Abstract

The article discusses the feasibility, operative setup, and surgical outcomes of Retzius‐sparing robot‐assisted radical prostatectomy (RS‐RARP) using the Hugo robot‐assisted surgery system. The study evaluated 26 patients who underwent RS‐RARP with the Hugo system, reporting on patient characteristics, operative setup, complications, and outcomes. The findings suggest that RS‐RARP with the Hugo system is feasible and safe, with high rates of urinary continence recovery, but also highlight the need for a learning process due to higher positive surgical margin rates compared to other systems. Further research is needed to assess long-term outcomes and compare the Hugo system with other robotic platforms. [Extracted from the article]

Abstract

Background: In patients with renal cell carcinoma (RCC) the role of the extent of tumor thrombus into the inferior vena cava (IVC) has never been addressed from a surgical and oncologic standpoint. This study aims to evaluate differences between level III–IV versus level I–II patients concerning peri- and postoperative morbidity, additional treatments and long-term oncological outcomes. Patients and Methods: Overall, 40 patients with RCC underwent radical nephrectomy (RN) with IVC thrombectomy at a single European institution between 2010 and 2023. Complications were reported according to the European Union (EAU) guidelines recommendations. Spider chart served as graphical depiction of surgical and oncologic outcomes. Results: Overall, 22 (55%) and 18 (45%) patients harbored level III–IV and I–II IVC thrombus. Level III–IV patients experienced significantly higher rates of intraoperative transfusions (68 vs 39%), but not significantly higher rates of intraoperative complications (32% vs 28%). Level III–IV patients had significantly higher rates of postoperative transfusions (82% vs 33%) and Clavien Dindo ≥3 complications (41% vs 15%). In level III–IV versus level I–II patients, median follow up was 482 and 1070 days, the rate of distant recurrence was 59% and 50%, the rate of systemic progression was 27% and 13%, and the rate of additional treatment/s was 64% and 61%, respectively (all p values > 0.05). Overall survival was 36% in level III–IV patients and 67% in level I–II (p = 0.001). Conclusions: Our findings suggest that patients with level III–IV RCC who are candidates for IVC thrombectomy should be counselled about the higher likelihood of postoperative severe adverse events and worse overall survival relative to level I–II counterparts. 12. A Brewed Awakening: A Visual Analysis of Craft Beer Labels and Their Use of Multimodality in the Transmission of Culture, Identity, and Taste

Abstract

This thesis is a comparative study that utilises theories of visual grammar and multimodality to determine how Norwegian craft beer breweries use their product labels to transmit culture, identity, and taste to their consumers. The study examines how a selection of craft breweries utilise verbal and visual elements on their product labels to communicate a beer’s flavour and aspects of culture to consumers whilst creating an identity and positioning themselves in a rapidly growing market. The main assumption was that craft breweries rely on a combination of modes such as text, image, colour, and layout, as well as using cultural references to convey the distinct characteristics of their beers and the brewery itself. The material for this study consists of 200 beer labels sourced from five Norwegian craft breweries: Amundsen and Cervisiam located in Oslo, Lervig and Salikatt located in Stavanger, and Monkey Brew located in Trondheim. These breweries were selected as they were the five top-rated Norwegian craft breweries on the Untappd beer rating website as of 1st September 2020.

Abstract

Background: Numerous surveys have been performed to determine the competence and the confidence of residents. However, there is no data available on the condition of Italian residents in urology. Objective: To investigate the status of training among Italian residents in urology regarding scientific activity and surgical exposure. Design, setting, and participants: A web-based survey that included 445 residents from all of the 25 Italian Residency Programmes was conducted between September 2015 and November 2015. Outcome measurements and statistical analysis: The main outcomes were represented by scientific activity, involvement in surgical procedures, and overall satisfaction. Results and limitations: In total, 324 out of 445 (72.8%) residents completed the survey. Overall, 104 (32%) residents had not published any scientific manuscripts, 148 (46%) published ≤5, 38 (12%) ≤10, 26 (8%) ≤15, four (1%) ≤20, and four (1%) >20 manuscripts, respectively. We did not observe any differences when residents were stratified by sex (p = 0.5). Stent positioning (45.7%), extracorporeal shock wave lithotripsy (30.9%), transurethral resection of bladder tumor (33.0%), hydrocelectomy (24.7%), varicocelectomy (17%), ureterolithotripsy (14.5%), and orchiectomy (12.3%) were the surgical procedures more frequently performed by residents. Overall, 272 residents (84%) expressed a good satisfaction for urology specialty, while 178 (54.9%) expressed a good satisfaction for their own residency programme. We observed a statistically decreased trend for good satisfaction for urology specialty according to the postgraduate year (p = 0.02). Conclusions: Italian Urology Residency Programmes feature some heavy limitations regarding scientific activity and surgical exposure. Nonetheless, satisfaction rate for urology specialty remains high. Further improvements in Residency Programmes should be made in order to align our schools to others that are actually more challenging. Patient summary: In this web-based survey, Italian residents in urology showed limited scientific productivity and low involvement in surgical procedures. Satisfaction for urology specialty remains high, demonstrating continuous interest in this field of study from residents. Italian Residency Programs in urology shows some limitation regarding scientific productivity and surgical activity. Resident involvement in many urological procedures still remains low and there is a risk in providing the near future urologists with no expertise.
