1. Characterization of microbial communities in seven wetlands with different anthropogenic burden using Next Generation Sequencing in Bogotá, Colombia

Abstract

Abstract Wetlands represent key ecosystems due to their remarkable biodiversity, ecological functions and multiple ecosystem services provided. In Colombia, there are 31,702 wetlands, 13 of which are in Bogotá, capital of the country. Despite the fundamental socioecological support of these aquatic ecosystems, a tremendous loss and degradation of these ecosystems has been observed due to anthropogenic perturbations. Therefore, the aim of this study was to describe the status of seven Bogotá wetlands with variable anthropogenic interventions by measuring organoleptic, physicochemical, and microbiological parameters, using commercial kits, highly sensitive equipment, and next-generation sequencing of the 16S- and 18S-rRNA genes. Our findings describe the status of seven wetlands with different anthropogenic burden in Bogotá-Colombia where physicochemical and microbiology signals of contamination were observed. Additionally, some profiles in the composition of the microbial communities, together with certain physicochemical characteristics, may represent an insight into the environmental dynamics, where Beta Proteobacteria such as Malikia represent a potential keystone in aquatic ecosystems impacted by wastewater effluent discharges; the presence of nitrates and phosphates explain the abundance of bacteria capable of oxidizing these compounds, such as Polynucleobacter. Moreover, the presence of specific prokaryotic and eukaryotic organisms, such as Clostridium, Cryptococcus, Candida, and Naegleria, reported in one or more of the wetlands assessed here, could represent a possible pathogenic risk for human and animal health. This study performed a complete evaluation of seven Bogotá wetlands with different anthropogenic impacts for the first time, and our findings emphasize the importance of maintaining continuous monitoring of these water bodies given their remarkable ecological importance and potential spill-over of several pathogens to humans and animals.

Abstract

Background: Antibiotic resistance is a worldwide concern. This study retrospectively analyzed patients admitted to the ICU of a tertiary hospital over a period of 7 months who were rectally colonized by multidrug-resistant microorganisms. The incidence of concomitant nosocomial infections was estimated, thus providing the risk of a colonizing microorganism producing a nosocomial infection. Methods: Infections with the same microorganism (concomitant) or different microorganisms (non-concomitant) were analyzed in order to adjust the empirical antibiotic treatment. Patients with rectal colonization by at least one multidrug-resistant bacterium (MDRB) on admission or after ICU admission were included. All patients had complete selective digestive decontamination (SDD) prophylaxis. For univariate analysis, categorical variables are expressed as frequencies and percentages and continuous variables as means and standard deviations, or as medians and interquartile ranges. For multivariate analysis, the model is summarized with p-values and hazard ratios with 95% confidence intervals. Survival analysis was conducted using the Kaplan–Meier method, which was performed to evaluate the time elapsed from colonization to infection by the same bacteria. Statistical significance was considered at p < 0.05. Results: Of the 130 patients with MDRB bacterial colonization analyzed, 98 remained free of infection, while 22 developed non-concomitant infections and 10 had infections concomitant to rectal colonizing bacteria. OXA-48-producing bacteria and MDR-Pseudomonas spp. incidences were 18.9% (95% CI: 7.96–35.2) and 44.4% (CI: 13.7–78.8), respectively. Conclusions: OXA-48-producing bacteria and MDR-Pseudomonas spp. were the only bacteria associated with the development of infections concomitant to rectal colonization in an SDD setting. The incidence of MDRB infections was low.

Abstract

SARS-CoV-2 infection is the cause of the disease named COVID-19, a major public health challenge worldwide. Differences in the severity, complications and outcomes of the COVID-19 are intriguing and, patients with similar baseline clinical conditions may have very different evolution. Myeloid-derived suppressor cells (MDSCs) have been previously found to be recruited by the SARS-CoV-2 infection and may be a marker of clinical evolution in these patients. We have studied 90 consecutive patients admitted in the hospital before the vaccination program started in the general population, to measure MDSCs and lymphocyte subpopulations at admission and one week after to assess the possible association with unfavorable outcomes (dead or Intensive Care Unit admission). We analyzed MDSCs and lymphocyte subpopulations by flow cytometry. In the 72 patients discharged from the hospital, there were significant decreases in the monocytic and total MDSC populations measured in peripheral blood after one week but, most importantly, the number of MDSCs (total and both monocytic and granulocytic subsets) were much higher in the 18 patients with unfavorable outcome. In conclusion, the number of circulating MDSCs may be a good marker of evolution in the follow-up of unvaccinated patients admitted in the hospital with the diagnosis of COVID-19.

Abstract

The definition of Insulin autoimmune syndrome includes the presence of high levels of blood insulin and insulin autoantibodies. We encountered a 45-years-old white man with a high insulin serum value that do not fit with the C-peptide result. To discard or to confirm an analytical interference and diagnose a possible Insulin Autoimmune Syndrome we performed the following investigations: dilution linearity test, heterophilic antibody blocking, polyethylene glycol precipitation, measurements with alternative assays, and gel filtration chromatography by size exclusion. The latter technique confirmed that most of the insulin was complexed with a 150-kDa protein, corresponding to immunoglobulin G, identified as insulin autoantibodies. These antibodies were responsible for hypoglycemia attacks in the patient, who had a previous autoimmune disease. This case highlights the importance of carefully analyzing the results and ruling out possible interferences, as well as considering all kinds of pathologies, even if they are infrequent.

ABSTRACT The anti-Müllerian hormone triggers the regression of uterus and fallopian tubes in male embryos; if there are problems in the synthesis or action of this protein, Müllerian structures persist in an otherwise phenotypic male. The most frequent clinical presentation of Persistent Mullerian Duct syndrome is cryptorchidism and inguinal hernia. The few cases reported in adults are incidental findings or inguinal hernias. However, we present an adult male with history of bilateral cryptorchidism with unsuccessful orchidopexy, who presents with a large abdominal mass with the finding of a seminomatous tumor and persistence of Müllerian structures, in whom the variant c.916delC (p.Leu306Cysfs*29) in the AMHR2 gene not previously reported was documented.

Abstract

The incidence of secondary infections in critically ill coronavirus disease 2019 (COVID-19) patients is worrisome. We investigated whether selective digestive decontamination (SDD) added to infection control measures during an intensive care unit (ICU) stay modified these infection rates. Methods: A retrospective observational cohort study was carried out in four ICUs in Spain. All consecutive ventilated patients with a SARS-CoV-2 infection engaged in national infection control programs between 1 March and 10 December 2020 were investigated. Patients were grouped into two cohorts according to the site of ICU admission. Secondary relevant infections were included. Infection densities corresponding to ventilator-associated pneumonia (VAP), catheter bacteremia, secondary bacteremia, and multi-resistant germs were obtained as the number of events per 1000 days of exposure and were compared between SDD and non-SDD groups using Poisson regression. Factors that had an independent association with mortality were identified using multidimensional logistic analysis. Results: There were 108 patients in the SDD cohort and 157 in the non-SDD cohort. Patients in the SDD cohort showed significantly lower rates (p < 0.001) of VAP (1.9 vs. 9.3 events per 1000 ventilation days) and MDR infections (0.57 vs. 2.28 events per 1000 ICU days) and a non-significant reduction in secondary bacteremia (0.6 vs. 1.41 events per 1000 ICU days) compared with those in the non-SDD cohort. Infections caused by MDR pathogens occurred in 5 patients in the SDD cohort and 21 patients in the non-SDD cohort (p = 0.006). Differences in mortality according to SDD were not found. Conclusion: The implementation of SDD in infection control programs significantly reduced the incidence of VAP and MDR infections in critically ill SARS-CoV-2 infected patients.

Considerando que la terapia antirretroviral no logra controlar la activación inmune que ocurre durante la infección por VIH-1, el estudio de moléculas inmunomoduladoras puede proporcionar estrategias alternativas para su control. En este sentido, este estudio busca evaluar la expresión transcripcional de moléculas asociadas con el metabolismo de las lipoproteínas de alta densidad y con la respuesta inflamatoria mediada por el inflamasoma NLRP3 en pacientes VIH-1. Este es un estudio transversal, donde se incluyeron 23 pacientes VIH-1 sin tratamiento antirretroviral, con diferentes estadios de progresión, 7 de los cuales son controladores (Carga viral 2000 copias/mL), además de 7 controles sanos. En células mononucleares de sangre periférica, se cuantificaron los niveles de la expresión transcripcional de ABCA-1, ABCA-3, Caspasa-5 y TXNIP mediante RT-PCR. Se evaluó la asociación de estos parámetros con variables clínicas y demográficas y se encontró que los individuos VIH-1 progresores mostraron niveles significativamente menores de TXNIP y ABCA-3, sugiriendo que durante la infección por VIH-1 se produce una alteración en la expresión de estas moléculas. Dada la complejidad de las interacciones inmuno-metabólicas durante la infección por VIH-1, se necesitan estudios adicionales para establecer los mecanismos precisos involucrados en estas alteraciones.

Abstract Background We examined whether long-term use of selective digestive tract decontamination (SDD) was effective in reducing intensive care unit (ICU)-acquired infection and antibiotic consumption while decreasing colistin-, tobramycin-, and most of the antibiotic-resistant colonization rates in a mixed ICU with a high endemic level of multidrug-resistant bacteria (MDRB). Methods In this cohort study, which was conducted in a 30-bed medical-surgical ICU, clinical outcomes before (1 year, non-SDD group) and after (4 years) implementation of SDD were compared. ICU patients who were expected to require tracheal intubation for > 48 hours were given a standard prophylactic SDD regimen. Abstract

This article presents the most relevant results found in the “Licenciatura” graduation research project for the BA in Teaching English offered by the University of Costa Rica, Campus Occidente. The work, entitled “Strengths and Weaknesses in Teaching English Literature in Experimental Bilingual High Schools of the Western Educational Region of Costa Rica: A Study from the Teachers’ Perspectives”, was conducted in 2012 and 2013. 36 EFL teachers participated. This paper presents results on the strengths and weaknesses of teachers’ academic and professional training in light of the implementation of the English literature syllabus of the Costa Rican Ministry of Public Education (known by the acronym MEP in Spanish). Qualitative and quantitative methodologies were used. Interviews were conducted with participants and English advisors and an online survey was also used. Results show that most EFL teachers do not feel trained to deal with the English literature syllabus because teaching literature is not their major, but the MEP has assumed that mastering the language is sufficient for an English teacher to teach literature. Most teachers reject this presupposition as they think that the MEP’S English literature syllabus focuses on literary content rather than using literature to teach and learn English. Teachers assert that there is a discrepancy between their academic training and what the MEP expects them to do.

Abstract

Objectives: To evaluate the Bio-Rad D-100®, an HPLC analyzer for glycated hemoglobin (HbA1c) determination, and to compare its performance with the Menarini HA-8180V® and Sysmex G8®. Methods: Method comparison was performed according to Clinical and Laboratory Standards Institute (CLSI) EP9-A2 guidelines. We selected 100 samples from the routine laboratory workload and analyzed them in duplicate with the three analyzers. The imprecision study was performed according to CLSI EP5-A2 guidelines for both inter-assay and intra-assay variability. Bias was assessed with external quality control material. To establish linearity, CLSI EP6-A protocol was followed. Results: Method comparison (95% confidence intervals in parentheses): D-100 vs G8: Passing-Bablok regression; y=0.973(0.963–0.983)×−0.07(−0.07−0.069); r=0.9989. Bland-Altman mean difference: −0.229%HbA1c (−0.256: −0.202); Relative bias plot: D-100/G8 vs D100-G8 mean ratio=0.971(0.967−0.975). D-100 vs HA-8180V: Passing-Bablok regression; y=0.944(0.932–0.958)×−0.078(0.024−0.173); r=0.9989. Bland-Altman mean difference: −0.363%HbA1c (−0.401: −0.325); Relative bias plot D-100/HA-8180V vs D100-HA-8180V mean ratio=0.955(0.952−0.958). Inter-assay coefficient of variation (CV): 0.81%. Intra-assay CV: 1.04% (low level), and 0.78% (high level). Bias against target value=2.332%. Linearity: r2=0.998 in the concentration range 4.4−13.9%HbA1c. Carry-over: 0.0024%. Conclusions: The Bio-Rad D-100 shows good correlation with G8 and HA-8180V. There is a small proportional systematic difference (2.7% and 5.6%, respectively) in both comparisons. Inter and intra-assay CVs are both lower than the lowest CV obtained in studies performed with D-100 and other instruments. Abstract

La muestra de arte Principio Potosí. ¿Cómo podemos cantar el canto del Señor en tierra ajena? (Creischer, Hinderer, Siekmann, 2010) vuelve a la explotación colonial de las minas de Potosí como proceso de acumulación originaria desde donde pensar el comienzo del capitalismo y las relaciones centro–periferia. A partir de esta idea, se proponen analogías entre la explotación colonial y la capitalista, entre la pintura colonial y el arte actual Desde Bolivia se plantea un contra–proyecto a la muestra, Principio Potosí Reverso (Rivera Cusicanqui y El Colectivo, 2010) bajo el precepto de que si hay algo así como un “Principio Potosí” este debe ser pensado desde la resignificación andina de los procesos coloniales. El foco está puesto menos en la circulación global de la plata que en el mercado interno y en la franja intermedia de negociaciones que este generó. Si en Principio Potosí “mercado”, “imagen” y “ritual” son pensados desde la lógica equivalencial de la acumulación capitalista, en Principio Potosí Reverso son tensionados desde la lógica inequivalente del ethos andino.

En el siguiente artículo propongo un análisis de el catálogo de la muestra de arte “Principio Potosí. ¿Cómo podemos cantar el canto del Señor en tierra ajena?”(Creischer, Siekmann y Hinderer, 2010) y un “ensayo visual” elaborado en disidencia a dicha muestra, Principio Potosí Reverso (Rivera Cusicanqui y El Colectivo). Tanto el catálogo cuanto el “ensayo visual” exponen la importancia de volver sobre Potosí como espacio clave en el que se reúnen las tensiones vinculadas a la colonialidad desde el siglo XVI y sus continuidades en el siglo XXI. Se retorna a Potosí como al lugar en donde se forja la modernidad y las relaciones centro–periferia que marcarán la historia de América Latina. Sin embargo, allí donde Principio Potosí ve el inicio de la modernidad, Principio Potosí Reverso ve la gestación de una modernidad alternativa. El campo de la disputa que se abre entre Principio Potosí y Principio Potosí Reverso tensa el arco que se abre entre lo textual y lo visual, pero a la vez entre arte y colonialidad, ofreciéndose así como espacio modélico para interrogar las alternativas actuales de un arte político poscolonial a la altura de los desafíos de nuestra cultura visual contemporánea. catálogo, ensayo visual, colonialidad, cultura visual.

18. Habitat use, richness, and abundance of native mice in the highlands of the Talamanca mountain range, Costa Rica

Abstract

The Costa Rican highlands are considered hotspots of diversity and endemism, but studies on rodents are scarce. We compared the richness and abundance of mice between the montane forest and the paramo at the summit of the Talamanca mountain range. We selected two study sites within the Talamanca mountain range: the Cerro de la Muerte Biological Station and the paramo. The former is a montane forest dominated by oaks, and the latter is dominated by an herbaceous layer, and some scattered bushy patches. We captured mice in two different microhabitats within each montane forest and paramo, so we had four different sampling microhabitats: (1) paramo–bush, (2) paramo–Chusquea, (3) montane forest–bush, and (4) montane forest–Chusquea. Mice were marked to identify recaptures. We captured four mouse species and their abundance varied largely between habitats and among microhabitats (Table 1). The most abundant species, representing 85 % of all mouse captures, was Peromyscus nudipes. Mice were more abundant in the montane forest than in the paramo. Within the montane forest, mice were more abundant in the microhabitat containing bushes. The montane forest has a more complex vegetation structure with more diversity of food resources and shelters than the paramo. As well as at the habitat level, we argue that differences in abundance among microhabitats are directly related with the structure of vegetation. A more complex habitat structure may provide rodents with better conditions.

Abstract

Este libro da cuenta de los procesos de construcción y profesionalización de la medicina y de la salud pública en la Real Audiencia de Quito y en el Virreinato del Nuevo Reino de Granada, y de la forma como estas se han relacionado en un proceso de doble vía con la sociedad y con el Estado, desde 1760 hasta 1830. Parte de un análisis histórico-comparativo y transcolonial sobre las reformas sanitarias borbónicas y la manera en la que los actores locales las negociaron. Se concentra, por un lado, en la caracterización de la Ilustración y la agenda reformista borbónica española, así como en las relaciones establecidas, por medio de ella, con los territorios coloniales. Por otro, se concentra en el análisis de los inicios de la transición hacia la nueva medicina francesa durante el periodo de independencia y la creación y muerte de la Gran Colombia. El libro, a pesar de estudiar el pasado, aporta elementos que podrían transportarse al presente para comprender la situación de conflicto derivada tanto del deterioro económico y social como sanitario, para obligarnos a pensar seriamente en la necesidad de construir políticas generales y sanitarias de carácter continental y global que nos permitan enfrentar este tipo de crisis.

Abstract

This article is the second part of a series that explores the historical process of the configuration of the internship in medical education and practice. In a previous article, we studied the emergence of the internship in Napoleonic France and its latter configuration trough the North American Flexnerian reforms to medical education. This second part deals with the process by which the internship was constituted in Colombia through five sections. The first one analyses the French influence at the beginning of the 20th century; the second part, the change towards the North American flexnerian model, which was motivated by the arrival of multiple medical missions and their suggestions regarding medical education; the third part studies the process by which these models assembled in the country, their regulation and its crisis; the fourth discusses the actual proposals; and the epilogue talks about the future of the internship in the COVID-19 pandemics.

Abstract

ResumenIntroducción Las mujeres han incursionado y aumentado en todas las áreas médico quirúrgicas y específicamente en la Urología, representando aproximadamente un 10% de esta fuerza laboral.Objetivo Describir el estado actual de las mujeres en la fuerza laboral en Urología a nivel mundial.Métodos Análisis bibliométrico basado en búsqueda retrospectiva de la literatura del año 1969 a 2019, mediante base de datos: MEDLINE, FABUMED y PubReminder®, utilizando términos de búsqueda: (workforce) AND (urology), incluyendo: artículos de revista, artículos originales de investigación, reseñas, ensayos y otros informes científicos. Se obtuvo el factor de impacto (FI) mediante Journal Citation Reports 2017/2018. A través de Scopus se obtuvo el h-index (HI). Las variables analizadas fueron: número total de publicaciones, revistas, autores, instituciones, porcentaje de publicaciones por revista y país, patrón de producción científica a lo largo del período estudiado. Se realizó análisis de mapeo bibliométrico con software VOSviewer®.Resultados Se encontraron un total de 352 revisiones desde el año 1969 hasta 2019. El mayor porcentaje de publicaciones fueron en la última década. Se encontraron en total 131 revistas, de ellas, 28 específicas de urología. La revista con mayor número de publicaciones fue el Journal of Urology. El 90% de los autores provienen de Estados Unidos y del total de artículos solo dos autores principales eran mujeres.Conclusiones El interés por determinar el papel de la mujer en la fuerza laboral urológica es reciente y se correlaciona con el aumento de la representación femenina en esta área.

Abstract

Resúmen Introducción y Objetivo En las últimas décadas, la incidencia y prevalencia de la incontinencia urinaria han venido en aumento. Existen numerosas publicaciones sobre este tema, se han introducido nuevos dispositivos, y se han modificado las líneas de manejo de esta patología. El objetivo de este estudio es realizar un análisis bibliométrico de las publicaciones sobre incontinencia urinaria masculina en las revistas indexadas. Métodos Se realizó un análisis bibliométrico descriptivo de corte retrospectivo de la literatura médica disponible en la base de datos Web of Science (WOS) en relación con incontinencia urinaria masculina entre 2002 y 2020. Por medio de la WOS, se obtuvieron las variables a analizar, y se elaboraron tablas y gráficas en el programa Microsoft Office Excel 2017. Resultados Encontramos un total de 498 publicaciones en 88 revistas. Hubo un crecimiento exponencial de las publicaciones a partir de 2006. Las 3 revistas con mayor porcentaje de publicaciones fueron: J Urol, con 25,2% (125), Neurol Urodyn, con 16,5% (82), European Urology Journal, con 11,1% (55); sólo 5 de las 20 revistas con más publicaciones tuvieron un factor de impacto ≥ 3. El país que tuvo mayor porcentaje fue Estados Unidos con 21,2% (105), seguido por países europeos. En América Latina, el país con mayor número de publicaciones fue Brasil, con 2,6% (13). No se encontraron estudios colombianos con esta búsqueda específica. Conclusión Este es el primer estudio bibliométrico sobre incontinencia urinaria masculina. Con el tiempo, han aumentado las publicaciones acerca de este tema, y la mayoría está concentrada en revistas de Estados Unidos y Europa. No es predominante la publicación en revistas de alto factor de impacto, y es muy baja la cantidad de publicaciones que aporta Colombia y los demás países latinoamericanos.

Abstract

Introducción: este artículo pretende estudiar las medidas tomadas en las epidemias de viruela de Santiago (1787), Concepción (1789) y Santafé (1782/1802), para compararlas y entender cómo el estudio de estas epidemias nos puede dar luces para el abordaje del reto de salud pública actual: la pandemia de covid-19. Desarrollo: el artículo está dividido en tres partes: en la primera se exponen las medidas de higiene que se tomaron para subsanar y prevenir estas epidemias, previas a la llegada de la vacunación, comparando el rol desempeñado por los actores locales en cada región; en la segunda se relata y se compara el proceso de llegada y búsqueda de la vacuna contra la viruela en cada territorio, y en la tercera se reflexiona brevemente sobre la pandemia actual. Conclusiones: en el análisis comparativo, se destaca la manera en la que la agenda transcolonial incluye una serie de similitudes para su aplicación en cada territorio, pero también las diferencias que los procesos locales y transcoloniales plantean para su domesticación en cada uno de ellos. Además, se resaltan las particularidades que ha tenido la pandemia de covid-19 y las lecciones que este estudio de caso deja para pensar en la necesidad de enfrentarla desde una perspectiva global.

Abstract

The anti-Müllerian hormone triggers the regression of uterus and fallopian tubes in male embryos; if there are problems in the synthesis or action of this protein, Müllerian structures persist in an otherwise phenotypic male. The most frequent clinical presentation of Persistent Mullerian Duct syndrome is cryptorchidism and inguinal hernia. The few cases reported in adults are incidental findings or inguinal hernias. However, we present an adult male with history of bilateral cryptorchidism with unsuccessful orchidopexy, who presents with a large abdominal mass with the finding of a seminomatous tumor and persistence of Müllerian structures, in whom the variant c.916delC (p.Leu306Cysfs*29) in the AMHR2 gene not previously reported was documented.

Resumen El avance tecnologico en el campo del diagnostico clinico ha generado una diversidad de pruebas de laboratorio aplicables en el lugar de asistencia al paciente (POCT), y ha permitido contar con una mayor calidad analitica de los procedimientos implementados. La elaboracion de un cuadro de mando integral es una herramienta util para el exito en la gestion de un proceso trasversal, complejo e interdisciplinario, como es el de obtener resultados analiticos fiables, transferibles de forma inmediata mediante sistemas POCT. Para la elaboracion de un cuadro de mando integral se deben considerar cuatro perspectivas: la de los clientes (pacientes, medicos, grupos de interes), la financiera (inversores privados o publicos), la de los procesos operativos internos (sistemas, procesos) y la de los profesionales (cultura organizativa). El objeto de este documento es establecer recomendaciones para la elaboracion de un cuadro de mando integral para gestionar los sistemas POCT disponibles en una institucion.

Abstract

This is the first survey of medium- and large-sized mammals for the Private Reserve of Natural Heritage Complex (RPPN) Serra Bonita, in southern Bahia, Brazil. RPPN Serra Bonita is one of the largest private reserves in the Central Corridor of the Atlantic Forest. This study aims to fill a knowledge gap in this area by describing the medium- and large-sized mammals species richness observed in the RPPN Serra Bonita. We installed 19 camera traps for two consecutive years, from November 2010 to November 2012 (sample effort 13.870 camera-days). In addition, some species were recorded using tracks and/or direct observations in the field. We recorded 22 species, including endemic and threatened species, distributed in seven orders: Cetartiodactyla, Carnivora, Cingulata, Lagomorpha, Pilosa, Primates and Rodentia. The crab-eating fox, Cerdocyon thous, was the most recorded species. We also recorded some threatened species, like Leopardus guttulus (VU) and Sapajus xanthosternos (CR). Species richness at RPPN Serra Bonita was higher than that found in other studies conducted in the Atlantic Forest of southern Bahia. The Clench model had a good fit with 96% of the expected mammals recorded in the reserve. This suggests that RPPN Serra Bonita protects a high species richness of medium- and large-sized mammals. Therefore, this small area should be considered a hotpoint within the Atlantic Forest hotspot.

Abstract

Background Overcrowding of the Emergency Department is rapidly becoming a global challenge and a major source of concern for emergency physicians. The desire to improve Emergency Department throughput requires novel approaches to patient flow. Materials and methods We conducted a prospective and cluster-randomized study, to evaluate the impact in patient outcomes of a new patient flow based on Point-of-Care Testing (POCT). A total of 380 Emergency Severity Level-3 patients were enrolled and studied in two different groups, interventional arm (laboratory analyses performed on POCT analyzers implemented in the Emergency Department) or control arm (central laboratory). The primary outcome was the Emergency Department length of stay. Secondary outcome included the time to first medical intervention, the laboratory turnaround time and the time to disposition decision. Readmission within the 7 days after discharge was also calculated. Results Length of stay significantly decreased by 88.50 min (from 247.00 to 158.50), time to disposition decision by 89.00 min (from 192.00 to 103.00) and laboratory turnaround time by 67.11 min (from 89.84 to 22.73) in the POCT group. No increase in readmission was found. Conclusion Our strategy based on POCT represents a good approach to optimize patient flow in the Emergency Department and it should be seen as a starting point for further studies focusing on improving throughput.

Abstract

International audience; Efforts to maintain and enhance reservoir permeability in geothermal systems can contribute to sourcing more sustainable energy, and hence to lowering CO 2 emissions. The evolution of permeability in geothermal reservoirs is strongly affected by interactions between the host rock and the fluids flowing through the rock's permeable pathways. Precipitation of secondary mineral phases, the products of fluidrock interactions, within the fracture network can significantly reduce the permeability of the overall system, whereas mineral dissolution can enhance reservoir permeability. The coupling between these two competing processes dictates the long-term productivity and lifetime of geothermal reservoirs. In this study, we simulate the conditions within a geothermal system from induced fracturing to the final precipitation stage. We performed batch and flow-through experiments on cores of the Carnmenellis granite, a target unit for geothermal energy recovery in Cornwall (UK), to understand the role of mineral dissolution and precipitation in controlling the permeability evolution of the system. The physico-chemical properties of the cores were monitored after each reaction-phase using ICP-OES, SEM, hydrostatic permeability measurements, and X-ray Computed Tomography. Results show that permeability evolution is strongly dependent on fluid chemistry. Undersaturated alkaline fluids dissolve the most abundant mineral phases in granite (quartz and feldspars), creating cavities along the main fractures and generating pressure-independent permeability in the core. Conversely, supersaturated alkaline fluids, resulting from extended periods of fluid-rock interactions, promote the precipitation of clay minerals, and decrease the permeability of the system. These results suggest that chemical dissolution during geothermal operations could generate permeable pathways that are less sensitive to effective stress and will remain open at higher pressures. Similarly, maintaining the circulation of undersaturated fluids through these granitic reservoirs can prevent the precipitation of pore-clogging mineral phases and preserve reservoir permeability in granite-hosted geothermal systems.

Abstract

The Husmuli zone of the SW-Iceland Hellisheidi geothermal field is currently being used for re-injection of geothermal fluids and geothermal CO2 for its permanent storage in the form of carbonate minerals. A fully coupled hydro-thermo-mechanical numerical model was employed to investigate the coupled impacts of these complex processes on the calibration of fluid flow paths, which can have significant implications for the long-term performance of this subsurface reservoir. Employing a combination of high-resolution fault mapping with laboratory measurements of stress dependent permeability coupled into a dual porosity field-scale model, the flow paths were calibrated using results of tracer tests performed at the site using stress-dependent permeability tensors. Although vertically extended faults are the primary fluid flow paths, fractures connecting the faults can play an important role in fluid transport. As the upward flow streamlines manifest, deep geological layers can also deviate the fluid flow towards the shallower layers provoking the vertical flow of geothermal fluids. This highlights the sweet spot for sustainable flow and heat extraction in vicinity of faults intercepting the geological layers at depth of 1100 m. It was also shown that the inclusion of the geomechanical calculations in the history matching of the tracer test could lead to changes in arrival time and peak of the tracer profiles. Results of an independent tracer test were used to validate the model and to demonstrate the predictive capability of the calibrated model. This verifies the consistency of our methodology to incorporate the stress-dependent permeability. The results of this comprehensive modelling study provide insight into the likely fluid flow paths, which can have profound impact on the evaluation of various processes such as CO2 mineralisation taking place in Hellisheidi geothermal reservoir.

Abstract

Abstract Recently, the tiger-cat species complex was split into Leopardus tigrinus and Leopardus guttulus, along with other proposed schemes. We performed a detailed analysis integrating ecological modeling, biogeography, and phenotype of the four originally recognized subspecies—tigrinus, oncilla, pardinoides, guttulus—and presented a new multidimensional niche depiction of the species. Species distribution models used > 1400 records from museums and photographs, all checked for species accuracy. Morphological data were obtained from institutional/personal archives. Spotting patterns were established by integrating museum and photographic/camera-trap records. Principal component analysis showed three clearly distinct groups, with the Central American specimens (oncilla) clustering entirely within those of the Andes, namely the pardinoides group of the cloud forests of the southern Central-American and Andean mountain chains (clouded tiger-cat); the tigrinus group of the savannas of the Guiana Shield and central/northeastern Brazil (savanna tiger-cat); and the guttulus group in the lowland forests of the Atlantic Forest domain (Atlantic Forest tiger-cat). This scheme is supported by recent genetic analyses. All species displayed different spotting patterns, with some significant differences in body measurements/proportions. The new distribution presented alarming reductions from the historic range of − 50.4% to − 68.2%. This multidimensional approach revealed a new species of the elusive and threatened tiger-cat complex.

Abstract

Introduction The midregional fragment of proadrenomedullin (MR-proADM) is known to provide accurate short-, mid- and long term prognostic information in the triage and multi-dimensional risk assessment of patients in the emergency department (ED). In two independent observational cohorts MR-proADM values identified low disease severity patients without risk of disease progression in the ED with no 28 days mortality that wouldn´t require hospitalization. In this interventional study we want to show that the combination of an MR-proADM algorithm with clinical assessment is able to identify low risk patients not requiring hospitalization to safely reduce the number of hospital admissions. Methods A randomized-controlled interventional multicenter study in 4 EDs in Spain. The study protocol was approved by Ethics Committees. Control arm patients received Standard Care. MR-proADM guided arm patients with low MR-proADM value (≤0.87 nmol/L) were treated as out-patients, with high MR-proADM value (>0.87 nmol/L) were hospitalized. The hospitalization rate was compared between the study arms. Results Two hundred patients with suspicion of infection were enrolled. In the MR-proADM guided arm the hospital admission rate in the intention-to-treat (ITT) population was 17% lower than in the control arm (40.6% vs. 57.6%, p=0.024) and 20% lower in the per protocol (PP) population (37.2% vs. 57.6%, p=0.009). No deaths of out-patients and no significant difference for the safety endpoints readmission and representation rates were observed. The readmission rate was only slightly higher in the MR-proADM guided arm compared to the control arm (PP population: at 14 days 9.3% vs. 7.1%, difference 2.1% (95% CI: -11.0% to 15.2%); and at 28 days 11.1% vs. 9.5%, difference 1.6% (95% CI: -12.2% to 15.4%)). The rate of 28 days representation was slightly lower in the MR-proADM guided arm compared to the control arm (20.4% vs. 26.2%, difference -5.8% (95% CI: -25.0% to 13.4%); PP population). Conclusions Implementing a MR-proADM algorithm optimizes ED workflows efficiently and sustainably. Hospitals can highly benefit from a reduced rate of hospitalizations by 20% using MR-proADM. Incluye contenido parcial de los autores Abstract.Mammalian carnivores are considered a key group in maintaining ecologicalhealth and can indicate potential ecological integrity in landscapes where they occur. Carni-vores also hold high conservation value and their habitat requirements can guide managementand conservation plans. The order Carnivora has 84 species from 8 families in the Neotropicalregion: Canidae; Felidae; Mephitidae; Mustelidae; Otariidae; Phocidae; Procyonidae; andUrsidae. Herein, we include published and unpublished data on native terrestrial Neotropicalcarnivores (Canidae; Felidae; Mephitidae; Mustelidae; Procyonidae; and Ursidae). NEOTRO-PICAL CARNIVORES is a publicly available data set that includes 99,605 data entries from35,511 unique georeferenced coordinates. Detection/non-detection and quantitative data wereobtained from 1818 to 2018 by researchers, governmental agencies, non-governmental organi-zations, and private consultants. Data were collected using several methods including cameratrapping, museum collections, roadkill, line transect, and opportunistic records. Literature(peer-reviewed and grey literature) from Portuguese, Spanish and English were incorporated inthis compilation. Most of the data set consists of detection data entries (n=79,343; 79.7%) butalso includes non-detection data (n=20,262; 20.3%). Of those, 43.3% also include count data(n=43,151). The information available in NEOTROPICAL CARNIVORES will contribute tomacroecological, ecological, and conservation questions in multiple spatio-temporal perspec-tives. As carnivores play key roles in trophic interactions, a better understanding of their distri-bution and habitat requirements are essential to establish conservation management plans andsafeguard the future ecological health of Neotropical ecosystems. Our data paper, combinedwith other large-scale data sets, has great potential to clarify species distribution and relatedecological processes within the Neotropics. There are no copyright restrictions and no restric-tion for using data from this data paper, as long as the data paper is cited as the source of theinformation used. We also request that users inform us of how they intend to use the data.

Abstract

The study analyses leadership styles and their influence on burnout and stress among basketball referees, applying a non-probabilistic sampling, convenience sampling, to 61 referees, 85.2% men, 14.8% women. Descriptive analysis, correlations and hierarchical multiple regression models were performed. By applying the Multifactor Leadership Questionnaire MLQ-5, Inventario de Burnout en deportistas Revisado IBD-R [Revised Athlete Burnout Inventory], and Escala de Estrés en el Ámbito Deportivo EEAD [Stress Scale for Sports]. Moderate stress level (26-50) as well as burnout (50-60). Transformational leadership is associated with low levels of stress (p = .003) and high levels of personal accomplishment (p = .026). The developer style, inverse and significant effect (p = .013) with stress. Corrective leadership is associated with low levels of stress (p = .005) and high personal accomplishment (p = .019) and emotional exhaustion (p = .006) of burnout. Passive leadership is associated with high levels of stress (p = .013) and low levels of personal accomplishment (p = .023). As a conclusion, referees mainly apply transformational leadership associated with low levels of stress and high levels of personal accomplishment. They have moderate level of stress and burnout, while there are differences between those at the national level and those at the local-regional level, in addition there are differences regarding the role played, in relation to stress.
