Acute myeloid leukemia (AML) patients with minimal residual disease and receiving allogeneic hematopoietic stem cell transplantation (HCT) have poor survival. Adoptive administration of dendritic cells (DCs) presenting the Wilms tumor protein 1 (WT1) leukemia-associated antigen can potentially stimulate de novo T and B cell development to harness the graft-versus-leukemia (GvL) effect after HCT. We established a simple and fast genetic modification of monocytes for simultaneous lentiviral expression of a truncated WT1 antigen (tWT1), granulocyte macrophage-colony-stimulating factor (GM-CSF), and interferon (IFN)-α, promoting their self-differentiation into potent “induced DCs” (iDCtWT1). A tricistronic integrase-defective lentiviral vector produced under good manufacturing practice (GMP)-like conditions was validated. Transduction of CD14+ monocytes isolated from peripheral blood, cord blood, and leukapheresis material effectively induced their self-differentiation. CD34+ cell-transplanted Nod.Rag.Gamma (NRG)- and Nod.Scid.Gamma (NSG) mice expressing human leukocyte antigen (HLA)-A∗0201 (NSG-A2)-immunodeficient mice were immunized with autologous iDCtWT1. Both humanized mouse models showed improved development and maturation of human T and B cells in the absence of adverse effects. Toward clinical use, manufacturing of iDCtWT1 was up scaled and streamlined using the automated CliniMACS Prodigy system. Proof-of-concept clinical-scale runs were feasible, and the 38-h process enabled standardized production and high recovery of a cryopreserved cell product with the expected identity characteristics. Einleitung: Das Auftreten von Rezidiven nach einer allogenen h��matopoetischen Stammzelltransplantation (alloSZT) z��hlt zu den h��ufigsten Ursachen f��r ein Therapieversagen und stellt eine enorme Herausforderung f��r die weitere Behandlung dar. Der Einsatz von Donorlymphozyten-Infusion (DLI) er��ffnet die M��glichkeit, ein Rezidiv bei verschiedensten h��matologischen Neoplasien zu therapieren oder dessen Entstehung vorzubeugen. Die vorliegende Arbeit untersucht, inwiefern verschiedene Einflussfaktoren im Rahmen einer DLI-Gabe Auswirkungen auf das ��berleben haben. Methodik: Es wurden 160 Patienten, die vom 30.09.1998 bis zum 31.12.2014 mit DLI nach einer alloSZT aufgrund einer h��matologischen Erkrankung an der Medizinischen Klinik mit Schwerpunkt H��matologie, Onkologie und Tumorimmunologie der Charit�� - Universit��tsmedizin Berlin behandelt wurden, retrospektiv untersucht. Zur Erfassung des Gesamt��berlebens (overall survival, OS) und ereignisfreien ��berlebens (event-free survival, EFS) wurde das Kaplan-Meier-Verfahren angewandt. Die kumulative Inzidenz der Rezidive (KI-R) und der Nicht-Rezidiv-Mortalit��t (KI-NRM) wurde in einem kompetitiven Risikomodel ermittelt. Zur Identifizierung unabh��ngiger Faktoren f��r das ��berleben erfolgten Cox-Regressionsanalysen. Ergebnisse: Das OS der gesamten Kohorte belief sich nach einem, drei und f��nf Jahren auf 81 %, 64 % und 58 %, das EFS auf 68 %, 54 % und 52%. Die KI-R betrug zu denselben Zeitpunkten 14 %, 31 % und 34 %, die KI-NRM 7 %, 12 % und 15 %. Das mediane Follow-up nach DLI belief sich auf 21,2 Monate. Eine DLI-Gabe erfolgte bei 29 % aufgrund eines h��matologischen Rezidivs, bei 54 % pr��emptiv und bei 17 % prophylaktisch. Die DLI-Indikation war ein starker unabh��ngiger Faktor f��r das OS (p = < 0,001), das EFS (p = < 0,001) sowie f��r die KI-R (p = < 0,001). Eine pr��emptive oder prophylaktische DLI-Gabe war mit einem signifikant l��ngeren OS, EFS und niedrigerer KI-R assoziiert. Eine aGvHD nach DLI trat bei 35,6 % der Patienten auf, eine cGvHD bei 32,6 %. Dabei bedingte eine aGvHD eine signifikant h��here KI-NRM (p = < 0,001). Bei Auftreten einer cGvHD zeigten sich ein signifikant l��ngeres OS (p = 0,040) sowie eine geringere KI-R (p = 0,001). Schlussfolgerungen: DLI sind eine wichtige Therapieoption nach alloSZT. Insbesondere der pr��emptive oder prophylaktische Einsatz von DLI kann durch den Graft-versus-Leukemia-Effekt (GvL-Effekt) Langzeitremissionen bei h��matologischen Neoplasien nach alloSZT induzieren. Die mit dieser Therapie assoziierte erh��hte Morbidit��t durch eine cGvHD ist ein positiver Pr��diktor f��r das ��berleben., Introduction: Relapse after allogeneic hematopoietic stem cell transplantation (alloSCT) remains the major cause for treatment failure and is an enormous challenge for further treatment. The use of donor lymphocyte infusion (DLI) gives the opportunity to cure a relapse or even more to prevent it. The present study examines how different factors affect the survival of patients who were treated with DLI after alloSCT. Methods: The data of 160 patients, who were treated with DLI after alloSCT at the Department of Hematology, Oncology and Tumor Immunology at the Charit�� - Universit��tsmedizin Berlin from September 30th, 1998 to December 31st, 2014 were analyzed retrospectively. The Kaplan-Meier method was applied to calculate overall survival (OS) and event free survival (EFS). The cumulative incidence of relapse (CI-R) and non-relapse mortality (CI-NRM) were calculated in a competitive risk setting. Independent factors were identified using COX regression models. Results: The OS of the entire cohort after 1, 3 and 5 years was 81 %, 64 % and 58 %, the EFS was 68 %, 54 % and 52 %. The CI-R at the same time points was 14 %, 31 % and 34 %; the CI-NRM was 7 %, 12 % and 15 %. The median follow-up after DLI of the entire cohort was 21,2 months. DLI were given in 29 % of patients because of a hematologic relapse, in 54 % in a preemptive setting and 17 % were given prophylactic. The indication for DLI was a strong independent predictor for OS (p = < 0,001), EFS (p = < 0,001) and CI-R (p = < 0,001). Preemptive and prophylactic use of DLI was associated with a significant longer OS, EFS and lower CI-R. The incidence of acute GvHD after DLI was 35,6 %, whereas chronic GvHD was noted in 32,6 % of the patients. Acute GvHD was associated a significantly higher CI-NRM (p = < 0,001). The occurrence of chronic GvHD was paralleled to a significantly longer OS (p = 0,040) and a lower CI-R (p = 0,001). Conclusions: DLI constitute an important therapeutic option after alloSCT. Our study shows that especially the preemptive and prophylactic use of DLI is feasible and capable of inducing long-term remission after alloSCT. This further underscores that the associated graft-versus-leukemia (GvL) effect is a potent mechanism to allow for long-term cure of the malignant disease. The increased morbidity of cGvHD associated with this therapy is a positive predictor of the survival.
