Muscle strength is necessary for the elderly population, to preserve their health and prevent injuries and illnesses, and to maintain their physical independence. With regard to this last point, lower-limb strength is essential, and it has been suggested that, among the large muscle groups, the gluteal muscles are the most important. Gluteus strength is necessary for elderly people. Different exercises yield differences in EMG data for the gluteal group. Background and Aims: Ustekinumab is an effective treatment for inflammatory bowel diseases. However, some patients do not respond to conventional doses. The aim of the study was to evaluate the effectiveness of intravenous maintenance ustekinumab in patients with secondary failure., Methods: Single-center, retrospective study in adult patients with intravenous maintenance ustekinumab. The reduction of biochemical activity markers, ustekinumab trough levels and clinical indices of activity were evaluated. Biological remission was defined as the percentage decrease fecal calprotectin ≥80% and/or final fecal calprotectin ≤250 and C reactive protein <5mg/L., Results: Thirty-one patients were included: Crohn's disease 77.4%. All included patients were bio-exposed and 61.3% had carried ≥2 biologics. Pre-intravenous maintenance mean Harvey-Bradshaw Index was 6.5±4.38 vs 5±3.1 at week 8 (p=0.024) vs 4.1±3.1 at week 24 (p=0.019). The median ustekinumab trough level pre-intravenous maintenance was 1.40μg/ml [IQR 2.3] vs 5.35μg/ml [IQR 4.1] at week 8 (p<0.001) vs 4.8μg/ml [IQR 3.9] at week 24 (p<0.001). The pre-intravenous maintenance median fecal calprotectin was 809μg/g [IQR: 2256] vs 423μg/g [IQR: 999] at week 8 (p=0.025) vs 333μg/g [508] (p=0.001) at week 24. At the end of follow-up 48% went into biological remission. The presence of perianal disease was associated with lower biological remission (70.6% vs 27.3%, p=0.025). Median intravenous ustekinumab maintenance time was 8.55 [IQR 23.9] months. In 83.9% of patients no serious infections or malignancy were documented., Conclusions: The use of maintenance intravenous ustekinumab appears to be an effective and safe strategy that can be evaluated as a salvage treatment especially in highly bio-exposed patients., (Copyright © 2024 Elsevier España, S.L.U. Introduction: MicroRNAs (miRNAs) are important epigenetic regulators in Crohn's disease (CD); however, their contribution to postoperative recurrence (POR) is still unknown. We aimed to characterize the potential role of miRNAs in predicting POR in patients with CD and to identify their pathogenic implications., Methods: Of 67 consecutively operated patients with CD, we included 44 with pure ileal CD. Peripheral blood samples were taken before surgery and during follow-up. The patients were classified according to the presence or absence of POR assessed by ileocolonoscopy or magnetic resonance imaging enterography. The miRNAs were profiled by reverse transcription polymerase chain reaction before surgery and during morphological POR or, for those who remained in remission, 1 year after surgery. R software and mirWalk were used., Results: Five human miRNAs (miR-191-5p, miR-15b-5p, miR-106b-5p, miR-451a, and miR-93-5p) were selected for discriminating between the 2 patient groups at presurgery (PS), with an area under the curve of 0.88 (95% confidence interval [0.79, 0.98]). Another 5 (miR-15b-5p, miR-451a, miR-93-5p, miR-423-5p, and miR-125b-5p) were selected for 1 year, with an area under the curve of 0.96 (95% confidence interval [0.91, 1.0]). We also created nomograms for POR risk estimation. CCND2 and BCL9L genes were related to PS miRNA profiles; SENP5 and AKT3 genes were related to PS and 1 year; and SUV39H1 and MAPK3K10 were related to 1 year., Discussion: Different plasma miRNA signatures identify patients at high POR risk, which could help optimize patient outcomes. We developed nomograms to facilitate the clinical use of these results. Background: Cytokines emerge as possible biomarkers of response in Crohn's disease (CD). We aimed to determine the plasmatic cytokine profiles of active CD patients who started infliximab (IFX) treatment and their capacity to predict the response to IFX., Methods: A total of 30 active CD patients receiving an induction therapy of IFX were enrolled in the study. Peripheral blood samples pretreatment were collected. Concentrations of 15 cytokines were measured by Luminex technology. Responses to IFX were evaluated by the drop in fecal calprotectin based on its logarithm-transformed values. A random forest (RF) predictive model was used for data analyses., Results: Samples of 22 patients were analyzed. The RF model ranked the following cytokines as the top predictors of the response: tumor necrosis factor alpha (TNFα), interleukin (IL)-13, oncostatin M (OSM), and IL-7 (p < 0.005). Partial dependency plots showed that high levels of IL-13 pretreatment, low TNFα levels, and low IL-7 levels were associated with a favorable IFX response. Increased levels of OSM and TNFα predicted unfavorable responses to IFX., Conclusions: We here show that a log drop in calprotectin strongly correlates with clinical parameters and it can be proposed as a useful objective clinical response predictor. Drawing on the recent advances in complex network theory, urban mobility flow patterns, typically encoded as origin-destination (OD) matrices, can be represented as weighted directed graphs, with nodes denoting city locations and weighted edges the number of trips between them. Such a graph can further be augmented by node attributes denoting the various socio-economic characteristics at a particular location in the city. In this paper, we study the spatio-temporal characteristics of "hotspots" of different types of socio-economic activities as characterized by recently developed attribute-augmented network centrality measures within the urban OD network. The workflow of the proposed paper comprises the construction of temporal OD networks using two custom data sets on urban mobility in Rome and London, the addition of socio-economic activity attributes to the OD network nodes, the computation of network centrality measures, the identification of "hotspots" and, finally, the visualization and analysis of measures of their spatio-temporal heterogeneity. Our results show structural similarities and distinctions between the spatial patterns of different types of human activity in the two cities. Our approach produces simple indicators thus opening up opportunities for practitioners to develop tools for real-time monitoring and visualization of interactions between mobility and economic activity in cities., Competing Interests: The authors have declared that no competing interests exist.

Introduction: The association between infliximab (IFX) and fecal calprotectin (FC) levels on one hand, and the clinical and endoscopic response of patients with inflammatory bowel disease on the other, is well established., Objective and Methods: To investigate the association between inflammatory biochemical parameters and serum concentrations of IFX during induction treatment with a primary nonresponse in a prospective cohort of Crohn's disease (CD) patients., Results: Of the 35 patients included, 8 (22.8%) had primary nonresponse at the end of induction. Induction IFX levels were lower among primary nonresponders at weeks 6 and 14 (week 6: median IFX level 7.3 vs. 11.2 μg/mL, respectively, p = 0.090; week 14: median IFX level 1.5 vs. 4.7 μg/mL, respectively, p = 0.020). FC levels were higher in patients with primary nonresponse versus primary response at weeks 0, 6, and 14 (week 0: median FC level 1,830 vs. 410 μg/g, -respectively, p = 0.030; week 6: median FC level 1,150 vs. 230 μg/g, respectively, p = 0.074; week 14: median FC level 1,210 vs. 208 μg/g, respectively, p = 0.060). For the multivariate analysis, the median IFX level at week 14 and median FC level at week 0 were independently associated with primary nonresponse. A significant inverse correlation was determined between FC level at week 0 and IFX level at week 14 (Spearman's rho correlation, 0.440; p < 0.05)., Conclusions: IFX levels (at week 14) and baseline FC levels could predict primary nonresponse after induction IFX therapy in patients with CD. A high baseline inflammatory load might modify the pharmacokinetic processes of anti-tumor necrosis factor drugs. Manual resistance training (MRT) has been widely used in the field of physical therapy. It has also been used as a strength training method due to the accommodating resistance nature of this modality. The aim of the present study was to compare the effects of an 8-week MRT program on maximum strength and muscular endurance in comparison to conventional resistance training in recreationally trained men. Twenty healthy recreationally trained male subjects were recruited and divided into a MRT training group and a conventional training (CT) group. CT group performed bench press and lat pull-down exercises, and the MRT group performed similar movements with resistance provided by a personal trainer. Both groups completed similar training protocol and training load: 2 training sessions weekly for 3 sets of 8 repetitions at an intensity of 8 to 10 on the perceived exertion scale of 0-10. Initial maximum strength differences were not significant between the groups. Neither group showed significant changes in muscular strength or endurance. Despite the statistically non-significant pre- to post differences, a trend for improvement was observed and effect size (ES) calculations indicated greater magnitude of effects for strength and endurance changes in the MRT group in lat pulldown (g=0.84) compared to CT group. Effectiveness of MRT is similar to CT for improving muscular strength and endurance. MRT can be used as a supplemental or alternative strength training modality for recreationally trained subjects, or be considered by personal trainers especially in low equipped facility conditions.

This paper analyzes success public spaces (specifically plazas ) in the urban fabric of the city of Murcia, Spain. Two approaches were adopted. Firstly, the city was visualized as a complex network whose nodes represent plazas . A centrality algorithm was applied to determine the importance of each node. Secondly, data sets were used from social networks Foursquare and Twitter, which provide different types of data as well as user profiles. Foursquare data indicates user preferences of urban public spaces, while in this respect Twitter offers less specific user generated data. Both perspectives have facilitated two rankings based on the most visited plazas in the city. The results enabled a comparative study to determine the potential differences or similarities between both approaches.
