Bei der Hälfte der Journale handelte es sich um traditionelle HNO-Journale, die eine Printausgabe herausbringen und die sich im Wesentlichen aus Abonnements und Anzeigen finanzieren. Bei der anderen Hälfte handelte es sich um Journale, die ausschließlich nach dem „Open Access“-Modell publizieren. Es wurden verschiedene Parameter der Journale und des Begutachtungsprozesses ausgewertet. Ergebnisse Alle 10 traditionellen HNO-Fachjournale lehnten eine Publikation des Fake-Manuskriptes ab. Unter den Journalen, die ausschließlich nach dem „Open Access“-Modell publizierten, gab es eine Ablehnung des Manuskripts, während 2 Journale eine Revision empfahlen und bei 7 Journalen das Manuskript direkt zur Publikation angenommen wurde. Schlussfolgerung Raubtier-Verlage und -Journale sind auch im HNO-Fachgebiet aktiv. Gerade bei Journalen, die Artikel ausschließlich „Open Access“ gegen eine Publikations- oder Bearbeitungsgebühr publizieren, sollten wissenschaftliche Autoren vorsichtig sein und die Seriosität der Zeitschrift vorab prüfen.

Objective Next to olfactory function, the nose can also perceive chemestetic sensations mediated by the trigeminal nerve. While olfactory dysfunction as a symptom of COVID-19 is well described, there has been little research on the limitation of other nasal sensory inputs due to SARS-CoV-2 infection. The aim of this study was to determine possible limitations of nasal chemesthesis after COVID-19 infection by a psychophysiological diagnostic tool. Methods In 65 patients with a PCR-confirmed, former COVID-19 disease, olfaction was tested by means of a sniffin' sticks test, tasting by taste sprays and chemesthesis with a menthol dilution series. The subjective self-assessment of the patients was recorded via a questionnaire. Results We found a restriction of nasal chemesthesis and the extent correlated with the loss of smell, as well as with the values of the taste score, but not with subjective self-assessment. Conclusion Not only the ability to smell and taste, but also nasal chemesthesis is affected by COVID-19.

Zusammenfassung Hintergrund Die HNO-App, eine Smartphone-App zur Fort- und Weiterbildung für HNO-Ärzte und HNO-Interessierte, ergänzt bestehende Angebote der ärztlichen Aus- und Weiterbildung. Das Prinzip des game-based E-Learning kann in Zeiten von Digitalisierung und Pandemie neue Perspektiven eröffnen. Herzstück der App ist ein großes HNO-Quiz, in welchem App-Nutzer im Duell-Modus gegeneinander antreten. Die vorliegende Arbeit analysiert die Leistung der App-Nutzer im Quizmodul unter Berücksichtigung der Fragenkategorien und dem Ausbildungsstand der Nutzer. Material und Methoden Retrospektiv erfolgte eine Auswertung der Quizfragen für die ersten 24 Monate nach Einführung der App. In 16 verschiedenen Kategorien standen 3593 verschiedene Fragen zur Auswahl. Anhand des Ausbildungsstandes wurde unterschieden zwischen Ärzten in Weiterbildung, Fachärzten, Oberärzten, Chefärzten und Ordinaria. Zudem standen Daten zu Studierenden und Nichtmedizinern zur Verfügung. Ergebnisse Es zeigte sich ein signifikanter Unterschied des Wissensstandes der Nutzer unter Berücksichtigung des Ausbildungsstandes. Die Gruppe der Ärzte in Weiterbildung (n=1013) stellte die größte Gruppe dar, hier wurden durchschnittlich 244 Fragen pro Nutzer gespielt und 65,1% der Fragen richtig beantwortet. Damit erreichten sie eine signifikant bessere Antwortquote als die Gruppe der Fachärzte (n=566), welche 61,0% der Fragen richtig beantworten konnte. Schlussfolgerung Insbesondere für Ärzte in Weiterbildung scheint die spielerische Form der Fortbildung durch das HNO-Quiz in der App attraktiv zu sein. Zudem erzielte diese Nutzergruppe bessere Antwortquoten als Fachärzte.

Pseudo-scientific predatory journals have been making a commercially lucrative business with scientific manuscripts for many years misusing the "open access" model by offering a fast and uncomplicated publication of scientific manuscripts in return for publication fees. The question arises as to whether the specialty field of otorhinolaryngology has already been infiltrated by this bad practice.A pseudo-scientific fake manuscript was sent to 20 journals in which the journal title indicated an otorhinolaryngological content. Half of the journals were traditional otorhinolaryngology journals, which publish a print edition and which are mainly financed by subscriptions and advertisements. The other half were journals that published exclusively according to the "open access" model. Various parameters of the journals and of the review process were evaluated.All 10 traditional ORL journals refused to publish the fake manuscript. Among the journals that published exclusively according to the "open access" model, there was one rejection of the manuscript, while 2 journals recommended a revision and 7 journals accepted the manuscript directly for publication.Predatory publishers and journals are also active in the field of otorhinolaryngology. ZusammenfassungHintergrund Verglichen mit dem Zugewinn an Lebensqualität, den viele Patienten durch ein Cochlea-Implantat (CI) erfahren, bringt das CI nur wenige Einschränkungen im täglichen Leben mit sich. Doch kann man mit einem CI auch Tauchen und Fliegen? Halten das operierte Ohr sowie das Implantat selbst den ausgeprägten Druckveränderungen stand? Bisher gibt es keine klaren Empfehlungen und sehr wenig Literatur über die Risiken des Gerätetauchens sowie des Fliegens mit CI.Material und Methoden In einer hypo-/hyperbaren Druckkammer wurden Menschen mit einem CI einem Druckprofil mit Phasen des Druckanstiegs sowie Phasen des Druckabfalls ausgesetzt, sodass zunächst ein Flug und dann ein Tauchgang simuliert wurden. Sowohl vor als auch nach der Druckkammermessung erfolgten eine Ohrmikroskopie sowie eine Messung der Impedanzen des Cochlea-Implantats. Zusätzlich füllte jeder Patient einen symptombezogenen Fragebogen vor sowie nach der Druckkammermessung aus.Ergebnisse In der Druckkammer wurden insgesamt 17 CI-implantierte Ohren bei 11 Probanden untersucht. Zwei Drittel der Probanden gaben an, während der Untersuchung leichte Beschwerden gehabt zu haben. Bei einem Probanden musste die Messung aufgrund von Schmerzen abgebrochen werden. Sieben Patienten zeigten im Anschluss an die Druckexposition eine Hammergefäßinjektion in der Ohrmikroskopie. Bei keinem der Patienten kam es nach der Untersuchung zu einer subjektiven Hörverschlechterung. Es kam zu keiner relevanten Änderung der Impedanzen des CI-Implantats.Schlussfolgerung Die Exposition mit Unter- und Überdruck bei Patienten mit Cochlea-Implantaten ergab in der objektiven Überprüfung der Impedanzen keine wesentliche Abweichung der Impedanzen, sodass keine erneute Anpassung notwendig war. Allerdings zeigten sich leichtgradige Barotraumata sowie subjektive, leichte Beschwerden. Weitere Studien sind erforderlich, um eine adäquate Risikoeinschätzung zur Druckexposition bei CI-Trägern geben zu können.

INTRODUCTION In round window vibroplasty the most efficient coupling technique for contact of the floating mass transducer (FMT) to the round window membrane (RWM) is yet to be determined. Various materials placed between the FMT and the RWM have been proposed to enable better stimulation of the cochlea. Collagenous scaffolds derived from decellularized extracellular cartilage matrices as a commercially available biomaterial are already used for other applications in ear surgery. We aimed to examine the coupling properties of collagen scaffolds compared with commonly used other materials (round window soft coupler, porcine perichondrium, and cartilage) as interponate between the FMT and the RWM in vibroplasty. METHODS A well-established in vitro temporal bone model was used. Volume velocities were measured with collagen scaffold compared with different interponates (round window soft coupler, porcine perichondrium, and cartilage) at 800, 1000, 1250, 1600, 2000, 2500, 3150, and 4000 Hz levels. RESULTS Statistical analysis revealed no superiority of commonly used materials compared with collagen scaffolds at all tested volume velocities (p > 0.05). DISCUSSION We could demonstrate that collagenous scaffolds of decellularized extracellular cartilage matrices have similar vibrational properties as conventional coupling materials of the FMT to the RWM in vibroplasty. Therefore, as a commercially available new material they display a suitable coupling option for round window vibroplasty.

ZusammenfassungDie Übersichtarbeit fasst die aktuellen Erkenntnisse der Auswirkung der COVID-19-Pandemie für die Arbeit der HNO-Ärztin und des HNO-Arztes zusammen. Die aktuell diskutierte Rolle einer Anosmie oder Hyposmie als COVID-19-assoziiertes Symptom wird dargestellt. Wir diskutieren das klinische Management aller HNO-Fälle, aber insbesondere von COVID-19-erkrankten Patienten aus Sicht der HNO-Heilkunde. Ein besonderes Augenmerk gilt den Auswirkungen auf die HNO-Untersuchung und auf HNO-ärztliche Operationen.

The need-based information transfer in education as well as for the recruitment of patients becomes more and more relevant. Here, the internet has emerged as an increasingly important factor in recent years and therefore information pages on hospital homepages can be very helpful. However, it is known that basic text comprehension skills are lacking among large populations.The aim of the present study was to evaluate the readability of the patient information available on the websites of trauma departments of German university hospitals. For this purpose, a search for information material on 10 different diagnoses was carried out. Out of 360 texts possible, 185 were found and assigned to 2 superordinate thematic areas (emergency vs. elective operations), subjected to a systematic text analysis via software "Text-Lab" and rated using 5 known readability indices (Amstad, G-SMOG, LIX, HIX, WSTF).The indices used for both thematic complexes consistently showed poor readability, so that the texts only seem sufficiently comprehensible to readers with higher education. (Amstad: 21.4 ± 20.8; G-SMOG: 11.6 ± 2.0; WSTF 13.3 ± 2.1; LIX: 60.9 ± 7.6; HIX: 4.1 ± 3.2).For the medical information provided by university hospitals, there is a clear need for improvement in order to make the adequate acquisition of knowledge accessible to a broader spectrum of patients.Sowohl die bedarfsgerechte Informationsübermittlung in der Aufklärung als auch zur Rekrutierung von Patienten wird immer relevanter. Hier hat sich in den letzten Jahren das Internet als immer wichtigerer Faktor herauskristallisiert, und Informationsseiten auf Klinikhomepages sollten hierzu hilfreich sein. Es ist jedoch bekannt, dass bei großen Teilen der Bevölkerung grundlegende Textverständnisfähigkeiten fehlen. Ziel der vorliegenden Untersuchung war es daher, die auf Internetseiten unfallchirurgischer Abteilungen deutscher Universitätskliniken zur Verfügung stehenden Patienteninformationen hinsichtlich ihrer Lesbarkeit zu evaluieren.Die Internetseiten der unfallchirurgischen Abteilungen deutscher Universitätskliniken wurden nach Patienteninformationen zu 10 Krankheitsbildern durchsucht. Die Recherche ergab 185 von maximal 360 möglichen Texten, die 2 übergeordneten Themenkomplexen (Notfall- vs. elektive Operationen) zugeteilt wurden. Texte wurden mit einer systematischen Textanalyse durch die Software „Text-Lab“ unterzogen und mittels 5 bekannter Lesbarkeitsindizes (Amstad-Lesbarkeitsindex, G-SMOG-Index, LIX-Lesbarkeitsindex, Hohenheimer Index, Wiener Sachtextformel) bewertet.Die Lesbarkeitsindizes zeigten, dass die analysierten Texte in beiden Themenkomplexen eine durchweg schlechte Lesbarkeit aufwiesen, sodass die Texte nur für Leser mit Hochschulbildung ausreichend verständlich sind. (Amstad-Lesbarkeitsindex: 21,4 ± 20,8; G-SMOG-Index: 11,6 ± 2,0; Wiener Sachtextformel: 13,3 ± 2,1; LIX-Lesbarkeitsindex: 60,9 ± 7,6; Hohenheimer Index: 4,1 ± 3,2).Für die durch Universitätskliniken bereitgestellten medizinischen Informationen ergibt sich ein deutlicher Verbesserungsbedarf, um den adäquaten Erwerb von Wissen einem breiteren Spektrum an Patienten zugänglich zu machen.

Die optimale Behandlung von Tumoren mit Orbitabeteiligung kann die Kompetenzen einzelner Fachgebiete uberschreiten und eine interdisziplinare Zusammenarbeit erfordern. Ziel dieser Arbeit ist die Darlegung eines interdisziplinaren Behandlungskonzepts anhand der Beispiele eines intraorbitalen Hamangioms und eines in die Orbita einwachsenden Plattenepithelkarzinoms der Nasennebenhohlen. Fur die primare Einordnung einer intraorbitalen pathologischen Veranderung ist neben einer ausfuhrlichen Anamnese und einer kompletten ophthalmologischen Untersuchung auch eine ausfuhrliche bildtechnische Untersuchung mit standardisierter Echographie sowie Schnittbildverfahren wie einer Orbita-Dunnschicht-Computertomographie (CT) und/oder in vielen Fallen auch Magnetresonanztomographie (MRT) unerlasslich. Abhangig von der Art der Pathologie ist ein rein operatives Vorgehen, wobei die verschiedenen Disziplinen wie Ophthalmologie, Hals-Nasen-Ohren-Heilkunde, Mund-Kiefer-Gesichts-Chirurgie, Neurochirurgie und Pathologie beteiligt sind oder ein uberdisziplinares Behandlungsregime mit (neo)adjuvanter Strahlen- oder Chemotherapie angezeigt. Tumoren der Orbita weisen ein weites Spektrum an moglichen Pathologien auf, welche mitunter komplexe, operative Zugangswege und multimodale Therapieansatze erfordern. Insbesondere bei Ausbreitung in die Nasennebenhohlen oder nach intrakraniell ist ein multiprofessionelles Team, bestehend aus Pathologen, Neuroradiologen, Mund-Kiefer-Gesichts-Chirurgen, Hals-Nasen-Ohren-Arzten, Neurochirurgen, Strahlentherapeuten, Ophthalmologen, Onkologen und Psychoonkologen fur eine erfolgreiche Therapie essenziell.

Zusammenfassung Hintergrund Die Sialendoskopie ist ein minimal-invasives Verfahren zur Diagnostik und Therapie von Erkrankungen des Speicheldrüsengangsystems. Sie kann in ITN und in LA durchgeführt werden. Vor diesem Hintergrund ist die postoperative Schmerzsituation interessant, über deren Ausmaß aktuell jedoch wenig bekannt ist. Daher wurden in dieser Studie die postoperativen Schmerzen nach Sialendoskopie in LA und ihr Schmerzmanagement in einer quantitativen Untersuchung erfasst und analysiert. Material und Methoden Es wurden 103 Patienten in die retrospektive Studie eingeschlossen, bei denen im Zeitraum von 2013 bis 2015 eine Sialendoskopie der Glandula parotis oder der Glandula submanibularis in Lokalanästhesie durchgeführt wurde. Analysiert wurden die postoperativen Schmerzen in Ruhe und bei Belastung am Operationstag, sowie am ersten und zweiten postoperativen Tag. Die Schmerzen wurden anhand einer numerischen Rating Skala (NRS 0–10) erfasst, welche im Rahmen des QUIPS-Projektes (Qualitätsverbesserung in der postoperativen Schmerztherapie) verwendet wird. Ergebnisse Am Operationstag betrug der Schmerz in Ruhe 0,9 ± 1,9 und bei Belastung 0,7 ± 1,4. Am ersten postoperativen Tag wurden Schmerzen von 1,2 ± 1,8 in Ruhe und 0,9 ± 1,9 bei Belastung festgestellt. Am zweiten postoperativen Tag zeigte sich eine Reduktion der Schmerzen auf 0,75 ± 1,6 in Ruhe und 0,73 ± 1,5 bei Belastung. Insgesamt wurden Metamizol (Novalgin®) und Ibuprofen als häufigste postoperative Analgetika eingesetzt. Am Operationstag erhielten 37,9 % der Patienten Schmerzmittel, am ersten postoperativen Tag 26,2 % der Patienten und am zweiten postoperativen Tag 12,6 % der Patienten. Schlussfolgerung Sialendoskopien der Glandula submandibularis und parotis sind mit moderaten postoperativen Schmerzen verbunden, welche sich gut mit Nicht-Opioiden behandeln lassen.

Background Otorhinolaryngologists (ORL) are said to be at high risk due to a close professionally contact with the mucosa of the upper airway where SARS-CoV-2 can be detected to a high degree. Anyhow, only few data is available for German ORLs. Methods The German Society of ORL, Head and Neck Surgery and the German ENT Association addressed German ORLs to participate in a web-based survey about infection with SARS-CoV-2. Data of infections and concomitant parameters in German ORLs were compared to the total number of infections in Germany. An initial survey was launched May 2020 and a monthly follow-up survey was active until January 2021. Results 970 out of 6383 German ORLs (15%) participated in the initial survey and the June follow-up. Testing positive for SARS-CoV-2 until June 2020 was reported by 54 ORLs. The relative risk of contracting SARS-CoV-2 for ORLs is calculated as an OR of 3.67 (95% CI 2.82;4.79) compared to the total population of Germany. As treatment, 2 individuals were admitted to hospital without intensive care and domestic quarantine was conducted in 96.3% of cases. No casualties were reported. In 31 cases the source of infection was not identifiable whereas 23 had a clear etiology: infected patients: n = 5, 9.26% ;medical staff: n = 13, 24.1% . 9.26% (n = 5) of the identified cases were related to contact to infected family members (n = 3), closer neighborhood (n = 1) or general public (n = 1). There does not seem to be an increased risk of infection performing surgery. The follow-up data for 12 month of COVID-19 will be implemented in the presentation. Conclusion There is an almost 3.7-fold risk of contracting SARS-CoV-2 for German ORLs compared to the population baseline level. Appropriate protection appears to be necessary for this occupational group.

Objective To report our initial clinical experience with N-hexyl cyanoacrylate (NHCA), a novel liquid agent for neurovascular embolization. Methods Four paragangliomas were treated with percutaneous embolization using NHCA as the sole embolic material. In one dural arteriovenous fistula (dAVF), NHCA was used in combination with other embolic materials. Procedural specifics, complications and angiographic results were retrospectively evaluated. Results Total or subtotal devascularization was obtained in 3 of the 4 paragangliomas. In the largest tumor, only partial devascularization could be achieved. The dAVF was completely occluded. Catheter entrapment did not occur. After dAVF treatment, the patient had an asymptomatic lacunar infarction, while there was no procedural morbidity related to paraganglioma treatment. Conclusions In this series, neurovascular embolization with NHCA was feasible and effective. It may be particularly beneficial for small and tortuous vessels that require low-profile catheterization and a slow and controlled polymerization. Further studies are necessary to prove the benefits of NHCA over established embolic agents.

The novel DERIVO 2heal Embolisation Device (Acandis, Pforzheim, Germany) is a flexible, fully radiopaque flow-diverter with a fibrin-based nano-coating, which is supposed to make the device inert to the coagulation cascade. We report a case of pseudoaneurysm treatment with this device under single anti-platelet therapy (SAPT). A female patient underwent endoscopic surgery for chronic rhinosinusitis. During surgery, the lateral wall of the sphenoid wall and the adjacent internal carotid artery was injured, leading to massive hemorrhage, which was treated with compression and tamponades. Eleven days after the procedure, the patient developed a rapidly growing pseudoaneurysm at the injury site with a relevant risk of upcoming aneurysm rupture. The aneurysm was treated by implantation of a DERIVO 2heal. A SAPT regimen was chosen due to the recent massive hemorrhage and anticipating enhanced progressive aneurysm occlusion. There were no thromboembolic complications and there was no morbidity. After 5 days, the pseudoaneurysm was completely occluded. The observations of the presented case are promising and warrant further investigation of this device.

Data of infections and concomitant parameters in German ORLs were collected and compared to the total number of infections in Germany. Results Out of 6383 German ORLs, 970 (15%) participated. 54 ORLs reported testing positive for SARS-CoV-2. Compared to the total population of Germany, ORLs have a relative risk of 3.67 (95% CI 2.82; 4.79) of contracting SARS-CoV-2. Domestic quarantine was conducted in 96.3% of cases. Two individuals were admitted to hospital without intensive care. No casualties were reported. In 31 cases, the source of infection was not identifiable whereas 23 had a clear medical aetiology: infected patients: n = 5, 9.26%; medical staff: n = 13, 14.1%. 9.26% (n = 5) of the identified cases were related to contact to infected family members (n = 3), closer neighbourhood (n = 1) or general public (n = 1). There was no identified increased risk of infection due to performing surgery. Conclusion German ORLs have an almost 3.7-fold risk of contracting SARS-CoV-2 compared to the population baseline level. Appropriate protection appears to be necessary for this occupational group.

HYPOTHESIS A handheld measuring probe was developed that analyzes the vibration characteristics of the stapes footplate after backward stimulation of the cochlea in round window vibroplasty. In temporal bone experiments, the measuring accuracy of the probe was tested. BACKGROUND In round window vibroplasty, the effectiveness of the transmitted vibrations into the inner ear is provided with limited visual and tactile information. Currently, there is no objective measuring tool available. METHODS In five unfixed temporal bones, a floating mass transducer was coupled to the round window membrane. During the excitation with different voltage levels (0, 5, 25, 100, 300 mV root mean square) corresponding to 0, 80, 94, 106, and 116 dB equivalent ear canal sound pressure respectively, the deflections of the footplate were recorded in parallel by laser Doppler vibrometry and the measuring probe. RESULTS The probe allowed for differentiation of the coupling efficiency. The measured footplate vibrations from the excitation levels of 106 dB (and 116 dB) were statistically significant compared with the testing without excitation. The footplate deflections determined in parallel by laser Doppler vibrometry showed comparable results. CONCLUSION In principal, the newly developed measuring probe allows for measuring the quality of retrograde cochlear excitation in a round window vibroplasty by detecting footplate vibrations. Further developments are directed for its application in clinical, intraoperative procedures.

We discuss the clinical management of all ENT patients, but especially of COVID-19 patients from the ENT perspective. Wir diskutieren das klinische Management aller HNO-Fälle, aber insbesondere von COVID-19-erkrankten Patienten aus Sicht der HNO-Heilkunde. Background The round window is an important portal for the application of active hearing aids and cochlear implants. The anatomical and topographical knowledge about the round window region is a prerequisite for successful insertion for a cochlear implant electrode. Objective of review To sum up current knowledge about the round window anatomy and to give advice to the cochlear implant surgeon for optimal placement of an electrode. Type of review Systematic Medline search. Search strategy Search term "round window[Title]" with no date restriction. Only publications in the English Language were included. All abstracts were screened for relevance, that is a focus on surgical anatomy of the round window. The search results were supplemented with hand searching of selected reviews and reference lists from included studies. Evaluation method Subjective assessment. Results There is substantial variability in size and shape of the round window. The round window is regarded as the most reliable surgical landmark to safely locate the scala tympani. Factors affecting the optimal trajectory line for atraumatic electrode insertion are anatomy of the round window, the anatomy of the intracochlear hook region and the variable orientation and size of the cochlea's basal turn. Conclusions The very close relation to the sensitive inner ear structures necessitates a thorough anatomic knowledge and careful insertion technique, especially when implanting patients with residual hearing. In order to avoid electrode migration between the scalae and to achieve protect the modiolus and the basilar membrane, it is recommended to aim for an electrode insertion vector from postero-superior to antero-inferior.

Hintergrund: Internetseiten von Krankenhausern werden zunehmend zur Informationseinholung und Beantwortung von Gesundheitsfragen herangezogen. Um den Anspruchen einer Leserschaft mit unterschiedlichem Bildungs- und Leseniveau zu genugen, mussen Internetseiten mit medizinischen Fachinformationen eine ausreichende Lesbarkeit aufweisen. Material und Methoden: Ausgewahlte Internetseiten aller Kliniken fur Allgemein- und Viszeralchirurgie an deutschen Universitatskliniken (n = 36) wurden einer systematischen Textanalyse unterzogen. Fur 9 unterschiedliche chirurgische Themengebiete wurden reprasentative Texte im Internetangebot jeder Klinik ausgewahlt. Anhand objektiver Textparameter und 5 bekannter Lesbarkeitsindizes wurden die Texte im Hinblick auf ihre Struktur und Lesbarkeit analysiert. Es wurde zudem untersucht, ob sich die Lesbarkeit der Texte in verschiedenen Regionen voneinander unterscheidet. Ergebnisse: Fur die endgultige Analyse konnten 196 Internetseiten herangezogen werden. Im Durchschnitt bestanden die Texte aus 25 Satzen und 368 Wortern. Ubereinstimmend zeigte sich bei den Lesbarkeitsindizes, dass die untersuchten Texte i. d. R. nur fur Leser mit einem hohen Bildungsniveau bzw. Akademiker verstandlich sind. Schlussfolgerung: Medizinische Experten-Laien-Texte auf Internetseiten deutscher universitarer allgemein- bzw. viszeralchirurgischer Kliniken sind fur den medizinischen Laien uberwiegend sehr schwer lesbar. Um die Allgemeinbevolkerung angemessen uber Krankheiten und Therapieoptionen sowie Schwerpunkte der Kliniken zu informieren, ist eine Uberarbeitung der meisten Texte empfehlenswert.

OBJECTIVE Assessing long-term results of patients treated with total ossicular replacement prosthesis (TORP)-vibroplasty. DESIGN Retrospective analysis. SETTING Tertiary referral center. PATIENTS A total of five patients (two women, three men; mean age, 66 yr) were eligible for evaluation after an average follow-up period of 5.1 years after TORP-vibroplasty. INTERVENTIONS Implantation of an active middle ear device in conjunction with a titanium coupler for oval window placement in patients with chronic middle ear disease with missing stapes suprastructure. MAIN OUTCOME MEASURES Audiometric outcomes and satisfaction of the patients. RESULTS The functional gain was 45.2 and 45.6 dB HL at 6 months and 5.1 years after implantation, respectively. The speech recognition using the Freiburg monosyllabic word test and speech intelligibility showed postoperatively a distinct improvement and revealed no statistically significant change across time for the entire follow-up period. According to the International Outcome Inventory for Hearing Aids questionnaire, the patients stated considerable subjective benefits and satisfaction with the device. CONCLUSION The good outcomes of TORP-vibroplasty in chronic disabled ears are stable. They provide long-term and long-lasting satisfying audiologic results combined with a high satisfaction of the patient. Prerequisite is the stable attachment to the cochlear windows.

In detecting a thin bony coverage of a superior semicircular canal (SSC), digital volume tomography (DVT) scans in Poeschl projection seem to be superior to high-resolution computed tomography (CT) scans. Still, a definite diagnosis of SSC dehiscence (SSCD) is not possible with any radiologic imaging technique.To compare CT and DVT to find out whether DVT is equal, better or worse in showing a thin bony layer on top of an SCC.In 11 human temporal bone specimens, the SSC was microscopically blue-lined leaving a thin bony coverage on top of it. All specimens were assessed with both high-resolution CT and DVT. After reconstructing the images in Stenvers and Poeschl projections, all images were evaluated by five independent examiners experienced in radiologic imaging of the temporal bone using a four-point ordinal scale, from 1 (distinct dehiscence) to 4 (distinct coverage).The mean score for all CT scans was 2.58 compared with 3.22 for DVT scans (p = 0.000). Poeschl projection showed a mean score of 3.25 compared with 2.55 for Stenvers projection (p = 0.000). The best imaging modality was found to be DVT scans in Poeschl projections, with a mean score of 3.60.

Hypothesis: Interposed cartilage and the round window coupler (RWC) increase the efficiency of cochlea stimulation with the floating mass transducer (FMT) of a single active middle ear implant (AMEI) placed against the round window membrane. Background: Treatment of mixed and conductive hearing loss with an AMEI attached to the round window is effective, yet the best placement technique of its EMT for the most efficient stimulation of the cochlea remains to be determined. Methods: Experimental study on human temporal bones with the FMT placed against firstly the unaltered round window niche and then subsequently against the fully exposed round window membrane with and without interposed cartilage and the RWC. Cochlea stimulation is measured by the volume velocities of the stapes footplate using LASER vibrometry. Results: At the undrilled round window niche, placement of the FMT by itself and with the RWC resulted in similar volume velocities. The response was significantly raised by interposing cartilage into the undrilled round window niche. Complete exposure of the round window membrane allowed for significantly increased volume velocities. Among these, coupling of the FMT with interposed cartilage yielded responses of similar magnitude compared with the RWC but significantly higher compared with the FMT by itself. Conclusion: Good contact to the round window membrane is essential for efficient stimulation of the cochlea. Therefore, interposing cartilage into the undrilled round window niche is a viable option. At the drilled round window membrane, the FMT with interposed cartilage and attached to the RWC are similarly effective.

Objectives Pleomorphic adenoma (PA) is reported to be the most common benign parotid tumour followed by Warthin tumour (WT), but the proportion of these two entities might have changed. Design Retrospective file analysis. Setting Tertiary referral head and neck centre. Participants Patients who underwent a parotidectomy within a period of 25 years (1990-2014). Main outcome measures Rate of occurrence of PA and WT as well as the development of the PA/WT ratio over the years. Results Overall, 1818 patients with WT (707, 38.9%) and PA (1111, 61.1%) were identified. There was a dominance of PA over WT in all years. An increase in percentage of WT, from 24% in 1990 to 48% in 2014, in comparison with PA was evident. Conclusions In our single-institution hospital-based material of parotidectomies, the percentage of WT in comparison with PA has significantly increased over the last 25 years.

OBJECTIVE To describe a method of cochlear implantation in which the opening of the cochlea and the electrode array insertion are performed under water (underwater technique). STUDY DESIGN Retrospective patient review. SETTING Academic tertiary referral center. PATIENTS Fifteen implantations in children and adult patients with residual hearing at the frequencies 250, 500, and 1000 Hz on the unaided preoperative pure-tone audiometry were included in this study. INTERVENTION(S) Cochlear implantation with a conventional full-length electrode, in which the opening of the cochlea and the electrode array insertion are performed after the tympanic cavity was filled with body-temperature Ringer solution. MAIN OUTCOME MEASURE(S) Changes on residual hearing 6 to 8 weeks after surgery and at subsequent follow-up appointments were analyzed. Preservation of residual hearing was defined as measurable postoperative threshold at the frequencies 250, 500, and 1000 Hz. RESULTS Overall postimplant hearing preservation 6 to 8 weeks after implantation was achieved in 13 of the patients (87%). Subsequent follow-up was performed on average 15.2 months after surgery (range, 7-32) in 14 out of 15 patients. At this late postoperative evaluation preservation of hearing was recorded in nine patients (64%), whereas in the remaining five patients (36%) no residual hearing was measured. CONCLUSION The underwater technique offers a reliable nontraumatic method for electrode array insertions during cochlear implantation. The method respects the physiology of the cochlea und minimizes the pressure variations during cochlear opening and implantation.

Objective: To evaluate the safety and effectiveness of round window (RW), oval window (OW), CliP and Bell couplers for use with an active middle ear implant. Methods: This is a multicenter, long-term, prospective trial with consecutive enrollment, involving 6 university hospitals in Germany. Bone conduction, air conduction, implant-aided warble-tone thresholds and Freiburger monosyllable word recognition scores were compared with unaided preimplantation results in 28 moderate-to-profound hearing-impaired patients after 12 months of follow-up. All patients had previously undergone failed reconstruction surgeries (up to 5 or more). In a subset of patients, additional speech tests at 12 months postoperatively were used to compare the aided with the unaided condition after implantation with the processor switched off. An established quality-of-life questionnaire for hearing aids was used to determine patient satisfaction. Results: Postoperative bone conduction remained stable. Mean functional gain for all couplers was 37 dB HL (RW = 42 dB, OW = 35 dB, Bell = 38 dB, CliP = 27 dB). The mean postoperative Freiburger monosyllable score was 71% at 65 dB SPL. The postimplantation mean SRT50 (speech reception in quiet for 50% understanding of words in sentences) improved on average by 23 dB over unaided testing and signal-to-noise ratios also improved in all patients. The International Outcome Inventory for Hearing Aids (IOI-HA)quality-of-life questionnaire was scored very positively by all patients. Conclusion: A significant improvement was seen with all couplers, and patients were satisfied with the device at 12 months postoperatively. These results demonstrate that an active implant is an advantage in achieving good hearing benefit in patients with prior failed reconstruction surgery.

INTRODUCTION The use of decongestants is common in otitis media eustachian tube (ET) dysfunction. However, the underlying mechanism and the type of action on the complex middle ear pressure equalization system are poorly understood. Here, by use of the pressure chamber, we investigated the impact of intranasal decongestive therapy (xylomethazoline) on ET function. MATERIALS AND METHODS Thirty healthy participants (60 ears) were exposed to a predetermined profile of phases of compression and decompression in a hypobaric and hyperbaric pressure chamber. ET opening pressure, ET opening duration, ET opening frequency, and ET closing pressure were determined before and after intranasal application of xylomethazoline. RESULTS A significantly higher number of ET openings (ET opening frequency) in passive equalization condition could be measured after application of decongestants than before. No significant difference could be found in the values of ET opening pressure, ET opening duration, and ET closing pressure parameters before in comparison with the values after application of xylomethazoline. CONCLUSION We conclude that xylomethazoline might only have a minor effect during active and passive middle ear pressure equalization. Larger cohorts and targeted application of decongestants should be tested to confirm these preliminary data and to find new evidence on the effects of decongestants.
