Abstract

Tatton-Brown-Rahman (TBRS) syndrome is a recently described overgrowth syndrome caused by loss of function variants in the DNMT3A gene. This gene encodes for a DNA methyltransferase 3 alpha, which is involved in epigenetic regulation, especially during embryonic development. Somatic variants in DNMT3A have been widely studied in different types of tumors, including acute myeloid leukemia, hematopoietic, and lymphoid cancers. Germline gain-of-function variants in this gene have been recently implicated in microcephalic dwarfism. Common clinical features of patients with TBRS include tall stature, macrocephaly, intellectual disability (ID), and a distinctive facial appearance. Differential diagnosis of TBRS comprises Sotos, Weaver, and Malan Syndromes. The majority of these disorders present other clinical features with a high clinical overlap, making necessary a molecular confirmation of the clinical diagnosis. We here describe seven new patients with variants in DNMT3A, four of them with neuropsychiatric disorders, including schizophrenia and psychotic behavior. In addition, one of the patients has developed a brain tumor in adulthood. This patient has also cerebral atrophy, aggressive behavior, ID, and abnormal facial features. Clinical evaluation of this group of patients should include a complete neuropsychiatric assessment together with psychological support in order to detect and manage abnormal behaviors such as aggressiveness, impulsivity, and attention deficit-hyperactivity disorder. TBRS should be suspected in patients with overgrowth, ID, tall stature, and macrocephaly, who also have some neuropsychiatric disorders without any genetic defects in the commonest overgrowth disorders. Molecular confirmation in these patients is mandatory.

Abstract

© 2018 Sociedad Española para el Procesamiento del Lenguaje Natural. The MOMENT project aims to contribute to a better understanding of severe mental disorders by analyzing the discourse of the two main groups involved, affected people and mental health professionals, in the light of the Conceptual Metaphor Theory and Corpus Linguistics methodology. In this framework, a corpus of first-person accounts from both groups will be designed, built and manually annotated. In turn, the corpus will serve as an experimental bank for automatic detection of metaphors. Therefore, MOMENT aims to improve both manual and automatic metaphor detection and annotation. From a social point of view, the goal of the project is twofold: on the one hand, to detect and systematize dominant conceptualizations and ideas about the disorder; and, on the other hand, to identify and promote the change of potential discourses which deny the agentive capacity of affected people.

Abstract

This study has been conducted with financial support received from Excellence Research Project P12-SEJ-1980 of the Andalusia Regional Government and Project ECO2012- 39576 of Spanish Ministry of Economy and Competitiveness., For financial institutions mobile banking has represented a breakthrough in terms of remote banking services. However, many customers remain uncertain due to its security. This study develops a technology acceptance model that integrates the innovation diffusion theory, perceived risk and trust in the classic TAM model in order to shed light on what factors determine user acceptance of mobile banking applications. The participants had to examine a mobile application of the largest European bank. In the proposed model, an approach to external influences was included, theoretically and originally stated by Davis et al. (1989). The proposed model was empirically tested using data collected from an online survey applying structural equation modeling (SEM). The results obtained in this study demonstrate how attitude determine mainly the intended use of mobile apps, discarding usefulness and risk as factors that directly improve its use. Finally, the study shows the main management implications and identifies certain strategies to reinforce this new business in the context of new technological advances., Para las entidades financieras la banca para móviles ha representado una innovación en términos de servicios de banca remota. Sin embargo, muchos clientes siguen considerando incierta su seguridad. Este estudio desarrolla un modelo de aceptación tecnológica que integra, en el modelo TAM clásico, la teoría de la difusión de la innovación, el riesgo percibido y la confianza, a fin de clarificar qué factores determinan la aceptación de las aplicaciones de banca para móviles por parte del usuario. Los participantes tuvieron que examinar una aplicación para móviles perteneciente al mayor banco europeo. En el modelo propuesto, se incluyó una aproximación hacia las influencias externas, que fue establecida de manera teórica y original por parte de Davis et al. (1989). El modelo propuesto se testó empíricamente utilizando la información recolectada mediante una encuesta online, aplicando el modelo de ecuaciones estructurales (SEM). Los resultados obtenidos en el estudio demuestran el modo en que la actitud determina principalmente el uso previsto de las aplicaciones para móvil, descartando la utilidad y el riesgo como factores que mejoran directamente su uso. Abstract

This paper presents a new energy-harvesting model for a network simulator that implements super-capacitor energy storage with solar energy-harvesting recharge. The model is easily extensible, and other energy-harvesting systems, or different energy storages, can be further developed. Moreover, code can be conveniently reused as the implementation is entirely uncoupled from the radio and node models. Real radiation data are obtained from available online databases in order to dynamically calculate super-capacitor charge and discharge. Such novelty enables the evaluation of energy evolution on a network of sensor nodes at various physical world locations and during different seasons. The model is validated against a real and fully working prototype, and good result correlation is shown. Furthermore, various experiments using the ns-3 simulator were conducted, demonstrating the utility of the model in assisting the research and development of the deployment of everlasting wireless sensor networks. Abstract

Trigger finger causes pain and a persistent functional limitation of the hand, which can lead to permanent blockage of the flexor tendon. Ultrasonography-guided percutaneous release has been widely reported as a successful technique for trigger finger involving the A1 pulley. This article describes for the first time the use of this technique in an unusual location, the A3 pulley of the fifth finger. A 71-year-old patient presented with a 3-month history of pain and blockage in the fifth finger of the right hand and was diagnosed with a grade III trigger finger, according to the Froimson scale. We performed an ultrasonography-guided percutaneous release technique on the A3 pulley to release the flexor tendon of the fifth finger. Ultrasonography-guided percutaneous polectomy to treat trigger finger in the A1 pulley is an effective alternative treatment to surgery and even has certain advantages over it. The anatomical similarity between the A1 and A3 pulleys was the key factor that supported the use of this technique in this clinical case. Based on past experience in similar cases, we conclude that ultrasonography-guided percutaneous polectomy of the A3 pulley of the fifth finger was a surgical technique which could lead to a satisfactory outcome in the treatment of this condition.
