This paper aims to evaluate the role and importance of print media in agricultural information dissemination among the farmers community. The mass media can be a powerful tool for the dissemination of information in rural areas. Print media can be used well if they are relevant to the targeted audience and provide the information they seek. For this study, data were collected using structured questionnaire, distributed randomly among the farmers in three selected villages of Gandhinagar district in Gujarat. The data was analyzed using the Microsoft Excel and bar charts were prepared for better understanding and interpretation. The finding shows that fellow farmers and print media are the important sources of agricultural information among the farmers community. However, based on the rating of various sources of information by respondents, the print media got first, fellow farmers got second and internet got third position. Among the different form of print media, the most used form of print media for dissemination of agricultural information was pamphlets followed by magazines, newspapers, posters, books/booklets, leaflets and journals. Introduction: Endodontic mistakes, also known as procedural accidents, are those bad events that occur during treatment, some of which are attributable to a lack of attention to detail and others of which are completely unforeseeable. The second most frequent reason for root canal failure is perforation. Aims and Objective: The current in vitro study's objective was to assess the furcal perforation's potential to be sealed with and without internal matrix by MTA flow, Biodentine, and pro-root MTA. Materials and Methods: 60 samples were allocated into six groups at random (10 each). In intact permanent mandibular first and second teeth, furcal perforation sites were made and sealed with various materials. Following perforation sealing, the specimens' capacity for sealing was evaluated using the dye penetration method. Result: The current study's findings indicated that Group 2 has the least amount of microleakage and Group 5 has the most. Photo‐electrochemical production of solar fuels from carbon dioxide, water, and sunlight is an appealing approach. Nevertheless, it remains challenging to scale despite encouraging demonstrations at low power input. Higher current densities require notable voltage input as ohmic losses and activation overpotentials become more significant, resulting in lower solar‐to‐CO conversion efficiencies. A concentrated photovoltaic cell is integrated into a custom‐made heat managed photo‐electrochemical device. The heat is transferred from the photovoltaic module to the zero‐gap electrolyzer cell by the stream of anodic reactant and produce synergetic effects on both sides. With solar concentrations up to 450 suns (i.e., 450 kW m−2) applied for the first time to photo‐electrochemical reduction of CO2, a partial current for CO production of 4 A is achieved. At optimal conditions, the solar‐to‐CO conversion efficiency reaches 17% while maintaining a current density of 150 mA cm−2 in the electrolyzer and a CO selectivity above 90%, representing an overall 19% solar‐to‐fuel conversion efficiency. This study represents a first demonstration of photo‐electrochemical CO2 reduction under highly concentrated light, paving the way for resource efficient solar fuel production at high power input. : Background: Abrasives are the insoluble components added to dentifrices to aid the physical removal of stains, plaque and food debris. However, replete of abrasivity cause enamel abrasion, dentin hypersensitivity, damaging the smile as well as masticatory apparatus. The aim of this study is to evaluate the enamel surface abrasivity using three different herbal dentifrices and a customised brushing machine under profilometer. Materials And Method: A total of 30 enamel blocks are prepared from buccal surface of maxillary central incisor which are randomly divided into three groups. Group-1 Specimens brushed with Colgate Vedshakti, Group-2 Specimens brushed with Patanjali Dant Kanti, Group-3 Specimens brushed with Dabur Red. All specimens were brushed using a customised brushing model with Oral-B powered brush for 30 minutes. Surface roughness of all three groups specimens were analysed using a surface roughness tester-profilometer. Statistical analysis used in this study was one-way analysis of variance (ANOVA) followed by post hoc turkey’s test. Results: Statistically significant difference was observed in value of enamel abrasion amongst Group1, 2 and 3. (p<0.05) Conclusion: Toothpaste with higher relative enamel abrasivity has the potential to abrade the enamel and cause hypersensitivity. The safety, effectiveness, and cost-effectiveness of molnupiravir, an oral antiviral medication for SARS-CoV-2, has not been established in vaccinated patients in the community at increased risk of morbidity and mortality from COVID-19. We aimed to establish whether the addition of molnupiravir to usual care reduced hospital admissions and deaths associated with COVID-19 in this population. PANORAMIC was a UK-based, national, multicentre, open-label, multigroup, prospective, platform adaptive randomised controlled trial. Eligible participants were aged 50 years or older—or aged 18 years or older with relevant comorbidities—and had been unwell with confirmed COVID-19 for 5 days or fewer in the community. Participants were randomly assigned (1:1) to receive 800 mg molnupiravir twice daily for 5 days plus usual care or usual care only. A secure, web-based system (Spinnaker) was used for randomisation, which was stratified by age (<50 years vs ≥50 years) and vaccination status (yes vs no). COVID-19 outcomes were tracked via a self-completed online daily diary for 28 days after randomisation. The primary outcome was all-cause hospitalisation or death within 28 days of randomisation, which was analysed using Bayesian models in all eligible participants who were randomly assigned. This trial is registered with ISRCTN, number 30448031. Between Dec 8, 2021, and April 27, 2022, 26 411 participants were randomly assigned, 12 821 to molnupiravir plus usual care, 12 962 to usual care alone, and 628 to other treatment groups (which will be reported separately). 12 529 participants from the molnupiravir plus usual care group, and 12 525 from the usual care group were included in the primary analysis population. The mean age of the population was 56·6 years (SD 12·6), and 24 290 (94%) of 25 708 participants had had at least three doses of a SARS-CoV-2 vaccine. Hospitalisations or deaths were recorded in 105 (1%) of 12 529 participants in the molnupiravir plus usual care group versus 98 (1%) of 12 525 in the usual care group (adjusted odds ratio 1·06 [95% Bayesian credible interval 0·81–1·41]; probability of superiority 0·33). There was no evidence of treatment interaction between subgroups. Serious adverse events were recorded for 50 (0·4%) of 12 774 participants in the molnupiravir plus usual care group and for 45 (0·3%) of 12 934 in the usual care group. None of these events were judged to be related to molnupiravir. This paper proposes an optimization approach for day-ahead reactive power planning to improve voltage security in transmission networks. The problem is formulated as a voltage-secure multi-period optimal reactive power dispatch (MP-ORPD) problem. The optimization approach searches for optimal set-points of dynamic and static reactive power (var) resources. Specifically, the output includes set-points for switching shunts, transformer taps, and voltage magnitudes at the regulated buses. The primary goal is to maximize the dynamic reactive power reserve of the system, by minimizing the reactive power supplied by synchronous generators. As the size of the MP-ORPD problem increases significantly with increasing number of contingencies and time periods, efficiency is crucial for practical applications. In this paper, a decomposition technique based on consensus and alternating optimization, where integer variable targets are obtained via MILP, is used to partition the MP-ORPD problem into a set of subproblems, which can be solved in parallel to reduce the computation time. The proposed MP-ORPD problem and its solution algorithm are integrated into the EPRI-VCA software. The results of various power networks of large electric utilities in the Eastern interconnection demonstrate the effectiveness of the proposed algorithm in providing preventive control schedules. Objective: To quality assess a sample of health behavior change apps from the NHS Apps Library using a rating tool based on the 2014 National Institute for Health and Care Excellence behavior change guidance (NICE BCG).Methods: A qualitative analysis of the NICE BCG identified themes and questions for a quality assessment of health behavior change apps. These were refined by further discussion and piloting, and applied by two independent raters to a sample of NHS Library apps (N=49). Disagreements were resolved following discussions with a third rater.Results: Themes identified were; purpose, planning, usability, tailoring, behavior change technique (BCT), maintenance, evaluation, data security and documentation. Overall, purpose of the apps was clear, but evidence for collaboration with users or professionals was lacking. Usability information was poor and tailoring disappointing. Most used recognized BCTs but paid less attention to behavior maintenance than initiation. Information on app evaluation and documentation was sparse.Conclusions: This study furthers the work of the NHS Apps Library, adapting the NICE (2014) behavior change guidance for quality assessment of behavior change apps.Practice Implications: This study helps lay the foundations for development of a quality assurance tool for mobile health apps aimed at health behavior change. Recent developments in restorative material, placement techniques, preparation designs, and an adhesive protocol allow clinicians to predictably restore fractured teeth. The share of inverter-connected renewable energy resources (RESs) is increasing in the grid, with these resources partially displacing conventional synchronous generators. This has resulted in increased variability of active power supply, reduced overall inertia, and increased spatial heterogeneity of inertia, leading to faster system frequency dynamics along with larger and more frequent frequency control events. These effects are expected to become increasingly more important in power system control in next-generation grids, which may conceivably be made up entirely of RESs. To mitigate these challenges, a fast, area-based hierarchical control strategy is proposed. This scheme partitions the power system into small areas, estimates local power imbalances, and corrects them by utilizing local inverter-based resources (IBRs). In cases where sufficient resources are not available locally, power is preferentially sourced from electrically close neighbours using an iterative distributed optimization scheme which preserves information privacy between areas. The proposed frequency control architecture can be retrofit onto existing control systems, and allows for flexibility in the amount of model information available to the designer. Background: A previous efficacy trial found benefit from inhaled budesonide for COVID-19 in patients not admitted to hospital, but effectiveness in high-risk individuals is unknown. We aimed to establish whether inhaled budesonide reduces time to recovery and COVID-19-related hospital admissions or deaths among people at high risk of complications in the community.Methods: PRINCIPLE is a multicentre, open-label, multi-arm, randomised, controlled, adaptive platform trial done remotely from a central trial site and at primary care centres in the UK. Eligible participants were aged 65 years or older or 50 years or older with comorbidities, and unwell for up to 14 days with suspected COVID-19 but not admitted to hospital. Participants were randomly assigned to usual care, usual care plus inhaled budesonide (800 μg twice daily for 14 days), or usual care plus other interventions, and followed up for 28 days. Participants were aware of group assignment. The coprimary endpoints are time to first self-reported recovery and hospital admission or death related to COVID-19, within 28 days, analysed using Bayesian models. The primary analysis population included all eligible SARS-CoV-2-positive participants randomly assigned to budesonide, usual care, and other interventions, from the start of the platform trial until the budesonide group was closed. This trial is registered at the ISRCTN registry (ISRCTN86534580) and is ongoing.Findings: The trial began enrolment on April 2, 2020, with randomisation to budesonide from Nov 27, 2020, until March 31, 2021, when the prespecified time to recovery superiority criterion was met. 4700 participants were randomly assigned to budesonide (n=1073), usual care alone (n=1988), or other treatments (n=1639). The primary analysis model includes 2530 SARS-CoV-2-positive participants, with 787 in the budesonide group, 1069 in the usual care group, and 974 receiving other treatments. There was a benefit in time to first self-reported recovery of an estimated 2·94 days (95% Bayesian credible interval [BCI] 1·19 to 5·12) in the budesonide group versus the usual care group (11·8 days [95% BCI 10·0 to 14·1] vs 14·7 days [12·3 to 18·0]; hazard ratio 1·21 [95% BCI 1·08 to 1·36]), with a probability of superiority greater than 0·999, meeting the prespecified superiority threshold of 0·99. For the hospital admission or death outcome, the estimated rate was 6·8% (95% BCI 4·1 to 10·2) in the budesonide group versus 8·8% (5·5 to 12·7) in the usual care group (estimated absolute difference 2·0% [95% BCI -0·2 to 4·5]; odds ratio 0·75 [95% BCI 0·55 to 1·03]), with a probability of superiority 0·963, below the prespecified superiority threshold of 0·975. The increasing integration of utility-scale renewable energy sources (RESs) brings emerging challenges to the classical problems of voltage control in transmission grids, including increased potential for voltage violations. To address this challenge, we present a novel measurement-based coordinated voltage control scheme that can enable fast participation of RESs. In this approach, RESs are coordinated with the traditional voltage control devices such as synchronous generators (SGs) and static var compensators (SVCs) to maintain all bus voltages within operational limits while respecting device power limits. The control scheme allows different priorities to be assigned to different control resources, and ensures that both voltage and reactive power constraints are met in steady-state whenever it is possible to do so. The controller design requires only an approximate model of the steady-state relationships between voltage and reactive power in the system, and in online operation, processes voltage and reactive power measurements to produce set-point updates for RESs, SVCs, and SGs; this feedback provides robustness against both model uncertainty and unmeasured disturbances. Background: Azithromycin, an antibiotic with potential antiviral and anti-inflammatory properties, has been used to treat COVID-19, but evidence from community randomised trials is lacking. We aimed to assess the effectiveness of azithromycin to treat suspected COVID-19 among people in the community who had an increased risk of complications.Methods: In this UK-based, primary care, open-label, multi-arm, adaptive platform randomised trial of interventions against COVID-19 in people at increased risk of an adverse clinical course (PRINCIPLE), we randomly assigned people aged 65 years and older, or 50 years and older with at least one comorbidity, who had been unwell for 14 days or less with suspected COVID-19, to usual care plus azithromycin 500 mg daily for three days, usual care plus other interventions, or usual care alone. The trial had two coprimary endpoints measured within 28 days from randomisation: time to first self-reported recovery, analysed using a Bayesian piecewise exponential, and hospital admission or death related to COVID-19, analysed using a Bayesian logistic regression model. Eligible participants with outcome data were included in the primary analysis, and those who received the allocated treatment were included in the safety analysis. The trial is registered with ISRCTN, ISRCTN86534580.Findings: The first participant was recruited to PRINCIPLE on April 2, 2020. The azithromycin group enrolled participants between May 22 and Nov 30, 2020, by which time 2265 participants had been randomly assigned, 540 to azithromycin plus usual care, 875 to usual care alone, and 850 to other interventions. 2120 (94%) of 2265 participants provided follow-up data and were included in the Bayesian primary analysis, 500 participants in the azithromycin plus usual care group, 823 in the usual care alone group, and 797 in other intervention groups. 402 (80%) of 500 participants in the azithromycin plus usual care group and 631 (77%) of 823 participants in the usual care alone group reported feeling recovered within 28 days. We found little evidence of a meaningful benefit in the azithromycin plus usual care group in time to first reported recovery versus usual care alone (hazard ratio 1·08, 95% Bayesian credibility interval [BCI] 0·95 to 1·23), equating to an estimated benefit in median time to first recovery of 0·94 days (95% BCI -0·56 to 2·43). The probability that there was a clinically meaningful benefit of at least 1·5 days in time to recovery was 0·23. 16 (3%) of 500 participants in the azithromycin plus usual care group and 28 (3%) of 823 participants in the usual care alone group were hospitalised (absolute benefit in percentage 0·3%, 95% BCI -1·7 to 2·2). There were no deaths in either study group. Safety outcomes were similar in both groups. Two (1%) of 455 participants in the azothromycin plus usual care group and four (1%) of 668 participants in the usual care alone group reported admission to hospital during the trial, not related to COVID-19.Interpretation: Our findings do not justify the routine use of azithromycin for reducing time to recovery or risk of hospitalisation for people with suspected COVID-19 in the community. Alcohol and drug abuse continue to be major causes of morbidity and mortality and have significant social and economic ramifications. Studies have shown that for every $1 spent on substance use disorder treatment, $4 are saved on healthcare costs. Characterizing the healthcare resource utilization of these patients may shed light on the burden of disease and opportunities for intervention. A retrospective chart review of all patients admitted to the ICU between July 1, 2017 and December 31, 2017 was completed. Variables regarding demographic and clinical characteristics as well as healthcare resource utilization were collected. Of 737 admissions to the ICU, 158 (21%) were due to acute or chronic complications of alcohol or drug abuse. Even though alcohol and drug users were significantly younger (average age 50 years) than the general ICU cohort (average age 66 years), resource utilization was similar between these patients. The median length of stay in the ICU was similar. The number of patients transferred to in-patient rehab was low (8%), and all of those were due to comorbid psychiatric illness. The total hospital charges for the alcohol and drug abuse cohort was over 7 million dollars for the 6 months observed. A significant number of patients had at least one ER visit (49%) during the previous year, and most of these had numerous visits. ICU resource utilization by patients with acute and chronic sequelae of drug or alcohol abuse disorders continues to be high. These patients utilize resources at rates similar to an older group with other disease processes. Patients are unlikely to receive intervention for their disorder unless they have a comorbid psychiatric illness. Patients admitted to the ICU with alcohol or drug-related illness were frequently seen in the ER or were admitted to the hospital in the year prior to ICU admission, providing opportunities for intervention. This paper presents a new framework to improve the quality of streaming synchrophasor measurements with the existence of missing data and bad data. The method exploits the low-rank property of the Hankel structure to identify and correct bad data, as well as to estimate and fill in the missing data. The method is advantageous compared to existing methods in the literature that only estimate missing data by leveraging the low-rank property of the synchrophasor data observation matrix. The proposed algorithm can efficiently differentiate event data from bad data, even in the existence of simultaneous and consecutive bad data. Several techniques have been developed to estimate the load parameters in power systems. Most of the existing algorithms mainly focus on estimating the parameters for offline studies. With on-going smart grid development, high-resolution data at faster rates are available to allow estimation of load parameters in real time. This paper addresses the challenges in online estimation of the load parameters using phasor measurement unit data. A novel adaptive search-based algorithm to estimate load model parameters is presented here. In this paper, a static load model is used with the Z (constant impedance), I (constant current), and P (constant power) components of the load. Developed estimation algorithms for the ZIP parameter estimation are validated using the IEEE 14-bus system and data provided by the industry collaborators. Simulation results demonstrate the accurate estimation of the ZIP load model using the developed method. We propose a novel hierarchical frequency and voltage control design for multi-area power system integrated with inverter-based resources (IBRs). The design is based on the idea of prioritizing the use of IBRs over conventional generator-based control in compensating for sudden and unpredicted changes in loads and generations, and thereby mitigate any undesired dynamics in the frequency or the voltage by exploiting their fast actuation time constants. A new sequential optimization problem, referred to as Area Prioritized Power Flow (APPF), is formulated to model this prioritization. It is shown that compared to conventional power flow APPF not only leads to a fairer balance between the dispatch of active and reactive power from the IBRs and the synchronous generators, but also limits the impact of any contingency from spreading out beyond its respective control area, thereby guaranteeing a better collective dynamic performance of the grid. This improvement, however, comes at the cost of adding an extra layer of communication needed for executing APPF in a hierarchical way. Results are validated using simulations of a 9-machine, 6-IBR, 33-bus, 3-area power system model, illustrating how APPF can mitigate a disturbance faster and more efficiently by prioritizing the use of local area-resources. • A new concept of "area-prioritization"-based power flow (APPF) dispatch is proposed to maximize the participation of inverter-based resources (IBRs) in frequency and voltage control. • A hierarchical computation architecture is developed to implement this prioritization. • Implementation steps are showcased via simulation of a 33-bus power system with 9 generators and 6 IBRs. The increasing availability of high-resolution, synchronized measurements from phasor measurement units (PMUs) presents an opportunity to monitor power system dynamics in real time. However, the degree of observability of power system dynamics depends upon the power system states monitored by the PMUs. This paper presents a new algorithm to assess the observability level of rotor angle dynamics, based on PMU locations and monitored states. An observability index is proposed, which can be tracked along the trajectory of system states over time. To overcome the difficulty in calculation of high-order derivatives, an automatic differentiation technique is applied for observability calculation of large-scale systems. The proposed observability algorithms and techniques are validated on a 179-bus system. The proposed method is also helpful in identifying locations where PMUs should be installed for effective observability of nonlinear power system dynamics. Wide-area measurement systems enable the wide-area damping controller (WADC) to use remote signals to enhance the small signal stability of large scale interconnected power systems. System operating condition variations and signal transmission time delays are the major factors to worsen the damping effect and even deteriorate the system stability. This paper proposes a novel measurement-based adaptive wide-area damping control scheme using oscillation mode prediction and system identification techniques. These techniques adjust the parameters of WADC as well as the time delay compensation in an online environment. To achieve fast online implementation, an identified high order multi-input multi-output (MIMO) model is deformed into a low order single-input single-output (SISO) model according to the residue of MIMO model. The SISO model can accurately represent the power system dynamics in the form of a transfer function, capturing the dominant oscillatory behaviors in the frequency range of interest. Moreover, the WADC has been implemented on a hardware test-bed (HTB) by adding its output signal to the excitation system of a selected generator. The effectiveness of the proposed measurement-based adaptive WADC has been demonstrated in a two-area four-machine system on the HTB under various disturbance scenarios. [ABSTRACT FROM AUTHOR]

Background: Chronic Lymphocytic Leukemia (CLL) is a lymphoproliferative disease characterized by multiple recurring clonal cytogenetic anomalies and is the most common leukemia in adults. Chromosomal abnormalities associated with CLL include trisomy 12 and IGH;BCL3 rearrangement [t(14;19)(q32;q13)] that juxtaposes a protooncogenic gene BCL3 and an immunoglobulin heavy chain, a translocation that may be associated with shorter survival. In addition to the IGH;BCL3 rearrangement, other translocations involving 14q32 locus are involved in various lymphoproliferative pathologies pointing toward the significance of IGH locus in oncogenic progression. Significantly, in the majority of B-cell neoplasms that carry an IGH;BCL3 rearrangement, it is a sole translocation involving an IGH locus. Case Presentation: We report a patient who, in addition to trisomy 12, carried a rare double-hit translocation characterized by the IGH;BCL3 translocation and an additional clonal IGH;BCL2 translocation involving IGH and another proto-oncogene BCL2, t(14;18)(q32;q21), commonly found in follicular lymphoma. Further single nucleotide polymorphism (SNP) array-based analysis detected a duplication of the 58.8 kb region at 19q13.32 adjacent to the BCL3 translocation junction on chromosome 19q13. Interestingly, the duplicated region contained ERCC2 gene, which encodes a DNA excision repair protein involved in the cancer-prone syndrome, xeroderma pigmentosum. Conclusions: Taken together our findings indicate the existence of double-translocation driven oncogenic events involving both IGH loci and proto-oncogenes BCL2 and BCL3. Importantly, the IGH;BCL3 translocation was characterized by the duplication of the genomic region adjacent to BCL3, containing a major DNA repair factor, ERCC2. [ABSTRACT FROM AUTHOR]

A 44-year-old woman was brought to the ED from John F. Kennedy International Airport. The patient was returning with her son from a 3-month visit to Bangladesh. Her journey started with a 4-h flight from Dhaka, Bangladesh to Dubai, United Arab Emirates. She consumed 240 mL of whiskey during the flight. This was followed by a 14-h flight from Dubai to New York. According to the patient's son, she did not consume any alcohol during the second flight. The patient was in her usual state of health with normal mentation throughout her journey. Upon landing, she started complaining of shortness of breath. After disembarking, she was witnessed to have seizure-like activity with involuntary passage of urine, following which she collapsed.
