Abstract

Contesto Questo lavoro di ricerca prende in esame pazienti affetti da Sclerosi Laterale Amiotrofica (SLA), valutando l’impatto di protocolli dietetici specifici nella gestione della malnutrizione. Lo studio intende verificare come la corretta gestione degli aspetti nutrizionali, mediante protocolli dietetici e scale di valutazione della disfagia, possa migliorare l’alimentazione e lo stato nutrizionale dei pazienti affetti da tale patologia, riducendo le complicanze ad essa associate e migliorando la loro qualità di vita. Materiali e Metodi Sono stati presi in esame n.11 pazienti con diagnosi di SLA, ai quali sono stati somministrati due distinti test di screening della deglutizione (Eating Attitude Test 10; Northwestern Dysphagia Patient Check Sheet), ripetuti a distanza di sei mesi. Risultati Dalle valutazioni effettuate si è rilevato un miglioramento (36% dei casi) nella storia di polmoniti ricorrenti e nella frequenza dei picchi febbrili. Per quanto concerne le variabili comportamentali è stato possibile osservare miglioramenti (36% dei casi) nella consapevolezza della disfagia e nella gestione delle secrezioni salivari. Nel 91% dei casi si è evidenziato un netto miglioramento riguardo il ritardo del riflesso faringeo della deglutizione. Discussione I risultati della ricerca, in linea alle evidenze scientifiche di settore, sottolineano quanto sia importante prendersi cura dello stato nutrizionale dei pazienti in relazione alla loro aspettativa e qualità di vita ed evidenziano l’importanza di un monitoraggio costante delle capacità deglutitorie per prevenire le complicanze ad essa associate (e.g. malnutrizione e/o disidratazione). Conclusioni Una corretta presa in carico multidisciplinare, che includa una valutazione continua della deglutizione e l’utilizzo di protocolli dietetici specifici, migliora la nutrizione di pazienti affetti dalla SLA, riducendo il rischio di sviluppare complicanze (e.g. malnutrizione, aspirazione durante il pasto), migliorando la qualità di vita del paziente. Abstract

The endocrine system is frequently altered after a major burn trauma. Besides the endocrine response to stress characterized by hypercortisolism, several hypothalamus-hypophysis-target gland axes are rapidly perturbed within a few days. These alterations can persist in the long term and deserve an appropriate treatment. Disturbances in water clearance and glucidic metabolism are also common and need to be diagnosed and corrected to decrease morbidity in such patients. Bone and mineral metabolism is deeply compromised and requires correction of mineral abnormalities in order to improve symptoms and prevent bone loss. No large prospective and/or intervention trials are available to date to elaborate age-related, evidence-based recommendations to monitor and treat burn-related endocrine alterations.

Abstract

Environmental factors, including nutrition and metal elements, are implicated in the pathophysiology of Alzheimer’s disease (AD). Several in vitro and in vivo data indicate a role for magnesium (Mg) in many biological and clinical aspects of AD. Mg deficiency, aside from having a negative impact on the energy production pathways required by the mitochondria to generate adenosine triphosphate, also affects many biochemical mechanisms vital for neuronal properties and synaptic plasticity, including the response of N-methyl- d -aspartate receptors to excitatory amino acids, stability, and viscosity of the cell membrane. Mg also has an action as a mild calcium antagonist, and as an antioxidant against free-radical damage of the mitochondria. Total and ionized Mg levels in plasma, as well as Mg concentrations in various tissues, were found to be decreased in AD patients and negatively associated with clinical deterioration. Chronic Mg deficiency results in excessive production of oxygen-derived free radicals and low-grade inflammation, these also being possible pathogenic factors in AD. Moreover, Mg was shown to have a role in the processing of amyloid-beta precursor protein, which plays a central role in the pathogenesis of AD. Whether Mg supplementation may help in preventing cognitive decline in the elderly remains to be demonstrated.

5. Vγ9Vδ2 T lymphocytes activation in osteoporotic patients treated with bisphosphonates

Abstract

Treatment with bisphosphonates induces differentiation and activation of Vγ9Vδ2 T lymphocytes obtained from peripheral blood showing also an antitumoral effect in both in vitro and in vivo models. Aim of the present study was to determine in vivo the effect of BPs treatment in patients affected with osteoporosis on Vγ9Vδ2 T lymphocytes. We have studied Vγ9Vδ2 T lymphocytes expansion and differentiation from PBMC obtained from osteoporotic patients treated with one of the following bisphosphonates zoledronate, alendronate, neridronate or risedronate. We have found that zoledronic acid, followed by alendronate was the most effective on reducing CM population (100%) and increasing TEM and TEMRA γδ population. Our results indicate that in vivo treatment with BPs induces Vγ9Vδ2 cells to mature toward the effector phenotype, which may induce more antiresorptive responses.

Abstract

Purpose: We evaluated the role of residual vein thrombosis (RVT) to assess the optimal duration of anticoagulants in patients with cancer who have deep vein thrombosis (DVT) of the lower limbs., Patients and Methods: Patients with active cancer and a first episode of DVT treated with low molecular weight heparin (LMWH) for 6 months were eligible. Patients were managed according to RVT findings: those with RVT were randomly assigned to continue LMWH for an additional 6 months (group A1) or to discontinue it (group A2), and patients without RVT stopped LMWH (group B). The primary end point was recurrent venous thromboembolism (VTE) during the 1 year after disconinuation of LMWH, and the secondary end point was major bleeding. Analyses are from the time of random assignment., Results: Between October 2005 and April 2010, 347 patients were enrolled. RVT was detected in 242 patients (69.7%); recurrence occurred in 22 of the 119 patients in group A1compared with 27 of 123 patients in group A2. The adjusted hazard ratio (HR) for group A2 versus A1 was 1.37 (95% CI, 0.7 to 2.5; P = .311). Three of the 105 patients in group B developed recurrent VTE; adjusted HR for group A1 versus B was 6.0 (95% CI, 1.7 to 21.2; P = .005). Three major bleeding events occurred in group A1, and two events each occurred in groups A2 and B. The HR for major bleeding in group A1 versus group A2 was 3.78 (95% CI, 0.77 to 18.58; P = .102). Overall, 42 patients (12.1%) died during follow-up as a result of cancer progression., Conclusion: In patients with cancer with a first DVT, treated for 6 months with LMWH, absence of RVT identifies a population at low risk for recurrent thrombotic events.
