Abstract

Aims To examine the effects of a 24-month treatment with ipragliflozin on carotid intima-media thickness (IMT) in type 2 diabetes patients. Methods and results In this multicenter, prospective, randomized, open-label, and blinded-endpoint investigator-initiated clinical trial, adults with type 2 diabetes and haemoglobin A1C (HbA1c) of 6.0–10.0% (42–86 mmol/mol) were randomized equally to ipragliflozin (50 mg daily) and non-sodium-glucose cotransporter-2 (SGLT2) inhibitor use of standard-care (control group) for type 2 diabetes and were followed-up to 24 months. The primary endpoint was the change in mean common carotid artery IMT (CCA-IMT) from baseline to 24 months. A total of 482 patients were equally allocated to the ipragliflozin (N = 241) and control (N = 241) groups, and 464 patients (median age 68 years, female 31.7%, median type 2 diabetes duration 8 years, median HbA1c 7.3%) were included in the analyses. For the primary endpoint, the changes in the mean CCA-IMT from baseline to 24 months were 0.0013 [95% confidence interval (CI), −0.0155–0.0182] mm and 0.0015 (95% CI, −0.0155–0.0184) mm in the ipragliflozin and control groups, respectively, with an estimated group difference (ipragliflozin-control) of −0.0001 mm (95% CI, −0.0191–0.0189; P = 0.989). A group difference in HbA1c change at 24 months was also non-significant between the treatment groups [−0.1% (95% CI, −0.2–0.1); P = 0.359]. Conclusion Twenty-four months of ipragliflozin treatment did not affect carotid IMT status in patients with type 2 diabetes recruited in the PROTECT study, relative to the non-SGLT2 inhibitor-use standard care for type 2 diabetes.

Abstract

Cardiac involvement has been reported in patients with coronavirus disease 2019 (COVID-19). We herein report a 41-year-old man who presented with recurrent paroxysmal atrioventricular block without showing significant cardiac injuries or comorbidities. The patient was diagnosed with COVID-19 and admitted to our hospital, where he was noted to have paroxysmal atrioventricular block. Cardiac biomarkers, echocardiography, and cardiac magnetic resonance imaging findings were fairly normal. An endomyocardial biopsy performed before the implantation of a permanent pacemaker revealed mild myocardial fibrosis without inflammatory infiltrates. The unusual myocardial involvement of the novel coronavirus was suspected.

5. Experimental and Numerical Analysis of Unstable Behavior of Melt Blowing Process.

Abstract

Melt blowing is a one step process which produces nonwoven fabrics by extending the extruded polymer melt with a jet of hot air that rapidly attenuates the extrudate into small diameter fibers. Unstable behavior of melt-blowing process was investigated through experimental analysis and numerical analysis. Polypropylene (PP) with different melt flow rate (MFR) were used in experimental analysis. Stability of the process was evaluated by measurement of fiber diameter distribution in the resultant web. In addition, thinning behavior of spin-line near the spinneret was observed by using a high-speed camera. Broadening of fiber diameter distribution was observed in the spinning conditions of small throughput rate or high airflow rate. Periodic fluctuation of fiber diameter was also observed in those spinning conditions. It indicated that the diameter distribution was caused by instability of spin-line. For the PP of low viscosity, instability of the process dominated by throughput rate. On the other hand, airflow rate was more effective on instability of high viscosity PP. These results suggested that the two different origins were existed. To investigate the mechanism of instability in melt-blowing process, numerical analysis was conducted using newly developed simulation system based on Lagrangian coordinate system. Results of numerical analysis suggested that the origin of instability of small throughput rate with low viscosity PP was surface tension of molten polymer. In addition, the other origin of instability noticeable in high airflow rate was resonance between spin-line and turbulence of blowing air. [ABSTRACT FROM AUTHOR]

6. Control of Fiber Diameter Fluctuation in the Melt-blowing Process by Introducing Long-chain Branching to Polypropylene.

Abstract

INTRODUCTION: Nonwoven webs prepared through the melt-blowing process of polypropylene (PP) are widely used for various applications such as the filtration layer in multi-layered masks. Preparation of webs consisting of fibers with thinner diameters is required for the improvement of filtration properties. Reduction of the through-put rate and increase of the speed of blowing air in the melt-blowing process generally leads to the production of thinner fibers, however, fiber diameter distribution tends to become wider under extreme conditions. In this research, PP for the melt-blowing grade was modified by blending a small amount of long-chain branched PP to improve the occurrence diameter fluctuation in the melt-blowing process and thereby reduce the average fiber diameter. EXPERIMENTAL: The polymers used in this research were linear polypropylene melt blowing grade (STD-PP, MFR = 1500 g/10 min) and long-chain branched polypropylene (LCB-PP, MFR=9 g/10 min). Melt-blowing of the blend of STDPP and LCB-PP was performed using an extrusion system equipped with a melt-blowing spinning head of 10 cm width, where 10 nozzles of 0.2 mm diameter were evenly arranged. Spinning behavior near the nozzle was observed using a high-speed camera. The diameter distribution of the fibers in the melt-blown web was analyzed using a scanning electron microscope. The specific surface area of the webs was analyzed using a constant volume adsorption apparatus. RESULTS AND DISCUSSION: Observation of the melt-blowing process near the spinneret using a high-speed camera revealed the occurrence of fiber diameter fluctuation of high frequency. The degree of diameter fluctuation intensified when production of the fibers of small diameter was attempted, and the melt-blown webs consisting of fibers with bi-modal diameter distribution were produced. It was found that the introduction of low MFR long chain branched PP suppressed the decrease in elongation viscosity in the high strain rate range, and eventually spinning was stabilized. The impact of each element, i.e. suppression of elongational viscosity reduction and enhancement of stabilization, changed with the concentration of LCB-PP. Stable spinning reduced the peak diameter of thick fibers in the web and thereby narrowed the fiber diameter distribution. Eventually, the optimum concentration of LCB-PP was found to exist, where the formation of webs with minimum average fiber diameter and maximum specific surface area was accomplished as shown in Figure 1 [1].
