- Abstract
Coronavirus disease 2019 (COVID-19) has negatively affected the delivery of respiratory diagnostic services across the world due to the potential risk of disease transmission during lung function testing. Community prevalence, reoccurrence of COVID-19 surges and the emergence of different variants of SARS-CoV-2 have impeded attempts to restore services. Finding consensus on how to deliver safe lung function services for both patients attending and for staff performing the tests are of paramount importance. This international statement presents the consensus opinion of 23 experts in the field of lung function and respiratory physiology balanced with evidence from the reviewed literature. It describes a robust roadmap for restoration and continuity of lung function testing services during the COVID-19 pandemic and beyond. Important strategies presented in this consensus statement relate to the patient journey when attending for lung function tests. We discuss appointment preparation, operational and environmental issues, testing room requirements including mitigation strategies for transmission risk, requirement for improved ventilation, maintaining physical distance and use of personal protection equipment. We also provide consensus opinion on precautions relating to specific tests, filters, management of special patient groups and alternative options to testing in hospitals. The pandemic has highlighted how vulnerable lung function services are and forces us to re-think how long-term mitigation strategies can protect our services during this and any possible future pandemic. This statement aspires to address the safety concerns that exist and provide strategies to make lung function tests and the testing environment safer when tests are required.

- Abstract
Background: Cisplatin (cDDP) has regained interest for metastatic breast cancer (MBC) patients, given the platinum sensitivity in subtypes and better manageable toxicity. Here, the primary aim was to determine whether molecular characteristics of circulating tumor cells (CTCs) could identify patients responding to cDDP and to describe the outcomes to cDDP monotherapy in a large group of MBC patients pretreated with anthracycline- and taxane-based treatments.Methods: Based on cell line data, a CTC-cDDP-sensitivity profile was generated. Applying an A'Herns single-stage phase II design, further investigation was considered worthwhile if 5/10 patients with a favorable profile responded to cDDP. Patients received 70mg/m 2 cDDP every three weeks, CTCs were enumerated and the CTC-cDDP-sensitivity profile was determined. In total, 65 heavily pretreated MBC patients (77% received ≥2 lines of previous chemotherapy for MBC) were eligible for the per-protocol analysis. Primary endpoint was response rate, secondary endpoints included best observed response, progression-free survival (PFS) and overall survival (OS). Results: The best observed response during cDDP therapy was a partial response in 7% and stable disease in 56% of the patients. None of the patients with a favorable CTC-cDDP-sensitivity profile had a response. The median baseline CTC count was 8 (range 0-3254). Patients with <5 CTCs had a better PFS and OS than patients with ≥5 CTCs (median PFS 4.5 months (95%CI 2.38-6.62) vs. 2.1 months [(95%CI 1.34-2.80)( p=0.009)] and median OS 13.1 months (95%CI 9.89-16.33) vs. 5.6 months [(95%CI 3.60-7.64)( p=0.003)]. No other factors than CTC count were associated with outcome to cDDP therapy, including triple-negative breast cancer versus ER-positive tumors. - Abstract
Aim To assess the safety and tolerability of adjuvant subcutaneous trastuzumab (Herceptin® SC, H SC), delivered from an H SC Vial via hand-held syringe (Cohort A) or single-use injection device (Cohort B), with or without chemotherapy, for human epidermal growth factor receptor 2 (HER2)-positive stage I to IIIC early breast cancer (EBC) in the phase III SafeHer study (NCT01566721). Methods Patients received 600 mg fixed-dose H SC every 3 weeks for 18 cycles. The chemotherapy partner was at the investigators' discretion (H SC monotherapy was limited to ≤10% of the population). Data from the first H SC dose until 28 days (plus a 5-day window) after the last dose are presented. Results are descriptive. Results In the overall population, 2282/2573 patients (88.7%) experienced adverse events (AEs). Of the above, 128 (5.0%) patients experienced AEs leading to study drug discontinuation; 596 (23.2%) experienced grade ≥ 3 AEs and 326 (12.7%) experienced serious AEs. Grade ≥ 3 cardiac disorders were reported in 24 patients (0.9%), including congestive heart failure in eight (0.3%). As expected, the AE rates varied according to the timing of chemotherapy in both cohorts, with higher rates in concurrent versus sequential chemotherapy subgroups. In the concurrent chemotherapy subgroup, AEs were more common during the actual period of concurrent chemotherapy compared with the period when patients did not receive concurrent chemotherapy. Conclusion SafeHer confirms the safety and tolerability of the H SC 600 mg fixed dose for 1 year (every 3 weeks for 18 cycles) as adjuvant therapy with concurrent or sequential chemotherapy for HER2-positive EBC. These primary analysis results are consistent with the known safety profile for intravenous H and H SC.

Pediatric asthma is one of the most common chronic disease in childhood. Treatment is focused on the control of asthma symptoms, enabling patients to fully participate in daily life. However, children’s expression of asthma symptoms is often difficult to assess and interpret. Additionally, accurate monitoring of pediatric asthma is challenging as symptoms are episodic and therefore often absent during outpatient evaluation. These aspects hamper the treatment of asthma and urge the development of a monitoring tool that can signal worsening of asthma control and offer an opportunity to anticipate the waves of asthma. Therefore, this study aimed to discover relevant home-monitoring parameters for asthma control in children using wearable technology.

Recently, this international task force reported the general considerations for bronchial challenge testing and the performance of the methacholine challenge test, a "direct" airway challenge test. Here, the task force provides an updated description of the pathophysiology and the methods to conduct indirect challenge tests. Because indirect challenge tests trigger airway narrowing through the activation of endogenous pathways that are involved in asthma, indirect challenge tests tend to be specific for asthma and reveal much about the biology of asthma, but may be less sensitive than direct tests for the detection of airway hyperresponsiveness. We provide recommendations for the conduct and interpretation of hyperpnoea challenge tests such as dry air exercise challenge and eucapnic voluntary hyperpnoea that provide a single strong stimulus for airway narrowing. This technical standard expands the recommendations to additional indirect tests such as hypertonic saline, mannitol and adenosine challenge that are incremental tests, but still retain characteristics of other indirect challenges. Assessment of airway hyperresponsiveness, with direct and indirect tests, are valuable tools to understand and to monitor airway function and to characterise the underlying asthma phenotype to guide therapy. The tests should be interpreted within the context of the clinical features of asthma.

- Abstract
This international task force report updates general considerations for bronchial challenge testing and the performance of the methacholine challenge test. There are notable changes from prior recommendations in order to accommodate newer delivery devices. Rather than basing the test result upon a methacholine concentration (provocative concentration (PC20) causing a 20% fall in forced expiratory volume in 1 s (FEV1)), the new recommendations base the result upon the delivered dose of methacholine causing a 20% fall in FEV1 (provocative dose (PD20)). This end-point allows comparable results from different devices or protocols, thus any suitable nebuliser or dosimeter may be used, so long as the delivery characteristics are known. Inhalation may be by tidal breathing using a breath-actuated or continuous nebuliser for 1 min (or more), or by a dosimeter with a suitable breath count. Tests requiring maximal inhalations to total lung capacity are not recommended because the bronchoprotective effect of a deep breath reduces the sensitivity of the test.

Summary: Background: Many patients with chronic obstructive pulmonary disease (COPD) experience dyspnoea during exercise, resulting in a reduction of physical activity (PA). Dynamic hyperinflation (DH) is seen as a major cause of dyspnoea in COPD. Objective: The objective of the current study was to investigate the relationship between DH, in terms of the amount of DH and the development and recovery rate of DH in patients with COPD, and PA. Methods: Thirty‐five patients with stable COPD were included from an outpatient clinic (14 GOLD II and 21 GOLD III, median age 65). PA was assessed using an accelerometer. Subjects underwent metronome‐paced tachypnoea (MPT) to induce DH. To quantify the amount of DH during MPT, a decrease in inspiratory capacity (IC) or a change in IC as percentage of total lung capacity was used. Results: No significant correlations were found between the parameters describing DH and PA. Secondary correlation analyses showed a negative correlation between static hyperinflation (SH) and PA (
r = −0·39;P = 0·02). The pattern of breathing during MPT and the test itself showed high interpatient variability. Conclusions: The absence of a significant correlation between DH and PA is contrary to previous studies. SH did show a correlation with PA. The variety in results and the technical difficulties in execution of the measurements ask for a new, more reliable, method to detect DH and investigate its relation with PA in patients with COPD. - Abstract
Transcutaneous electromyography (tc-EMG) has been used to measure the electrical activity of respiratory muscles during inspiration in various studies. Processing the raw tc-EMG signal of these inspiratory muscles has shown to be difficult as baseline noise, cardiac interference, cross-talk and motion artefacts can influence the signal quality. In this review we will discuss the most important sources of signal noise in tc-EMG of respiratory muscles and the various techniques described to suppress or reduce this signal noise. Furthermore, we will elaborate on the options available to develop or improve an algorithm that can be used to guide the approach for analysis of tc-EMG signals of inspiratory muscles in future research. - Abstract
The electrical activity of the main breathing muscle, the diaphragm, can be measured with transcutaneous electromyography (dEMG). As this technique provides insight in breathing effort, relevant information on the patient’s respiratory condition can be obtained. This thesis consists of two parts. The first part studied the clinical usability of dEMG in (preterm) infants. The second part investigated innovative dEMG techniques, which could ease routine diaphragm activity measurement, as it is currently measured with separate hardware and software, and could improve patient/healthcare provider friendliness. This thesis shows the great potential of dEMG in terms of cardiorespiratory monitoring based on breathing effort, assessing (changes in) diaphragmatic function and the potential to individualize respiratory support. The second part of the thesis shows that dEMG could be measured with the standard cardiorespiratory monitoring equipment by placing its electrodes at the height of the diaphragm. Next to using the standard monitoring equipment, dEMG could be measured with a wireless and non-adhesive cardiorespiratory monitoring belt based on dEMG. This belt could improve parent-infant interaction as it is wireless. Moreover, removal of adhesive electrodes is prevented which is likely to improve comfort. Both methods could facilitate widespread clinical implementation of dEMG. In conclusion, this thesis shows the great potential of dEMG and large steps were made to implement this technique in the clinical practice of the Neonatal Intensive Care Unit.

The studies presented in this thesis investigated the use of transcutaneous electromyography of the diaphragm (dEMG) in (preterm) infants and children. The underlying objective was to improve current cardiorespiratory monitoring and possibly implement this technique as an aid in the titration of respiratory support in critically-ill children admitted to the neonatal (NICU) or pediatric intensive care unit (PICU). With dEMG the electrical activity of the diaphragm is measured, using skin electrodes. As the diaphragm is the main respiratory muscle, monitoring its function throughout ICU admission could provide relevant information on the patient’s respiratory condition. The thesis investigated the methodology of dEMG, its potential use for patient monitoring in the delivery room and its use as a clinical tool in the ICU to monitor work of breathing, investigate weaning of respiratory support and assess patient-ventilator interaction. The methodological studies show that while interpreting the measurements, the positioning of the electrodes and analysis of the recordings should be considered, as these factors impact signal strength. The clinical studies show dEMG is a feasible technique in both the NICU- and the acute setting (in the delivery room). In both settings the effect of respiratory support on the activity of the diaphragm has been investigated. In the NICU, the correlation between diaphragm activity and work of breathing has been assessed. Lastly, dEMG has shown to be useful to investigate the interaction between the patient and the ventilator during non-invasive respiratory support, showing high levels of patient-ventilator asynchrony. All studies support further research to implement dEMG as a continuous monitoring tool for respiratory effort in the NICU and PICU. In the not too distant future dEMG might be useful to monitor work of breathing and improving synchronization of respiratory support with spontaneous breathing in these critically-ill infants.

Background The phase III CLinical Evaluation Of Pertuzumab And TRAstuzumab (CLEOPATRA) trial established the combination of pertuzumab, trastuzumab and docetaxel as standard first-line therapy for human epidermal growth factor receptor 2 (HER2)-positive locally recurrent/metastatic breast cancer (LR/mBC). The multicentre single-arm PERtUzumab global SafEty (PERUSE) study assessed the safety and efficacy of pertuzumab and trastuzumab combined with investigator-selected taxane in this setting. Patients and methods Eligible patients with inoperable HER2-positive LR/mBC and no prior systemic therapy for LR/mBC (except endocrine therapy) received docetaxel, paclitaxel or nab-paclitaxel with trastuzumab and pertuzumab until disease progression or unacceptable toxicity. The primary endpoint was safety. Secondary endpoints included progression-free survival (PFS) and overall survival (OS). Prespecified subgroup analyses included subgroups according to taxane, hormone receptor (HR) status and prior trastuzumab. Exploratory univariable analyses identified potential prognostic factors; those that remained significant in multivariable analysis were used to analyse PFS and OS in subgroups with all, some or none of these factors. Results Of 1436 treated patients, 588 (41%) initially received paclitaxel and 918 (64%) had HR-positive disease. The most common grade ≥3 adverse events were neutropenia (10%, mainly with docetaxel) and diarrhoea (8%). At the final analysis (median follow-up: 5.7 years), median PFS was 20.7 [95% confidence interval (CI) 18.9-23.1] months overall and was similar irrespective of HR status or taxane. Median OS was 65.3 (95% CI 60.9-70.9) months overall. OS was similar regardless of taxane backbone but was more favourable in patients with HR-positive than HR-negative LR/mBC. In exploratory analyses, trastuzumab-pretreated patients with visceral disease had the shortest median PFS (13.1 months) and OS (46.3 months). Conclusions Mature results from PERUSE show a safety and efficacy profile consistent with results from CLEOPATRA and median OS exceeding 5 years. Results suggest that paclitaxel is a valid alternative to docetaxel as backbone chemotherapy. Exploratory analyses suggest risk factors that could guide future trial design.

- Abstract
Rising number of patients affected by asthma, COPD or lung cancer in most parts of the world, combined with the emerging systems medicine concept and the increasing availability of biological therapies for respiratory diseases, call for further improvement in diagnosis and management of disease. In particular this holds for better phenotyping of individual patients, which is a prerequisite for the development and optimization of precision medicine approaches. This calls for new (non-invasive) biomarkers for early detection, biological phenotyping, monitoring of disease activity, selection of therapies, individualized dosing of therapies and prediction of outcome. Exhaled breath contains thousands of volatile organic compounds (VOCs) that are reflective of metabolic processes in the body. Comprehensive analysis of expired VOCs (breathomics) includes many favouring aspects that fit the unmet need of (non-invasive, point-of-care) tools for the detection of biomarkers. The aim of this thesis was to develop breathomics (by electronic nose technology) further towards a point-of-care tool in (chronic) respiratory diseases. Studied areas of application were: diagnosis, phenotyping, therapeutic drug level measurement, screening and disease activity monitoring. From an engineering perspective there were advancements on sampling and analyses strategies to support (international) multicentre research. Furthermore, the foundation of SpiroNose, a concept which merges breathomics with well-established spirometry, has enabled to run breathomics research in busy outpatient settings. eNose technology is heading towards clinical application step by step, however there are important challenges as it concerns metabolic evidence, confirmation of findings and dealing with confounders like drug usage and smoking.

Disorders of the respiratory system can lead to serious physiological damage. Pulmonary diseases are typically divided into obstructive and restrictive disorders, which are being investigated in this work with a focus on chronic obstructive pulmonary disease, bronchial asthma, and cardiac pulmonary edema. In this work, an object-oriented physiological model of the human respiratory system was developed for the analysis of lung diseases and various methods of pulmonary function diagnostics. The approach of non-linear compartment modeling was followed. The developed model offers a new possibility for the model-based analysis of lung mechanics, which enables the re-evaluation of conventional lung function diagnostics measurement methods. To model cardiopulmonary diseases, the model has been expanded to include the cardiovascular system and the fluid balance of the lung. For model validation, simulation results were compared with the clinical data published in the literature. Finally, the work discussed the potential use of electrical impedance tomography (EIT) for the assessment of regional lung mechanics. Aim To assess the safety and tolerability of adjuvant subcutaneous trastuzumab (Herceptin ® SC, H SC), delivered from an H SC Vial via hand-held syringe (Cohort A) or single-use injection device (Cohort B), with or without chemotherapy, for human epidermal growth factor receptor 2 (HER2)-positive stage I to IIIC early breast cancer (EBC) in the phase III SafeHer study (NCT01566721). Methods Patients received 600 mg fixed-dose H SC every 3 weeks for 18 cycles. The chemotherapy partner was at the investigators' discretion (H SC monotherapy was limited to ≤10% of the population). Data from the first H SC dose until 28 days (plus a 5-day window) after the last dose are presented. Results are descriptive. Results In the overall population, 2282/2573 patients (88.7%) experienced adverse events (AEs). Of the above, 128 (5.0%) patients experienced AEs leading to study drug discontinuation; 596 (23.2%) experienced grade ≥ 3 AEs and 326 (12.7%) experienced serious AEs. Grade ≥ 3 cardiac disorders were reported in 24 patients (0.9%), including congestive heart failure in eight (0.3%). As expected, the AE rates varied according to the timing of chemotherapy in both cohorts, with higher rates in concurrent versus sequential chemotherapy subgroups. In the concurrent chemotherapy subgroup, AEs were more common during the actual period of concurrent chemotherapy compared with the period when patients did not receive concurrent chemotherapy. Conclusion SafeHer confirms the safety and tolerability of the H SC 600 mg fixed dose for 1 year (every 3 weeks for 18 cycles) as adjuvant therapy with concurrent or sequential chemotherapy for HER2-positive EBC. These primary analysis results are consistent with the known safety profile for intravenous H and H SC.

- Abstract
The European Sleep Research Society (ESRS) is the leading voice for sleep medicine and research in Europe and includes in its mission the objective to promote and develop sleep education. As part of this objective, the Society established a Multidisciplinary Task Force on sleep education, which proposed a workshop on this topic to define the role of ESRS in the future of sleep education in Europe. This includes defining the domains of operation by the ESRS, its most important links and networks, and to recognise areas for development. A concrete goal was to make an action plan with suggestions for eventual Task Forces. Recommendations from the workshop include active collaboration with other Societies and University degree courses in sleep with the objective to position the ESRS as the central focus for sleep education in Europe. Predefined learning objectives and Entrustable Professional Activities were identified as key aspects of future direction. The ESRS Textbook in Sleep Medicine provides a valuable educational resource, and the next edition should expand on the traditional format to include digital material and an integrative platform. The workshop also identified the need to develop a more compact publication to inform non-sleep specialists, university students, and interested members of the public with basic information on sleep and sleep disorders., (© 2025 European Sleep Research Society.)

- Abstract
There is no universally accepted method for positive end expiratory pressure (PEEP) titration approach for patients on spontaneous mechanical ventilation (SMV). Electrical impedance tomography (EIT) guided PEEP-titration has shown promising results in controlled mechanical ventilation (CMV), current implemented algorithm for PEEP titration (based on regional compliance measurements) is not applicable in SMV. Regional peak flow (RPF, defined as the highest inspiratory flow rate based on EIT at a certain PEEP level) is a new method for quantifying regional lung mechanics designed for SMV. The objective is to study whether RPF by EIT is a feasible method for PEEP titration during SMV. Single EIT measurements were performed in COVID-19 ARDS patients on SMV. Clinical (i.e., tidal volume, airway occlusion pressure, end-tidal CO
2 ) and mechanical (cyclic alveolar recruitment, recruitment, cumulative overdistension (OD), cumulative collapse (CL), pendelluft, and PEEP) outcomes were determined by EIT at several pre-defined PEEP thresholds (1-10% CL and the intersection of the OD and CL curves) and outcomes at all thresholds were compared to the outcomes at baseline PEEP. In total, 25 patients were included. No significant and clinically relevant differences were found between thresholds for tidal volume, end-tidal CO2 , and P0.1 compared to baseline PEEP; cyclic alveolar recruitment rates changed by -3.9% to -37.9% across thresholds; recruitment rates ranged from - 49.4% to + 79.2%; cumulative overdistension changed from - 75.9% to + 373.4% across thresholds; cumulative collapse changed from 0% to -94.3%; PEEP levels from 10 up to 14 cmH2 O were observed across thresholds compared to baseline PEEP of 10 cmH2 O. A threshold of approximately 5% cumulative collapse yields the optimum compromise between all clinical and mechanical outcomes. EIT-guided PEEP titration by the RPF approach is feasible and is linked to improved overall lung mechanics) during SMV using a threshold of approximately 5% CL. 48. Changes in Small Airway Physiology Measured by Impulse Oscillometry in Subjects with Allergic Asthma Following Methacholine and Inhaled Allergen Challenge.

- Abstract
Background : Small airway dysfunction (SAD) is associated with impaired asthma control, but small airway physiology is not routinely assessed in clinical practice. Previously, we demonstrated impulse oscillometry (IOS)-defined small airway dysfunction (SAD) in dual responders (DRs) upon bronchoprovocation with various allergens. Aim : To compare lung physiology using spirometry and IOS following bronchoprovocation with methacholine (M) and inhaled house dust mite (HDM) extract in corticosteroid-naïve asthmatic subjects. Methods : Non-smoking, clinically stable HDM-allergic asthmatic subjects (18-55 years, FEV
1 > 70% of pred.) underwent an M and inhaled HDM challenge on two separate days. Airway response was measured by IOS and spirometry, until a drop in FEV1 ≥ 20% (PC20 ) from post-diluent baseline (M), and up to 8 h post-allergen (HDM). Early (EAR) and late asthmatic response (LAR) to HDM were defined as ≥20% and ≥15% fall in FEV1 from post-diluent baseline during 0-3 h and 3-8 h post-challenge, respectively. IOS parameters (Rrs5, Rrs20, Rrs5-20, Xrs5, AX, Fres) were compared between mono-responders (MRs: EAR only) and dual responders (EAR + LAR). Correlations between maximal % change from baseline after the two airway challenges were calculated for both FEV1 and IOS parameters. Results : A total of 47 subjects were included (11 MRs; 36 DRs). FEV1 % predicted did not differ between MR and DR at baseline, but DR had lower median PC20 M (0.84 (range 0.07-7.51) vs. MR (2.15 (0.53-11.29)); p = 0.036). During the LAR, DRs had higher IOS values than MRs. For IOS parameters (but not for FEV1 ), the maximal % change from baseline following M and HDM challenge were correlated. PC20 M was inversely correlated with the % change in FEV1 and the % change in Xrs5 during the LAR (r= -0.443; p = 0.0018 and r= -0.389; p = 0.0075, respectively). Conclusions : During HDM-induced LAR, changes in small airway physiology can be non-invasively detected with IOS and are associated with increased airway hyperresponsiveness and changes in small airway physiology during methacholine challenge. DRs have a small airways phenotype, which reflects a more advanced airway disease.- Published

Among the most discussed issues in the field of pulmonary function testing are reference equations and standardised methods of interpreting pulmonary function test (PFT) results. The Global Lung Function Initiative (GLI) task force gathered information from multiple countries with diverse ethnicities to create a multi-ethnic reference formula suitable for a broad age range. Their first output was the GLI-2012 spirometry reference equations. Together with the GLI-2012 reference equations, the GLI document recommended the use of z-scores to standardise the interpretation of PFT results. Z-scores were introduced as an alternative to per cent predicted and fixed thresholds in interpreting PFTs.
