Abstract

Background: The most appropriate endo-therapeutic approach to biliary anastomotic strictures is yet to be defined. Aim: To retrospectively report on the endo-therapy of duct-to-duct anastomotic strictures during 2013 in Italy. Methods: Data were collected from 16 Endoscopy Units at the Italian Liver Transplantation Centers (BASALT study group). Results: Complete endo-therapy and follow-up data are available for 181 patients: 101 treated with plastic multistenting, 26 with fully covered self-expandable metal stenting and 54 with single stenting. Radiological success was achieved for 145 patients (80%), that is, 88% of plastic multistenting, 88% of self-expandable metal stenting and 61% of single stenting (P < 0.001 vs plastic multistenting; P < 0.05 vs self-expandable metal stenting). After first-line endo-therapy failure, the patients underwent a second-line endo-therapy with plastic multistenting for 25%, fully covered self-expandable metal stenting for 53% and single stenting for 22% of cases, and radiological success was achieved for 84%, that is, 100%, 85% and 63% with plastic multistenting, self-expandable metal stenting and single stenting (P < 0.05 vs plastic multistenting or self-expandable metal stenting) respectively. Procedure-related complications occurred in 7.8% of endoscopic retrograde cholangiopancreatographies. Overall, clinical success was achieved in 87% of patients after a median follow-up of 25 months. Conclusion: Plastic multistenting is confirmed as the preferred first-line treatment, while fully covered self-expandable metal stenting as rescue option for biliary anastomotic strictures. Single stenting has sub-optimal results and should be abandoned.

Abstract

Background. Liver transplantation is the most extreme form of surgical management of patients with hepatic trauma, with very limited literature data supporting its use. The aim of this study was to assess the results of liver transplantation for hepatic trauma. Methods. This retrospective analysis based on European Liver Transplant Registry comprised data of 73 recipients of liver transplantation for hepatic trauma performed in 37 centers in the period between 1987 and 2013. Mortality and graft loss rates at 90 days were set as primary and secondary outcome measures, respectively. Results. Mortality and graft loss rates at 90 days were 42.5% and 46.6%, respectively. Regarding general variables, cross-clamping without extracorporeal veno-venous bypass was the only independent risk factor for both mortality (P = 0.031) and graft loss (P = 0.034). Regarding more detailed factors, grade of liver trauma exceeding IV increased the risk of mortality (P = 0.005) and graft loss (P = 0.018). Moreover, a tendency above the level of significance was observed for the negative impact of injury severity score (ISS) on mortality (P = 0.071). The optimal cutoff for ISS was 33, with sensitivity of 60.0%, specificity of 80.0%, positive predictive value of 75.0%, and negative predictive value of 66.7%. Conclusions. Liver transplantation seems to be justified in selected patients with otherwise fatal severe liver injuries, particularly in whom cross-clamping without extracorporeal bypass can be omitted. The ISS cutoff less than 33 may be useful in the selection process.

Abstract

Ecosystem services indicators are important to measure the trends and state of ecosystem service delivery; with recent years seeing a wide indicator base being developed. Indicators are powerful tools to inform and improve the knowledge base on ecosystems and ecosystem services, which are useful for decision makers at any spatial scale for basing their decisions on evidence and also at a higher levels. A selection of Protected Areas (PAs) from mountain, coast and marine, and arid ecosystems were used as case study areas to investigate a selection of ecosystem indicators and how they can be used to monitor ecosystem services in PAs. In total twelve PAs participated in this report, five from mountain areas, two from arid/semi-arid areas and five from coastal/marine areas. The types of ecosystem service are concurrent with the CICES classification, but the selection of most relevant ecosystem service varies between different PAs. Where possibly, supply, demand and beneficiaries were defined for each of the ecosystem service.

© 2017, The Author(s). Aims/hypothesis: Pancreatic islet beta cell failure causes type 2 diabetes in humans. To identify transcriptomic changes in type 2 diabetic islets, the Innovative Medicines Initiative for Diabetes: Improving beta-cell function and identification of diagnostic biomarkers for treatment monitoring in Diabetes (IMIDIA) consortium (www.imidia.org) established a comprehensive, unique multicentre biobank of human islets and pancreas tissues from organ donors and metabolically phenotyped pancreatectomised patients (PPP). Methods: Affymetrix microarrays were used to assess the islet transcriptome of islets isolated either by enzymatic digestion from 103 organ donors (OD), including 84 non-diabetic and 19 type 2 diabetic individuals, or by laser capture microdissection (LCM) from surgical specimens of 103 PPP, including 32 non-diabetic, 36 with type 2 diabetes, 15 with impaired glucose tolerance (IGT) and 20 with recent-onset diabetes (<1 year), conceivably secondary to the pancreatic disorder leading to surgery (type 3c diabetes). Bioinformatics tools were used to (1) compare the islet transcriptome of type 2 diabetic vs non-diabetic OD and PPP as well as vs IGT and type 3c diabetes within the PPP group; and (2) identify transcription factors driving gene co-expression modules correlated with insulin secretion ex vivo and glucose tolerance in vivo. Selected genes of interest were validated for their expression and function in beta cells. Results: Comparative transcriptomic analysis identified 19 genes differentially expressed (false discovery rate =0.05, fold change =1.5) in type 2 diabetic vs non-diabetic islets from OD and PPP. Nine out of these 19 dysregulated genes were not previously reported to be dysregulated in type 2 diabetic islets. Abstract

Patterns and trends of declining reed-beds in four freshwater ecosystems in central Italy are investigated through an aerial orthoimagery-based diachronic analysis over a period of 24 years. Extent variations and retreat from the waterfront are detected in all sites, compensated only in few cases by backwards enlargements. These shape and size modifications suggest that reed die-back is associated with retreat and with a notable fragmentation process. Long-term analysis performed in one of the sites, compared to water levels, shows a drastic extent decrease co-occurring with an artificial rise of the water level, confirming the regulation of freshwater bodies as a driver of biodiversity loss. Results also suggest that reed-dominated ecosystems may reach a threshold of tolerance towards stressing conditions beyond which they cannot regenerate. The carried out study testifies for a valuable role of landscape metrics in analysing the spatial processes related to declining reed-beds. Spatial metrics prove to be a suitable tool in monitoring processes, working as early warning signals of ongoing decline and helping to model future changes. [ABSTRACT FROM AUTHOR]

Abstract

We report the spatio-temporal dynamics of land cover changes from 1954 to 2008 in a hilly mountainous area in the Italian northern Apennine, and ecological factors and processes associated with these dynamics. Land cover patches were mapped and analysed at three time periods (1954, 1980 and 2008). Changes over time were detected by analysing a combination of multitemporal source data from airborne images and from field surveys. Relationships between cover classes and environmental variables were analysed for the year 2008. Grasslands and scrublands (corresponding to the Natura 2000 Habitats 6210 and 5130, respectively) were found to be the most threatened communities; they occupy small portions of the studied area and are subject to a marked dynamic towards the higher stages of the vegetation successions including a mesophilous and a thermophilous succession. The ability to estimate persistence times and evolution trends of vegetation types following land abandonment is an important tool in land management. Our combined approach yielded a good picture of the vegetation involved in biodiversity loss consequent to land abandonment, as well as estimation of the expected time for this loss to occur. Abstract

During the past twenty years, the number of research projects involving people not trained as scientists, the so-called citizen science, has increased consistently, including mosquito monitoring and control projects. The involvement of citizens in mosquito monitoring programmes not only helps scientists during the data collection phase, but also raises public awareness on mosquito-transmitted diseases and educates citizens about virtuous behaviours that can help in reducing mosquito populations and their spread. The Asian tiger mosquito Aedes albopictus (Skuse) is an invasive species that became established in Europe starting in 1979, with Italy representing currently one of the most infested countries. Procida, a small Mediterranean island in the Naples gulf (Campania region, southern Italy) has unique and very interesting features facilitating the field testing of mosquito integrated vector management (IVM) approaches and control methods, including the Sterile Insect Technique (SIT). With the help of the local municipal administration, the Procida citizens are actively involved as volunteers in monitoring the seasonal and spatial distribution of the Asian tiger mosquito. The collected baseline data will be useful to implement a future island-wide integrated suppression trial of Ae. albopictus, including the release of sterile males, to be carried out in collaboration with the local municipal administration and with the technical support of the Joint FAO/IAEA Division in Vienna.

Abstract

In the last decades, the colonization of Mediterranean Europe and of other temperate regions by Aedes albopictus created an unprecedented nuisance problem in highly infested areas and new public health threats due to the vector competence of the species. The Sterile Insect Technique (SIT) and the Incompatible Insect Technique (IIT) are insecticide-free mosquito-control methods, relying on mass release of irradiated/manipulated males, able to complement existing and only partially effective control tools. The validation of these approaches in the field requires appropriate experimental settings, possibly isolated to avoid mosquito immigration from other infested areas, and preliminary ecological and entomological data. We carried out a 4-year study in the island of Procida (Gulf of Naples, Italy) in strict collaboration with local administrators and citizens to estimate the temporal dynamics, spatial distribution, and population size of Ae. albopictus and the dispersal and survival of irradiated males. We applied ovitrap monitoring, geo-spatial analyses, mark-release-recapture technique, and a citizen-science approach. Results allow to predict the seasonal (from April to October, with peaks of 928–9,757 males/ha) and spatial distribution of the species, highlighting the capacity of Ae. albopictus population of Procida to colonize and maintain high frequencies in urban as well as in sylvatic inhabited environments. Irradiated males shown limited ability to disperse (mean daily distance travelled, Author summary Mosquitoes represent a nuisance as well as public health risk due to their ability to transmit pathogens to humans. The Asian tiger mosquito Aedes albopictus is an invasive species nowadays established in all Mediterranean countries, reaching highest abundance in Italy. Innovative control approaches have been proposed to complement conventional ones and to increase the success of the fight against this aggressive day-biting species, but still need to be validated in the field. Small islands are ideal places for these validation experiments as they are naturally protected by the spill over of mosquitoes from neighbouring untreated areas. The island of Procida, in the gulf of Naples (Italy), has the right ecological characteristics (e.g., homogeneous landscape and high densities of human and Ae. albopictus populations) to represent an ideal experimental site. In collaboration with the Municipality and the residents we obtained relevant data on the mosquito distribution and seasonality on the island and performed preliminary experiments, creating an instrumental baseline information which will facilitate the effective testing of novel control schemes.

Abstract

Mosquitoes represent a considerable nuisance and are actual/potential vectors of human diseases in Europe. Costly and labour-intensive entomological monitoring is needed to correct planning of interventions aimed at reducing nuisance and the risk of pathogen transmission. The widespread availability of mobile phones and of massive Internet connections opens the way to the contribution of citizen in complementing entomological monitoring. ZanzaMapp is the first mobile &ldquo, mosquito&rdquo, application for smartphones specifically designed to assess citizens&rsquo, perception of mosquito abundance and nuisance in Italy. Differently from other applications targeting mosquitoes, ZanzaMapp prioritizes the number of records over their scientific authentication by requesting users to answer four simple questions on perceived mosquito presence/abundance/nuisance and geo-localizing the records. The paper analyses 36,867 ZanzaMapp records sent by 13,669 devices from 2016 to 2018 and discusses the results with reference to either citizens&rsquo, exploitation and appreciation of the app and to the consistency of the results obtained with the known biology of main mosquito species in Italy. In addition, we provide a first small-scale validation of ZanzaMapp data as predictors of Aedes albopictus biting females and examples of spatial analyses and maps which could be exploited by public institutions and administrations involved in mosquito and mosquito-borne pathogen monitoring and control.

Abstract

Background: The most appropriate endo-therapeutic approach to biliary anastomotic strictures is yet to be defined. Aim: To retrospectively report on the endo-therapy of duct-to-duct anastomotic strictures during 2013 in Italy. Methods: Data were collected from 16 Endoscopy Units at the Italian Liver Transplantation Centers (BASALT study group). Results: Complete endo-therapy and follow-up data are available for 181 patients: 101 treated with plastic multistenting, 26 with fully covered self-expandable metal stenting and 54 with single stenting.
