INTRODUÇÃO: A HD é um dos mecanismos responsáveis pela dispneia e redução da tolerância ao exercício em pacientes com condições que levam à limitação ao fluxo expiratório, como a asma e a DPOC. Os estudos realizados em pacientes com diagnóstico de DPOC demonstraram redução progressiva da capacidade inspiratória durante teste de esforço. Pacientes asmáticos, mesmo com espirometria normal, quadro clínico estável e sem asma induzida por exercício, podem apresentar limitação ao fluxo expiratório e HD durante esforço, justificando a presença de dispneia e menor capacidade para a realização de exercícios. A limitação ao exercício é um achado frequente em pacientes com bronquiectasias não fibrose cística e a HD pode ser um dos mecanismos envolvidos. A prevalência de HD em bronquiectasias é desconhecida, assim como fatores associados à sua ocorrência. Os principais objetivos deste estudo foram avaliar a prevalência de HD e a redução de capacidade aeróbica, investigar fatores associados à sua ocorrência em bronquiectasias e analisar mecanismos responsáveis pela redução da tolerância ao exercício nessa população. METODOLOGIA: Cento e quatorze pacientes com diagnóstico de bronquiectasias realizaram TECP, prova de função pulmonar completa, OF e TCAR. Foram avaliados CI durante o exercício e a capacidade aeróbica, além das associações entre redução de capacidade aeróbica e variação da CI durante o exercício com dados clínicos, funcionais e tomográficos, considerando a gravidade medida por diferentes escores prognósticos (FACED, E-FACED e BSI). RESULTADOS: Em uma população com idade média de 43±15 anos, VEF1 médio 48.7±19.8 (% pred), foi encontrada uma prevalência de HD de 68,4%. Avaliado pela OF, o grupo de pacientes com HD tem pior função pulmonar, maior grau de AE e acometimento de pequenas vias aéreas. Há redução da capacidade aeróbica em parcela significativa dessa população, 78% com V\'O2 < 84% pred, sendo que esses são mais jovens e apresentam pior função pulmonar, e a DLCO é um marcador independente de redução do V\'O2 pico. Pacientes com redução do V\'O2 pico apresentam redução de VE e VT, menores valores de CI no repouso e no esforço e pior pontuação em todos os escores de gravidade. Não houve associação entre presença de HD e redução da capacidade aeróbica e a extensão do acometimento radiológico não se associou com presença de HD ou pior capacidade aeróbica. CONCLUSÕES: A prevalência de HD durante o TECP, em pacientes com bronquiectasias, é elevada e se associa com pior função pulmonar e maior gravidade, indicando VEF1 como principal fator associado a essa condição. Por ser uma condição complexa, a combinação de testes de função pulmonar, imagem e teste de exercício deve ser utilizada. A redução da tolerância ao exercício também é frequente nessa população, sendo importante testar intervenções terapêuticas voltadas para pacientes com HD e redução da capacidade aeróbica BACKGROUND: Dynamic hyperinflation (DH) is one of the mechanisms responsible for dyspnea and reduction of exercise tolerance in patients with conditions that limit expiratory flow, such as asthma and COPD. The studies performed in patients diagnosed with COPD demonstrated progressive reduction of inspiratory capacity (IC) during exercise test. Asthmatic patients, even with normal spirometry, stable clinical status and without exercise-induced asthma, may present limitation of expiratory flow and DH during exercise, justifying the presence of dyspnea and less ability to perform exercises. Exercise limitation is a frequent finding in patients with non-Cystic Fibrosis Bronchiectasis and DH may be one of the mechanisms involved. The prevalence of DH in bronchiectasis is unknown, as well as factors associated with its occurrence. The main objectives of this study were to evaluate the prevalence of DH and aerobic capacity reduction, to investigate factors associated to its occurrence in bronchiectasis and to analyze mechanisms responsible for the exercise tolerance reduction in this population. METHODS: One hundred and fourteen patients with a diagnosis of Bronchiectasis performed cardiopulmonary exercise test (CPET), lung function tests including spirometry, pletismography and impulse oscillometry. High resolution chest computed tomography (HRCT), IC measurements during exercise and aerobic capacity was also performed. Associations between aerobic capacity reduction and IC variation during exercise with clinical, functional and tomographic data, considering the severity measured by different prognostic scores (FACED, E-FACED and BSI) were analyzed. RESULTS: A prevalence of DH of 68.4% was found in a population with a mean age of 43 ± 15 years, mean FEV1 48.7 ± 19.8 (predicted%). The group of patients who presented DH had worse pulmonary function, a greater degree of air trapping and small airway involvement evaluated by impulse oscillometry. There is a reduction in aerobic capacity in a significant portion of this population, 78% with V\'O2 < 84% pred. These patients are younger and have a worse lung function, with DLCO being an independent marker of V\'O2 peak reduction. Patients with V\'O2 peak reduction present reduced VE and VT, lower IC values at rest and effort, and worse scores on all severity scores. There was no association between DH and reduction of aerobic capacity, and the extent of radiological involvement was not associated with DH or worse aerobic capacity. CONCLUSIONS: The prevalence of DH during CPET in patients with bronchiectasis is high and is associated with worse lung function and greater severity. FEV1 is the main factor associated with this condition. Because it is a complex condition, the combination of lung function tests, imaging, and exercise test should be used in the management of these patients. The reduction of exercise tolerance is also frequent in this population, and it is important to test therapeutic interventions in patients with DH and aerobic capacity reduction
