Vancomycin-resistente Enterokokken (VRE) gehören zu den in Deutschland gemäß § 23 Abs. 4 IfSG zu erfassenden Erregern und werden in vielen deutschen Kliniken häufig beobachtet. Der Nachweis von VRE ist in den zurückliegenden Jahren auf einem stabil hohen Niveau in Deutschland und bewegt sich leicht oberhalb des EU-Durchschnitts. Die Krankheitslast durch invasive VRE-Infektionen ist hingegen nachweislich ansteigend. Das Nationale Referenzzentrum für Staphylokokken und Enterokokken beobachtet diese Entwicklungen und berichtet über die Situation von VRE und Enterokokken mit besonderen Antibiotikaresistenzen im Zeitraum 2021/2022. Darüber hinaus wird auf allgemeine VRE-Resistenzsta¬tistiken nationaler Erhebungssysteme und -studien hingewiesen.

Um die Belastung der Krankenh��user durch die COVID-19-Pandemie darzustellen und auch nosokomiale SARS-CoV-2-Infektionen zu erfassen, hat das RKI in Zusammenarbeit mit dem Nationalen Referenzzentrum f��r die Surveillance von nosokomialen Infektionen an der Charit�� Berlin eine systematische Krankenhaus-Surveillance von SARS-CoV-2-Infektionen in Deutschland, COSIK, entwickelt.

Im Rahmen der Bew��ltigung der COVID-19-Pandemie spielt die SARS-CoV-2/COVID-19-Diag��nostik eine entscheidende Rolle. Das RKI erfasst w��chentlich die Anzahl der in Deutschland durchgef��hrten SARS-CoV-2-PCR-Tests, sowie einige Begleitinfor��mationen. Hierf��r werden deutschlandweit Daten von Universit��tskliniken, Forschungseinrichtungen sowie klinischen und in der ambulanten Versor��gung t��tigen Laboren zusammengef��hrt. Wie das Epidemiologische Bulletin 47/2021 berichtet, wurden seit Beginn der Testungen bis ein��schlie��lich Kalenderwoche 45/2021 bisher 81.975.529 SARS-CoV-2-PCR-Tests erfasst, da��von waren 5.677.751 positiv.

Prinzipiell können zum Nachweis von SARS-CoV-2 bei nicht beatmeten Patientinnen und Patienten Pro¬ben von mehreren Orten entnommen werden. Methode der Wahl ist der vom Fachpersonal entnommene oropharyngeale, nasopharyngeale oder kombinierte Abstrich. Diese Proben können jedoch auch von Patientinnen und Patienten selbst abgenommen werden, was zahlreiche Vorteile birgt (z. B. die Vermeidung einer Virusübertragung auf das Personal). Das Epidemiologische Bulletin 17/2021 gibt anhand einer Literaturübersicht und Studienergebnissen des RKI einen Überblick über die prinzipiell zur Verfügung stehenden Probenahmeorte und -modi. Darüber hinaus werden von Patientinnen und Patienten selbst abgenommene Proben in Hinblick auf Praktikabilität und Akzeptanz mit den vom Fachpersonal abgenommenen Proben vergleichen.

Aus einer Analyse der an das RKI übermittelten COVID-19-Meldedaten ging hervor, dass 70% der hospitalisierten und 95% der verstorbenen Patientinnen und Patienten in Deutschland ≥ 60 Jahre alt waren und Alten- und Pflegeeinrichtungen in besonderem Maße von Ausbrüchen betroffen sind. Einen wesentlichen Einfluss auf das Auftreten von Ausbrüchen hat dabei die Entwicklung der COVID-19-Inzidenzen in der Gesamtbevölkerung. Um Kenntnisse über Ausdehnung, Verlauf und Charakteristika der Ausbruchsgeschehen zu gewinnen, wurden drei verschiedene Zeitphasen des Pandemieverlaufs betrachtet und die jeweiligen Ausbruchsdaten aus diesen Phasen in den Kontext der Entwicklung des Infektionsgeschehens in der Gesamtbevölkerung gestellt. Hieraus lassen sich die Hauptstrategien zur Eindämmung von COVID-19 in Pflegeheimen ableiten.

Für den Nachweis einer akuten SARS-CoV-2-Infektion stehen in Deutschland aktuell zwei unterschiedliche Testverfahren zur Verfügung: PCR-Methoden und Antigentests. Beide verwenden Untersuchungsmaterial aus den oberen Atemwegen und sind zur Anwendung durch Fachpersonal vorgesehen. Der Nachweis von SARS-CoV-2 mittels RT-PCR ist der Goldstandard und zeichnet sich durch eine sehr hohe Sensitivität und Spezifität aus. Allerdings sind bei hohem Probeaufkommen PCR-basierte Tests eine begrenzte Ressource. Laborbasierte Antigentests oder sogenannte „Antigen-Schnelltests“ lassen sich mit deutlich weniger Aufwand durchführen und liefern ein Ergebnis in kurzer Zeit. Sie können allerdings zu einer höheren Anzahl falsch negativer bzw. falsch positiver Testergebnisse führen. Falsch positive Ergebnisse können durch einen nachfolgenden PCR-Test erkannt werden. Derzeit werden Antigentests, bei denen Probennahme, Testung und Bewertung des Ergebnisses durch medizinische Laien vorgesehen sind, im Rahmen der CE-Kennzeichnung geprüft. Das Epidemiologische Bulletin 8/2021 beschreibt die Chancen, aber auch Risiken und Limitationen bei der Eigenanwendung von Antigen-Selbsttests zum Nachweis einer akuten Infektion mit SARS-CoV-2.

Abwägung der Dauer von Quarantäne und Isolierung bei COVID-19 Angesichts weltweit zunehmender Fallzahlen mit SARS-CoV-2 ist die konsequente Einhaltung von Maßnahmen zur Verlangsamung der Ausbreitung erforderlich. Grundlage bildet die AHA+L-Regel. Falls es zu SARS-CoV-2-Infektionen kommt, stehen individuelle Maßnahmen im Vordergrund: eine mindestens 10-tägige (Selbst-)Isolierung von Erkrankten und Personen, bei denen eine Virusausscheidung festgestellt worden ist, und die 14-tägige Quarantäne derjenigen, bei denen nach Kontakt zu einer ansteckenden Person die Wahrscheinlichkeit besteht, dass es zu einer Ansteckung gekommen ist. Im Epidemiologischen Bulletin 39/2020 wird der Unterschied von Quarantäne und Isolierung basierend auf den Grundlagen des Infektionsverlaufs näher erläutert. Erste Ergebnisse aus internen Rechenmodellen des Robert Koch-Instituts verdeutlichen, wie sich das Restrisiko einer Infektion Dritter in Abhängigkeit von der Quarantänedauer mit oder ohne abschließende Testung verhält.

Die 2008 etablierte Antibiotika-Resistenz-Surveillance (ARS) wurde für die Surveillance von SARS-CoV-2 erweitert. Über ARS werden Daten zu jedem einzelnen durchgeführten Test an das RKI übermittelt. Es werden Daten zur virologischen Diagnostik zu SARS-CoV-2 tagesaktuell elektronisch übermittelt. Die besondere Wertigkeit der laborbasierten Surveillance zu SARS-CoV-2 besteht darin, dass über ein etabliertes Surveillancesystem alle in der Routine durchgeführten Testungen erfasst werden, d. h. auch negative Befunde, und damit eine Aussage zum Umfang der in den teilnehmenden Laboren durchgeführten Diagnostik gemacht werden kann.

Weiterhin weniger klar ist Umfang und Art des klinischen Problems, das sich hinter dieser Beobachtung verbirgt. Der vermehrte Nachweis in den Einrichtungen ist auch am Nationalen Referenzzentrum für Staphylokokken und Enterokokken zu bemerken, wie z. B. an (i) steigenden Einsendezahlen seit 2015 und (ii) einem hohen Bedarf an Typisierungen; letzteres ist nahezu gleichbedeutend mit einem Verdacht auf Transmissionen/Cluster von Infektionen und Besiedlungen in betroffenen Einrichtungen („Ausbrüche“). In den Einsendungen an das NRZ wird zudem, wie im Epidemiologischen Bulletin 35/2019 ausgeführt wird, ein Anstieg an Isolaten mit Resistenzen gegen Reserveantibiotika, dabei vor allem Isolate mit Linezolid-Resistenz, z. T. auch aus lokalen Häufungen, beobachtet.

Background Infections due to antibiotic-resistant bacteria are threatening modern health care. However, estimating their incidence, complications, and attributable mortality is challenging. We aimed to estimate the burden of infections caused by antibiotic-resistant bacteria of public health concern in countries of the EU and European Economic Area (EEA) in 2015, measured in number of cases, attributable deaths, and disability-adjusted life-years (DALYs). Methods We estimated the incidence of infections with 16 antibiotic resistance-bacterium combinations from European Antimicrobial Resistance Surveillance Network (EARS-Net) 2015 data that was country-corrected for population coverage. We multiplied the number of bloodstream infections (BSIs) by a conversion factor derived from the European Centre for Disease Prevention and Control point prevalence survey of health-care-associated infections in European acute care hospitals in 2011-12 to estimate the number of non-BSIs. We developed disease outcome models for five types of infection on the basis of systematic reviews of the literature. Findings From EARS-Net data collected between Jan 1, 2015, and Dec 31, 2015, we estimated 671 689 (95% uncertainty interval [UI] 583 148-763 966) infections with antibiotic-resistant bacteria, of which 63.5% (426 277 of 671 689) were associated with health care. An aim of the ECDC point prevalence survey (PPS) in European Union/European Economic Area acute care hospitals was to acquire standardised healthcare-associated infections (HAI) data. We analysed one of the most common HAIs in the ECDC PPS, healthcare-associated pneumonia (HAP). Standardised HAI case definitions were provided and countries were advised to recruit nationally representative subsets of hospitals. We calculated 95% confidence intervals (CIs) around prevalence estimates and adjusted for clustering at hospital level. Of 231,459 patients in the survey, 2,902 (1.3%; 95% CI: 1.2-1.3) fulfilled the case definition for a HAP. HAPs were most frequent in intensive care units (8.1%; 95% CI: 7.4-8.9) and among patients intubated on the day of the survey (15%; 95% CI: 14-17; n = 737 with HAP). The most frequently reported microorganism was Pseudomonas aeruginosa (17% of 1,403 isolates), followed by Staphylococcus aureus (12%) and Klebsiella spp. (12%). Antimicrobial resistance was common among isolated microorganisms. The most frequently prescribed antimicrobial group was penicillins, including combinations with beta-lactamase inhibitors. HAPs occur regularly among intubated and non-intubated patients, with marked differences between medical specialities. HAPs remain a priority for preventive interventions, including surveillance. Our data provide a reference for future prevalence of HAPs at various settings.

Background: Clostridium difficile infections (CDI) in humans range from asymptomatic carriage to life-threatening intestinal disease. Findings on C. difficile in various animal species and an overlap in ribotypes (RTs) suggest potential zoonotic transmission. However, the impact of animals for human CDI remains unclear. Methods: In a large-scale survey we collected 1,447 fecal samples to determine the occurrence of C. difficile in small companion animals (dogs and cats) and their owners and to assess potential epidemiological links within the community. The Germany-wide survey was conducted from July 2012-August 2013. PCR ribotyping, Multilocus VNTR Analysis (MLVA) and PCR detection of toxin genes were used to characterize isolated C. difficile strains. A database was defined and logistic regression used to identify putative factors associated with fecal shedding of C. difficile. Results: In total, 1,418 samples met the inclusion criteria. The isolation rates for small companion animals and their owners within the community were similarly low with 3.0% (25/840) and 2.9% (17/578), respectively. PCR ribotyping revealed eight and twelve different RTs in animals and humans, respectively, whereas three RTs were isolated in both, humans and animals. RT 014/0, a well-known human hospital-associated lineage, was predominantly detected in animal samples. Moreover, the potentially highly pathogenic RTs 027 and 078 were isolated from dogs. Even though, C. difficile did not occur simultaneously in animals and humans sharing the same household. The results of the epidemiological analysis of factors associated with fecal shedding of C. difficile support the hypothesis of a zoonotic potential. Conclusions: Molecular characterization and epidemiological analysis revealed that the zoonotic risk for C. difficile associated with dogs and cats within the community is low but cannot be excluded.
