Abstract

One of the main objectives of the project "Integrating multimodal connections in the Adriatic-Ionian region" (ADRIPASS) is to deal with the commonly recognized lack of efficient maritime -- hinterland connections, which are mainly caused by the existence of various bottlenecks at borders. This is achieved by identifying and analysing these -- physical and non-physical -- bottlenecks along the Trans-European Transport Networks (TEN-T) corridor sections in the Adriatic -- Ionian (ADRION) region, with specific attention paid to those corridors that have been indicatively extended to the Western Balkans (WB) region, where the issue of border crossings is quite relevant. The Covid-19 pandemic outbreak affects the entire globe, including Europe, in many and different ways. In the fight against the spread of Covid-19, national governments of the European Union (EU) introduced temporary restrictions to border traffic of various degrees, ranging from border controls to outright closure. In this framework, ADRIPASS took duly into account the experience gained during the period of Covid-19 in the Transnational Strategy for the improvement of multimodal transport and accessibility in the ADRION region, developed as the final output of the project. The current paper presents in a summarized way the activities, results and outputs, including the defined Strategy and specific proposals related to transport facilitation for the period to follow. Abstract

Background. Sepsis is the major cause of mortality from any infectious disease worldwide. Sepsis may be the result of a healthcare associated infection (HAI): the most frequent adverse events during care delivery especially in Intensive Care Units (ICUs). The main aim of the present study was to describe the epidemiology of ICU-acquired sepsis and related outcomes among patients enrolled in the framework of the Italian Nosocomial Infections Surveillance in ICUs - SPIN-UTI project. Study design. Prospective multicenter study. Methods. The SPIN-UTI network adopted the European protocols for patient-based HAI surveillance. Results. During the five editions of the SPIN-UTI project, from 2008 to 2017, 47.0% of HAIs has led to sepsis in 832 patients. Overall, 57.0% episodes were classified as sepsis, 20.5% as severe sepsis and 22.5% as septic shock. The most common isolated microorganisms from sepsis episodes were Acinetobacter baumannii, Klebsiella pneumoniae and Pseudomonas aeruginosa. The case fatality rate increased with the severity of sepsis and the mean length of ICU-stay was significantly higher in patients with ICU-acquired sepsis than in patients without. Conclusions. Our study provides evidence that ICU-acquired sepsis occurs frequently in Italian ICU patients and is associated with a high case fatality rate and increased length of stay. However, in order to explain these findings further analyses are needed in this population of ICU patients.

Abstract

Il trasporto merci in contesti urbani risulta rilevante per la necessità di mantenere un’alta qualità della vita, correlata alla disponibilità ed all’accesso a beni e servizi (merci o trasformazioni di merci). L’offerta di tali beni e servizi comporta, però, la generazione di esternalità negative, quali inquinamento atmosferico ed acustico, consumo di energia, congestione stradale ed incidentalità. Garantire un’alta qualità della vita urbana non può prescindere dalla contestuale riduzione delle esternalità negative generate dal trasporto merci, necessario per offrire beni e servizi. Recentemente, la crescita e l’incentivazione della mobilità ciclistica in ambienti urbani, visti i comprovati benefici ambientali, economici e sociali, ha stimolato anche la nascita della mobilità merci ciclistica, ovvero della ciclologistica. La ciclologistica è una soluzione di logistica urbana sostenibile, che prevede il trasporto di merci attraverso l’utilizzo di mezzi a trazione umana, la cui spinta propulsiva è ottenuta mediante pedalata, sia essa assistita o meno. La relativa novità di questo modo di trasporto merci rende necessarie ricerche volte alla comprensione e al monitoraggio nella sua evoluzione, per poter informare gli amministratori locali ed i professionisti della mobilità (architetti, economisti, ingegneri, politologi, ecc.) nei loro processi decisionali e giudizi professionali. L’approfondimento di questa ricerca ha come obiettivo, quindi, la comprensione del fenomeno della ciclologistica attraverso una raccolta sistematica delle informazioni reperite nella letteratura scientifica relative ad esperienze internazionali.

25. Laminar Origin of Corticostriatal Projections to the Motor Putamen in the Macaque Brain.

Abstract

In the macaque brain, projections from distant, interconnected cortical areas converge in specific zones of the striatum. For example, specific zones of the motor putamen are targets of projections from frontal motor, inferior parietal, and ventrolateral prefrontal hand-related areas and thus are integral part of the so-called "lateral grasping network." In the present study, we analyzed the laminar distribution of corticostriatal neurons projecting to different parts of the motor putamen. Retrograde neural tracers were injected in different parts of the putamen in 3 Macaca mulatta (one male) and the laminar distribution of the labeled corticostriatal neurons was analyzed quantitatively. In frontal motor areas and frontal operculum, where most labeled cells were located, almost everywhere the proportion of corticostriatal labeled neurons in layers III and/or VI was comparable or even stronger than in layer V. Furthermore, within these regions, the laminar distribution pattern of corticostriatal labeled neurons largely varied independently from their density and from the projecting area/sector, but likely according to the target striatal zone. Accordingly, the present data show that cortical areas may project in different ways to different striatal zones, which can be targets of specific combinations of signals originating from the various cortical layers of the areas of a given network. These observations extend current models of corticostriatal interactions, suggesting more complex modes of information processing in the basal ganglia for different motor and nonmotor functions and opening new questions on the architecture of the corticostriatal circuitry. 26. Projections to the putamen from neurons located in the white matter and the claustrum in the macaque.

Abstract

Continuing investigations of corticostriatal connections in rodents emphasize an intricate architecture where striatal projections originate from different combinations of cortical layers, include an inhibitory component, and form terminal arborizations which are cell‐type dependent, extensive, or compact. Here, we report that in macaque monkeys, deep and superficial cortical white matter neurons (WMNs), peri‐claustral WMNs, and the claustrum proper project to the putamen. WMNs retrogradely labeled by injections in the putamen (four injections in three macaques) were widely distributed, up to 10 mm antero‐posterior from the injection site, mainly dorsal to the putamen in the external capsule, and below the premotor cortex. Striatally projecting labeled WMNs (WMNsST) were heterogeneous in size and shape, including a small GABAergic component. We compared the number of WMNsST with labeled claustral and cortical neurons and also estimated their proportion in relation to total WMNs. Since some WMNsST were located adjoining the claustrum, we wanted to compare results for density and distribution of striatally projecting claustral neurons (ClaST). ClaST neurons were morphologically heterogeneous and mainly located in the dorsal and anterior claustrum, in regions known to project to frontal, motor, and cingulate cortical areas. The ratio of ClaST to WMNsST was about 4:1 averaged across the four injections. These results provide new specifics on the connectional networks of WMNs in nonhuman primates, and delineate additional loops in the corticostriatal architecture, consisting of interconnections across cortex, claustralstriatal and striatally projecting WMNs.
