Abstract

Design / Build / Fly (DBF) is an annual and international remote controlled aircraft design competition that provides engineering students an opportunity to develop an aircraft that meets the competition requirements set by AIAA. This year’s competition requires teams to develop a short takeoff, carrier based aircraft with remotely folding wings and capable of carrying and releasing cargo. After analyzing the score analysis, our team determined that the highest scoring potential is to maximize the amount of stores carried while completing an equal number of laps within a 10 minute window. The team explored a wide range of options and finalized on an aircraft design that meets the given constraints while also excelling in mission performance. Our aircraft consists of a 75 in wingspan, an aspect ratio of 6.25, a maximum takeoff weight of 16 lbs, and a cargo capacity of 12 stores. Faculty Advisors: Professor Robert H. 12. Paralumbar compartment syndrome, a rare sequela of deadlifting: a case report and review of current literature.

Abstract

Background: Compartment syndrome is a well-known phenomenon that is most commonly reported in the extremities. However, paralumbar compartment syndrome is rarely described in available literature. The authors present a case of paralumbar compartment syndrome after high intensity deadlifting. Case presentation: 53-year-old male who presented with progressively worsening low back pain and paresthesias one day after high-intensity deadlifting. Laboratory testing found the patient to be in rhabdomyolysis; he was admitted for intravenous fluid resuscitation and pain control. Orthopedics was consulted, and Magnetic Resonance Imaging revealed significant paravertebral edema and loss of muscle striation. Given the patient's lack of improvement with intravenous and oral pain control, clinical and radiographic findings, there was significant concern for acute paralumbar compartment syndrome. The patient subsequently underwent urgent fasciotomy of bilateral paralumbar musculature with delayed closure. Conclusion: Given the paucity of literature on paralumbar compartment syndrome, the authors' goal is to promote awareness of the diagnosis, as it should be included in the differential diagnosis of intractable back pain after high exertional exercise. The current literature suggests that operative cases of paralumbar compartment syndromes have a higher rate of return to pre-operative function compared to those treated non-operatively. This case report further supports this notion. The authors recommend further study into this phenomenon, given its potential to result in persistent chronic exertional pain and irreversible tissue damage. [ABSTRACT FROM AUTHOR]

Abstract

Simple Summary: Colorectal cancer (CRC) is the second leading cause of cancer deaths worldwide. Genetic alterations promote cancer development and its spread to distant sites. Medications that counteract the effects of these alterations/mutations, called targeted therapies, are used to treat CRC. Immunotherapy, another class of medications that promotes immune system mediated recognition and destruction of cancer cells, is used for treatment of a small subset of CRC patients. Here we review the current state of knowledge and ongoing research into biomarkers, targeted therapy, and immunotherapy for CRC treatment. Colorectal cancer (CRC) is the second leading cause of cancer deaths worldwide. Although the overall incidence of CRC is decreasing, the incidence of young-onset CRC, characterized by a diagnosis of CRC before age 50, is increasing. Outcomes for CRC patients are improving, partly due to comprehensive molecular characterization of tumors and novel therapeutic strategies. Advances in genomic and transcriptomic analyses using blood- and tumor-tissue-based sequencing have facilitated identification of distinct tumor subtypes harboring unique biological characteristics and therapeutic vulnerabilities. These insights have led to the development and incorporation of targeted therapies and immunotherapy in CRC treatment. In this review, we discuss the molecular landscape and key oncogenes/tumor suppressors contributing to CRC tumorigenesis, metastasis, and therapeutic resistance. We also discuss personalized therapeutic strategies for subsets of CRC patients and provide an overview of evolving novel treatments being evaluated in clinical trials. [ABSTRACT FROM AUTHOR]

Abstract

The risk of ventilator induced lung injury in mechanically ventilated (MV) critically ill patients can be mitigated by patient-specific optimisation of ventilator settings. Recent studies have shown that driving pressure, i.e. the difference between plateau pressure (Pplat) and PEEP, is a strong indicator for survival in MV patients suffering from ARDS. However, to measure Pplat, an extended end-inspiratory pause (EIP) has to be applied, possibly interrupting ventilation therapy. This study presents a method for predicting Pplat from normal breaths in MV patients.

37. Analysis of combustion chamber deposit growth on temperature swing thermal barrier coatings in a spark ignition engine.

Abstract

Temperature swing thermal barrier coatings (TBCs) have the potential to improve efficiency and performance through thermal management of the combustion chamber. However, the effects of combustion chamber deposits (CCDs) on the surface of the TBCs are unclear. Therefore, this paper analyzes the impact of CCD formation on a temperature swing TBC. A piston and a heat flux probe coated with a novel material are installed in a single-cylinder research engine at a low-load condition for 62.5 h to promote CCD growth. Every 12.5 h, the performance at a knock limited condition is assessed and thermophysical property measurements on the heat flux probe are performed. Net thermal efficiency increased by 0.4% absolute after 12.5 h relative to the baseline condition, but further CCD growth caused a dithering of efficiency between the 12.5 h and baseline points. The KLSA retarded consistently throughout this period. External property measurements with the coated heat flux probe showed an improvement in the thermophysical properties of the TBC/CCD layer. However, female gout patients are more likely to have comorbidities such as hypertension, diabetes mellitus, and renal dysfunction. Furthermore, they experience a greater disease burden due to gout than males. While nonbiological causes may possibly contribute to this sex discrepancy in burden, this raises questions regarding whether current gout pharmacotherapies are as efficacious in females as they are in males. In this review, we examine how the clinical profile of female gout patients differs from male patients; we then survey the literature for data on outcomes for female gout patients treated with urate-lowering therapies for chronic management of gout as well as commonly used agents for acute flares.
