Abstract

REGISTRY Investigators of the European Huntington's Disease Network and COHORT Investigators of the Huntington Study Group., Studying individuals with extreme phenotypes could facilitate the understanding of disease modification by genetic or environmental factors. Our aim was to identify Huntington's disease (HD) patients with extreme symbol digit modality test (SDMT) scores. We first examined in HD the contribution of cognitive measures of the Unified Huntington's Disease Rating Scale (UHDRS) in predicting clinical endpoints. The language-independent SDMT was used to identify patients performing very well or very poorly relative to their CAG and age cohort. We used data from REGISTRY and COHORT observational study participants (5,603 HD participants with CAG repeats above 39 with 13,868 visits) and of 1,006 healthy volunteers (with 2,241 visits), included to identify natural aging and education effects on cognitive measures. Separate Cox proportional hazards models with CAG, age at study entry, education, sex, UHDRS total motor score and cognitive (SDMT, verbal fluency, Stroop tests) scores as covariates were used to predict clinical endpoints. Quantile regression for longitudinal language-independent SDMT data was used for boundary (2.5% and 97.5% quantiles) estimation and extreme score analyses stratified by age, education, and CAG repeat length. Ten percent of HD participants had an extreme SDMT phenotype for at least one visit. In contrast, only about 3% of participants were consistent SDMT extremes at two or more visits. The thresholds for the one-visit and two-visit extremes can be used to classify existing and new individuals. Abstract

Achieving the international goals for climate and resource protection is a more and more important task in the 21st century. To assess GHG reduction potential of bio-based products and bioenergy specific data and a pro-found knowledge about how to calculate are necessary. The evaluation of rapeseed production in Bavaria pointed out that GHG emissions and nitrogen balances show significant farm-specific and site-specific differences. For example, farm-specific GHG emissions range from 0.541 to 1.039 kg kg-1 rapeseed. Influencing key factors are the site-specific biomass yield and the nutrient management. In this context a needs-based using of organic fertilizers is one important factor to reduce GHG emissions in cultivation rapeseed. Default values e.g. specified by RED are not practicable for deriving recommendations for action. Abstract

Modern plant oil compatible tractors with emission control systems show high technical maturity. Limited exhaust gas components, as there are nitrogen oxides, carbon monoxide, hydrocarbons and particle matter, determined at a tractor test stand and under real driving conditions are at a very low level and similar to diesel operation. However, with rapeseed oil fuel from decentral production a greenhouse gas (GHG) reduction of up to nearly 90 % compared to diesel can be achieved. Additionally, a bunch of ecological benefits are associated. 33. Geschlechtsspezifische Therapieerfolg – Einschätzung nach Lid-Frontalis-Suspension

Abstract

Einleitung: Beim Blepharospasmus mit Lidapraxie stellt die Lid-Frontalis-Suspension (LFS) einen minimal invasiven Eingriff dar, um eine gute Lidhebung zu erreichen. Dies gilt, wenn die symptomatische Behandlung mit Botulinumtoxin eine unzureichende Wirkung erbracht hat. Diese retrospektive Analyse hat sich mit der subjektiv durch die Patienten eingeschätzten Erfolgsrate beschäftigt. Methode: Der Therapieerfolg wurde anhand eines Behandlungskalenders für jedes operierte Auge separat mittels einer Skala (0%=kein OP-Effekt, 100%=keine Beschwerden mehr) erhoben. In diesem Bogen wurde durch die Patienten der Schweregrad der Beschwerden ab dem Tag der Operation und für fünf zeitliche Abstände angegeben und statistisch ausgewertet. Ergebnisse: Bezogen auf alle Patienten (n=15 ; 9 Frauen, 6 Männer) zeigte sich ein gut eingeschätzter Therapieerfolg mit einem mittleren initialen Wert (Tag 0–10) von 74.6% und nach über einem Jahr von 68.1% (>365 Tage). Der Mittelwert der subjektiven Therapie-Erfolgeinschätzung lag bei den Männern initial bei 65,0% und stieg im Verlauf auf 78,6%. Bei den Frauen ergab sich im zeitlichen Verlauf eine Abnahme von initial 81,1% auf einen Wert von 62,1% nach dem ersten postoperativen Jahr. Schlussfolgerung: Die LFS ist eine effektive Maßnahme zur Verbesserung der Lidöffnung bei Patienten mit Blepharospasmus und Lidapraxie. Interessanterweise scheint es geschlechtsbezogene Unterschiede in der Therapieerfolgseinschätzung zu geben. Abstract

Cervical dystonia is a neurological movement disorder causing abnormal posture of the head. It may be accompanied by involuntary movements which are sometimes tremulous. The condition has marked effects on patients' self-image, and adversely affects quality of life, social relationships and employment. Botulinum neurotoxin (BoNT) is the treatment of choice for CD and its efficacy and safety have been extensively studied in clinical trials. However, current guidelines do not provide enough practical information for physicians who wish to use this valuable treatment in a real-life setting. In addition, patients and physicians may have different perceptions of what successful treatment outcomes should be. Consequently, an international group of expert neurologists, experienced in BoNT treatment, met to review the literature and pool their extensive clinical experience to give practical guidance about treatment of CD with BoNT. Eight topic headings were considered: the place of BoNT within CD treatment options ; patient perspectives and desires for treatment ; assessment and goal setting ; starting treatment with BoNT-A ; follow- up sessions ; management of side effects ; management of non- response ; switching between different BoNT products. One rapporteur took responsibility for summarising the current literature for each topic, while the consensus statements were developed by the entire expert group. These statements are presented here along with a discussion of the background information.

Botulinum neurotoxin (BoNT) inhibits the release of acetylcholine from cholinergic nerve terminals in muscles or salivary glands. With reduced activation, the muscle activity or secretion decreases. Indications for medical, non‐cosmetic use of BoNT in the orofacial area include among others oromandibular dystonia, painful masseter hypertrophy, Frey's syndrome, and severe drooling. The chapter covers anamnestic characteristics of these conditions as well as clinical, electromyographic (EMG) and laboratory findings, treatment methods with guided injections, and outcome from systematic treatment controls and follow‐up examinations.

Abstract

A recent consensus update classifies dystonia according to two main axes describing the motor features and the different etiologies. Dystonia with no other neurological signs is called õisolated,ö whereas dystonia associated with other neurological signs is called õcombined.ö Since its first description by Oppenheim, dystonia has been considered a pure motor disorder, but recently non–motor features have been recognized as part of clinical phenotype. In isolated dystonias psychiatric disturbances including depression, anxiety disorders, and obsessive–compulsive disorder have been frequently reported. Also cognitive deficits, mainly of executive and visuospatial domains, have been observed, but no major cognitive impairment or dementia was reported. In combined dystonias, psychiatric and cognitive dysfunctions are described, with differences among specific syndromes. The topic of neuropsychiatric features in dystonia is evolving and further studies are needed to elucidate the observed phenomenological diversities.

Abstract

Cervical dystonia is a neurological movement disorder causing abnormal posture of the head. It may be accompanied by involuntary movements which are sometimes tremulous. The condition has marked effects on patients' self-image, and adversely affects quality of life, social relationships and employment. Botulinum neurotoxin (BoNT) is the treatment of choice for CD and its efficacy and safety have been extensively studied in clinical trials. However, current guidelines do not provide enough practical information for physicians who wish to use this valuable treatment in a real-life setting. In addition, patients and physicians may have different perceptions of what successful treatment outcomes should be. Consequently, an international group of expert neurologists, experienced in BoNT treatment, met to review the literature and pool their extensive clinical experience to give practical guidance about treatment of CD with BoNT. Eight topic headings were considered: the place of BoNT within CD treatment options; patient perspectives and desires for treatment; assessment and goal setting; starting treatment with BoNT-A; follow-up sessions; management of side effects; management of non-response; switching between different BoNT products. One rapporteur took responsibility for summarising the current literature for each topic, while the consensus statements were developed by the entire expert group. These statements are presented here along with a discussion of the background information.

50. Spasmodic dysphonia: the need for a combined neurological and phoniatric approach.

Abstract

Spasmodic dysphonia (SD) is now generally considered to be a task-specific focal dystonia. For the first time, we wanted to explore the relationship between SD and dystonia from a combined neurological and phoniatric perspective. For this, we studied 115 patients with non-psychogenic SD by a combined neurological and phoniatric evaluation. Onset of SD was 49.7 ± 19.0 (6-68) years. The female/male ratio was 2. 63% had additional dystonia manifestations (cervical dystonia 35%, arm dystonia 15%, blepharospasm 11%, oromandibular dystonia 11%, writer's cramp 11%, pharyngeal dystonia 10%, generalised dystonia 4%, axial dystonia 2%, spasmodic dyspnoea 2% and segmental dystonia 1%). 71% occurred before, 25% after and 4% together with SD. 17% had a family history of dystonia and 6% a history of exposure to dopamine receptor blocking agents. 41% had mixed SD (SD-M), 31% abductor SD (SD-AB) and 28% adductor SD (SD-AD). SD-M was significantly correlated with additional dystonia manifestations and tremulous SD. No patient showed essential tremor or Parkinsonian syndromes. Two third of SD patients have additional dystonia manifestations and one fifth have a family history of dystonia, considerably more than previously described. In half of all patients, SD starts with non-SD dystonia. Our combined approach revealed a high prevalence of SD-M associated with frequent additional dystonia manifestations including dystonic tremor and a family history of dystonia. Patients presenting with SD should be evaluated for additional dystonia manifestations and dystonia patients should be evaluated for SD.
