Abstract

Abstract Objective The first international consensus criteria for optic neuritis (ICON) were published in 2022. We applied these criteria to a prospective, global observational study of acute optic neuritis (ON). Methods We included 160 patients with a first‐ever acute ON suggestive of a demyelinating CNS disease from the Acute Optic Neuritis Network (ACON). We applied the 2022 ICON to all participants and subsequently adjusted the ICON by replacing a missing relative afferent pupillary defect (RAPD) or dyschromatopsia if magnetic resonance imaging pathology of the optical nerve plus optical coherence tomography abnormalities or certain biomarkers are present. Results According to the 2022 ICON, 80 (50%) patients were classified as definite ON, 12 (7%) patients were classified as possible ON, and 68 (43%) as not ON (NON). The main reasons for classification as NON were absent RAPD (52 patients, 76%) or dyschromatopsia (49 patients, 72%). Distribution of underlying ON etiologies was as follows: 78 (49%) patients had a single isolated ON, 41 (26%) patients were diagnosed with multiple sclerosis, 25 (16%) patients with myelin oligodendrocyte glycoprotein antibody‐associated disease, and 15 (9%) with neuromyelitis optica spectrum disorder. The application of the adjusted ON criteria yielded a higher proportion of patients classified as ON (126 patients, 79%). Interpretation According to the 2022 ICON, almost half of the included patients in ACON did not fulfill the requirements for classification of definite or possible ON, particularly due to missing RAPD and dyschromatopsia. Abstract

Abstract Background Mentoring is important for a successful career in academic medicine. In online matching processes, profile texts are decisive for the mentor-selection. We aimed to qualitatively characterize mentoring-profile-texts, identify differences in form and content and thus elements that promote selection. Methods In a mixed method study first, quality of texts in 150 selected mentoring profiles was evaluated (10-point Likert scale; 1 = insufficient to 10 = very good). Second, based on a thematic and content analysis approach of profile texts, categories and subcategories were defined. We compared the presence of the assigned categories between the 25% highest ranked profiles with the 25% lowest ranked ones. Finally, additional predefined categories (hot topics) were labelled on the selected texts and their impact on student evaluation was statistically examined. Results Students rated the quality of texts with a mean of 5.89 ± 1.45. 5 main thematic categories, 21 categories and a total of 74 subcategories were identified. Ten subcategories were significantly associated with high- and four with low-rated profiles. The presence of three or more hot topics in texts significantly correlated with a positive evaluation. Conclusion The introduced classification system helps to understand how mentoring profile texts are composed and which aspects are important for choosing a suited mentor. more...

3. Peripapillary hyper-reflective ovoid mass-like structures (PHOMS) in AQP4-IgG-positive neuromyelitis optica spectrum disease (NMOSD) and MOG-IgG-associated disease (MOGAD)

Abstract

Background Peripapillary hyperreflective ovoid mass-like structures (PHOMS) have recently been described as new optical coherence tomography (OCT) marker. It is not yet clear whether the occurrence of PHOMS is disease-specific or disease-spanning. PHOMS have been described in 16–18% of patients with multiple sclerosis (MS). Currently, no data on the prevalence of PHOMS in other demyelinating diseases including aquaporine-4-IgG-positive neuromyelitis optica spectrum disease (AQP4 + NMOSD) or myelin oligodendrocyte glycoprotein-IgG-associated disease (MOGAD) are reported. Methods We performed a cross-sectional, retrospective spectral domain OCT study evaluating the frequency of PHOMS in AQP4 + NMOSD (n = 47) and MOGAD (n = 44) patients. To test the association with retinal neuroaxonal damage, we compared demographic and clinical data as well as retinal layer thicknesses between eyes with vs. eyes without PHOMS. Results PHOMS were detected in 17% of AQP4 + NMOSD and 14% of MOGAD patients. Intra-cohort analysis revealed that AQP4 + NMOSD patients with PHOMS were significantly older [mean (years): 57.5 vs. 50.0; p value = 0.04]. We found no association of PHOMS with retinal neuroaxonal degeneration. In addition, in subjects with only one eye affected by PHOMS compared with the unaffected fellow eye, no differences in retinal parameters were observed (n = 4). Conclusions In summary, we found PHOMS in 17% of AQP4 + NMOSD and 14% of MOGAD patients. This is comparable to the prevalence of published MS PHOMS data. Therefore, a disease-specific occurrence of PHOMS is unlikely. Interestingly, PHOMS do not seem to depend on retinal neuroaxonal degeneration. more...

Abstract

Infections caused by pathogens of the Mycobacterium tuberculosis complex, i. e., tuberculosis (TB), and the non-infectious, autoimmune disease sarcoidosis are among the most common granulomatous diseases worldwide. Typically, the lung is the primary site of infection and manifestation, respectively which makes the two diseases important differential diagnoses. Both diseases can affect virtually all organ systems, albeit with significantly lower incidence. CASE PRESENTATION: We report the case of a 50-year-old Indian man presenting with a tuberculous perihepatic abscess and a systemic inflammatory response after being diagnosed with neurosarcoidosis presenting as a single granuloma in the frontal lobe with lymphadenopathy in 2014. On day of admission the patient presented with right upper abdominal pain and fever for two weeks. With increased inflammatory parameters in serum and after finding of external CT images, a perihepatic abscess was suspected. This encapsulated cave was drained percutaneously under CT control. A high concentration of acid-fast rods was detected using ZN, PCR was positive for M. tuberculosis. Several samples of sputum and urine were microscopically negative but yielded growth of Mycobacteria after four weeks. DISCUSSION: This is a case presenting with two different granulomatous diseases, each of which manifested itself in an atypical form. The tuberculous liver abscess might either be explained as a flare-up of latent tuberculosis under azathioprine therapy or as a reinfection acquired during one of several visits in the high-prevalence country India. In addition, it must be discussed whether the cerebral granuloma in 2014 could have been an early stage of tuberculous granuloma. Sensitivity of ZN staining is significantly reduced in cerebral samples, and negative PCR-results might be due to low germ load or methodical issues, e. g., decreased sensitivity in formalin fixated samples.Infektionen ausgelöst durch pathogene Mykobakterien des Mykobakterium tuberculosis-Komplexes (wie beispielweise die Tuberkulose) oder die Autoimmunerkrankung Sarkoidose, zählen zu den häufigsten granulomatösen Erkrankungen weltweit. Typischerweise kommt es bei beiden Entitäten zunächst zu einem pulmonalen Befallsmuster, was wiederum eine Stellung als gegenseitige Differentialdiagnose mitbedingt. Eine Ausbreitung auf multiple Organsysteme ist möglich (wenn auch mit deutlich geringerer Inzidenz verbunden). FALLVORSTELLUNG: Wir berichten den Fall eines 50-jährigen indischen Mannes, der sich mit einem perihepatischen tuberkulösen Abszess und einer systemischen Entzündungsreaktion vorstellte, nachdem 2014 eine Neurosarkoidose bei singulärem Granulomnachweis im Frontallappen und begleitender Lymph-adenopathie diagnostiziert worden war. Am Aufnahmetag berichtete der Patient über seit zwei Wochen bestehende rechtsseitige abdominelle Beschwerden und intermittierende Fieberschübe. Nach CT-gesteuerter Drainierung des perihepatischen Abszesses erbrachten PCR und Ziehl-Neelsen Färbung den Nachweis von Mykobakterium tuberculosis. Auch die angelegten Sputum- und Urinkulturen wurden nach vier Wochen positiv. DISKUSSION: Bei dem Patienten kam es vermutlich zur sequentiellen Erkrankung mit zwei verschiedenen granulomatösen Entitäten, die sich beide auf eine atypische Art und Weise klinisch manifestierten. Die neu aufgetretene Tuberkulose könnte dabei entweder mitbedingt durch die immunsuppressive Wirkung des Azathioprins, eingesetzt zur Behandlung der 2014 diagnostizierten Neurosarkoidose oder im Rahmen eines Indienaufenthalts neu entstanden sein. Möglicherweise hatte es sich auch bereits 2014 um ein frühes Stadium eines tuberkulösen zerebralen Granuloms gehandelt und eine reduzierte Sensitivität der Ziehl-Neelsen Färbung bzw. eine damals negative PCR-Testung bei reduzierter Sensitivität der in Formalin fixierten zerebralen Biopsate hatte eine Diagnostik konterkariert. more...

Abstract

Objectives: Due to the COVID-19 pandemic, medical curricula face major challenges. This also applies to mentoring programs, where face-to-face meetings are considered essential. Methods: The LMU Munich medical faculty mentoring program (MeCuM-Mentor) adapted to counteract the unforeseen pause of conventional course formats and associated uncertainty of many students. We here present an approach to transform the established large scale or group mentoring events of our program into online formats. Three projects are presented as examples: 1. HowTo Klink (HK), mainly informative in nature and with peer-mentoring character, 2. FacharztDuell (FAD) and 3. “Auf ein Gespräch mit... (AEGM)”, both with a focus on career counseling. Results: Initial evaluations show a similarly high participation rate and a high level of satisfaction among the participating students. Students' evaluation of whether the projects presented should take place in presence or in online format has so far shown no clear trend. Conclusion: Prospective studies are necessary to investigate the effectiveness of these online formats and analyse differences in participant behaviour. The extent to which online mentoring can replace classic mentoring functions has to be discussed anew., Ziele: Aufgrund der COVID-19-Pandemie stehen medizinische Curricula aktuell vor großen Herausforderungen. Dies gilt auch für Mentoring-Programme, bei denen persönlicher Austausch als wesentlich angesehen wird. Methoden: Das Mentoring-Programm der Medizinischen Fakultät der LMU München (MeCuM-Mentor) wurde angepasst, um der unvorhergesehenen Pause der bisherigen Kursformate und der damit verbundenen Unsicherheit vieler Studierender entgegenzuwirken. Wir stellen hier einen Ansatz vor, um die etablierten Groß- oder Gruppen-Mentoring-Veranstaltungen unseres Programms in Online-Formate zu transformieren. Beispielhaft werden drei Projekte vorgestellt: 1. HowTo Klink (HK), wesentlich mit informativem sowie mit Peer-Mentoring-Charakter, 2. FacharztDuell (FAD) und 3. Auf ein Gespräch mit ... (AEGM), beide mit Schwerpunkt Berufsberatung. Ergebnisse: Initiale Bewertungen zeigen eine ähnlich hohe Teilnahmequote und Zufriedenheit der Studierenden. Die Einschätzung der Studierenden, ob die genannten Projekte in Präsenz- oder Online-Format stattfinden sollen, hat bisher keinen klaren Trend gezeigt.
