Abstract

Transcriptomics, regional cerebral blood flow (rCBF), and a spatial motor virtual reality task were integrated using mediation analysis in a novel demonstration of "imaging omics". Data collected in NCAA Division I football athletes cleared for play before in-season training showed significant relationships in a) elevated levels of miR-30d and miR-92a to elevated putamen rCBF, (b) elevated putamen rCBF to compromised balance scores, and (c) compromised balance scores to elevated miRNA levels. rCBF acted as a mediator variable (minimum 70% mediation, significant Sobel's test) between abnormal miRNA levels and compromised balance scores. Given the involvement of these miRNAs in inflammation and immune function, and that vascular perfusion is a component of the inflammatory response, these findings support a chronic inflammatory model of repetitive head acceleration events (HAEs). rCBF, a systems biology measure, was necessary for miRNA to affect behavior. Abstract

Objective: To characterize neurocognitive response to cerebrospinal fluid (CSF) diversion during a multiday external lumbar drainage (ELD) trial in patients with suspected normal pressure hydrocephalus (NPH). Methods: Inpatients (N = 70) undergoing an ELD trial as part of NPH evaluation participated. Cognition and balance were assessed using standardized measures before and after a three-day ELD trial. Cognitive change pre- to post-ELD trial was assessed in relation to change in balance, baseline neuroimaging findings, NPH symptoms, demographics, and other disease-relevant clinical parameters. Results: Multiday ELD resulted in significant cognitive improvement (particularly on measures of memory and language). This improvement was independent of demographics, test-retest interval, number of medical and psychiatric comorbidities, NPH symptom duration, estimated premorbid intelligence, baseline level of cognitive impairment, cerebrovascular disease burden, degree of ventriculomegaly, or other NPH-related morphological brain alterations. Balance scores evidenced a greater magnitude of improvement than cognitive scores and were weakly, but positively correlated with cognitive change scores. Conclusions: Findings suggest that cognitive improvement associated with a multiday ELD trial can be sufficiently captured with bedside neurocognitive testing. These findings support the utility of neuropsychological consultation, along with balance assessment, in informing clinical decision-making regarding responsiveness to temporary CSF diversion for patients undergoing elective NPH evaluation. Abstract

Mental health and traumatic stress symptoms are prevalent in juvenile offenders, yet research on diversion programs rarely examine these important outcomes. Program duration and the role of protective factors are also understudied in this literature. This study examined the impact of program duration and youth protective factors on mental health and traumatic stress symptoms of 713 youth participating in an Illinois diversion program. Overall, the more time youth spent in the diversion program, the more improvement they demonstrated in mental health and trauma symptoms. Both program duration and number of youth strengths at program entry were significantly associated with improvement in mental health and traumatic stress symptoms. Findings highlight the importance of program duration and suggest that youth protective factors should be identified early on and targeted to maximize the impact of juvenile justice diversion programs on mental health and trauma symptoms. Our study aimed to determine: i) the best fitting factor structure of individual treatment components; ii) the association of child characteristics (i.e., demographics, treatment dosage, environmental risk factors) with mental health outcomes (i.e., post-treatment internalizing and externalizing symptoms); and iii) the association of individual treatment factors (i.e., sets of treatment components) with mental health outcomes. Methods: A sample of 459 violence-exposed children aged 1.5–5 years was examined. Principal component analyses were conducted to factor-analyze 22 child treatment components and 18 caregiver treatment components, respectively. Multiple linear regression analyses were conducted to determine the relationship between child and caregiver treatment factors and outcomes. Results: Children who received grief work showed significant improvement in externalizing symptoms. Children of caregivers who received parent training, attachment skills building, psychoeducation about domestic violence, safety planning, and anger management training showed significantly less externalizing symptom improvement. Gender, race/ethnicity, and treatment dosage were also associated with outcomes. Conclusion: This is the first study to examine treatment components in a sample of young children exposed to violence with a standardized quantitative measure. Our findings suggest that acknowledging children’s loss and sorrow expressed through externalizing behavior and helping them process bereavement may help alleviate their symptoms. Clinical recommendations are discussed.

Abstract

This study explored whether youth protective factors become more or less developed overtime for 4811 youth involved in child welfare using the Child and Adolescent Need and Strengths (CANS) Assessment. Using child welfare administrative data, analyses investigated if the improvement of youth protective factors while in the child welfare system was associated with reducing the risk of justice system involvement. Study findings demonstrated that youth with improved community life strengths, talents/interests, educational strengths, and spiritual/religious strengths were at a significantly lower risk of becoming justice involved while in child welfare. This speaks to the protective effect that positive self-concept in the form of individual talents or creativity, community resources, educational supports, and spiritual/religious encouragement have on youth development and functioning while in the child welfare system, in addition to the potential role of the child welfare system in helping to develop these specific protective factors.
