Abstract

Zusammenfassung: Grundlegende Erkenntnisse über die immunologischen Mechanismen, besonders über die Bedeutung verschiedener Immunzellen und proinflammatorischer Zytokine, in der Ätiopathogenese von entzündlichen Dermatosen und Autoimmunerkrankungen der Haut führten in den letzten Jahren zur Entwicklung zahlreicher neuer Behandlungsstrategien. B-Lymphozyten besitzen aufgrund ihrer Eigenschaften als Antigen präsentierende Zellen und Vorläufer Autoantikörper sezernierender Plasmazellen eine außerordentliche Bedeutung im Autoimmunprozess. Sie stellen daher einen wichtigen Angriffspunkt in der Therapie dieser Erkrankungen dar. Die durch B-Zell-Depletion mit dem monoklonalen anti-CD20-Antikörper Rituximab beobachtete klinische Besserung bei Patienten mit rheumatoider Arthritis forcierte das Interesse am Einsatz B-Zell-gerichteter Therapien bei verschiedenen Autoimmunerkrankungen. Die Bedeutung einer beeinträchtigten B-Zell-Funktion bei der Entstehung und Progression von dermatologischen Autoimmunerkrankungen ist bisher nicht vollständig geklärt und aktuell Gegenstand zahlreicher klinischer und immunologischer Untersuchungen.

Abstract

This article discusses a rare autoimmune bullous disease called lichen planus pemphigoides (LPP) that developed in a 50-year-old Caucasian male two weeks after a COVID-19 infection. LPP is characterized by lichenoid and/or bullous skin lesions and the presence of IgG autoantibodies against certain proteins in the skin and mucous membranes. The patient in this case study exhibited the clinical, histopathological, and serological criteria for LPP. The article also mentions the overlap between LPP, bullous lichen planus, and bullous pemphigoid, and highlights the importance of detecting autoantibodies to support the diagnosis. This is the first reported case of LPP after a COVID-19 infection. [Extracted from the article]

Abstract

Background: Bullous pemphigoid (BP) is the most common autoimmune subepidermal blistering disease of the skin and mucous membranes. This disease typically affects the elderly and presents with itch and localized or, most frequently, generalized bullous lesions. A subset of patients only develops excoriations, prurigo-like lesions, and eczematous and/or urticarial erythematous lesions. The disease, which is significantly associated with neurological disorders, has high morbidity and severely impacts the quality of life. Objectives and methodology: the Autoimmune blistering diseases Task Force of the European Academy of Dermatology and Venereology sought to update the guidelines for the management of BP based on new clinical information, and new evidence on diagnostic tools and interventions. The recommendations are either evidence-based or rely on expert opinion. The degree of consent among all task force members was included. Results: treatment depends on the severity of BP and patients' comorbidities. High-potency topical corticosteroids are recommended as the mainstay of treatment whenever possible. Oral prednisone at a dose of 0.5 mg/kg/day is a recommended alternative. In case of contraindications or resistance to corticosteroids, immunosuppressive therapies, such as methotrexate, azathioprine, mycophenolate mofetil or mycophenolate acid, may be recommended. The use of doxycycline and dapsone is controversial. They may be recommended, in particular, in patients with contraindications to oral corticosteroids. B-cell-depleting therapy and intravenous immunoglobulins may be considered in treatment-resistant cases. Omalizumab and dupilumab have recently shown promising results. The final version of the guideline was consented to by several patient organizations. Conclusions: the guidelines for the management of BP were updated. They summarize evidence- and expert-based recommendations useful in clinical practice., The guideline update was partly supported by the European Academy of Dermatology and Venereology (EADV) and the European Network for Rare Skin Disorders (ERN).

Abstract

Pemphigus vulgaris (PV) is a severe autoimmune blistering disease affecting both skin and mucous membranes. Its pathogenesis is related to IgG autoantibodies primarily targeting the cellular adhesion protein desmoglein (Dsg) 3, one of the major desmosome components. Impaired redox regulation is considered a major player in the pathogenesis of autoimmune diseases such as pemphigus by enhancing inflammation and breakdown of immunological tolerance by structural protein modifications. Despite many recent advances, local and systemic redox profiles that characterize the immune response in pemphigus are virtually unknown but potentially crucial in further advancing our understanding of redox-dependent modifications that eventually lead to clinical manifestation. Here, we have analyzed the individual expression pattern of four major redox enzymes that are members of the thioredoxin (Trx) fold superfamily (peroxiredoxins (Prxs) 1 and 4, glutaredoxin (Grx) 2, and Trx1) in serum and PBMCs as well as their distribution in the skin of pemphigus patients compared to healthy controls. We show that in groups of five pemphigus patients, Prx1 is upregulated in both serum and PBMCs, while its epithelial distribution remains within the spinous epithelial layer. Expression of Grx2 and Prx4 is both reduced in serum and PBMCs, while their distinct and similar expression in the skin changes from an even distribution throughout the basal layer (healthy) to ubiquitous nuclear localization in pemphigus patients. In PV patients, Trx1 is secreted into serum, and cellular distribution appears membrane-bound and cytosolic compared to healthy controls. We furthermore showed that a 3D ex vivo human skin model can indeed be used to reproduce similar changes in the protein levels and distribution of redox enzymes by application of cold atmospheric plasma. Deciphering the relationship between redox enzyme expression and autoimmunity in the context of pemphigus could be critical in elucidating key pathogenic mechanisms and developing novel interventions for clinical management.

Abstract

The extent of liver resection for tumours is limited by the expected functional reserve of the future liver remnant (FRL), so hypertrophy may be induced by portal vein embolization (PVE), taking 6 weeks or longer for growth. This study assessed the hypothesis that simultaneous embolization of portal and hepatic veins (PVE/HVE) accelerates hypertrophy and improves resectability. All centres of the international DRAGON trials study collaborative were asked to provide data on patients who had PVE/HVE or PVE on 2016-2019 (more than 5 PVE/HVE procedures was a requirement). Liver volumetry was performed using OsiriX MD software. Multivariable analysis was performed for the endpoints of resectability rate, FLR hypertrophy and major complications using receiver operating characteristic (ROC) statistics, regression, and Kaplan-Meier analysis. In total, 39 patients had undergone PVE/HVE and 160 had PVE alone. The PVE/HVE group had better hypertrophy than the PVE group (59 versus 48 per cent respectively; P = 0.020) and resectability (90 versus 68 per cent; P = 0.007). Major complications (26 versus 34 per cent; P = 0.550) and 90-day mortality (3 versus 16 per cent respectively, P = 0.065) were comparable. Multivariable analysis confirmed that these effects were independent of confounders. PVE/HVE achieved better FLR hypertrophy and resectability than PVE in this collaborative experience.
