In 2023, a second wave of the global mpox epidemic, which is mainly affecting men who have sex with men (MSM), was observed in some countries. Herein, we benefited from a large viral sequence sampling (76/121; 63%) and vast epidemiological data to characterise the re-emergence and circulation of the Monkeypox virus (MPXV) in Portugal during 2023. We also modelled transmission and forecasted public health scenarios through a compartmental susceptible-exposed-infectious-recovered (SEIR) model. Our results suggest that the 2023 mpox wave in Portugal resulted from limited introduction(s) of MPXV belonging to C.1.1 sublineage, hypothetically from Asia, followed by sustained viral transmission and potential exportation to other countries. We estimated that the contribution of the MSM high sexual activity group to mpox transmission was 120 (95% CrI: 30–3553) times higher than that of the low sexual activity group. However, among the high sexual activity group, vaccinated individuals likely contributed approximately eight times less [0.123 (95% CrI: 0.068–0.208)] than the unvaccinated ones. Vaccination was also linked to potential reduced disease severity, with a Mpox Severity Score of 6.0 in the vaccinated group compared to 7.0 in unvaccinated individuals. Scenario analysis indicated that transmission is highly sensitive to sexual behaviour, projecting that a slight increase in the MSM sub-population with high sexual activity can trigger new mpox waves. This study strongly supports that continued vaccination, targeted awareness among risk groups and routine genomic epidemiology is needed to anticipate and respond to novel MPXV threats (e.g. global dissemination of clade I viruses).

A infeção por Chlamydia trachomatis é a infeção bacteriana sexualmente transmissível mais frequente a nível global. A sua abordagem diagnóstica é desafiante pela existência de um grande número de portadores assintomáticos, e requer uma disponibilização apropriada de testes laboratoriais à população em risco. Em Portugal, a incidência da infeção tem crescido de forma consistente nos últimos anos, pelo que se impõe a necessidade de cuidados redobrados na identificação de casos, rastreio de contactos sexuais e aplicação de medidas terapêuticas eficazes. As presentes recomendações resultam da adaptação à realidade portuguesa dos consensos internacionais em termos de diagnóstico e terapêutica da infeção por Chlamydia trachomatis, e foram formuladas com o objetivo de uniformizar a gestão clínica e laboratorial dos casos sintomáticos e portadores não sintomáticos da infeção em Portugal à luz dos conhecimentos atuais.

Objective: Genital herpes (GH) is one of the most common sexual transmitted infections. Herpes simplex virus type 1 (HSV-1) genital infections have been increasing, with a shift toward its predominance in many developed countries. The aim of this study was to characterize the population with GH diagnosed in Centro Hospitalar Universitário de Lisboa Central, Lisbon, Portugal. Methods: A retrospective analysis of all laboratory-confirmed diagnosis of GH between 2017 and 2021 was conducted. The diagnosis was established by real-time polymerase chain reaction, in samples collected by swabbing suspicious mucocutaneous lesions. Medical records of included patients were reviewed and data of interest analyzed. Results: During the studied period, a total of 239 patients were diagnosed with GH, from which 76.6% (n = 183) were caused by HSV type 2 (HSV-2). Most patients were men (68%; n = 163), with a mean age of 35.7 years. Compared to the group diagnosed with HSV-2, the mean age was significantly lower in the HSV-1 group (28.6 years vs. 37.9; p < 0.001) and the proportion of patients with first clinical manifestations of GH was significantly higher in the latest (67.8% vs. 30%; p

Abstract Background Infection by the monkeypox virus classically causes a cutaneous rash that is preceded by fever and lymph node swelling, as well as other nonspecific systemic symptoms. A recent outbreak occurred and spread in Europe and other regions, especially among patients who declare themselves as men who have sex with men. Current reports have shown that cutaneous lesions may be limited to the anogenital area. We report on a case of proctitis caused by monkeypox virus, without visible typical lesions of this virus. Case presentation A 29-year-old Caucasian male presented with a monkeypox virus proctitis that recurred after treatment for a documented Neisseria gonorrhoeae and Chlamydia trachomatis coinfection, likely acquired at the same time. The proctitis was preceded by fever and a swollen inguinal lymph node, and was associated with a hemorrhoid. The monkeypox virus polymerase chain reaction of a rectal swab documented high viral loads, although no typical lesion was visible. After resolution of the rectitis, the patient developed a single dermatome herpes zoster, despite the absence of usual risk factors. The patient evolved well without further specific treatment. Conclusion This case shows that monkeypox virus can be responsible for proctitis, without any typical lesion, along with the important rectal shedding of the virus. It raises the concern of contagion during anal intercourse through body fluids and gives further credit that monkeypox virus can be a sexually transmitted infection. This should prompt routine rectal screening in patients with proctitis accompanied by fever and swollen lymph nodes, and in patients who have a history of unprotected receptive anal sex, even in presence of other sexually transmitted infections, and especially during a monkeypox virus outbreak. The potential link between monkeypox virus infection and shingles warrants further investigations.

O risco infecioso associado a terapêutica biológica é cada vez mais estudado e conhecido, existindo estratégias de rastreio e profilaxia bem estabelecidas. O mesmo não se verifica na utilização de corticoide sistémico nem dos tradicionais modificadores de doença autoimune, de que são exemplo o metotrexato e a ciclosporina, apesar de usados há largos anos. A dose e tempo de terapêutica com corticoide sistémico relacionam-se com a ocorrência de infeção oportunista. Doses superiores a 5 mg/dia associam-se a infeção bacteriana, superiores a 10 mg/dia a reativação de infeção por vírus herpes zoster (VHZ), e superiores a 15 mg/dia ou por mais de 2 a 4 semanas a reativação de tuberculose latente, o que implica rastreio e aplicação de quimioprofilaxia de forma adequada. O corticoide sistémico surge ainda como dos principais fatores de risco para o desenvolvimento de pneumocistose no doente não-VIH e doses prolongadas (por mais de 4 semanas) podem levar a reativação de infeção por vírus da hepatite B (VHB) e justificar a profilaxia com tenofovir disoproxil fumarato ou entecavir. Foram ainda descritos casos de estrongiloidíase complicada de síndrome de hiperinfeção em doentes sob corticoide. O grau de imunossupressão conferido pode contraindicar a realização de vacinas vivas atenuadas. O metotrexato e ciclosporina comportam baixo risco infecioso quando utilizados em monoterapia, sendo a vigilância de sintomas a principal estratégia preventiva. Não obstante, ambos são imunomoduladores, contraindicam a realização de vacinas vivas atenuadas e associam-se a risco de infeção bacteriana e reativação de infeção por VHZ, no caso da ciclosporina, ou reativação de infeção por VHZ e VHB no caso do metotrexato, sobretudo se administrado em dose >0,4 mg/kg/semana. Ambos se associam a evolução para tuberculose ativa quando em associação terapêutica com outros imunossupressores. Compreender e estudar o risco de infeção na utilização de terapêutica imunossupressora permite a sua aplicação de forma mais informada e segura.

Introduction: Recent studies worldwide reveal a significant prevalence of extragenital infections by Neisseria gonorrhoeae among men who have sex with men. We aimed to analyse the frequency and characteristics of extragenital gonococcal infections diagnosed in men who have sex with men in a walk-in Sexually Transmitted Infection clinic in Lisbon, Portugal. Material and Methods: We conducted a cross-sectional, retrospective study of the anorectal and/or oropharyngeal Neisseria gonorrhoeae infections in men who have sex with men, diagnosed in our Sexually Transmitted Infection clinic between January 2014 and December 2016. Results: We found extragenital infection in 87 cases of gonorrhoea identified in men who have sex with men in this period, including: 49 cases of anorectal disease, 9 of oropharyngeal disease, 13 cases of infection at both extragenital sites, and 16 of simultaneous extragenital and urogenital gonorrhoea. Patients’ ages ranged from 17 to 64 years (median: 28 years). Forty-seven (54%) of the patients did not present with any extragenital symptoms. Thirty (35%) were human immunodeficiency virus-1-positive. Discussion: Since most extragenital Neisseria gonorrhoeae infections are asymptomatic, they may be missed and go untreated unless actively investigated. Current international guidelines recommend the screening of gonorrhoea at extragenital sites in men who have sex with men because anorectal and oropharyngeal infections constitute a potential disease reservoir, and may facilitate transmission and/or acquisition of human immunodeficiency virus infection. Conclusion: Our results highlight the relevance of testing men who have sex with men for Neisseria gonorrhoeae at extragenital sites, regardless of the existence of local complaints. The implementation of adequate screening programmes in Portugal should be considered. We also reinforce the need to raise awareness in the population regarding the adoption of prophylactic measures against transmission of sexually transmitted infections during anal and/or oral sexual exposure.

Syphilis is an old disease and a major public health concern, which is re-emerging in the western world. Atypical syphi- litic presentations can be challenging to diagnose. We present a 23-year-old, HIV-positive man who has sex with men who presented with a 2-week history of an asymptomatic lesion on the glans. Previous topical and systemic treatments proved unsuccessful. Physical examination demonstrated a slightly indurated, ill-defined erythematous plaque over most of the glans. Enlarged, non-tender, bilateral inguinal lymph nodes were palpable. Serological tests revealed a fourfold increase in the VDRL titre (1:8, previously 1:2). A diagnosis of syphilitic balanitis of Follmann was made and complete resolution was achieved with an intramuscular single dose of benzathine penicillin 2.4 MU. Syphilitic balanitis of Follmann represents an uncommon manifestation of primary syphilis, with less than 100 cases reported. Untrea- ted, it will resolve spontaneously, but the patient will remain highly infectious and at-risk for long-term morbidity. In an era in which the number of syphilis cases is increasing sharply both in Western Europe and the United States, all physicians should be aware for the typical and atypical early syphilitic manifestations as they represent opportunities of paramount importance for treatment and epide- mic control.

Introduction: Sexually transmitted infections (STIs) are a significant public health problem worldwide. They are more frequent in young men; however the prevalence in older individuals is increasing. The aim of this study was to access the prevalence of STIs diagnosis in men aged 50 years and older. We also aimed to compare the demographic and behavioral characteristics of those diagnosed with STIs and those with non-STI conditions. Methods: We conducted a retrospective study including all men aged 50 years and older who have attended a Sexual Transmitted Diseases (STD) clinic of a central hospital in Lisbon, Portugal, between July 2015 and December 2017. Results: Between July 2015 and December of 2017, 1722 men attended the STD clinic, of which 323 (18.8%) were aged 50 years and older. Out of these 323 men, 137 (41.4%) were diagnosed with a STI, more often anogenital warts (32.9%), genital herpes simplex virus infection (19.7%) and early syphilis (19.0%). The mean age of the patients with a STI was 59.9 years and almost all (90.5%) reported an inconsistent use of a condom (90.5%). The number of men that have sex with men (MSM) was significantly higher in patients with a STI (p = 0,003) than those with a non-STI diagnosis. Additionally, 40 (29.2%) of these STI patients had been previously infected with the human immunodeficiency virus (HIV) and 33 (24.1%) had a past history for syphilis, which were significantly higher than in patients in patients with a non-STI diagnosis (p = 0.015 and p = 0.001, respectively). Conclusion: Existing data on STI has mainly focused younger population with few studies performed in older individuals. Our study reports a large number of men aged 50 years and older that allows a better understanding of this age group. High levels of unsafe sex and previous diagnosis of HIV and syphilis emphasizes the importance of sexual health promotion in older individuals.

Fungal invasive infections are rare in general population but are an emergent cause of infection in the immunocompromized population, especially in the solid organ transplant recipients. Herein the authors report a clinical case of a liver transplanted patient suffering a cutaneous co-existent infection with A. alternata as well as A. infectoria. To our knowledge this is the first case of cutaneous concomitant infection due to those two species reported not only in Portugal but also worldwide. The patient was treated with surgical excision of the lesions and oral itraconazol without relapse.

A infecção genital por Chlamydia trachomatis (Ct) é frequentemente assintomática e quando não tratada precocemente constitui um problema major de saúde pública. Os autores realizaram um estudo cujo objectivo foi determinar a prevalência de infecção genital por Ct numa consulta de Infecções Sexualmente Transmissíveis (ISTs) e realizar uma análise de diferentes variáveis. Para tal foi realizado um estudo retrospectivo dos doentes com diagnóstico de infecção por Ct confirmado por técnica de amplificação de ácidos nucleicos, observados na consulta de ISTs do Serviço de Dermatologia e Venereologia do Hospital Curry Cabral entre Janeiro de 2006 e Dezembro de 2009. Encontrou-se uma prevalência de infecção de 10,5% (81 doentes com infecção genital por Ct em 771 indivíduos testados). Na população com infecção por Ct foram encontradas diferenças com significado estatístico entre o sexo masculino e feminino na idade média dos doentes (homens=28,9 vs mulheres=24,3) p= 0,018; no número de companheiros sexuais nos últimos 6 meses (homens=3,32 vs mulheres=1,50) p=0,009; e no tempo em semanas decorrido entre o início dos sintomas e o diagnóstico (homens=2,65 vs mulheres=6,64) p= 0,032. Os doentes com co-infecção pelo Vírus da Imunodeficiência Humana (VIH) tinham um maior número de companheiros sexuais nos últimos 6 meses do que os não VIH p=0,003. Assim, existem diferenças demográficas, comportamentais e clínicas entre o sexo masculino e feminino e entre a população VIH e não VIH. Além disso, a elevada prevalência de infecção por Ct justifica os programas de diagnóstico precoce instituídos.

Introdução: Os homens que têm sexo com homens, particularmente os infectados pelo vírus da imunodeficiência humana (VIH) têm risco aumentado de cancro anal. Ao contrário da maior parte das doenças oportunistas e/ou definidoras de SIDA, os dados são contraditórios no que diz respeito ao efeito da terapêutica antirretrovírica, não sendo claro o impacto da supressão virológica e reconstituição imunitária na evolução da infecção por vírus do papiloma humano (HPV) e prevenção das neoplasias associadas. Actualmente é já recomendada a vacinação de doentes com infecção por VIH contra o HPV; os programas de rastreio do cancro anal, recomendados por algumas organizações, carecem ainda de validação definitiva da sua eficácia. Material e Métodos: Os autores fizeram um estudo retrospectivo dos casos de cancro anal diagnosticados entre 2000-2015 em doentes com infecção por VIH em seguimento no serviço de Infecciologia do Hospital de Curry Cabral, em Lisboa. São avaliadas as características clínicas e epidemiológicas dos doentes com cancro do canal anal ao longo de um período de quinze anos. Resultados: O cancro anal foi diagnosticado em dez doentes, maioritariamente homens HSH, com infecção por VIH com uma duração média de 15,1 anos, com contagem média de linfócitos TCD4+ de 441 células/uL. O diagnóstico foi mais frequentemente realizado no estádio III e o tratamento mais frequente foi cirúrgico, ocasionalmente conjugado com radioterapia. Faleceram 4 doentes. Conclusões: São necessários programas de rastreio consensuais do cancro anal, a par da necessidade de reforço da vacinação contra o HPV nos homens, nomeadamente nos homens que têm sexo com homens, com ou sem infecção por VIH.

A sarcoidose consiste numa doença inflamatória multissistémica, de etiologia desconhecida, que se caracteriza pela formação de granulomas não-caseosos nos órgãos envolvidos, predominantemente pulmões e gânglios intratorácicos. O envolvimento cutâneo ocorre em cerca de 25-35% dos casos, é habitualmente um achado precoce e constitui um local acessível e seguro para estudo histológico. As lesões cutâneas podem ter várias apresentações, incluindo pápulas, nódulos, placas e cicatrizes infiltradas. O diagnóstico baseia-se no reconhecimento das características clínicas, imagiológicas e histológicas e exclusão de outras doenças granulomatosas. O diagnóstico precoce e tratamento sistémico (quando necessário) poderão prevenir complicações futuras.

Introdução: Assistiu-se nas últimas décadas a um aumento do número de casos de gonorreia notificados na Europa. Material e métodos: Foram revistos de forma retrospectiva os processos clínicos dos doentes observados na Consulta de IST do Hospital de Curry Cabral durante o período compreendido entre Janeiro de 2006 e Dezembro de 2013 (8 anos). O rastreio para a infecção por Neisseria gonorrhoeae é realizado em todos os doentes de forma sistemática através de técnica de diagnóstico molecular (BD Probetec-SDA system). Os autores procederam à caracterização epidemiológica e sociodemográfica bem como determinação da frequência da infecção por N. gonorrhoeae e respectivas localizações. Resultados: Durante o período em análise, num universo de 1735 doentes, foi diagnosticada gonorreia em 114 casos (6,6%), 104 em homens e 10 em mulheres, com uma mediana de idades de 29 anos. O diagnóstico de gonorreia verificou-se em 63 homens que têm sexo com homens (HSH) e 41 homens que têm sexo com mulheres. Dos doentes com diagnóstico de gonorreia, 26 eram seropositivos para o VIH (23%). A localização uro-genital foi a mais frequente, com 103 casos, seguindo-se a infecção do recto em 12 doentes e infecção da orofaringe em 9 doentes. De todos estes diagnósticos, 17% ocorreram em localizações nas quais os doentes não referiram sintomas (13 homens e 5 mulheres). Conclusões: Os resultados deste trabalho foram semelhantes aos dados publicados recentemente na literatura. Salienta-se o elevado número de casos em HSH. Neste contexto epidemiológico, a identificação deste agente em localizações sem queixas não é negligenciável, realçando a importância do rastreio extragenital em doentes assintomáticos nessas localizações.

Apresenta-se o caso de um doente de 46 anos, com coinfecção VIH-1/VHC e cirrose hepática, internado por febre, prostração e astenia. Ao exame objetivo, observavam-se escassas pústulas necróticas em base eritematosa acantonadas aos dedos das mãos e pés com edema, dor e limitação funcional dos mesmos e da articulação tíbio-társica esquerda. Analiticamente, apenas elevação ligeira da enzimologia hepática e da PCR. Admitiram-se as hipóteses diagnósticas de endocardite, meningococcémia ou gonococcémia. Após isolamento de Neisseria gonorrhoeae em hemocultura, iniciou-se ceftriaxone 1g/dia EV com melhoria clínica. A positividade da PCR para N. gonorrhoeae no exsudado orofaríngeo confirmou o diagnóstico de gonococcémia disseminada de ponto de partida orofaríngeo. A gonorreia é uma infeção sexualmente transmitida causada pelo diplococo Gram negativo Neisseria gonorrhoeae. A gonococcémia disseminada na forma da síndrome clássica “dermatite-artrite” acompanha apenas 1-2% das infeções mucosas. A gonorreia orofaríngea é geralmente assintomática em homens e mulheres, constituindo provavelmente um reservatório importante do agente. O aumento da incidência de gonorreia torna este caso pertinente na prática clínica actual do dermatologista.
