected in 1997, yet despite the constitutional significance of this programme it was undertaken without particular enthusiasm by the then prime minister, Tony Blair. Nowhere was this blend of significant change, yet pervasive lack of passion (or interest) more apparent than devolution to Wales. Drawing upon recently released archival documents, this article will explore the New Labour government's approach to devolution in Wales. These papers provide fresh evidence of the second order nature, and treatment, of Welsh devolution compared to the attention provided to Scottish devolution, as well as of the narcissism of small details which pervaded Whitehall's discussions of Welsh devolution (with considerable energy, for example, dedicated to questions about the titles which leading figures in the Assembly would be given). These papers also bear testament to the struggles which Ron Davies, the then secretary of state for Wales, faced in securing even a minor scheme of devolution and the tensions he experienced with the centre of government. Purpose: The purpose of this paper is to place on record the impact made on government policy and research by Autism Cymru, a small charity that existed in Wales between 2001 and 2014. The success of Autism Cymru resulted directly from philanthropic funding, applied with strategic vision and genuine ambition. Design/methodology/approach: A retrospective, chronological viewpoint highlighting the creative process, drawing upon records held by the charity. Findings: Autism Cymru initiated the concept and played a crucial role in steering the development of government policy for autism in Wales between 2001 and 2011. The charity also drove forward the initiative, which led to the establishment of the Wales Autism Research Centre at Cardiff University in 2010. This paper demonstrates that with astute philanthropic support, small learning disability/autism charities can elicit structural and sustainable change at the national level, leading to wide-ranging benefits for the communities they represent. Originality/value: The strategic approach taken over 20 years ago in Wales by Autism Cymru, which led to the Welsh Government's ASD Strategic Action Plan for Wales (Welsh Government, 2011), set in place a national policy model, which was then followed by The Scottish Strategy for Autism and the Northern Ireland Autism Strategy (Department of Health, 2013>; Scottish Government, 2011). The insightful and tenacious method used by Autism Cymru remains relevant today, demonstrating that any small charity supported by shrewd philanthropic funding can punch well above its weight by taking a planned, ambitious and strategic approach to policy, research and practice. Purpose: For over 15 years, Mental Health First Aid™ (MHFA) has successfully been delivered in Wales, United Kingdom, with growing interest in the MHFA programme and increasing course attendees. Trainers, aware of the need for support, know the importance of MHFA being accessible for different communities and learner groups. MHFA has always focused on increasing mental health literacy. One marginalised group, with lower mental health literacy than the general population, is Deaf people, a group with increased risk of mental health problems. This paper aims to provide insights about why Deaf people are twice as likely as hearing people to experience mental health problems. Design/methodology/approach: During this paper, the authors have used four focal points i) exploring situational contexts for Deaf people; ii) reasons why Deaf individuals are at greater risk of mental health problems; iii) the authors explore a project, "Hear Deaf", and implementation of MHFA Wales by Deaf MHFA trainers; and iv) initiatives to influence and impact on policymakers. Findings: During the project, nine MHFA courses were delivered to Deaf communities across different locations in Wales, often with information and advertisements circulated directly to Deaf communities through Deaf clubs, resulting in 120 Deaf people trained. The authors conclude with their own reflections as a researcher and an MHFA who work predominantly with Deaf communities. Originality/value: This paper provides a discussion on the specific risks for Deaf people around mental health and the importance of mental health promotion programmes for Deaf communities. Purpose: This paper is a commentary that aims to address themes arising from the article by Morgan titled "Wales and Autism: The Impact of Philanthropy Matched with Ambition". Design/methodology/approach: This opinion piece considers the broader impact of stakeholder engagement in the field of autism, including how the dissemination of evidence-based support could be made more accessible for autistic people, their families and carers and philanthropists. Findings: This paper highlights the link between stakeholder engagement and meaningful change in policy and practice in the field of autism. It highlights the need for philanthropists to invest in evidence-based support but highlights the difficulty in identifying and accessing such support. This piece supports the use of evidence and gap maps (EGMs) to increase the accessibility of autism research and evidence. Originality/value: While stakeholder engagement is encouragingly rising in this field, most of the evidence is held in academic journals, which are behind paywalls and, therefore, inaccessible to many autistic people, their families, carers and associated professionals. Displaying evidence using EGMs provides an accessible way for autistic people and those who support them to identify possible avenues of support quickly and allows philanthropists to direct research funding to areas of high priority. Purpose: Parental advocacy is an emerging area of research and policy interest in Wales and across the UK. Although there is little research in the UK context to date, international research has indicated that parental advocacy can improve the relationship between parent and professional in the field of child protection social work. This paper aims to ascertain how the implementation of a parental advocacy programme supports parents to play a meaningful role in decision-making when children's services are working with them and their families. Design/methodology/approach: This study used interviews, surveys and focus groups to obtain qualitative data from 18 parents, seven parent advocates, two advocacy managers and four social workers, to explore the potential impact of parental advocacy on decision-making. The study identified challenges in implementing parental advocacy, particularly relating to awareness of the service. Participants also discussed experiences of the child protection system and how parents are supported by advocates. Findings: Despite challenges surrounding implementation, initial findings were encouraging, and generated examples of how parental advocacy services have helped parents to understand children's services and develop relationships of trust with social care professionals. In doing so, this study identified potential mechanisms that may be useful to support future service delivery. Originality/value: This paper and research is novel as it explores parental advocacy within the Welsh context. Although there has been research conducted into parental advocacy, this has largely come from the USA. Purpose: This study aims to critically examine the effects of COVID-19 social discourses and policy decisions specifically on older adult volunteers in the UK, comparing the responses and their effects in England, Scotland, Wales and Northern Ireland, providing perspectives on effects of policy changes designed to reduce risk of infection as a result of COVID-19, specifically on volunteer involvement of and for older adults, and understand, from the perspectives of volunteer managers, how COVID-19 restrictions had impacted older people's volunteering and situating this within statutory public health policies. Design/methodology/approach: The study uses a critical discourse approach to explore, compare and contrast accounts of volunteering of and for older people in policy, and then compare the discourses within policy documents with the discourses in personal accounts of volunteering in health and social care settings in the four nations of the UK. This paper is co-produced in collaboration with co-authors who have direct experience with volunteer involvement responses and their impact on older people. Findings: The prevailing overall policy approach during the pandemic was that risk of morbidity and mortality to older people was too high to permit them to participate in volunteering activities. Disenfranchising of older people, as exemplified in volunteer involvement, was remarkably uniform across the four nations of the UK. However, the authors find that despite, rather than because of policy changes, older volunteers, as part of, or with the help of, volunteer involving organisations, are taking time to think and to reconsider their involvement and are renewing their volunteer involvement with associated health benefits. Research limitations/implications: Working with participants as co-authors helps to ensure the credibility of results in that there was agreement in the themes identified and the conclusions. A limitation of this study lies in the sampling method, as a convenience sample was used and there is only representation from one organisation in each of the four nations. In an era of accelerating biomedical innovations, partial ectogenesis looms as a double-edged sword for female bodily autonomy and reproductive rights. Rather than simplifying or 'solving' abortion access and the ethical issues inherent to that right, this technology wields the alarming potential to curtail women's right to safe and legal abortions. This article investigates the potential of partial ectogenesis as a 'solution' to the abortion debate, focusing on two significant hypotheses. Foremost, the paper scrutinizes the dilemma of ectogenesis abortion; specifically, instances where a woman intending to terminate her pregnancy is obliged to undergo foetal transfer surgery. This scrutiny engages with Räsänen's concept of the right to the death of a foetus, yet ultimately concludes that this supposed right is unconvincing. Despite there being no such right, this paper argues that the prospect of partial ectogenesis is incapable of resolving the core issues of the abortion debate, not least because of the inherent conflict between a woman's right to her own body and the proposed technology or rather, compulsion of such surgeries. Instead, this article posits that a more significant ethical and logical concern lies in misconceptions surrounding the term 'foetus' and how such misconceptions impact considerations of autonomy. Thereafter, this paper examines the potential for partial ectogenesis to alter the legal viability threshold set by the Abortion Act 1967-in particular, this paper explores the impact a shift in the viability threshold would have on female bodily autonomy. In the aim of resolving these concerns, this article concludes by advocating for the decriminalisation of abortion to safeguard female bodily autonomy against constraints on abortion access were the viability threshold legally revised. By addressing these crucial topics, this article aims to provide a nuanced analysis of the dialogue surrounding reproductive rights, bodily autonomy, and the complex interplay between emerging biotechnologies, medical ethics and legislative frameworks. South Asians constitute Britain's largest ethnic minorities, yet very little is known about their migration history in the prewar era. Recent literature has begun to challenge the widely held view that South Asian migration to Britain was a post-war phenomenon in response to Britain's need for labour as it emerged from two costly wars. The literature has demonstrated that the South Asian presence in Britain can be traced back to the seventeenth century. This paper adds to that literature by analysing the occupation distribution and mobility of South Asian-born migrants during the early twentieth century. Regression results from the 1901 and 1911 Censuses of England and Wales suggest that relative to other foreign-born and British-born, South Asian-born immigrants were primarily employed in white-collar jobs, a position they largely retained until at least 1911. These results suggest that the South Asian diaspora was economically well assimilated into the British economy. [ABSTRACT FROM AUTHOR]

The 2019 ICC Cricket World Cup was held from May 30 to July 14 in England and Wales with a new tournament format being used. The competition was reduced to ten teams, in a single pool, with each team scheduled to play every one of the other nine teams. The purpose of such a tournament is to identify the strongest or most dominant teams, but this was thwarted in the 2019 Cricket World Cup by the scoring system, in particular the use of net run rate to rank teams tied on points awarded for wins, draws and ‘no result’ matches. Concepts from discrete mathematics, in particular graph theory, are applied to create a ranking system better suited to this type of tournament. The dominance graph for matches between the top five teams shows that the team which came fourth, under the current system, was dominated by the team placed in fifth position. This paper shows how the use of net run rate may be replaced by calculating one stage, two stages and if necessary three stage dominances, to rank teams. These calculations are easily done using a dominance matrix. Recommendations to ensure that the strongest teams play in the final match, for the cup, are made and it is suggested that the group stage of the FIFA Football World Cup use the scoring method explained in this paper. The question of land ownership is fundamental to any modern society and has been blamed as the root of numerous social inequalities. This paper explores one potential solution to the land question, Land Value Tax (LVT), which was first popularised in the 1880s by Henry George. Despite its long history, LVT has not been widely implemented, but recently proposals to replace existing property taxes with this form of taxation have re-emerged on the political scene. While the Welsh Government has recently explored the idea of introducing an LVT, there are limited studies on this form of taxation in the context of a discretionary planning system, such as the Welsh planning system. This paper conducts exploratory qualitative research using Wales as a case study. Data was collected via semi-structured interviews with experts across various related fields, primarily within taxation and planning, to establish the potential effects of LVT in practice, with a particular focus on how it might affect the equity of planning outcomes. Overall, the research showed that despite the strong theoretical potential, fundamental practical issues would diminish the ability of LVT to effectively influence planning. LVT is argued to be largely incompatible with a discretionary planning system (e.g., the Welsh planning system), and therefore, the research concluded that many of the potential benefits of LVT could be better achieved through reforms to existing taxes and planning processes, particularly land value capture mechanisms. This paper explores whether children and young people (CYP) with additional learning needs (ALN) are at a systemic disadvantage regarding Welsh language opportunities. Justification pertaining to why this should be on the radar of educational psychologists (EPs) in Wales is provided. Congruent with critical realism, quantitative and qualitative research is reviewed, examining micro, eco and macrosystems. A systemic lens illuminates the context and process factors that CYP with ALN and their families may encounter regarding Welsh language opportunities. This paper is informed by the bioecological Person Process Context Time (PPCT) model, and the Constructionist Model of Informed and Reasoned Action (COMOIRA). Limitations demarcating interpretation of identified issues are highlighted, without negating their significance. Implications for EPs are offered, contemplating their role in facilitating inclusion through supporting the development of greater cohesion between Welsh Government (WG) legislation (macrosystem), and the systemic realities of CYP with ALN in Wales. Despite a widespread focus on grandparents, a large proportion of kinship care in the UK is provided by older siblings. What drives older siblings to become kinship carers, and how this might differ from other kinship carers, is not well represented in academic literature. In this study, narrative interviews were carried out with thirteen adults across England, Scotland, and Wales who had experience being the main carer for their younger sibling(s) when their parents could not care for them sufficiently. The narrative method elicited holistic accounts of participants experiences of being a sibling carer, and the analysis generated three groups of narrative accounts highlighting how and why some sibling kinship care arrangements come about—with siblings wanting to bring their younger siblings back into their family; siblings trying to keep their younger siblings in their family; or older siblings stepping in to fill a gap in parenting at home. The paper draws on the narrative accounts of participants to build the groups, presenting an illustrative narrative account to represent each group. Importantly, these accounts demonstrate how becoming a kinship carer as an older sibling may, or may not, be recognised or fit into wider narratives of what becoming a kinship carer looks like. It is hoped that these accounts will prompt practitioners and policymakers to look more closely at the role of siblings when considering who is and who should be involved in deciding how to support children to remain within their family network. Motivated by demand prediction for the custodial prison population in England and Wales, this paper describes an approach to the study of service systems using infinite server queues, where the system has non-empty initial state and the elapsed time of individuals initially present is not known. By separating the population into initial content and new arrivals, we can apply several techniques either separately or jointly to those sub-populations, to enable both short-term queue length predictions and longer-term considerations such as managing congestion and analysing the impact of potential interventions. The focus in the paper is the transient behaviour of the M t / G / ∞ queue with a non-homogeneous Poisson arrival process and our analysis considers various possible simplifications, including approximation. An increasing body of research suggests that bilingualism is possible and perhaps even advantageous for autistic individuals. However, several factors might influence parents' decisions about raising their autistic child bilingually, including national language policies, educational contexts, advice received from key professionals, and the child's individual strengths and needs. Accordingly, there is a clear imperative to understand how the views of different stakeholders converge and diverge when language decisions are made in the context of autism. This paper brings new insights by synthesising the findings of three qualitative studies that used interpretative phenomenological analysis (IPA) to explore the perspectives and experiences of bilingual autistic children (n = 11), parents (n = 16), and educational practitioners (n = 13) of bilingualism in autism in England and Wales. Despite wide variation between and within groups, a striking tension emerged between individuals' beliefs about bilingualism in general, which were positive, and their experiences of bilingualism in autism specifically, which often foregrounded more monolingual approaches. This paper examines this tension, with a particular focus on stakeholders' attitudes towards the feasibility of bilingualism, families' language choices in the context of autism, and how notions of contextual linguistic diversity accentuated differences between England and Wales. We conclude by arguing that greater awareness of both bilingualism and neurodiversity in educational and clinical settings may improve the experiences of bilingual autistic children and, crucially, the language advice families receive. A growing body of scholarship draws attention to prisons and environmental justice, pointing out the propensity for prisons to be located on contaminated sites and to be in close proximity to polluting industries, as well as for prisons themselves to contribute to local environmental degradation. Prisoners' immobility renders them unable to relocate away from harmful environments, and there are now numerous suggestions that their wellbeing suffers as a result of the poor quality of many local environments. However, since a relationship between environmental quality and prisoner wellbeing is yet to be robustly demonstrated, there is currently no firm evidence base from which to argue for positive change. This paper therefore examines the effect of the environmental quality of the locations of prisons, approximated as the presence of greenspace in the immediate vicinity of prisons, local species biodiversity and local levels of air and noise pollution, on wellbeing outcomes in a set of prisons in England and Wales. It finds that good environmental quality, in the form of high biodiversity and/or low air pollution, enhances the already-recognized positive effects of greenspace on the wellbeing of incarcerated populations. Background: A COVID-19 hospital guideline was implemented across all 18 acute hospitals in Wales in March 2020, promoting ward management of COVID pneumonitis and data collected across the first 3 Waves of the pandemic (Wave 1 March 1st 2020 to November 1st 2020, Wave 2 November 2st 2020 to February 21st 2021 and Wave 3 June 1st 2021 to December 14th 2021). The aim of this paper is to compare outcomes for patients by admission setting and type of ventilatory support given, with a particular focus on CPAP therapy. Methods: This is a retrospective observational study of those aged over 18 admitted to hospital with community acquired COVID-19 between March 2020 and December 2021. The outcome of interest was in-hospital mortality. Univariate logistic regression models were used to compare crude outcomes across the waves. Multivariable logistic regression models were used to assess outcomes by different settings and treatments after adjusting for Wave, age, sex, co-morbidity and deprivation. Results: Of the 7,803 records collected, 5,887 (75.4%) met the inclusion criteria. Analysis of those cases identified statistically significant outcome improvements across the waves for all patients combined (Waves 1 to 3: 31.5% to 18.8%, p<0.01), all ward patients (28.9% to 17.7%, p<0.01), and all ICU patients (44.3% to 32.2%, p = 0.03). Sub group analyses identified outcome improvements in ward patients without any oxygen therapy (Waves 1 to 3: 22.2% to 12.7%, p<0.01), with oxygen therapy only (34.0% to 12.9%, p<0.01) and with CPAP only (63.5% to 39.2%, p<0.01). The outcome improvements for ICU patients receiving CPAP only (35.7% to 24.6%, p = 0.31) or invasive ventilation (61.6% to 54.6%, p = 0.43) were not statistically significant though the numbers being admitted to ICU were small. The logistic regression models identified important age and comorbidity effects on outcomes. The multivariable model that took these into account suggested no statistically significantly greater risk of death for those receiving CPAP on the ward compared to those receiving CPAP in ICU (OR 0.89, 95% CI: 0.49 to 1.60). Conclusions: There were successive reductions in mortality in inpatients over the three Waves reflecting new treatments and better management of complications. Project HELIX 2016-2020 was developed and implemented in Wales, to improve technical and safety/science knowledge; and sustainable innovation in food manufacturing small to medium enterprises (SMEs). The paper aims to place Project HELIX within the wider context of regional and food sector development and then examine the project's rationale and delivery methods relative to this. It reflects on the effectiveness of the project (or otherwise) in constructing regional advantage from knowledge transfer and knowledge spillovers. This paper employs data from the 2021 UK census to initially explore sub-state (English, Welsh) national identities among minority ethnic groups. This shows that these identities remain much more exclusive of people in minority groups than is a British identity, and that this exclusion is particularly marked with respect to English identity. The analysis then builds on this observation using similar data to examine English identification among the White British ‘majority’ in a ‘superdiverse’ city – London. Attributes which are typically shared by London boroughs in which identification as English deviates most from the national average, and multi-variable analysis which considers the ethnic structure of the borough in which an individual lives alongside other key factors (age, education, social class) suggest differences in identification between people living in boroughs that are characterised by more established and extensive ethnic diversity and those in boroughs transitioning from a previously more homogeneous (white) ethnic structure. In exploring how the articulation of a specific national identity might relate to ethnically-diverse or ‘superdiverse’ contexts, the paper uniquely contributes to recent research which calls for a stronger focus on how people who do not belong to migrant-minority groups might respond to living in such contexts. This paper examines the transformative potential of inward investment on cluster development. It undertakes this through the lens of the evolving semiconductor cluster in South Wales (UK) and within a context whereby the semiconductor industry is under pressure with government interventions to improve prospects being implemented at a time of increasing global tension in the industry. The paper reveals the potential of inward investors as agents of regional economic change and cluster development. It shows that the emerging South Wales cluster distinguishes itself in terms of investment motivation, commodities and services produced. These factors result in growing embeddedness in relation to elements such as joint research with higher education and intra-firm collaboration to develop new products and processes. The paper concludes that investments based on risky and fast-changing technologies in industries such as the semiconductor sector necessarily create challenges for policy makers. These are best managed by placing greater emphasis on developing the conditions to satisfy the needs of such knowledge-intensive industries in relation to labour market conditions, factor conditions and market access. This paper sheds light on the substantial challenges faced by schools today in the UK, especially in the aftermath of a negative school inspection report. Stigmatised and labelled as 'faliing' and with limited support, staff within these schools are often emotionally exhausted and professionally traumatised (Jeffrey & Woods, 1996), when they begin the arduous journey towards school improvement. A longitudinal study, implemented within a secondary school located in an economically and socially disadvantaged region of Wales, focuses on the outcomes of a school-wide talent development program, with a strong emphasis on coaching, made available to staff in all roles, from caretaking to leadership. Through the adoption of a data-driven, strategy-aligned, professional development programme, this paper describes how the internal talent of the whole school was harnessed and directed towards a transformative shift: one that took the school from an initial inspection assessment of 'in significant need of improvement' to a more favourable 'satisfactory' rating and beyond. Central to this programme was a coaching initiative aimed at bolstering staff self-efficacy, which over time resulted in significant improvements in staff wellbeing, team resilience and overall performance, benefiting both staff and students across the school. Policy classifies and shapes people/subjects in particular kinds of ways. Focusing on the context of Health and Physical Education (HPE), this paper analyses policy documents from Australia, the United States of America (USA) and Wales. We pay particular attention to how learners are represented within and across the three policy documents, and we apply Bacchi's 'What's the Problem Represented to be?' approach to guide the analysis. For us, problematization is a fruitful and positive process that enables educators to engage with a critical dialogue regarding the policies they are expected to enact. Our analysis highlighted that common across the policies were overlapping discourses of idealism, neoliberalism, healthism, and individualism, which serve to reinforce deficit language and a focus on what learners 'lack'. The problem of 'learner as lacking' is represented within the policies via at least three subject positions: 'the sedentary learner', 'the un-educated learner' and 'the naïve learner'. The findings suggest that the three policies were producing an ideal and perhaps impossible learner (subject) whilst at the same time representing the learner as a problem that the policy could 'fix'. This paper is important because it: (i) demonstrates how certain discourses and voices are amplified and silenced within curriculum policy documents and policy work more broadly; (ii) makes educational and health politics visible; and (iii) creates space for the profession to develop greater critical consciousness related to policy. In terms of future directions, we urge curriculum policy writers and other stakeholders to carefully consider how learners are categorised, represented and governed in and through policy. If curriculum policies, by their very nature, need to produce problems – in this case, the (im)possible subject - we invite educators to engage in critical conversations regarding the policies they are expected to enact. Billions of historical climatological observations remain unavailable to science as they exist only on paper, stored in numerous archives around the world. The conversion of these data from paper to digital could transform our understanding of historical climate variations, including extreme weather events. Here we demonstrate how the rescue of such paper observations has improved our understanding of a severe windstorm that occurred in February 1903 and its significant impacts. By assimilating newly rescued atmospheric pressure observations, the storm is now credibly represented in an improved reanalysis of the event. In some locations this storm produced stronger winds than any event during the modern period (1950–2015) and it is in the top-4 storms for strongest winds anywhere over land in England and Wales. As a result, estimates of risk from severe storms, based on modern period data, may need to be revised. Examining the atmospheric structure of the storm suggests that it is a classic Shapiro–Keyser-type cyclone with "sting-jet" precursors and associated extreme winds at locations and times of known significant damage. Comparison with both independent observations and qualitative information, such as photographs and written accounts, provides additional evidence of the credibility of the atmospheric reconstruction, including sub-daily rainfall variations. Simulations of the storm surge resulting from this storm show a large coastal surge of around 2.5 m, comparing favourably with newly rescued tide gauge observations and adding to our confidence in the reconstruction. Combining historical rescued weather observations with modern reanalysis techniques has allowed us to plausibly reconstruct a severe windstorm and associated storm surge from more than 100 years ago, establishing an invaluable end-to-end tool to improve assessments of risks from extreme weather. This paper examines how the spaces through which legal trials take place shape attitudes towards justice. There has been a growing academic interest in the role of courts in configuring the relationships between trial participants and consequently reproducing pre‐established hierarchies of power. This paper builds on this work to consider the spatiality of trials from the perspective of a particular group of court users: defendants. Drawing on qualitative data drawn from a longitudinal study conducted in prisons in England and Wales, the paper examines how defendants perceive trial spaces, exploring in particular how such insights expand beyond the narrow focus on the courtroom to draw in experiences of transportation, holding cells, bodily restrictions, and the provision of food. In doing so, we seek to contribute to debates concerning penal consciousness, whereby the subjectivity and embodiment of individual participants illuminate differential experiences of the material and legal nature of trial processes. The paper concludes by emphasising how a focus on penal consciousness can help to address perceptions of the injustice of trial outcomes while improving the accessibility of courts. This paper's aim is to explore the challenges of developing an education 'evidence ecosystem' for Wales. Methodology. The analysis is derived from ongoing research and debates with key stakeholders in higher education and the Welsh Government and draws on policy research from other small countries. Results. It might be argued that Wales provides the perfect environment for developing such an ecosystem because of its size, commitment to evidencebased policy, political consensus, collaborations between policy-makers, researchers and the education profession. That it has not yet been able to do so illustrates the constraints of wider political and historical circumstances which structure provision, particularly in small countries. Purpose: The purpose of this papers is to set out the advantages to local communities of independent representation of older people by older people through active and vibrant older people's forums and groups and the rationale for Councils in Wales to support their revitalisation and development. Design/methodology/approach: This paper describes how semi-structured action research and follow-up discussion and analysis over a two-year period was undertaken by older people with representatives of Older Peoples Forums and groups to identify a strategy to restart and revitalise independent forums post pandemic to represent the voice of older people to government at all levels. Findings: Facilitation of the independent and self-determined voice of older people through genuine co-production with Forums can help Councils meet statutory duties and ensure their policies and services for older people are developed and informed from the direct experience of those receiving them. This will also help address the impacts of an ageing population and meet the national aim to create age-friendly communities across Wales. Social implications: This research provides a basis for Councils to work with Independent Older Peoples Forums and groups to restart and regenerate the forums and promote the self-determined voice of older people to government at all levels. Originality/value: In an ageing society older people should be empowered and enabled to make their own decisions. The principle of self-determination by older people for older people directly and through co-productive engagement with them by local authorities has meaning and purpose that should be promoted and implemented. Fieldwork encounters are not only contingent to biographical subjectivities, but are mediated by a confluence of identity, place and embodiment. This paper offers reflexive accounts of researchers with various socio-cultural and disciplinary backgrounds, who collaborated as a team to examine the varied funerary experiences and needs of established minorities and recent migrants in England and Wales. Focusing on the researchers' varied personal experiences with death and bereavement and on their performances of minority and majority ethnic and migrant identities, the paper highlights the mediated and embodied nature of fieldwork. It argues that reflection on the various aspects of intersectional researcher identity is necessary for a rigorous fieldwork practice that takes transparency and politics into account. This facilitates a deeper understanding of the positionality of both researchers and interlocutors, and the situated co-production of knowledge. In doing so, the paper illustrates that conducting research with a diverse team of researchers contributes to better understanding the complexity and multifacetedness of social phenomena. Background: Treatment for people with kidney disease is often associated with complicated combinations of medicines. Logistical challenges with traditiona paper-based prescribing means that these patients are particularly susceptible to medication-relation errors and harm. Aim: To improve the quality of care that people with kidney disease receive across Wales through a Value-Based digital transformation programme. Setting: Renal units within the National Welsh Renal Clinical Network (WRCN). Development: A novel Electronic Prescribing & Medicines Administration (EPMA) system, integrated into a patient care record and linked to a patient portal was developed in South West Wales (SWW) region of the WRCN, enabled by the Welsh Government (WG) Efficiency Through Technology Fund. National upscale was enabled through the WG Transformation Fund. Implementation: EPMA was designed and rolled out initially in SWW region of the WRCN (2018). A dedicated delivery team used the blueprint to finalise and implement a strategy for successful national roll-out eventually across all Wales (completed 2021). Evaluation: A multi-factorial approach was employed, as both the technology itself and the healthcare system within which it would be introduced, were complex. Continuous cycles of action research involving informal and formal qualitative interviews with service-users ensured that EPMA was accessible and optimally engaging to all target stakeholders (patients and staff). Results confirmed that EPMA was successful in improving the quality of care that people with kidney disease receive across Wales, contributed to Value-Based outcomes, and put people who deliver and access care at the heart of transformation. Conclusion: Key findings of this study align directly with the national design principles to drive change and transformation, put forward by the WG in their plan for Health and Social Care: prevention and early intervention; safety; independence; voice; seamless care. This paper addresses a gap in the literature on youth justice in England and Wales by examining disaggregated patterns of juvenile court processing (i.e. proven offences) and custody. It provides new evidence to show that gendered effects are best observed over time. Looking at juvenile justice data over time allows us to see the effects of policy that are obscured in the short-term. This is especially important when considering small and specific populations, such as girls. It is often assumed that policy impacts smaller groups in the justice system (in this case, girls) in the same way as the larger group (in this case, boys), with boys' experiences representing the norm. In this paper, we call into question that assumption by considering female and male proven offences and juvenile custody over time in England and Wales and show why gendered impacts should be given proper consideration. The Becoming Breastfeeding Friendly (BBF) in Great Britain study was conducted during 2017–2019 comprising three country studies: BBF England, Wales and Scotland. It was part of an international project being coordinated during the same period by the Yale School of Public Health across five world regions to inform countries and guide policies to improve the environment for the promotion, protection and support of breastfeeding. This paper reports on the application of the BBF process that is based on an implementation science approach, across the countries that constitute Great Britain (England, Wales and Scotland). The process involves assessing 54 benchmarks across eight interlocking gears that drive a country's 'engine' towards a sustainable policy approach to supporting, promoting and protecting breastfeeding. It takes a consensus‐oriented approach to the evaluation of benchmarks and the development of recommendations. This paper provides a critical overview of how the process was conducted, the findings and recommendations that emerged and how these were managed. We draw on critical theory as a theoretical framework for explaining the different outcomes for each country and some considerations for future action. Key messages: Undertaking an international, comparable approach to develop evidence‐based policy recommendations for scaling up the breastfeeding environment can provide useful data on which to draw explanations and conclusions on national variation.The findings from the BBF process across England, Scotland and Wales suggest that improving breastfeeding in Great Britain is dependent on the degree of political will and having a coordinated national breastfeeding strategy in place or not, along with access to robust breastfeeding data.A critical theory lens helps to bring to light some differences in the research and policy process that can explain differences between countries in the United Kingdom. The Police Uplift Programme in England and Wales aims to recruit more than 20,000 new police officers into the service. There is little known about the potential consequences associated with a large-scale recruitment drive and the effect of introducing a younger cohort of officers into the ranks of the police. This article is based on a narrative review of 2,110 English-language titles including grey literature exploring the implications for the police who are younger in age and service workforce profile. Titles were identified through electronic and handsearching methods. Fifty-nine titles were assessed for inclusion, of which 48 were subject to a full-text review. The majority of papers were from North America. Three themes were identified from the review: (1) issues related to cultural considerations including younger officers' preference for transparent and linear relationships and highlighting an emphasis on public service; (2) well-being and work–life balance considerations; and (3) harnessing the skill base through effective recruitment and retention practices. The findings suggest that police forces can create a narrative for new recruits as adult learners committed to public service combined with enhanced leadership styles that prioritize teamwork, transparency, and personal development. This paper examines the radio drama Danger (1924) by Richard Hughes. Danger, which is known as the world's first radio drama, was broadcast by the BBC (British Broadcasting Corporation) in January 1924. The play's importance is not limited to its pioneering role in creating the genre of radio drama. Danger is set in a Welsh coal mine, a backdrop that invokes the social issues engaging Britain in the 1920s. By pursuing these issues, I would like to examine this work's mission as a public medium that appeals to the masses, as well as its innovation in drama broadcasting. [ABSTRACT FROM AUTHOR]

Purpose: In England and Wales, Domestic Homicide Reviews (DHRs) are conducted into domestic abuse-related killings. In 2016, deaths by suicide were brought into the scope of this review system and, to distinguish them from reviews into domestic homicides, we describe these as 'Suicide Domestic Abuse-Related Death Reviews' (S-DARDR). To date, S-DARDRs have been little considered and, in response, this empirical paper seeks to unpack this process. Method: In a larger study, 40 DHR participants were interviewed, and a reflexive thematic analysis was undertaken. 18 participants discussed S-DARDRs. These interviews were re-read, with relevant extracts identified and re-analysed thematically. Through a shared critical reflection, we drew on our practice experience to interrogate the themes generated from the interviews and offer insight into the underlying challenges. Results: From the interviews, we generated four themes relating to commissioning and delivery; the involvement of stakeholders; intersections with other statutory processes; and purpose. Based on our shared critical reflection, we identified the underlying challenges as an under conceptualisation of S-DARDRs, alongside their de-mooring from the criminal justice system. Taken together, these challenges have implications for the conduct of S-DARDRs. We identify recommendations for policy and practice to address these challenges. Conclusion: The development of S-DARDRs has been little considered and challenges arise around when and how they should be undertaken. A shared understanding of key concepts and expectations around delivery is necessary if S-DARDRs are to enable robust learning and be a driver for systems change while also being accessible and understood by all stakeholders. Background: The 2022/23 influenza season in the United Kingdom saw the return of influenza to prepandemic levels following two seasons with low influenza activity. The early season was dominated by A(H3N2), with cocirculation of A(H1N1), reaching a peak late December 2022, while influenza B circulated at low levels during the latter part of the season. From September to March 2022/23, influenza vaccines were offered, free of charge, to all aged 2–13 (and 14–15 in Scotland and Wales), adults up to 49 years of age with clinical risk conditions and adults aged 50 and above across the mainland United Kingdom. Methods: End‐of‐season adjusted vaccine effectiveness (VE) estimates against sentinel primary‐care attendance for influenza‐like illness, where influenza infection was laboratory confirmed, were calculated using the test negative design, adjusting for potential confounders. Methods: Results In the mainland United Kingdom, end‐of‐season VE against all laboratory‐confirmed influenza for all those > 65 years of age, most of whom received adjuvanted quadrivalent vaccines, was 30% (95% CI: −6% to 54%). VE for those aged 18–64, who largely received cell‐based vaccines, was 47% (95% CI: 37%–56%). Overall VE for 2–17 year olds, predominantly receiving live attenuated vaccines, was 66% (95% CI: 53%–76%). Conclusion: The paper provides evidence of moderate influenza VE in 2022/23. It seems probable that some form of medically-assisted dying will become legal in England and Wales in the foreseeable future. Assisted dying Bills are at various stages of preparation in surrounding jurisdictions (Scotland, Republic of Ireland, Isle of Man, Jersey), and activists campaign unceasingly for a change in the law in England and Wales. There is generally uncritical supportive media coverage, and individual autonomy is seen as the unassailable trump card: 'my life, my death'. However, devising a law which is 'fit for purpose' is not an easy matter. The challenge is to achieve an appropriate balance between compassion and patient autonomy on the one hand, and respect for human life generally and medical autonomy on the other. More people should benefit from a change in the law than be harmed. In relation to medically-assisted dying, this may not be possible. Protecting the vulnerable is a key issue. Likewise, not impacting negatively on societal attitudes towards the disabled and frail elderly, particularly those with dementia. This paper compares three existing models of physician-assisted suicide: Switzerland, Oregon (USA), and Victoria (Australia). Vulnerability and autonomy are discussed, and concern expressed about the biased nature of much of the advocacy for assisted dying, tantamount to disinformation. A 'hidden' danger of assisted dying is noted, namely, increased suffering as more patients decline referral to palliative-hospice care because they fear they will be 'drugged to death'. Finally, suggestions are made for a possible 'least worse' way forward. One solution would seem to be for physician-assisted suicide to be the responsibility of a stand-alone Department for Assisted Dying overseen by lawyers or judges and operated by technicians. Doctors would be required only to confirm a patient's medical eligibility. Palliative-hospice care should definitely not be involved, and healthcare professionals must have an inviolable right to opt out of involvement. Description Within the current climate of health care pressures, services are exploring ways in which to use resources to the best of their ability to ensure service users have timely, safe, and effective care as well as having positive outcomes and good experiences of the care they receive. The current paper explores a service level agreement between the private sector and a child and family service within the HSC. Within this agreement, has been the development of a pathway through which families on a psychological service waiting list, could be triaged, and allocated to a private clinic within which, psychological care could be provided from assessment through to discharge. The subjective experiences and initial outcomes of those involved have been largely positive. The paper describes, explores, and reflects on the process of collaborative practice. Drawing on the creative group and individual interviews with young people aged 11–18 years old in England and Wales in 2018, this paper assembles data that addresses the changing nature of intimacies in young people's digitally-networked lives. Informed by Lauren Berlant's work (2008, viii), it maps how platforms such as Instagram and YouTube circulate certain 'fantasies of the good life' that orient young people towards heteronormative future imaginaries. It argues that publicising intimate relationships on social media works to both reify and unsettle fantasies of monogamy, marriage and domesticity by highlighting their inherent precarity. It also illustrates how smart technologies such as the virtual personal assistants Alexa and Siri reconfigure the good life fantasy. Moving beyond simplistic moral panics that seek to discipline young people's digital intimacies, this paper aims to engage with the nuances of young people's digital sexual cultures. Overall, this paper argues that attending to this complexity can enable practitioners to better attune to and support young people to navigate contemporary digital relationships and sexuality issues.
