Abstract

The corrosion resistance of a Magnesium alloy is low and needs to be improved. This research aimed at corrosion-resistance improvement by supatterd deposition aluminium film, which is formed on the surface of AZ91 Magnesium-alloy. Corrosion resistance performed polarization curve measurement, was evaluated in quest of the corrosion rate using the Tafel extrapolation method, and conducted surface observation and EDS analysis by SEM. Although corrosion resistance is not improved only by film forming because of defects in film, corrosion resistance is improved by heat treatment for 3 hours by 553K after sputtering. In the case of heat treated at 623K and 673K for 3 hours, magnesium diffuses through the alminium film and reached the surface of the film. Thus, heat treatment at high temperature degrade the corrosion resistance of the film. The optimization of heat treatment after sputtering is important in this method.

Abstract

FIn this work, AlN/SiCN composite coatings were deposited by r.f.-reactive sputtering method using a facing target-type sputtering (FTS) apparatus with composite targets consisting of Al plate and SiC chips in a gaseous mixture of Ar and N2, and investigated their mechanical properties and microstructure. The indentation hardness (HIT) of AlN/SiCN coatings prepared from composite targets consisting of 8 ~32 chips of SiC and Al plate showed the maximum value of about 29~32 GPa at a proper nitrogen gas flow rate. X-ray diffraction (XRD) patterns for the AlN/SiCN composite coatings indicated the presence of the only peeks of hexagonal (B4) structured AlN phase. AlN coatings clarified the columnar structure of the cross sectional view TEM observation. On the other hand, microstructure of AlN/SiCN composite coatings changed from columnar to equiaxed structure with increasing SiCN content. HR-TEM observation clarified that the composite coatings consisted of very fine equiaxial grains of B4 structured AlN phase and amorphous phase.

Abstract

In sputtering deposition process of TiO2, metal Ti or sintered TiO2 target is used as deposition source. In this study, we have compared the characteristic of target materials. When TiO2 target was used, stoichiometric TiO2 films was deposited under the Ar atmosphere containing 1.0% of oxygen. The highest sputtering rate under this atmosphere was 3.9nm/min at 3.4W/cm2. But, sintered TiO2 target is fragile and cannot endure higher density of input power than 3.4W/cm2. On the other hand, Ti target needs higher oxygen concentration (8%) in sputtering gas atmosphere for obtaining rutile/anatase. Even though Ti target can be input twice power density of 7.9W/cm2, the highest deposition rate for Ti target was 1.4/nm, which was ~35% of the highest rate for TiO2 target. Then we have study out the composite target consisting of Ti plate and TiO2 chips. Using the composite target, stoichiometric TiO2 films were prepared in the rate of 9.6nm/min at 6.8 W/cm2 under the atmosphere of Ar/2.5%O2. Furthermore, we have found that the TiO2 films obtained from the composite target consisted of about 100% anatase, whereas TiO2 films obtained from other target have rutile dominant structure. The optical band gap energy of the film is determined by using the Tauc plot. The calculated band gap energies for the films deposited by Ti target and composite target were 2.95 and 3.24eV, which are equivalent to that of rutile and anatase structure, respectively.

Abstract

We tried to fabricate the Ge/TiO2 composite films with the differential pressure (pumping) co-sputtering (DPCS) apparatus in order to improve the optical properties. In the study, the micro structure of these thin films has been evaluated. TEM image revealed that the thin film was alternately layered with TiO2 and Ge, lattice fringes were observed both of Ge layer and TiO2 layer. There were portions that lattice fringe of Ge was disturbed near the interface of Ge and TiO2. X-ray photoelectron spectroscopy elucidated that there were few germanium oxides and a part with the thin film after annealed.

Abstract

With the advent of the Heliophysics/Geospace System Observatory (H/GSO), a complement of multi-spacecraft missions and ground-based observatories to study the space environment, data retrieval, analysis, and visualization of space physics data can be daunting. The Space Physics Environment Data Analysis System (SPEDAS), a grass-roots software development platform (www.spedas.org), is now officially supported by NASA Heliophysics as part of its data environment infrastructure. It serves more than a dozen space missions and ground observatories and can integrate the full complement of past and upcoming space physics missions with minimal resources, following clear, simple, and well-proven guidelines. Free, modular and configurable to the needs of individual missions, it works in both command-line (ideal for experienced users) and Graphical User Interface (GUI) mode (reducing the learning curve for first-time users). Both options have "crib-sheets," user-command sequences in ASCII format that can facilitate record-and-repeat actions, especially for complex operations and plotting. Crib-sheets enhance scientific interactions, as users can move rapidly and accurately from exchanges of technical information on data processing to efficient discussions regarding data interpretation and science. SPEDAS can readily query and ingest all International Solar Terrestrial Physics (ISTP)-compatible products from the Space Physics Data Facility (SPDF), enabling access to a vast collection of historic and current mission data. The planned incorporation of Heliophysics Application Programmer's Interface (HAPI) standards will facilitate data ingestion from distributed datasets that adhere to these standards. Although SPEDAS is currently Interactive Data Language (IDL)-based (and interfaces to Java-based tools such as Autoplot), efforts are under-way to expand it further to work with python (first as an interface tool and potentially even receiving an under-the-hood replacement). We review the SPEDAS development history, goals, and current implementation. We explain its "modes of use" with examples geared for users and outline its technical implementation and requirements with software developers in mind. Abstract

Recent results by the Van Allen Probes mission showed that the occurrence of energetic ion injections inside geosynchronous orbit could be very frequent throughout the main phase of a geomagnetic storm. Understanding, therefore, the formation and evolution of energetic particle injections is critical in order to quantify their effect in the inner magnetosphere. We present a case study of a substorm event that occurred during a weak storm $\textit{ Dst }$ $\sim$ -40nT on 14 July 2013. Van Allen Probe B, inside geosynchronous orbit, observed two energetic proton injections within 10min, with different dipolarization signatures and duration. The first one is a dispersionless, short-timescale injection pulse accompanied by a sharp dipolarization signature, while the second one is a dispersed, longer-timescale injection pulse accompanied by a gradual dipolarization signature. We combined ground magnetometer data from various stations and in situ particle and magnetic field data from multiple satellites in the inner magnetosphere and near-Earth plasma sheet to determine the spatial extent of these injections, their temporal evolution, and their effects in the inner magnetosphere. Our results indicate that there are different spatial and temporal scales at which injections can occur in the inner magnetosphere and depict the necessity of multipoint observations of both particle and magnetic field data in order to determine these scales.

11. Excitation of drift-compressional mode waves by drift resonance with ring current ions: Arase observation

Abstract

Compressional Pc 5 waves are important for the energy dynamics in the terrestrial inner magnetosphere because they can accelerate the relativistic electrons (Ukhorskiy et al., 2009). One of the origins of compressional Pc 5 waves is local plasma instability, but the generation mechanism of the compressional Pc 5 waves is still controversial. Recently, drift compressional instability has been focused on because it can generate compressional waves even if plasma beta is less than unity (Rubtsov et al., 2018; Mager et al., 2019; Takahashi et al., 2022). However, the wave property and excitation mechanism of the drift compressional mode are not fully understood due to the lack of observation.This study reports the role of drift resonance in the excitation of drift compressional waves observed by the Arase satellite on 19th November 2018. From the theoretical calculations of the wave frequency (Mager et al., 2013) and pressure perturbation (Takahashi et al., 2022), we concluded that the observed wave is the drift compressional mode. We estimated the azimuthal wave number (m number) from the finite Larmor radius effect and obtained m ~ -140. This is consistent with the drift resonance with protons at 20-30 keV, whose fluxes showed the most coherent oscillations. In addition to a positive radial gradient of ion temperature, we found that an inward phase space density gradient and positive df/dW sometimes appeared, which indicates that the drift resonance gives free energy to the wave, where f and W are phase space density and energy, respectively., The 28th IUGG General Assembly (IUGG2023) (Berlin 2023)

Abstract

Background: Recent guidelines suggested a wider use of long-acting injectable antipsychotics (LAI) than previously, but naturalistic data on the consequences of LAI use in terms of discontinuation rates and associated factors are still sparse, making it hard for clinicians to be informed on plausible treatment courses. Objective: Our objective was to assess, under real-world clinical circumstances, LAI discontinuation rates over a period of 12 months after a first prescription, reasons for discontinuation, and associated factors. Methods: The STAR Network ‘Depot Study’ was a naturalistic, multicentre, observational prospective study that enrolled subjects initiating a LAI without restrictions on diagnosis, clinical severity or setting. Participants from 32 Italian centres were assessed at baseline and at 6 and 12 months of follow-up. Psychopathology, drug attitude and treatment adherence were measured using the Brief Psychiatric Rating Scale, the Drug Attitude Inventory and the Kemp scale, respectively. Results: The study followed 394 participants for 12 months. The overall discontinuation rate at 12 months was 39.3% (95% confidence interval [CI] 34.4–44.3), with paliperidone LAI being the least discontinued LAI (33.9%; 95% CI 25.3–43.5) and olanzapine LAI the most discontinued (62.5%; 95% CI 35.4–84.8). The most frequent reason for discontinuation was onset of adverse events (32.9%; 95% CI 25.6–40.9) followed by participant refusal of the medication (20.6%; 95% CI 14.6–27.9). Medication adherence at baseline was negatively associated with discontinuation risk (hazard ratio [HR] 0.853; 95% CI 0.742–0.981; p = 0.026), whereas being prescribed olanzapine LAI was associated with increased discontinuation risk compared with being prescribed paliperidone LAI (HR 2.156; 95% CI 1.003–4.634; p = 0.049). Abstract

Importance: the cost-effectiveness of the Self-Help Plus (SH+) program, a group-based, guided, self-help psychological intervention developed by the World Health Organization for people affected by adversity, is unclear. Objective: to investigate the cost-utility of providing the SH+ intervention combined with enhanced usual care vs enhanced usual care alone for Syrian refugees or asylum seekers hosted in Turkey. Design, setting, and participants: this economic evaluation was performed as a prespecified part of an assessor-blinded randomized clinical trial conducted between October 1, 2018, and November 30, 2019, with 6-month follow-up. A total of 627 adults with psychological distress but no diagnosed psychiatric disorder were randomly assigned to the intervention group or the enhanced usual care group. Interventions: the SH+ program was a 5-session (2 hours each), group-based, stress management course in which participants learned self-help skills for managing stress by listening to audio sessions. The SH+ sessions were facilitated by briefly trained, nonspecialist individuals, and an illustrated book was provided to group members. Th intervention group received the SH+ intervention plus enhanced usual care; the control group received only enhanced usual care from the local health care system. Enhanced usual care included access to free health care services provided by primary and secondary institutions plus details on nongovernmental organizations and freely available mental health services, social services, and community networks for people under temporary protection of Turkey and refugees. Main outcomes and measures: the primary outcome measure was incremental cost per quality-adjusted life-year (QALY) gained from the perspective of the Turkish health care system. An intention-to-treat analysis was used including all participants who were randomized and for whom baseline data on costs and QALYs were available. Abstract

Aims: as refugees and asylum seekers are at high risk of developing mental disorders, we assessed the effectiveness of Self-Help Plus (SH + ), a psychological intervention developed by the World Health Organization, in reducing the risk of developing any mental disorders at 12-month follow-up in refugees and asylum seekers resettled in Western Europe. Methods: refugees and asylum seekers with psychological distress (General Health Questionnaire-12 > 3) but without a mental disorder according to the Mini International Neuropsychiatric Interview (M.I.N.I.) were randomised to either SH + or enhanced treatment as usual (ETAU). The frequency of mental disorders at 12 months was measured with the M.I.N.I., while secondary outcomes included self-identified problems, psychological symptoms and other outcomes. Results: of 459 participants randomly assigned to SH + or ETAU, 246 accepted to be interviewed at 12 months. No difference in the frequency of any mental disorders was found (relative risk [RR] = 0.841; 95% confidence interval [CI] 0.389-1.819; p-value = 0.659). In the per protocol (PP) population, that is in participants attending at least three group-based sessions, SH + almost halved the frequency of mental disorders at 12 months compared to ETAU, however so few participants and events contributed to this analysis that it yielded a non-significant result (RR = 0.528; 95% CI 0.180-1.544; p-value = 0.230). SH + was associated with improvements at 12 months in psychological distress (p-value = 0.004), depressive symptoms (p-value = 0.011) and wellbeing (p-value = 0.001). Conclusions: the present study failed to show any long-term preventative effect of SH + in refugees and asylum seekers resettled in Western European countries. Abstract

Introduction: Depression is a highly prevalent condition in the elderly, with a vast impact on quality of life, life expectancy, and medical outcomes. Selective serotonin reuptake inhibitors (SSRIs) are the most commonly prescribed agents in this condition and, although generally safe, tolerability issues cannot be overlooked. Vortioxetine is an antidepressant with a novel mechanism of action. Based on studies to date, it may have a promising tolerability profile in the elderly, as it does not adversely affect psychomotor or cognitive performance and does not alter cardiovascular and endocrine parameters. The present study aims to assess the tolerability profile of vortioxetine in comparison with the SSRIs considered as a single group in elderly participants with depression. The rate of participants withdrawing from treatment due to adverse events after 6 months of follow up will be the primary outcome. Methods and analysis: This is a pragmatic, multicentre, open-label, parallel-group, superiority, randomized trial funded by the Italian Medicines Agency (AIFA - Agenzia Italiana del Farmaco). Thirteen Italian Community Psychiatric Services will consecutively enrol elderly participants suffering from an episode of major depression over a period of 12 months. Participants will be assessed at baseline and after 1, 3 and 6 months of follow up. At each time point, the following validated rating scales will be administered: Montgomery-Åsberg Depression Rating Scale (MADRS), Antidepressant Side-Effect Checklist (ASEC), EuroQual 5 Dimensions (EQ-5D), Short Blessed Test (SBT), and Charlson Age-Comorbidity Index (CACI). Outcome assessors and the statistician will be masked to treatment allocation. A total of 358 participants (179 in each group) will be enrolled. Abstract

We investigate the magnetic field dipolarization in the inner magnetosphere and its associated ion flux variations, using the magnetic field and energetic ion flux data acquired by the Van Allen Probes. From a study of 74 events that appeared at L= 4.5-6.6 between 1 October 2012 and 31 October 2013, we reveal the following characteristics of the dipolarization in the inner magnetosphere: (1) its time scale is approximately 5 min; (2) it is accompanied by strong magnetic fluctuations that have a dominant frequency close to the O+ gyrofrequency; (3) ion fluxes at 20-50 keV are simultaneously enhanced with larger magnitudes for O+ than for H+; (4) after a few minutes of the dipolarization, the flux enhancement at 0.1-5 keV appears with a clear energy-dispersion signature only for O+; and (5) the energy-dispersed O+ flux enhancement appears in directions parallel or antiparallel to the magnetic field. From these characteristics, we discuss possible mechanisms that can provide selective acceleration to O+ ions at > 20 keV. We conclude that O+ ions at L= 5.4-6.6 undergo nonadiabatic local acceleration caused by oscillating electric field associated with the magnetic fluctuations and/or adiabatic convective transport from the plasma sheet to the inner magnetosphere by the impulsive electric field. At L= 4.5-5.4, however, only the former acceleration is plausible. We also conclude that the field-aligned energy-dispersed O+ ions at 0.1-5 keV originate from the ionosphere and are extracted nearly simultaneously to the onset of the dipolarization.

Abstract

Most chambered sponges (the polyphyletic group of "Sphinctozoa") are hypercalcified types and most of them probably belong to the Demospongia. "Spinctozoa" occur from the Cambrian to the Recent and are the most abundant sponges in Late Palaeozoic and Triassic reefs and shallow water limestones. Among hexactinellid sponges, chambered forms are very rare including taxa only from the Late Jurassic and the Late Triassic of Europe, Russia, Tadjikistan, Iran or China. There are five genera described Casearia Quenstedt, Caucasocoelia Boiko, Dracolychnos Wu & Xiao, Pseudo-verticillites Boiko and Innaecoelia Boiko, the latter of which is synomised with Casearia by most authors.

Abstract

Pc1 geomagnetic pulsations, equivalent to electromagnetic ion cyclotron waves in the magnetosphere, display a specific amplitude modulation, though the region of the modulation remains an open issue. To classify whether the amplitude modulation has a magnetospheric or ionospheric origin, an isolated proton aurora (IPA), which is a proxy of Pc1 wave–particle interactions, is compared with the associated Pc1 waves for a geomagnetic conjugate pair, Halley Research Base in Antarctica and Nain in Canada. The temporal variation of an IPA shows a higher correlation coefficient (0.88) with Pc1 waves in the same hemisphere than that in the opposite hemisphere. This conjugate observation reveals that the classic cyclotron resonance is insufficient to determine the amplitude modulation. We suggest that direct wave radiation from the ionospheric current by IPA should also contribute to the amplitude modulation.

Abstract

We studied O+ drift-bounce resonance using Magnetospheric Multiscale (MMS) data. A case study of an event on 17 February 2016 shows that O+ flux oscillations at similar to 10-30 keV occurred at MLT similar to 5 hr and L similar to 8-9 during a storm recovery phase. These flux oscillations were accompanied by a toroidal Pc5 wave and a high-speed solar wind (similar to 550 km/s). The azimuthal wave number (m-number) of this Pc5 wave was found to be approximately -2. The O+/H+ flux ratio was enhanced at similar to 10-30 keV corresponding to the O+ flux oscillations without any clear variations of H+ fluxes, indicating the selective acceleration of O+ ions by the drift-bounce resonance. A search for similar events in the time period from September 2015 to March 2017 yielded 12 events. These events were mainly observed in the dawn to the afternoon region at L similar to 7-12 when the solar wind speed is high, and all of them were simultaneously identified on the ground, indicating low m-number. Correlation analysis revealed that the O+/H+ energy density ratio has the highest correlation coefficient with peak power of the electric field in the azimuthal component (E-a). Abstract

A new record of halimedacean algae is reported from the Upper Triassic (Carnian) of the Dolomites (Southern Alps) near Cortina d’Ampezzo. Based on the gross morphology (non-segmented non-branched thalli) and the internal skeletal arrangement (e.g., medulla with thicker siphons, sometimes bifurcating at high angles; cortex with thinner siphons with multiple Y-like branching), the material can be assigned to the genus Boueina. It is the first record of halimedacean algae from the Upper Triassic of the Southern Alps (Dolomites) and the highly diverse San Cassiano Formation sensu lato (Heiligkreuz Formation). Abstract

With the advent of the Heliophysics/Geospace System Observatory (H/GSO), a complement of multi-spacecraft missions and ground-based observatories to study the space environment, data retrieval, analysis, and visualization of space physics data can be daunting. The Space Physics Environment Data Analysis System (SPEDAS), a grass-roots software development platform (www.spedas.org), is now officially supported by NASA Heliophysics as part of its data environment infrastructure. It serves more than a dozen space missions and ground observatories and can integrate the full complement of past and upcoming space physics missions with minimal resources, following clear, simple, and well-proven guidelines. Free, modular and configurable to the needs of individual missions, it works in both command-line (ideal for experienced users) and Graphical User Interface (GUI) mode (reducing the learning curve for first-time users). Both options have “crib-sheets, ” user-command sequences in ASCII format that can facilitate record-and-repeat actions, especially for complex operations and plotting. Crib-sheets enhance scientific interactions, as users can move rapidly and accurately from exchanges of technical information on data processing to efficient discussions regarding data interpretation and science. SPEDAS can readily query and ingest all International Solar Terrestrial Physics (ISTP)-compatible products from the Space Physics Data Facility (SPDF), enabling access to a vast collection of historic and current mission data. The planned incorporation of Heliophysics Application Programmer’s Interface (HAPI) standards will facilitate data ingestion from distributed datasets that adhere to these standards. Abstract

Chorus waves, among the most intense electromagnetic emissions in the Earth’s magnetosphere, magnetized planets, and laboratory plasmas, play an important role in the acceleration and loss of energetic electrons in the plasma universe through resonant interactions with electrons. However, the spatial evolution of the electron resonant interactions with electromagnetic waves remains poorly understood owing to imaging difficulties. Here we provide a compelling visualization of chorus element wave–particle interactions in the Earth’s magnetosphere. Through in-situ measurements of chorus waveforms with the Arase satellite and transient auroral flashes from electron precipitation events as detected by 100-Hz video sampling from the ground, Earth’s aurora becomes a display for the resonant interactions. Our observations capture an asymmetric spatial development, correlated strongly with the amplitude variation of discrete chorus elements. This finding is not theoretically predicted but helps in understanding the rapid scattering processes of energetic electrons near the Earth and other magnetized planets.

Abstract

In vivo anti-nuclear antibody positive phenomenon in the renal tissues except for necropsy/autopsy cases or very severe cases of systemic lupus erythematosus is a controversial issue until today. However, we experienced a case of lupus nephritis that showed in vivo anti-nuclear antibody-positive cells in the renal tissues. Four successive renal biopsies over 17 years were studied with immunofluorescence as well as immunohistochemical and immuneelectron microscopic staining. The first, second and third biopsies did not reveal any in vivo anti-nuclear antibodypositive cells. However, the fourth biopsy in relapse showed positive cells, and the transfer of the patient’s serum into the mice provoked in vivo anti-nuclear antibody-positive cells in the mouse intestines. We present this case in detail and discuss the implications of these observations.

Abstract

This paper reports two unique auroral features: postmidnight purple auroral rays and global Pc1 geomagnetic pulsations, observed before the onset of the corotating interaction region (CIR) storm of 21 March 2017, at the beginning of the first campaign of the new Particles and Waves in the Inner magnetosphere using Ground‐based network observation (PWING) longitudinal ground network with the Arase satellite. The purple auroral rays were observed from ~0315 to 0430 UT (~03–04 magnetic local time) in the northeastern sky at Husafell, Iceland (magnetic latitude: 64.9°N). We newly propose that the entry of high‐density CIR plasma into the magnetotail created purple auroral rays in the sunlit ionosphere. Abstract

A Pc5 wave is observed by the Exploration of energization and Radiation in Geospace Arase satellite in the inner magnetosphere (L similar to 5.4-6.1) near postmidnight (L-magnetic local time similar to 1.8-2.5 hr) during the storm recovery phase on 27 March 2017. Its azimuthal wave number (m-number) is estimated using two independent methods with satellites and ground observations to be -8 to -15. The direct measurement of the m-number enables us to calculate the resonance energy. The flux oscillations of H+ and O+ ions at >= 56.3 keV are caused by drift resonance and those of O+ ions at <= 18.6 keV by bounce resonance. Resonances of O+ ions at multiple energies are simultaneously observed for the first time. The enhancement of the O+/H+ flux ratio at <= 18.6 keV indicates selective acceleration of O+ ions through bounce resonance. Plain Language Summary Geomagnetic pulsations are magnetic fluctuations excited by solar wind or plasma instabilities in the magnetosphere. Pc5 waves are continuous geomagnetic pulsations with a period of 150-600 s. A Pc5 wave was observed in the inner magnetosphere during a magnetic storm on 27 March 2017. It propagated westward with a wave number of 8 to 15 and resonated with charged particles, resulting in oscillations of the H+ and O+ ion fluxes at >= 56.3 keV and the O+ ion fluxes at <= 18.6 keV. Abstract

The plasmas (electrons and ions) in the inner magnetosphere have wide energy ranges from electron volts to mega-electron volts (MeV). These plasmas rotate around the Earth longitudinally due to the gradient and curvature of the geomagnetic field and by the co-rotation motion with timescales from several tens of hours to less than 10 min. They interact with plasma waves at frequencies of mHz to kHz mainly in the equatorial plane of the magnetosphere, obtain energies up to MeV, and are lost into the ionosphere. In order to provide the global distribution and quantitative evaluation of the dynamical variation of these plasmas and waves in the inner magnetosphere, the PWING project (study of dynamical variation of particles and waves in the inner magnetosphere using ground-based network observations, http://www.isee.nagoya-u.ac.jp/dimr/PWING/) has been carried out since April 2016. This paper describes the stations and instrumentation of the PWING project. We operate all-sky airglow/aurora imagers, 64-Hz sampling induction magnetometers, 40-kHz sampling loop antennas, and 64-Hz sampling riometers at eight stations at subauroral latitudes (~ 60° geomagnetic latitude) in the northern hemisphere, as well as 100-Hz sampling EMCCD cameras at three stations. These stations are distributed longitudinally in Canada, Iceland, Finland, Russia, and Alaska to obtain the longitudinal distribution of plasmas and waves in the inner magnetosphere. This PWING longitudinal network has been developed as a part of the ERG (Arase)-ground coordinated observation network. The ERG (Arase) satellite was launched on December 20, 2016, and has been in full operation since March 2017. We will combine these ground network observations with the ERG (Arase) satellite and global modeling studies. Abstract

Introduction: Information on the off–label use of Long–Acting Injectable (LAI) antipsychotics in the real world is lacking. In this study, we aimed to identify the sociodemographic and clinical features of patients treated with on– vs off–label LAIs and predictors of off–label First– or Second–Generation Antipsychotic (FGA vs. SGA) LAI choice in everyday clinical practice. Method: In a naturalistic national cohort of 449 patients who initiated LAI treatment in the STAR Network Depot Study, two groups were identified based on off– or on–label prescriptions. A multivariate logistic regression analysis was used to test several clinically relevant variables and identify those associated with the choice of FGA vs SGA prescription in the off–label group. Results: SGA LAIs were more commonly prescribed in everyday practice, without significant differences in their on– and off–label use. Approximately 1 in 4 patients received an off–label prescription. In the off–label group, the most frequent diagnoses were bipolar disorder (67.5%) or any personality disorder (23.7%). FGA vs SGA LAI choice was significantly associated with BPRS thought disorder (OR = 1.22, CI95% 1.04 to 1.43, p= 0.015) and hostility/suspiciousness (OR = 0.83, CI95% 0.71 to 0.97, p= 0.017) dimensions. The likelihood of receiving an SGA LAI grew steadily with the increase of the BPRS thought disturbance score. Conversely, a preference towards prescribing an FGA was observed with higher scores at the BPRS hostility/suspiciousness subscale. Conclusion: Our study is the first to identify predictors of FGA vs SGA choice in patients treated with off–label LAI antipsychotics. Demographic characteristics, i.e. age, sex, and substance/alcohol use co–morbidities did not appear to influence the choice towards FGAs or SGAs. Despite a lack of evidence, clinicians tend to favour FGA over SGA LAIs in bipolar or personality disorder patients with relevant hostility. Further research is needed to evaluate treatment adherence and clinical effectiveness of these prescriptive patterns.

Abstract

Importance: the cost-effectiveness of the Self-Help Plus (SH+) program, a group-based, guided, self-help psychological intervention developed by the World Health Organization for people affected by adversity, is unclear. Objective: to investigate the cost-utility of providing the SH+ intervention combined with enhanced usual care vs enhanced usual care alone for Syrian refugees or asylum seekers hosted in Turkey. Design, setting, and participants: this economic evaluation was performed as a prespecified part of an assessor-blinded randomized clinical trial conducted between October 1, 2018, and November 30, 2019, with 6-month follow-up. A total of 627 adults with psychological distress but no diagnosed psychiatric disorder were randomly assigned to the intervention group or the enhanced usual care group. Interventions: the SH+ program was a 5-session (2 hours each), group-based, stress management course in which participants learned self-help skills for managing stress by listening to audio sessions. The SH+ sessions were facilitated by briefly trained, nonspecialist individuals, and an illustrated book was provided to group members. Th intervention group received the SH+ intervention plus enhanced usual care; the control group received only enhanced usual care from the local health care system. Enhanced usual care included access to free health care services provided by primary and secondary institutions plus details on nongovernmental organizations and freely available mental health services, social services, and community networks for people under temporary protection of Turkey and refugees. Main outcomes and measures: the primary outcome measure was incremental cost per quality-adjusted life-year (QALY) gained from the perspective of the Turkish health care system. An intention-to-treat analysis was used including all participants who were randomized and for whom baseline data on costs and QALYs were available. Data were analyzed September 30, 2020, to July 30, 2021. Results: of 627 participants (mean [SD] age, 31.3 [9.0] years; 393 [62.9%] women), 313 were included in the analysis for the 51-1+ group and 314 in the analysis for the enhanced usual care group. An incremental cost-utility ratio estimate of T 6068 pound ($1147) per QALY gained was found when the SH+ intervention was provided to groups of 10 Syrian refugees. At a willingness to pay per QALY gained of T 14 pound 831 ($2802), the SH+ intervention had a 97.5% chance of being cost-effective compared with enhanced usual care alone. conclusions and relevance: this economic evaluation suggests that implementation of the SH+ intervention compared with enhanced usual care alone for adult Syrian refugees or asylum seekers hosted in Turkey is cost-effective from the perspective of the Turkish health care system when both international and country-specific willingness-to-pay thresholds were applied., European Union (EU); Horizon 2020

Abstract

Aims: as refugees and asylum seekers are at high risk of developing mental disorders, we assessed the effectiveness of Self-Help Plus (SH + ), a psychological intervention developed by the World Health Organization, in reducing the risk of developing any mental disorders at 12-month follow-up in refugees and asylum seekers resettled in Western Europe. Methods: refugees and asylum seekers with psychological distress (General Health Questionnaire-12 > 3) but without a mental disorder according to the Mini International Neuropsychiatric Interview (M.I.N.I.) were randomised to either SH + or enhanced treatment as usual (ETAU). The frequency of mental disorders at 12 months was measured with the M.I.N.I., while secondary outcomes included self-identified problems, psychological symptoms and other outcomes. Results: of 459 participants randomly assigned to SH + or ETAU, 246 accepted to be interviewed at 12 months. No difference in the frequency of any mental disorders was found (relative risk [RR] = 0.841; 95% confidence interval [CI] 0.389-1.819; p-value = 0.659). In the per protocol (PP) population, that is in participants attending at least three group-based sessions, SH + almost halved the frequency of mental disorders at 12 months compared to ETAU, however so few participants and events contributed to this analysis that it yielded a non-significant result (RR = 0.528; 95% CI 0.180-1.544; p-value = 0.230). SH + was associated with improvements at 12 months in psychological distress (p-value = 0.004), depressive symptoms (p-value = 0.011) and wellbeing (p-value = 0.001). Conclusions: the present study failed to show any long-term preventative effect of SH + in refugees and asylum seekers resettled in Western European countries. Abstract

Introduction Depression is a highly prevalent condition in the elderly, with a vast impact on quality of life, life expectancy, and medical outcomes. Selective serotonin reuptake inhibitors (SSRIs) are the most commonly prescribed agents in this condition and, although generally safe, tolerability issues cannot be overlooked. Vortioxetine is an antidepressant with a novel mechanism of action. Based on studies to date, it may have a promising tolerability profile in the elderly, as it does not adversely affect psychomotor or cognitive performance and does not alter cardiovascular and endocrine parameters. The present study aims to assess the tolerability profile of vortioxetine in comparison with the SSRIs considered as a single group in elderly participants with depression. The rate of participants withdrawing from treatment due to adverse events after 6 months of follow up will be the primary outcome. Methods and analysis This is a pragmatic, multicentre, open-label, parallel-group, superiority, randomized trial funded by the Italian Medicines Agency (AIFA - Agenzia Italiana del Farmaco). Thirteen Italian Community Psychiatric Services will consecutively enrol elderly participants suffering from an episode of major depression over a period of 12 months. Participants will be assessed at baseline and after 1, 3 and 6 months of follow up. At each time point, the following validated rating scales will be administered: Montgomery–Åsberg Depression Rating Scale (MADRS), Antidepressant Side-Effect Checklist (ASEC), EuroQual 5 Dimensions (EQ-5D), Short Blessed Test (SBT), and Charlson Age-Comorbidity Index (CACI). Outcome assessors and the statistician will be masked to treatment allocation. A total of 358 participants (179 in each group) will be enrolled. Ethics and dissemination This study will fully adhere to the ICH E6 Guideline for Good Clinical Practice. Participants’ data will be managed and safeguarded according to the European Data Protection Regulation 2016/679. An external Ethical Advisory Board will help guarantee high ethical standards. Abstract

This study, based on the 'Servizi Territoriali Associati per la Ricerca' (STAR) Network Depot Study nationwide baseline data, explored whether individual symptoms severity and clusters might influence the prescription of paliperidone palmitate 1-month (PP1M) vs. aripiprazole monohydrate. The Brief Psychiatric Rating Scale (BPRS) was used to assess psychopathology and relevant symptoms clusters. Drug Attitude Inventory, 10 items, was used to test attitude towards medications. Adherence to treatments was rated according to the Kemp seven-point scale. We assessed for eligibility 451 individuals and, among them, we included 195 subjects (n = 117 who started PPM1 and n = 78 aripiprazole monohydrate). Individuals were comparable in terms of age, gender, treatment years, recent hospitalizations, previous long-acting injectable antipsychotic treatments, additional oral treatments, attitude toward drugs, medication adherence, and alcohol/substance-related comorbidities. Subjects starting PP1M presented higher BPRS overall (P = 0.009), positive (P = 0.015), and negative (P = 0.010) symptom scores compared to subjects starting aripiprazole monohydrate. Results were confirmed by appropriate regression models and propensity score matching analysis. No differences were found comparing the other BPRS subscale scores: affect, resistance, and activation. Clinicians may be more prone to prescribe PPM1, rather than aripiprazole monohydrate, to subjects showing higher overall symptom severity, including positive and negative symptoms. No additional clinical factors influenced prescribing attitudes in our sample.

Abstract

The selection of plant genotypes characterized by wellness and stable growth under drought-stress conditions amid ongoing climate change is an important challenge in forest tree breeding. The introduction of molecular markers will enable efficient selection of breeding materials that are resistant to drought stress in forest trees as well as in crop species. Japanese cedar, Cryptomeria japonica, the most dominant forest species in Japan, grows well on mesic sites and is characterized by intraspecific variation in its drought-stress response. Here, we analyzed the physiological characteristics and gene expression profiles of multiple C. japonica genotypes under both control and drought-stress conditions to identify drought-responsive genes and develop gene expression markers that could assess the physiological status of cuttings based on their gene expression responses to dehydration. The two representative genes that showed a large expressional change in the later period after water withholding were both homologs of COR47, a gene to the dehydrin protein family, and of ERF016, a member of the dehydration-responsive element-binding (DREB) transcription factor family in Arabidopsis. We developed both intermediate- and small-scale sets of the gene-expression analysis platforms that allowed us to correlate gene expression with the physiological responses of cuttings to drought. 49. Fatal visceral disseminated varicella zoster virus infection during initial remission induction therapy in a patient with lupus nephritis: a case report and review of the literature.

Abstract

Visceral disseminated varicella zoster virus (VZV) infection is a severe complication, characterized by a notably high mortality rate. Herein, we present a case of a 36-year-old-man involving visceral disseminated VZV infection that emerged during remission induction therapy involving high-dose prednisolone (PSL), mycophenolate mofetil (MMF), and hydroxychloroquine for lupus nephritis. Two months after starting the immunosuppressive therapy, he experienced a rapid onset of severe upper abdominal pain. The following day, clinical manifestations and laboratory abnormalities rapidly deteriorated. Hyperferritinemia and hypertriglyceridemia, indicative of hemophagocytic lymphohistiocytosis (HLH), emerged, along with escalating liver and renal impairment and newly appeared disseminated intravascular coagulation, and multiple organ failure is suggested. The patient developed widespread blistering predominantly on the trunk and face, patient's condition failed to ameliorate, ultimately culminating in his demise a few hours later. At autopsy, the cutaneous lesions with blisters revealed positive immunostaining with anti-VZV antibody, and similar findings were detected in multiple organs. HLH was confirmed in lymph nodes. It is crucial to emphasize the awareness of visceral disseminated VZV, particularly in cases patients are undergoing concurrent PSL therapy alongside MMF for SLE. The progression of this fatal condition usually begins with abdominal pain, followed by a skin rash a few days later. The present case is the first to show evidence of HLH occurring as a result of visceral disseminated VZV infection. This disease is extremely rare but extremely serious, therefore, VZV-DNA should be measured in cases where you suspect this disease for early diagnosis and treatment., Competing Interests: Declarations. Recent research has focused on immunotherapy with no side effects as an innovative medical treatment for cancer. However, typical drugs for immunotherapy are very expensive. Here, we propose the use of immunoceramics that activate immune cells by contact with their surface. Previous studies demonstrated that polymers, including the phenylboronic acid group, could activate lymphocytes. This activation may be due to the interaction between the sugar chains in cells and the OH group in B(OH) 3 formed via the dissociation of the BO 2 group. We have clarified that boron-containing apatite (BAp) activated lymphocytes in vitro. In this study, we fabricated the ceramic surfaces using the CaO-P 2 O 5 -SiO 2 -B 2 O 3 system (CPSB ceramics) containing BAp as a main crystalline phase. The results of the in vitro evaluation indicated that killer T cells in splenocytes cocultured with the CPSB ceramics were more numerous than in splenocytes cocultured on a control surface. The results of the in vivo evaluation indicated that the CPSB ceramics significantly inhibited tumor growth when CD8-positive T cells were cultured on individual ceramics and subsequently injected into tumor-bearing mice. The present CPSB ceramics are expected to be a valuable biomaterial for immunotherapy.
