Flash pulmonary oedema can occur as a result of multiple triggers that may act independently or in concert. One such precipitating factor is bilateral renal artery stenosis which can be treated either with revascularisation or with medical therapy. Unilateral renal artery stenosis, however, is a rare cause of flash pulmonary oedema, especially when the contralateral kidney is still functional. We describe a case of an elderly woman with a history of heart failure with preserved ejection fraction and multiple hospitalisations for hypertensive crisis and flash pulmonary oedema who was found to have right, ostial renal artery stenosis that was treated with stent placement.

3. Hemodynamics and functional state of the contralateral kidney in the early postoperative period after surgical treatment of kidney cancer

AIM: To study the hemodynamics and functional state of the renal tissue of the contralateral kidney in the early postoperative period after surgical treatment of kidney cancer. MATERIALS AND METHODS: The prospective study included 58 patients with renal cell carcinoma, 36 (62.1%) of whom underwent radical nephrectomy, and 22 (37.9%) underwent partial nephrectomy. Tumor sizes ranged from 1.0 to 12.0 cm. All patients before surgery and in the early postoperative period underwent ultrasound examination of the structure and size of the kidneys, Doppler ultrasonography of the renal vessels, biomicroscopy of the bulbar conjunctiva, measured peripheral blood pressure, determined the glomerular filtration rate (GFR) and performed a coagulogram. The control group included 16 healthy adults. RESULTS: In 83.3% of patients after radical nephrectomy and in 13.6% of patients after partial nephrectomy a tendency towards an increase in blood pressure compared with the initial values was noted by the 2-4th day after the operation. By the 5th day after surgery, the volume of the only kidney remaining after radical nephrectomy increased by an average of 4% (from 126.1 1.4 to 131.2 2.1 cm3, p 0.05), while after partial nephrectomy has not changed reliably. After surgery, a decrease in GFR was detected in 34 (58.6%; p 0.05) patients, including after radical nephrectomy (n = 28) up to 73.4 8.2 ml / min / 1.73 m2, after partial nephrectomy (n = 6) up to 98.2 3.4 ml / min / 1.73 m2. Doppler ultrasonography of the vessels of a single kidney in patients after radical nephrectomy showed a moderate increase in linear blood flow, an increase in the resistance index in the main trunk of the renal artery, and a decrease in the pulsation index in the segmental and arc arteries. In patients after partial nephrectomy in the contralateral kidney these changes were not observed. When performing biomicroscopy of the bulbar conjunctiva in 83.3% of patients after radical nephrectomy and in 13.6% of patients after partial nephrectomy, changes in the microvasculature were revealed: narrowing of arterioles, expansion of venules, slowing of venular and capillary blood flow. Before the operation and in the early postoperative period, the content of fibrinogen and soluble fibrin-monomer complex in the blood of patients with renal cell carcinoma was significantly higher than in the control group. CONCLUSIONS: In patients with renal cell carcinoma, changes in the contralateral kidney in the early postoperative period after radical nephrectomy are significantly more pronounced than after partial nephrectomy, and are accompanied by changes in systemic and local hemodynamics and kidney function. The results of the study confirm the feasibility of performing organ-preserving surgeries in patients with renal cell carcinoma.

We reviewed the records of 144 patients. The mean gestational age at first US diagnosis was 27.5 ± 4.3 weeks. An anomaly of the contralateral kidney was detected in 25% of cases. An extrarenal anomaly was detected in 13.8%. Karyotype analysis was performed in 16.6% of cases and revealed trisomy 18 in 2 cases with extrarenal defects. Karyotype analysis was normal in all the patients who had isolated multicystic dysplastic kidney (MCDK). The diagnostic accuracy of prenatal ultrasound was 92.2%. Contralateral kidney anomaly was detected 33.9% of patients, and half of these were vesicoureteral reflux. Antihypertensive therapy was required in 2.6% of cases. Nephrectomy was performed in 8%, and partial or total involution of MCDK was achieved in 33.9% of patients. MCDK can be accurately diagnosed by prenatal sonography, and prognosis depends on extrarenal and contralateral renal abnormalities. In isolated cases, require of surgery is rare, and serial follow-up is suggested to determine involution.Impact statement

Aim: Assess the tumors and compensatory capacity of the kidneys on the basis of determining the volume of functioning kidney tissue according to single-photon emission scintigraphy (SPECT). Materials and Methods: 65 patients with kidney parenchyma cancers, 32 patients with cysts, and 15 patients with no kidney pathology were included in our study. In addition, we looked into 57 children with a variety of non-cancerous pathologies. The ratio of the volumes of functioning tissue and particular activities between the afflicted and contralateral kidneys, as well as the ratio of the values of the obtained kidney volumes to the appropriate volume for the patient under research, based on his anthropometry data (sex and age), were employed. Results: According to the findings, the highest compensatory renal reserve occurs when the volume of functioning tissue of the afflicted kidney decreases and the volume of the contralateral kidney increases. Backgroung/Aim: Salivary gland malignancies are rare, with only a fraction due to metastases. We report a rare case of the Clear Cell Renal Cell Carcinoma (CCRCC), a subtype of the Renal Cell Carcinoma (RCC), metastasizing to the parotid gland more than five years following nephrectomy. Case report: A 75-year-old female presented with a unilateral left parotid mass that was growing over the period of 18 months. After clinical and laboratory examinations, the patient underwent left partial superficial parotidectomy with preservation of the facial nerve. The significance of the patient's history of the CCRCC was pivotal to the histopathological diagnosis of a metastatic CCRCC to the parotid. Conclusions: The unpredictable nature of the RCC results in the need for a long period of follow-up, as well as having a high degree of suspicion of metastasis in a patient presenting with a medical history of the RCC and a parotid mass.

Introduction and objectives. Exceptionally clinically important is clear understanding of the pathophysiological mechanisms for the intact kidney involvement of in course of the unilateral inflammatory process. The study aimed to identify the presence and severity of the intact kidney tissue changes in course of rat acute unilateral kidney inflammation. Materials and methods. The experiment was carried out on 52 mature nonlinear white rats Rattus Norwegicus. In 32 animals (main group) an acute unilateral kidney inflammation has been modeled. For modeling of the acute unilateral kidney inflammation in experimental rats, an E.Coli strain was injected in the kidney parenchyma according to our own method. The pathogen was previously isolated from urine in patients with the diagnosed urinary tract infection. The control group consisted of 20 animals, the same amount of sterile 0,9% NaCl was injected in the kidney. The observation period lasted 10 days. Results. Starting from the 3
rd day, signs of hematogenous dissemination of infection / inflammation into contralateral kidney were registered. Starting from the 5th day urinogenous dissemination process was observed. By the 3rd day of study a cortical hyperemia in contralateral kidney was seen, which possibly was of compensatory origin. Conclusions. Background/aim The criteria for surgical management of ureteropelvic junction obstruction are not well-defined, and there is a risk for loss of renal function before the operation. In this context, certain changes in contralateral kidney had been investigated in order to increase the sensitivity of diagnosis. In this study, we aimed to investigate whether contralateral transient minimal hydronephrosis (CTMH) can be considered as an “early alarm” sign for worsening of the affected kidney in infants with hydronephrosis. Materials and methods A total of 182 infants (92 surgically treated and 90 conservatively followed-up) with unilateral hydronephrosis were retrospectively analyzed. Ultrasonography and renal scan findings were evaluated. Correlation between the appearance of CTMH, contralateral compensatory hypertrophy (CCH) on ultrasonography, and prognosis of the affected kidney were evaluated. Results Among the surgically treated patients, 18 (19.6%) patients developed CTMH on average 7 months (0–13 months) before surgery. Among these 18 patients with CTMH, 12 patients (66.6%) had loss of renal function preoperatively, while this ratio was 29.7% on their counterparts (p = 0049). CCH was observed in 31 (33.7%) individuals in surgically treated patient group including all 18 patients with CTMH, while none of the conservatively followed-up patients developed CCH and/or CTMH. In the multiple logistic regression analysis, among the variables investigated, CTMH was found as an independent predictor of the deterioration in the affected kidney and of the poor prognosis (p = 0.011 and p = 0.0004, respectively). Conclusion In our study, among the variables investigated, CTMH was found as an independent predictor of the deterioration in the affected kidney and poor prognosis in infants followed-up with isolated unilateral hydronephrosis. Additionally, CTMH can be considered as an “early alarm” sign for worsening of the affected kidney and the need for surgical intervention.

Introduction Renal leiomyosarcoma is a rare entity in the world and its understanding is based on reports and various cases; however, the prognosis is bleak for its malignant potential and an average survival of 18 months. Clinical case A 54-year-old woman with a clinical picture of 6 months of pain in the right flank and a tomographic image of a bilateral renal lesion underwent right radical nephrectomy and left conservative surgery. The definitive histopathological study reported right primary renal leiomyosarcoma with left metastasis. Conclusion Renal leiomyosarcoma is an entity of low incidence and high mortality; however, our case represents the minority of patients with contralateral kidney metastasis reported in the literature.

The laparoscopic approach to tumour nephrectomy in children is controversial. We therefore reviewed our institution's cases of tumour nephrectomy (laparoscopic, open, and converted) to better understand which is suitable for this approach, what factors prevent it, and whether one can excise tumours greater than the CCLG recommendation of 300 ml.All tumour nephrectomies performed between 2002 and 2016 were identified using our surgical database. Further data were gathered from radiology and pathology databases. Those with nonrenal tumours or having a partial nephrectomy were excluded. Tumour maximum diameters, volumes, and ratios to contralateral kidneys were calculated. A Mann-Whitney U was used to compare the groups.Forty-three cases were included. Fifteen procedures were completed laparoscopically (35%), and a further 3 converted. The median age at surgery was 2.5 years (range 0-10) in the laparoscopic group and 2 years (range 0-15) in the open group. There was a significant difference (P0.05) between the laparoscopic and open groups for: median maximum diameter (10cm vs 12.25cm), median volume (155 ml vs 459 ml), maximum diameter ratio (1.22 vs 1.75), and volume ratio (3.8 vs 11.2).Tumours in the laparoscopic group were significantly smaller, but it was possible to excise tumours more than 300 ml. Difficulties in excision related to tumour size relative to the abdomen. Therefore, a ratio of tumour to contralateral kidney may be a better guide to safe excision than an overall volume cutoff. From our series, the laparoscopic approach is likely to be achievable if the volume ratio is ≤ 8.1.Level 3.

Renal cell carcinoma has a propensity to metastasize to lung, bone, liver, brain, adrenal glands and the contralateral kidney. The proportion of metastatic RCC at diagnosis is estimated to be about 30–40%, and about 20–50% patients will develop metastasis after radical nephrectomy. Gallbladder metastasis from RCC is rare and is found in only about 0.6% of patients at autopsy.1 We present a case of metachronous metastasis of clear cell carcinoma to the gallbladder in a 55-year-old woman one year after radical nephrectomy.

INTRODUCTION AND OBJECTIVES:Replacement of healthy renal parenchyma by renal cell carcinoma (RCC) can contribute to preexisting CKD and thus complicate management. However the magnitude and factors associated with this phenomenon have not been described.METHODS:A retrospective review was performed of all radical nephrectomies (RN) performed at our center from 2006-2016. 155 patients with available nuclear renal scan and imaging demonstrating a single RCC tumor with normal contralateral kidney were included for analysis. To analyze the volumetric changes of the ipsilateral and contralateral kidneys and their effect on functional outcome post partial nephrectomy using segmentation analysis.We have analyzed the data of 119 patients from a single surgeon series of partial nephrectomy patients. Median follow-up was 11.40 months. Patients with bilateral tumors, and solitary kidney were excluded from analysis. Volumetric measurements were performed using a semiautomated tissue segmentation tool. A simple linear regression model to assess the predictors for parenchymal volume loss (PVL). A multivariable linear regression model was used to evaluate the association between PVL and warm ischemia time (WIT), controlling for other factors.Mean WIT was 12.09 ± 4.40 minutes and the mean percentage decrease in the volume of the operated kidney was 16.99 ± 13.49%. WIT (β = 1.24, P0.001) and tumor complexity (simple vs. intermediate, β = 0.06, P = 0.984; simple vs. high, β = 11.62,P = 0.007) were associated with PVL. A 1 minute increase in WIT was associated with an increase in the percentage volume loss in the operated kidney by 1.38% (β = 1.20, P0.001). Patients with high tumor complexity (β = 11.17, P = 0.009) had a significantly higher percentage volume loss compared to patients with simple tumor complexity. Ipsilateral PVL (β = -0.35, P = 0.015) and male gender (β = -9.89, P = 0.021) were associated with change in eGFR. After adjusting for confounders, % volume loss (β = -0.32, P0.001) remained a significant predictor for contralateral hypertrophy.Tumor complexity results in higher WIT and increased PVL as measured by volumetric segmentation. PVL is a key factor associated with functional outcome, and is directly linked to WIT. Increased PVL is also associated with decreased contralateral hypertrophy. Prospective studies with larger samples sizes will be required to validate our findings.

Traditionally, conventional studies, in example ultrasonography (US), contrast enhanced computed tomography (ceCT) and magnetic resonance imaging (MRI) are employed for detecting and characterizing renal masses. International guidelines recommend the use of ceCT in order to asses the tumour extension, including the widespread of disease in the local and distant lymph nodes, the assessment of the surrounding and distant organs and the function of the contralateral kidney.

To evaluate all possible risk factors that can cause impairment of overall renal function in patients with unilateral ureteral calculus and a normal contralateral kidney.This is a prospective study of 90 patients who presented to our institute complaining of renal colic secondary to unilateral ureteral calculus. All patients were evaluated with a thorough history, physical examination, and laboratory and radiological investigations including renal function testing, urine analysis, non-contrast computed topography, and radionucleotide scan. Patients were divided into two groups according to their calculated creatinine clearance using the Modification of Diet in Renal Disease (MDRD) formula. Group I (favorable group) had a creatinine clearance60 ml/min, while group II (unfavorable group) had a creatinine clearance60 ml/min.The patients' mean age ± SD was 38.8 ± 11.4 years. Group I included 54 patients (60 %), while group II included 36 patients (40 %). On univariate analysis, factors that were associated with overall renal function impairment were patients' age, urea-to-creatinine ratio (UCR), use of nonsteroidal anti-inflammatory drugs, stone location, and presence of obstruction. However, using binary logistic regression analysis, only patients' age, UCR, and presence of obstruction sustained statistical significance in association with renal function impairment.The study of factors that help explain the presence of renal impairment in patients with unilateral ureteral calculus is important in the clinical setting. Patients' age, urea-to-creatinine ratio, and degree of obstruction seem to be significantly associated with overall renal function impairment.

To investigate the usefulness of the ellipsoid formula for assessing compensatory hypertrophy of the contralateral kidney on pre-operative and post-operative CT in renal cell carcinoma (RCC) patients.We retrospectively identified 389 patients who had radical nephrectomy for RCC between 2011 and 2015. Contrast-enhanced CT was performed within 3 months pre-operative and at 1 year post-operative. The kidney volumes were calculated from CT using the ellipsoid formula. We subdivided patients into three groups based on tumour size (I: ≤4 cm, II: 4-7 cm, III: 7 cm). Volumetric renal parameters were compared and multivariate analyses were performed to determine predictors associated with pre-operative and post-operative compensatory hypertrophy.Kidney volume calculation using the ellipsoid method took a median of 51 s. Group III had a significantly larger median pre-operative contralateral renal volume than Groups I and II (I: 140.4, II: 141.6, III: 166.7 ml, p0.05). However, the median ratio of post-operative contralateral renal volume change was significantly higher in Groups I and II than Group III (I: 0.36, II: 0.23, III: 0.12, p0.001). On multivariate analysis, tumour size revealed the strongest positive association with pre-operative contralateral kidney volume (partial regression coefficient: β = 30.8, 7 cm) and ratio of post-operative contralateral kidney volume change (β = 0.214, I vs III; β = 0.168, II vs III).Kidney volume calculation for assessing pre- and post-operative compensatory hypertrophy of the contralateral kidney in RCC patients can be easily and rapidly performed from CT images using the ellipsoid formula. Advances in knowledge: The ellipsoid formula allows reliable method for assessing pre-operative and post-operative compensatory hypertrophy of the contralateral kidney in RCC.

Exceptionally clinically important is clear understanding of the pathophysiological mechanisms for localization and migration of pathologic microorganisms towards the intact kidney in course of the unilateral inflammatory process. The study aimed to determine the intact kidney’s pathogenic species composition and numerosity in course of rat acute unilateral kidney inflammation, different time frames after the affected organ removal. The experiment was carried out on 54 mature nonlinear white rats Rattus Norwegicus. In all animals an acute unilateral kidney inflammation has been modeled. For modeling of the acute unilateral kidney inflammation in experimental rats, an E.Coli strain was used. The pathogen was previously isolated from urine in patients with the diagnosed urinary tract infection. In order to determine the affected kidney influence on the intact organ microbial contamination, nephrectomy on the affected side was performed on the 10th, 14th and 21st days. Bacterial concentrations in the residual kidney tissue were determined 4, 7 and 14 days after nephrectomy. Control group constituted animals without nephrectomy. Scores were compared to the respective ones of the intact kidney in control group, on the same day of experiment. Reliable changes of almost all microbiologic contamination scores were detected after removal of the affected organ. Infection of the inoculated kidney spread onto intact contralateral organ by the hematogenous and/or lymphogenous routes and persisted there for some specific time. The nature of the infection in the contralateral kidney changed between the 10th and 14th days after inoculation – the agent apparently penetrated the kidney tissue and developed inflammation. Removal of the inoculated kidney timely, before the infection of the intact organ has progressed to its invasive stage, facilitated complete elimination of the pathogen in the contralateral kidney by the 28th day., Виняткове клінічне значення має чітке розуміння патофізіологічних механізмів локалізації (нерозповсюдження) та міграції мікроорганізмів в інтактну нирку за умов однобічного запального процесу. Дослідження переслідувало мету визначити видовий популяційний склад та кількісні характеристики патологічної мікробіоти в тканині інтактної нирки за однобічного запального процесу нирок у різні терміни після видалення ураженого органа. Експеримент здійснено на 54 статевозрілих нелінійних білих щурах Rattus Norwegicus. Всім тваринам моделювали гострий однобічний запальний процес нирок. Для моделювання запального процесу в нирках дослідних щурів був використаний штам E.Coli, виділений з сечі хворої з діагностованою інфекцією сечової системи. З метою визначення ступеню впливу ураженої нирки на мікробну контамінацію інтакного органа, проводили нефректомію з ураженого боку на 10-ту, 14-ту та 21-шу добу експерименту. Концентрації збудників у тканині резидуальної нирки визначали через 4, 7 та 14 діб після видалення ураженого органа. Тваринам контрольної групи також моделювали гостре однобічне запалення нирок, проте нефректомію з ураженої сторони не проводили. Показники порівнювали з аналогічними в інтактної нирки на таку ж добу експерименту у щурів контрольної групи. Визначено, що після видалення ураженої нирки у всіх підгруп тварин змінювалися практично всі показники контамінованості контрлатерального органа по відношенню до таких, де нефректомія не проводилася. Інфекція з інокульованої нирки гематогенним або/і лімфогенним шляхами потрапляла у інтактний контрлатеральний орган і певний час персистувала в ньому. Між 10-ю і 14-ю добою після інокуляції характер інфекційного процесу у контрлатеральному органі змінювався – збудник проникав у тканини нирки спричиняв запальний процес в останній. Видалення інокульованої нирки до переходу інфекційного процесу у контрлатеральній нирці в інвазивну стадію сприяло повній елімінації збудника з контрлатерального органу., Исключительное клиническое значение имеет четкое понимание патофизиологических механизмов локализации (нераспространении) и миграции микроорганизмов в интактную почку в условиях одностороннего воспалительного процесса. Исследование преследовало цель определить видовой популяционный состав и количественные характеристики патологической микробиоты в ткани интактной почки при одностороннем воспалительном процессе почек в разные сроки после удаления пораженного органа. Эксперимент проводился на 54 половозрелых нелинейных белых крысах Rattus Norwegicus. Всем животным моделировали острый односторонний воспалительный процесс почек. Для моделирования воспалительного процесса в почках исследуемых крыс был использован штамм E.Coli, выделенный из мочи больного с диагностированной инфекцией мочевой системы. С целью определения степени влияния пораженной почки на контаминацию контралатерального органа, проводили нефрэктомию с пораженной стороны на 10-е, 14-е и 21-е сутки эксперимента. Концентрации возбудителей оставшейся почки определяли через 4, 7 и 14 суток после удаления пораженного органа. Животным контрольной группы также моделировали острое одностороннее воспаление почек, однако нефрэктомию с пораженной стороны не проводили. Показатели сравнивали с аналогичными в интактной почке в те же сутки эксперимента у крыс контрольной группы. Определено, что после удаления пораженной почки у всех подгрупп животных изменялись практически все показатели контаминации контралатерального органа по отношению к таким, где нефрэктомия не проводилась. Инфекция из инокулированной почки гематогенным и/или лимфогенным путем попадала в интактный контралатеральный орган и некоторое время в нем персистировала. Между 10-ми и 14-ми сутками после инокуляции характер инфекционного процесса в контралатеральном органе менялся – возбудитель, по-видимому, проникал в ткани почки и вызывал воспалительный процесс в органе. Раннее удаление пораженного органа, до перехода инфекционного процесса в контралатеральной почке в инвазивную стадию, способствовало полной элиминации возбудителя из контралатерального органа.

Purpose We investigated the effect of preoperative contralateral kidney (CK) volume on renal function after radical nephroureterectomy (RNU). Methods We reviewed 630 patients who underwent RNU between September 1994 and December 2013. Of these 630 patients, 135 patients with advanced upper tract urothelial cancer were ultimately included. Preoperative CK volume was measured from computed tomography images of the venous phase using a specialized volumetric program. Glomerular filtration rate (GFR) was calculated using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations and checked preoperatively, 7 days, 1, 3, and 12 months postoperatively. Results The mean age at surgery was 62.1 (10.1) years and the mean CKD-EPI GFR was 73.8 (75.5) ml/min/1.73 m 2 . After RNU, new-onset CKD was identified in 69 (51.1%) patients. On multivariate logistic regression analysis, high body mass index (95% CI: 1.08–1.51, P = 0.005), low preoperative CKD-EPI GFR (95% CI: 0.92–0.98, P = 0.037), and low CK volume (95% CI: 0.96–0.99, P ρ = 0.495, P P = 0.001). Conclusions High body mass index, low preoperative CKD-EPI GFR, and low CK volume are significantly associated with new-onset CKD. Therefore, they could be used to identify patients eligible for neoadjuvant chemotherapy in upper tract urothelial cancer.

An affected kidney was removed in various terms of the experiment in order to determine its influence on functional activity of the intact kidney. Nephrectomy on the affected side was performed on the 10th, 14th and 21st days. Functional scores of the residual kidney were evaluated 4, 7 and 14 days after nephrectomy. Control group constituted animals without nephrectomy. Scores were compared to the respective ones of the intact kidney in control group, on the same day of experiment. Reliable changes of almost all functional kidney scores were detected after removal of the affected organ. Early nephrectomy (on the10th and 14th days) evoked compensatory reaction of the residual kidney and resulted in normalization of functional parameters on the 28th day., С целью определения степени влияния пораженной почки на функциональную активность контралатерального органа, первую удаляли в разные термины от начала эксперимента. Нефрэктомию с пораженной стороны проводили на 10-ые, 14-ые и 21-ые сутки эксперимента. Функциональные показатели оставшейся почки исследовали через 4, 7 и 14 суток после нефрэктомии. Показатели сравнивали с аналогичными в интактной почке на такие самые сутки эксперимента, у крыс, которым не проводилась нефрэктомия с пораженной стороны. Определено, что после удаления пораженной почки у всех подгрупп животных изменялись практически все показатели функции контралатерального органа по отношению к таким, где нефрэктомия не проводилась. Ранняя нефрэктомия – на 10-ые и 14-ые сутки – приводила к нормализации функциональных показателей и развития компенсаторной реакции со стороны резидуальной почки до 28 суток., З метою визначення ступеня впливу ураженої нирки на функціональну активність контрлатерального органа, першу видаляли в різні терміни від початку експерименту. Нефректомію з ураженого боку проводили на 10-ту, 14-ту та 21-шу добу експерименту. Функціональні показники резидуальної нирки досліджували через 4, 7 та 14 діб від нефректомії. Показники порівнювали з аналогічними в інтактної нирки на таку ж добу експерименту у щурів, що не піддалися нефректомії з ураженого боку. Визначено, що після видалення ураженої нирки у всіх підгруп тварин змінювалися практично всі показники функції контрлатерального органа по відношенню до таких, де нефректомія не проводилася. Рання нефректомія – на 10-ту та 14-ту добу – призводила до нормалізації функціональних показників та розвитку компенсаторної реакції з боку резидуальної нирки до 28-ї доби.

A bilateral S-shaped kidney is a rare anomaly in which both the kidneys are in their normal position, in contrast to the commonly reported S-shaped fusion anomaly, in which the contralateral kidney crosses the midline to fuse with opposite kidney leaving the ipsilateral renal fossa empty. Here we present the diagnosis and management of a case of bilateral S-shaped renal anomaly with associated left pelviureteric junction obstruction and nonfunctioning kidney and right renal stones. Left kidney was managed by open nephrectomy and right kidney by PNL.

- Abstract
Background: The progression of chronic renal failure in patients with unilateral renal injury is associated with loss of function in the contralateral kidney, but the molecular mechanism remains unclear. The activation of mineralocorticoid receptor (MR) in the kidney contributes to renal cell damage, leading to apoptosis, pyroptosis, and necrosis. Pyroptosis is a programmed cell death induced by caspase-1, which is usually activated by nod-like receptor pyrin-containing 3 (NLRP3) inflammasomes. Our study aimed to investigate the effects of eplerenone (EPL) on cell pyroptosis in the contralateral kidneys of unilateral ureteral obstruction (UUO) rats., Methods: Sprague-Dawley rats were randomly divided into 3 groups: sham group, UUO group (10 days of left ureter ligation), and UUO treated with EPL (UUO + EPL) group. The contralateral kidneys of all rats were collected for studies., Results: We observed evidently increased number of pyroptosis cells in the contralateral kidneys of UUO rats compared to those from Sham rats. The expression of NLRP3, caspase-1, interleukin-1β, serum and glucocorticoid-inducible protein kinase-1, and nuclear factor kappa B were also upregulated in the contralateral kidneys of UUO rats compared to Sham kidneys, and these effects were reduced by MR blocker EPL., Conclusion: Our data suggest that the activation of MR is involved in NLRP3/caspase-1 pathway-induced cell pyroptosis in the contralateral kidney of UUO model., (© 2019 S. - Abstract
Background: The unilateral ureteral obstruction (UUO) model not only induces renal interstitial fibrosis in the obstructed kidney but also induces injury in the contralateral kidney. We hypothesized that activation of the mineralocorticoid receptor (MR) may induce fibrosis in the early stage of UUO., Methods: Thirty male Sprague-Dawley rats weighting 200 ± 10 g were used in this study and randomly divided into 3 groups: a UUO group, a UUO and eplerenone group, and a sham group. The contralateral kidney and plasma were harvested for further study 10 days after surgery., Results: The level of plasma aldosterone (869.95 ± 55.851 pg/mL) was significantly higher in the UUO group than that in the sham group (478.581 ± 36.186 pg/mL vs. UUO, p < 0.05). The infiltrated inflammatory cells (F4/80) and deposited collagens were increased significantly in the contralateral kidneys in the UUO group compared to those in the sham group, which were decreased by eplerenone. However, proliferating cell nuclear antigen was increased 2.47 times in the UUO group compared to the sham group in the contralateral kidney (p < 0.01), and those changes are attenuated by eplerenone. The expression of SGK-1 protein and mRNA was upregulated in the contralateral kidney in the UUO group, which is suppressed by eplerenone treatment. NF-κB pathway effecters were also changed markedly in the contralateral kidney in the UUO group and partly reversed by eplerenone., Conclusion: Aldosterone induces inflammatory cell proliferation via the MR/SGK-1 and NF-κB pathways and eventually leads to fibrosis in the contralateral kidney., (© 2017 S.
